実施例1:低用量のRAD001およびBEZ235またはCCG168の組み合わせはR6/2脳切片においてオートファジーを誘導する
最近の研究は、オートファジーがmHtt蓄積にとって重要なクリアランス経路であること、またmTOR経路阻害がオートファジーを誘導するのに十分であることを示している。特に、mTOR経路の阻害剤が、ハンチントン病の様々な細胞モデル、ハエ目および動物モデルにおいて、オートファジーを誘導し、mHtt蓄積を低下させて、神経変性を防止することを示した(Ravikumar et al., 2004;Ravikumar et al., 2006;Levine et al., 2008; Sakar et al., 2009)。アロステリックおよび触媒性mTOR阻害剤がオートファジーを誘導し、線条体変性を防止できるかどうかを評価するために、R6/2切片を、DIV14〜DIV21まで、RAD001、BEZ235およびCCG168({5-[2,4-ビス-((S)-3-メチル-モルホリン-4-イル)-ピリド[2,3-d]ピリミジン-7-イル]-2-メトキシ-フェニル}-メタノール)を用いて処理し、DIV21でのmTOR活性およびLC3-II/I転換を分析した(図1)。300 nM BEZ235および300 nM CCG168はオートファジーを誘導したが、一方で250 nM RAD001および50 nM BEZ235は効果を示さなかった。興味深いことに、50 nM BEZ235は、250 nM RAD001および250 nM RAD001/50 nM BEZ235とは対照的に、p-S6 S240/244のレベルを低下させなかった。驚くべきことに、250 nM RAD001/50 nM BEZ235および250 nM RAD001/50 nM CCG168の組み合わせは、250 nM RAD001および50 nM BEZ235単一濃度と比較して、オートファジーを誘導するのに十分であり、BEZ235およびCCG168が、LC3-IIレベルを増加する際にRAD001と相乗的に働くということを示す(図1)。
RAD001/BEZ235の組み合わせが、ポリQ含有物の低下において相乗的に働くかどうかを調べるために、DIV21時の処理済R6/2切片のポリQ密度を、R6/2切片中のポリQ含有物および核の免役組織化学的検出を比較することにより測定した。250 nM RAD001または50 nM BEZ235で処理した切片は、コントロールのR6/2と比較して、同様のポリQ分布を示したが、RAD001/BEZ235およびRAD001/CCG168の組み合わせは、ポリQ密度を低下させた。単一の低濃度RAD001およびBEZ235はポリQ含有物を低下させないが、この組み合わせは有意な効果を示し、R6/2切片と比べてポリQ密度を40%±11低下させたことを観察した。さらに、有効な単一の300 nM BEZ235の濃度は、同様の低下を導き、BEZ235がRAD001と共に、ポリQ密度を低下する際に相乗的に働くことを示唆する(図2)。
低用量のRAD001およびBEZ235またはCCG168の組み合わせは、R6/2脳切片中の線条体変性を防止する
HD進行は、線条体DARPP-32の漸増的損失および軸索変性を特徴とする(The Huntigton's Disease Collaborative Research Group. 1993;Davies et al., 1997;Bibb et al., 2000;Luthi-Carter et al., 2000)。R6/2切片において、DIV21に開始する線条体DARPP-32および神経フィラメントの漸増的損失を追跡した。それ故に、RAD001/BEZ235の組み合わせが、線条体変性の開始を相乗的に防ぐことができるかどうかを評価するために、DIV21時のDARPP-32および神経フィラメント染色を、WTおよびR6/2切片中のDARPP-32および神経フィラメントの免役組織化学検出により分析した。単一の低濃度のRAD001およびBEZ235は、DARPP-32および神経フィラメントレベルの保護において有効ではなかった。しかし、この組み合わせは、95%±12の神経フィラメントレベルおよび80%±5のDARPP-32レベルを保護した。250 nM RAD001/30 nM BEZ235は、最も低い有効な組み合わせ濃度であった(図3および4)。250 nM RAD001および50 nM CCG168の組み合わせは、DARPP-32および神経フィラメントレベルを保護する際に有効であった。
これらの結果を合わせると、アロステリックmTOR阻害剤および触媒性mTOR阻害剤の組み合わせ、例えばRAD001/BEZ235またはCCG168は、相乗的に働いてオートファジーを誘導し、ポリQ含有物を減少させ、線条体変性を防止することを示しており、驚くべきことに、この組み合わせは、触媒性mTOR阻害剤単独と比べて低濃度で、例えば300 nM BEZ235と比べて10倍低い濃度で有効であることを示す。したがって、低用量のアロステリックmTOR阻害剤および触媒性mTOR阻害剤の組み合わせは、HDおよび凝集傾向のタンパク質を原因とする他の神経変性疾患を、少量の薬物暴露により治療する機会を提示する。
上記実施例1〜3を、以下の実験方法を用いて行った:
マウスおよび切片の培養物
ヒトのハンチントン・エクソン-1Q200を発現する遺伝子組み換えマウス(Davies et al., 1997に記述したR6/2マウスモデル)を、温度制御された室内で飼育し、12時間の明/暗サイクルで維持した。食事および水は自由摂取とし、実験を、研究室動物の使用および飼育のための地域承認のガイドラインに従い行った。切片培養物を、ストッピニ(Stoppini)ら(Stoppini et al., 1991 および Gogolla et al., 2006)に記述された方法にしたがって確立し、特別な切断角を利用して、保持された大脳皮質-線条体経路を有する脳切片を作成する。最終的に、切片を選択してMillicel上に置き(Millipore, PICM03050)、培養培地(1 ml)において、35℃で5%CO2にて6ウェルディッシュで培養した。
切片を、DIV14〜DIV21の間、様々なmTOR阻害剤により処理した。該培養培地を、2日毎に交換して、薬剤を新しい培養培地に加えた。我々は、このプロトコールを使用して、後記mTOR阻害剤の作用を評価した:BEZ235(DMSO中、50および300 nM)、RAD001(DMSO中、250 nM)、CCG168(DMSO中、300 nM)、RAD001+BEZ235(DMSO中、250 10、250 30および250 50 nM)およびRAD001+CCG168(DMSO中、250および50 nM)。
切片を、PBSで洗浄して、Complete Mini(Roche, #04693124001)およびPhosSTOP(Roche, #04906837001)を含有する1% Triton X-100/PBS中に溶解した。この溶解液を、超音波処理し、β-アクチン(Sigma, #A5441)、pS6 Ser 240/244(Cell Signalling, #2215)、およびLC3B(Cell Signalling, #2775)についてウェスタンブロッティングにより分析した。イムノブロッドをECL検出試薬により発色させた(Amersham Biosciences)。
切片を、4%PFA中で10分間固定して、PBSで洗浄し、0.3% トライトンX-100 20% ウマ血清/PBS(ブロッキング溶液)中で4時間室温にてブロッキングした。DARPP-32(Cell Signaling, #2306S, 1:200)、神経フィラメント(NeuF, Developmental Studies Hybridoma Bank, University of Iowa;#2H3, 1:200)およびEM48(Millipore, #MAB5374, 1:200)に対する抗体を、ブロッキング溶液中で、48時間4℃でインキュベートした。その後、切片をPBSで洗浄して、Alexa 488(Invitrogen, 1:500)および二次抗体結合Alexa 555(Invitrogen, 1:500)と共に0.3% Triton X-100/PBS中で2時間インキュベートした。最終的に、切片を、PBSで洗浄して、10分間DAPIと共にインキュベートして(Invitrogen, #D1306 1:10000)、ProLong(Invitrogen, #P36934)を用いてガラス皿上に組み込んだ。
高分解能画像を、Plan-Neofluar 40x/1.3油浸対物レンズを用いて正立Zeiss LSM700共焦点顕微鏡で得た。ポリQ密度およびDARPP-32/NeuFシグナル強度の定量のために、各実験について少なくとも3つの、共焦点3Dのスタック/薄片を線条体にて得て(5つの切片/症状)、Imaris 4.2(Bitplane AG)およびImage J ソフトウェアを用いて分析した。
全てのデータを、平均±SEMとして表した。統計学的分析を、分散分析(ANOVA)に続いてスチューデントt検定により行った(Excel, Microsoft, USA)。有意レベルを、p<0.05に設定した。
試薬および機器:TR−FRET mTOR結合アッセイ成分を、Invitrogen Corporation(Carlsbad/CA, USA)から購入できる:GFP-FKBP12(サンプル691-145-3B, 47 μM, MW=38KDa)、Tb3+−α-GST抗体(Cat. No. PV4216)、専用TR-FRET希釈緩衝液 (Cat. No. PV3574)、GFP-4EBP1(Cat. No. PV4759)、DMSO(Fluka, Cat. - 該アロステリックmTOR阻害剤が、RAD001であり、該触媒性mTOR阻害剤がBEZ235またはCCG168である、神経変性疾患の処置または予防において使用するための、請求項2記載の組み合わせ。
- −該RAD001が、1.4〜1.6 mg/kg/日の用量で投与され、該BEZ235または該CCG168が、2.4〜2.6 mg/kg/日の用量で投与される;

神経変性疾患の処置または予防において使用するための、請求項3記載の組み合わせ。 - 該RAD001が、約1.5 mg/kg/日の用量で投与され、該BEZ235または該CCG168が、約2.5 mg/kg/日の用量で投与される、神経変性疾患の処置または予防において使用するための、請求項3記載の組み合わせ。
- 該神経変性疾患が、ハンチントン疾患、パーキンソン疾患、脊髄小脳失調症3型、アルツハイマー疾患、運度神経疾患および末梢神経障害から選択される、神経変性疾患の処置または予防において使用するための、請求項1〜5のいずれか一項記載の組み合わせ。
- 該ハンチントン疾患の処置または予防における使用のための、請求項6記載の組み合わせ。
- 該アロステリックmTOR阻害剤および該触媒性mTOR阻害剤が経口投与される、神経変性疾患の処置または予防において使用するための、請求項1〜7のいずれか一項記載の組み合わせ。
- 神経変性疾患の処置または予防の必要のある対象において、かかる対象に、有効量のアロステリックmTOR阻害剤および触媒性mTOR阻害剤の組み合わせを投与することを含む、神経変性疾患の処置または予防するための方法。
- 該アロステリックmTOR阻害剤が、RAD001、シロリムス、テムシロリムスおよびデフェロリムスから選択され、該触媒性mTOR阻害剤が、BEZ235、CCG168、Ku-0063794、WYE-354および8-(6-メトキシ-ピリジン-3-イル)-3-メチル-1-(4-ピペラジン-1-イル-3-トリフルオロメチル-フェニル)-1,3-ジヒドロ-イミダゾ[4,5-c]キノリン-2-オンから選択される、請求項9記載の方法。

−該RAD001が、0.01〜10.0 mg/kg/日の用量で投与され、該BEZ235または該CCG168が、0.01〜10.0 mg/kg/日の用量で投与される、請求項11記載の方法。 - 該RAD001が、約1.5 mg/kg/日の用量で投与され、該BEZ235または該CCG168が、約2.5 mg/kg/日の用量で投与される、請求項11記載の方法。
- 該神経変性疾患が、ハンチントン疾患、パーキンソン疾患、脊髄小脳失調症3型、アルツハイマー疾患、運動神経疾患および末梢神経障害から選択される、請求項9〜13のいずれか一項記載の方法。
- 該神経変性疾患が、ハンチントン疾患である、請求項14記載の方法。
- 該アロステリックmTOR阻害剤および該触媒性mTOR阻害剤が経口投与される、請求項9〜15のいずれか一項記載の方法。
- 神経変性疾患の処置または予防において、同時に、別々に、または逐次的に使用するためのアロステリックmTOR阻害剤および触媒性mTOR阻害剤を含む、組み合わせ製品。

- 神経変性疾患の処置または予防において、同時に、別々に、または逐次的に使用するための、個別調製物または組み合わせ調製物のいずれかとして、1または複数回用量の90〜110 mg RAD001および165〜185 mg BEZ235またはCCG168; 80〜120 mg RAD001および155〜195 mg BEZ235またはCCG168;70〜130 mg RAD001および145〜205 mg BEZ235またはCCG168;60〜140 mg RAD001および135〜215 mg BEZ235またはCCG168;50〜150 mg RAD001および125〜225 mg BEZ235またはCCG168;50〜200 mg RAD001および100〜300 mg BEZ235またはCCG168;25〜175 mg RAD001および100 〜 250 mg BEZ235またはCCG168;1〜200 mg RAD001および50〜300 mg BEZ235またはCCG168;1〜300 mg RAD001および1〜500 mg BEZ235またはCCG168;または1〜500 mg RAD001および1〜800 mg BEZ235またはCCG168を含む、請求項19記載の組み合わせ製品。
- 神経変性疾患の処置または予防において、同時に、別々に、または逐次的に使用するための、個別調製物または組み合わせ調製物のいずれかとして、1または複数回用量の約100 mg RAD001および約175 mg BEZ235またはCCG168を含む、請求項19記載の組み合わせ製品。
- 該神経変性疾患が、ハンチントン疾患、パーキンソン疾患、脊髄小脳失調症3型、アルツハイマー疾患、運動神経疾患および末梢神経障害から選択される、神経変性疾患の処置または予防において、同時に、別々に、または逐次的に使用するための、請求項17〜21のいずれか一項記載の組み合わせ製品。

- 神経変性疾患の処置または予防において使用するための、アロステリックmTOR阻害剤、触媒性mTOR阻害剤および医薬上許容し得る担体または希釈剤を含む、医薬組成物。
