Abstract
Description
抗腫瘍ワクチンは、ヒト乳頭腫ウイルス (HPV), カポジ肉腫 ヘルペスウイルス (KSHV), エプスタイン Bar ウイルス誘発性のリンパ腫(EBV)を含むウイルス誘発性の悪性疾患のみならず、腫瘍抗原(例えば、MAGE, BAGE, RAGE, GAGE, SSX-2, NY-ESO-1, CT-抗原, CEA, PSA, p53 またはPRAME)を提示する散発性の悪性疾患などの抗癌治療から悪性疾患の治療または予防法にわたる多くの治療分野に適用が見出される。任意の抗腫瘍ペプチドワクチンで得られる最も好適な免疫応答は、前記ペプチド内のT細胞エピトープで惹起されるT細胞応答である。好ましい結果がえられる抗腫瘍T-細胞応答は、HLA クラス I拘束性CTL応答および同時にHLA クラス II 拘束性Th応答の両方からなるだろう、また有利なB-細胞応答を伴なってもよい。幾つかの文献によって、クラス II エピトープ提示樹状細胞(DC)との相互作用でCD4+ T細胞がCD40 リガンドをアップレギュレートすることが実証されている。
本発明において、患者におけるPRAME発現癌細胞に対する効率的および成功したワクチン-誘導性のT細胞応答の誘導に要求される異なる側面は、至適なPRAME由来ワクチン ペプチドの設計および選択に関する組み合わせである。至適なPRAMEワクチンペプチドは、少なくとも一つ、好ましくはそれ以上のCD4+ Th リンパ球 応答を誘発する能力が証明された少なくとも一つのPRAME由来ペプチドとともに含まれるべきである。実験セクションは、配列および長さ/サイズに関してワクチン接種のためのPRAME由来ペプチドの至適な設計および選択に必要とされるパラメータを提供する。実験セクションは、全長 PRAME タンパク質に存在し、19-45 アミノ酸の至適な 長さを有しているペプチドに組み合わせることができる、HLA クラス I 提示CTL エピトープおよびCD4+ Th リンパ球反応性誘導ペプチドのインビトロおよびインビボでの同定および確認の両方を開示する。
ペプチドの合成での生産
これらの研究に使用された全てのペプチドを、標準のFmoc化学を用いて自動化マルチプルペプチドシンセサイザー(Abimed AMS 422)における固相ストラテジーにより合成した。CTL誘導のための短いペプチドを、20 μlのDMSOに溶解し、0.9% NaClで1 mg/mlのペプチド濃度に希釈し、使用前に-20℃で貯蔵した。HLA クラス I ペプチド 結合 アッセイに使用されたフルオレッセイン標識参照ペプチドを、Cys-誘導体として合成した。標識化を、5-(ヨードアセトアミド)フルオレッセイン(Fluka Chemie AG, Buchs, Switzerland)でpH 7.5 (Na-リン酸水溶液/アセトニトリル 1:1 v/v)で行った。標識ペプチドを、セファデックス G-10で脱塩し、さらにC18 RP-HPLCで精製した。標識ペプチドを、質量分析で分析した。インビトロプロテアソーム 消化 分析およびCD4+ Th リンパ球反応性の分析に使用される27-merおよび22-merのポリペプチドを、上記に記載のとおり合成し、アセトニトリル-水のグラディエントでの逆相-HPLCで精製し、アセトニトリル-水から一晩凍結乾燥した。純度を、質量分析で確認した。
最も優勢であるHLA クラス I 分子への潜在的な結合能を有する8, 9, 10 または11 アミノ酸の長さを有するPRAMEペプチドの選択を、ペプチド 結合 予測 アルゴリズム BIMAS (http://bimas.cit.nih.gov/molbio/hla_bind/) (Parker, et al., 1994, J. Immunol. 152:163) および SYFPEITHI (http://www.syfpeithi.de/)で行った。これらのコンピュータアルゴリズムは、所望のHLA クラス I 分子の結合モチーフを満たしている全長 PRAME タンパク質に含まれるペプチドを探索する。HLA クラス I 分子を、ヒトの集団において高度または少なくとも中等度の普及率(prevalences)で選択した(HLA-A1, HLA-A2, HLA-A3, HLA-A24, HLA-A68, HLA-B7, HLA-B8, HLA-B35, HLA-B60, HLA-B61 および HLA-B62である)。これらの HLA クラス I 分子のヒト集団の間の普及率は、表 1に示される。
HLA クラス I 結合能の実験的な測定に関して、HLA クラス I の競合ベース(competition-based)の細胞結合アッセイをHLA-A1, HLA-A2, HLA-A3, HLA-A24, HLA-A68, HLA-B7, HLA-B8, HLA-B35, HLA-B60, HLA-B61 および HLA-B62に関して発生させたものに使用した(Kessler et al., 2003, Hum Immunol. 64:245)。EBVで形質転換したヒトB細胞(B-LCL)は、それらの自然に提示されたHLA クラス I ペプチドから穏やかな酸処理で「除かれた(stripped)」ものを使用した。B-LCLを収穫し、リン酸緩衝食塩水 (PBS)で洗浄した。そして、ペレット(2 - 15x106 細胞)を、氷に5 min置いた。溶出を、氷冷したクエン酸緩衝剤(0.263 M クエン酸 および 0.123 M Na2HPO4の1:1の混合物, 表 2にリストされたpHに調整した)に正確に90 sで細胞をインキュベートして行った。その後すぐに、細胞を、2% FCSを含んでいる氷冷IMDMで緩衝し、同じ培地でもう一回洗浄し、2% FCS および 2 μg/ml ヒト β2-microglublin (β2M)を含んでいるIMDM培地(Sigma, St. Louis, MO, USA)に4x105 細胞/mlの濃度で再懸濁した。

実際の結合測定によって、49のPRAME ペプチド(9または10 aa. 長)がHLA-A2に対して高度または中等度の親和性(表 3a)を提示し、表 3bに示されたように、93のペプチド(8-, 9-, 10-, 11-mers)が他の HLA クラス I 分子 (HLA-A1, HLA-A3, HLA-A24, HLA-A68, HLA-B7, HLA-B8, HLA-B35, HLA-B60, HLA-B61 および HLA-B62)に高度または中等度の結合能を有することが明らかとされた。これらのHLA クラス I 結合能が証明されたペプチドを、さらにそれらのフランキングタンパク質配列からのプロテアソーム切断での酵素的な解放に関してプロテアソーム消化分析の結果を用いて分析した(図 1)。表 4にリストされるとおり、この分析によって、(1)高親和性のHLA クラス I 結合能を有し、(2)プロテアソーム切断で産生されたC末端であり、(3)プロテアソーム消化分析においてインタクトであると認められるペプチドの選択が可能となる。

インビトロでのプロテアソーム媒介性切断の分析の材料および方法
20S プロテアソームを、Groettrup等〔Groettrup et al. (J.Biol.Chem. 270:23808-23815.;1995)〕に記載のとおりB-LCL 細胞株から精製した。この細胞タイプは、免疫プロテアソームを含有することが知られている。高いLMP2 および 7 含有量が、2-D イムノ-ブロッティングで確認された。動力学を評価するために、消化を異なるインキュベーション期間で行った。ペプチド(27 mers, 20 μg)を、記載(Eggers, et al. 1995. J. Exp. Med. 182:1865)のとおり、1 μgの精製プロテアソームと300 μlのプロテアソーム消化緩衝剤中で37℃で1 h, 4 h および24 hインキュベーションした。トリフルオロ酢酸を添加して消化を中止し、サンプルを質量分析前-20℃で貯蔵した。
ほとんど全体のPRAME aa. 配列をカバーする二十九のオーバーラップするPRAME ペプチド(大抵は27-mers)を、精製した20S プロテアソームでインビトロで消化した。消化のインターバルは、1 hr, 4 hr および 24 hrであった。消化断片の質量スペクトル分析によって、消化したPRAMEペプチド内の多頻度(abundant)および低頻度(low abundant)のプロテアソーム切断部位が明らかとされた。

PRA(16-24), PRA(150-158), PRA(150-159), PRA(253-262) および PRA(254-262)のC-末端は、Nardilysinの多頻度の切断部位で直接的に作られることが予測された。加えて、後者の二つのペプチド〔PRA(253-262), および PRA(254-262)〕は、それらのC-末端でプロテアソーム切断により生じることが実験的に示された。
幾つかの臨時的(occasional)なCTLエピトープ(大抵はそれらのC-末端に塩基性の残基を有する)は、それらのC-末端を解放するための付加的な酵素での非-プロテアソーム切断を必要とする(Tenzer et al., 2005; Cell. Mol. Life Sci 62:1025 および Seifert et al., 2003, Nat. Immunol. 4:375)。本発明は、C-末端がプロテアソームと独立にNardilysin (EC 3.4.24.61) および Thimet オリゴペプチダーゼ (TOP; EC 3.4.24.15)の二つの連続的な切断で生じる一つの係るCTL エピトープ(aa. 配列 ELFSYLIEKを有するPRAMEにおけるポジション 190-198)を含む。
プロテアソーム, NardilysinおよびThimetオリゴペプチダーゼ (TOP)の精製調製物を20 nMの濃度で使用して、無細胞系において合成のHLA-A3提示CTL エピトープ ELFSYLIEK (PRA190-198)にその天然のフランキング領域を伴うものを包含している27-mer (PRA182-208), 19-mer (PRA190-208), 13-mer (PRA190-202), 12-mer (PRA190-201) および 11-mer (PRA190-200)のペプチド(20 uMの濃度で)を消化した。図 2に要約されるとおり、この包括的な消化分析によって、PRA190-198のN-末端が効率的にプロテアソーム切断部位で解放されることが明らかとされた。しかしながら、大多数のCTL エピトープと対照的にC-末端の解放は、Nardilysinでの最初の切断を必要とし、11-mer, 12-mer および 13-merの前駆体エピトープペプチドPRA190-200; 190-201; 190-202と続く11-, 12- および 13-merの前駆体ペプチドから最小の9-mer ELFSYLIEK エピトープへの更なるTOP-媒介性デグラデーションの両方が生じる。
免疫原性の分析を、同定された推定上の HLA クラス I 提示 CTL エピトープのサブセットに関して行った。免疫原性を、合成で生産したCTL エピトープに対するCTLのインビトロ誘導で決定した。さらにまた、発生させたCTL (クローン)を、PRAMEおよび正しいHLA クラス I 分子を共発現している腫瘍細胞を認識する能力に関して試験した。
CTL誘導のための末梢血単核球(PBMC)を、Ficoll-Paque法で健常ドナーの血液から得た。PBMCに存在する全てのAPCを至適に使用するために、我々は活性化B細胞および一次誘導工程の間にAPCとして使用される成熟DCの混合を生じる培養系を開発した。PBMCを、SRBC-ロゼット形成でT細胞フラクションとB細胞および単球を含んでいるフラクションとに分離した。T細胞フラクションを、低温保存した。単球およびB細胞の混合物を、24 ウェルプレートで1x106 細胞/ウェルの濃度を800 U/ml GM-CSF, 500 U/ml IL-4 (PeproTech Inc.) および 500 ng/ml CD40 mAb (クローン B-B20; Serotec)を含んでいる完全(complete)な培養培地中で6 日間培養した。この培養系によって、三重(threefold)の効果が達成された:すなわち、
i) GM-CSFおよびIL-4によって、単球の未成熟樹状細胞への分化が誘導された、
ii) IL-4およびCD40 mAbによって、B細胞の活性化および増殖が生じた(Schultze, et al. 1997, J Clin. Invest. 100:2757)および
iii) CD40 mAbによって、未成熟樹状細胞の成熟が媒介された(Cella, et al. 1996. J Exp Med 184:747)。3日目に、サイトカイン および CD40 mAbを補充した。CTL誘導能をさらに促進するために、APC-混合物を付加的な 2 日間で0.4 ng/ml LPS (Difco Labs), 500 U/ml IFN (Boehringer Mannheim) および 500 ng/ml CD40 mAbで培養した。3日目に、APC-混合物を、50 μg/ml ペプチド (各ペプチドを別々に)で4 hでRTでパルスし, 照射(30 Gy)し、フリーのペプチドを洗浄して除去した。低温保存した自己のT細胞フラクションを、解凍し、磁性ビーズ (Dynal)を用いてCD4+ T細胞を枯渇させた。一次誘導を、96ウェルのU-bottem プレートで行った。10,000/ウェルの濃度でのAPCを、50,000 CD8+ T細胞/ウェルで10% ヒト プール血清 (HPS), 5 ng/ml IL-7 (PeproTech) および 0.1 ng/ml IL-12 (Sigma)を含んでいる培養培地で共培養した。誘導の開始後7日目に、CTLのミクロ培養物(micro-cultures)を、収穫し(プールし)、洗浄し、40,000 応答細胞(responder cells)/ウェルの濃度で96-ウェルU-bottem プレートで10% HPS, 5 ng/ml IL-7 および 0.1 ng/ml IL-12を含んでいる培養培地中で再刺激した。Schultze等〔Schultze et al. (1997, J Clin. Invest. 100:2757)〕によって記載されたプロトコールで作り、照射(75 Gy)し、MHC I 分子から自然に提示されたペプチドを除去するための穏やかな酸溶出の後に、2% FCS および 3 μg/ml β2-microglublin (Sigma)を含んでいる培養培地で4 hでRTでペプチドパルス(50 μg/ml)した自己活性化B細胞(MHC結合実験の材料および方法を参照されたい)を、再刺激APC(restimulator APC)として10,000 細胞/ウェルの濃度で使用した。再刺激を、類似する様式〔20 IU/ml lL-2(Chiron Corp.)で置換されるIL-7は例外〕で14 および 21日で繰り返した。29日目に、CTLバルク培養を、標準の限界希釈手順でクローン化した。CTLクローンを、10% FCS, 1.5% 白血球凝集素(leucoagglutinin)(Sigma) および 240 IU/ml IL-2を含んでいる培養培地に同種のPBMC および B-LCLからなるフィーダー混合物を用いて7〜12日間ごとの特異的な刺激で維持した。
PRAME発現腫瘍細胞を根絶する能力があるワクチン誘導性の抗-腫瘍 CD8+ CTL 応答の至適な誘導および維持に関して、同時のCD4+ Th 応答の誘導が必要とされる(例えば、Bourgeois, et al, 2002. Eur.J.Immunol. 32:2199; Kumaraguru, et al, 2004. J.Immunol. 172:3719; Janssen, et al, 2003. Nature 421:852; Hamilton, et al, 2004. Nat.Immunol. 5:873)。この現象に寄与する一次的な機構は、CD40-リガンドCD40相互作用を介した専門の抗原提示細胞〔主に、樹状細胞(DCs)〕の成熟におけるCD4+ ヘルパー T 細胞集団により提供される援助であり、これは「ライセンシングモデル(licensing model)」と称される(Schoenberger, et al., 1998. Nature 393:480; Lanzavecchia. 1998. Nature 393:413)。幾つかの証拠によって、係るCD4+ Th応答なしで、CD8+ 応答がないか又は最適以下で誘導され、メモリー CD8+ T細胞応答の維持 および リコール(recall)が損なわれることが示された(Belz, et al., 2002. J.Virol. 76:12388)。従って、CD4+ Th 細胞を誘導する能力のあるPRAME タンパク質におけるHLA クラス II 結合 ペプチドを同定することが重要である。これらの PRAME ペプチドは、二つの異なる スクリーニングアッセイを用いて同定された。CD4+ Th 細胞増殖およびTh 細胞により産生されるIFNγの両方を使用して、HLA クラス II 結合に必要な長さ (22-merまたは27-mer ペプチド)を有する51のオーバーラップ PRAME ペプチドのパネルに対する反応性を評価した。最初に、これらの オーバーラップ PRAME ペプチドに予測された結合能を有するHLA クラス II 分子を同定した。
HLA クラス II ペプチド 結合は、HLA クラス I 結合よりも厳密性が低い。HLA クラス IIにおけるペプチド結合は、少なくとも 13 aa.長であり、非常に長くてもよい。というのも、HLA クラス II 結合溝のオープンエンドによって、クラス II 分子へのペプチド結合が許容されて、両方の端で溝をこえて伸びるからである。従って、HLA クラス II 結合 ペプチドの長さの要求性は、HLA クラス I 分子におけるペプチド結合の要求性よりも非常にフレキシブルである。さらにまた、この趣旨に沿って、HLA クラス IIにおけるペプチド結合は、HLA クラス Iにおける結合よりもより無差別(promiscuous)である。しばしば、13〜25 aa.の長さのポリペプチドは、複数の HLA クラス II 分子に結合する能力を有する。HLA クラス II 分子のこれらのフレキシブルなペプチド結合特性の利点は、予測されたペプチド結合を検証するために実際の実験の結合アッセイがあまり必要とされないことである。
CD4+ T 細胞増殖 アッセイに関して、健常ドナーまたはPRAME-陽性癌の患者のいずれかから得た総PBMC (1.5x10e5 細胞/ウェル)を、10% 自己血清(autologous serum)および10 μg/mlの51 オーバーラップ 27-merまたは22-mer PRAME ペプチドを補充したRPMI 培養培地中でU-底面 96-ウェルプレートの8ウェルに播種した。6日目に、50 μlの3H-チミジン (1 mCi/50 ml)を添加し、7日目に3H-チミジンの取込みを測定した。
8健常ドナーおよび7PRAME陽性癌患者の末梢における51のオーバーラップPRAMEペプチドに対するCD4+ Th 細胞反応性の分析によって、51 ペプチドのうちの28がCD4+ Th 細胞でのIFNγ 産生を誘導し、36 ペプチドがCD4+ Th 細胞増殖を誘導することが明らかとされた(表 5B)。
一または二以上のPRAME由来ペプチドを含んでおり、PRAME陽性腫瘍に対する免疫応答を誘導する至適で規定されたT細胞誘導組成物は、HLA クラス I 拘束性CD8+ CTL 応答および, 同時に, HLA クラス II 拘束性CD4+ T ヘルパー 応答の両方を誘発しなければならない。Th 細胞応答は、誘導を増強するため及びCTL 応答を維持するために必要とされる。
1) 少なくとも一つの CTL エピトープ、好ましくは二以上、最も好ましくは免疫原性がCTL誘導によって確認されたCTL エピトープ、より好ましくはHLA-A2によって提示可能なCTL エピトープを含んでいる、
2) 少なくとも一つの CD4+ T ヘルパー細胞反応性のペプチド, 好ましくはPRAME 陽性の悪性疾患を有している患者及び健常ドナーの両方に反応性のものを含んでいる、および
3) 19-45の長さのaa., 好ましくは 30〜35のアミノ酸。
