Abstract
Description
本出願は、2010年8月17日に出願した米国出願第61/374,460号の、35U.S.C.の119条(e)の下での優先権の利益を主張するものである。本出願は、2010年11月30日に出願した米国出願第12/957,011号の優先権の利益も主張する。
ヒト促進細胞(hFC)が同定され、本明細書に記載される。hFCは、一般的に、CD8+およびアルファベータTCR-として特徴付けられる。hFCは、ガンマデルタTCR-またはガンマデルタTCR+でもあり得る(すなわちガンマデルタTCR細胞の非存在は必要とされない)。CD8+/アルファベータTCR- hFCは、以下のマーカーを発現する細胞の存在により特徴付けることができる:CD3イプシロン(hFCの約48%により発現される)、CD19(hFCの約33%により発現される)、CD11c(hFCの約44%により発現される)、CD11b(hFCの約40%により発現される)、Foxp3(hFCの約42%により発現される)、HLA-DR(hFCの約30%により発現される)およびCD123(hFCの約8%により発現される)(図1A)。hFCは、IFN-ガンマ(hFCの約25%)およびCXCR4(hFCの約31%)を発現する細胞の存在により特徴付けることもできる(図1B)。さらに、hFCの約65%は、免疫寛容誘発性形質細胞様樹状細胞(B220+/CD11c+/CD11b-)に似ており、hFCは、抗原特異的Treg細胞を誘導できる。さらに、hFCは、限定することなく、CD16、CD52、NKp30、NKp44、NKp46、CD162、CD11aおよびCD62Lのようなマーカーのより低いレベルでの存在により特徴付けることができる。
レシピエントにおけるドナー骨髄細胞の生着を増進するhFCの能力は、hFCが様々な治療プロトコールの促進において有用であることを示す。hFCが濃縮された(例えば約5%〜約12%のhFCを含有する)細胞組成物は、耐久性のある生着を著しく改善し、移植片対宿主病(GVHD)を排除する。いずれの特定の機構にも結び付けられないが、一旦投与されると、hFCは、骨小腔、脾臓、胎児または成体の肝臓および胸腺を含むレシピエントの体内の様々な造血細胞部位に帰ると考えられる。hFCは、正しい部位に播種され、生着し、キメラ免疫系を確立し始める。幹細胞およびhFCの両方が複合体を形成して、生着のための適当な部位に播種される可能性がある。
項A-コロニー形成細胞アッセイ
(実施例1)
HSCおよびhFCの精製
HSCおよびhFCを、ヒト脊椎骨髄(VBM)または動員末梢血(MPB)から、マルチパラメータ滅菌生細胞選別により単離した(FACSVantage SE:Becton Dickinson)。簡単に述べると、VBMまたはMPBを、直接標識モノクローナル抗体(mAb)で飽和濃度にて30分間染色した。HSC:CD34+/CD45+;およびhFC:CD8+/TCR-/CD56dim/neg。両方の細胞集団を選別し、純度について分析した。85%以上の純度レベルだけを許容した。
HSCおよびhFC選別および計数
HSCを、ISHAGEプロトコールに基づいて選別および計数した。Sutherlandら、1996、「The ISHAGE guidelines for CD34+ cell determination by flow cytometry」、J. Hematotherapy、5:213〜26頁を参照されたい。簡単に述べると、CD45-FITC/CD34-PE組み合わせパラメータは、末梢血幹細胞/前駆細胞区画を臨床的に適切に反映した。プロット1は、前方散乱(FSC;x軸)対側方散乱(SSC;y軸)を用いてフォーマットし、領域(R1)は、デブリを除くリンパ球、単球および顆粒球の集団の周囲に描いた。R1から、プロット2を、CD45 FITC対側方散乱を用いてフォーマットし、CD45-イベントを除くようにR1を描いた。R2から、プロット3を、CD34 PE対側方散乱を用いてフォーマットし、R3を、CD34+集団の周囲にだけ描いた。R3から、プロット4を、CD45-FITC対CD34+細胞のSSCを用いてフォーマットした。特徴的な低SSCおよび低〜中間のCD45蛍光を有するクラスタを形成する細胞をゲーティングし、R4とした。非特異的染色イベントは、この領域から除いた。R4から、プロット5を、FSC(x軸)対SSC(y軸)を用いてフォーマットした。CD34+幹細胞/前駆細胞の全ての蛍光および光散乱基準を満たすイベントのクラスタがプロット5に出現した。
hFCを用いるコロニー形成細胞アッセイ
細胞選別の後に、HSC(CD45+/CD34+)単独またはHSCとhFC(CD45+/CD34+とCD8+/TCR-/CD56dim/neg)または対照としてのHSCとT細胞を、メチルセルロースに直ちに播種(0時間)したかまたは細胞培養培地中で18時間予備インキュベートした後にメチルセルロースに播種した。全ての細胞試料を、4重で培養した。37℃および5%CO2にて14日間培養した後に、50より多い細胞を含有するコロニーを得た。
hFCの亜集団を用いるコロニー形成細胞アッセイ
コロニー形成培養(CFC)アッセイ:15,000のHSCを、30,000のCD8+/アルファベータTCR-/CD56dim/neg hFCとともにまたはなしで、96ウェルプレート中の培養培地中で0時間または18時間培養し、37℃にてインキュベートした。培養後、細胞をメチルセルロースに再懸濁し、CFCアッセイに用いた。コロニーは、14日目に数えた。
(実施例1)
マウスモデルにおけるキメリズムおよび生着
CD8+/TCR- hFCが同種および同系のマウスレシピエントにおける精製HSCの生着を増進することが以前に示されている(Fugierら、2005、J. Exp. Med.、201(3):373〜383頁)。さらに、マウスにおいて、hFCがコロニー形成能を増進し、より多くの原始多能性HSC前駆細胞のin vitroでの発生を助長することが示されている(Rezzougら、2008、J. Immunology、180(1):49〜57頁)。
マウスモデルにおけるCD8+/アルファベータTCR-/CD56dim/negの生着
動物:5〜6週齢の雄の非肥満糖尿病(NOD)/SCID/インターロイキン-2受容体(IL-2r)ガンマ鎖ノックアウト(NSG)マウスを、Jackson Laboratory(Bar Harbor、ME)から購入した。
(実施例1)
鎌状赤血球症(SCD)予備実験
2名の鎌状赤血球症(SCD)患者を、パイロット実験において予め処置して、混合キメリズムを確立することを試みた。これらのSCD患者はともに、彼らの疾患からの合併症について高い危険性があった。200cGyのTBIとフルダラビン、MMFおよびCyAとの組み合わせが、SCD患者における生着を確立できるかを評価した。しかし、一過性の生着だけが達成された。前処置は耐容性がよく、重度の有害事象は発生しなかった。しかし、内因性の造血が再出現した。
SCD患者#3-2005年11月に移植
SCD#3(誕生日98年2月11日)は、アフリカ系米国人女性であり、複数の疼痛発作および急性胸部症候群のエピソードを経験した。彼女は輸血療法を維持した。彼女の鎌状赤血球形質を有するHLA同一の姉妹が、彼女のドナーとなった。この患者は、4回の用量のCampath-1H(30mg/日)での前処置を-53日目に開始し、4回の用量のCampath-1H(30mg/日)で2回目を-24日目に開始した。彼女は、3回の用量のフルダラビン(30mg/m2 IV)を-4日目に開始し、次いで200cGyのTBIを0日目に受けた。

SCD#4(誕生日96年5月23日)は、ナイジェリア人の男性であり、複数の疼痛発作と2回の急性胸部症候群に罹患した後に、1999年に赤血球交換を開始した。この患者の鎌状赤血球形質を有するHLA同一の同胞が、彼のドナーとなった。患者は、SCD#3と同じ非骨髄破壊的前処置を受けた。彼のHSC+hFCの用量は、5.24×106/kgのCD34細胞、0.55×106/kgのαβ-TCR細胞および0.35×106/kgのhFCであった。彼は、前処置に非常によく耐容性があり、FISHに基づいて1ヶ月で生着してキメラであった(88%ドナー細胞)。彼のドナーキメリズムは、697日目に28%であった(図8A)。ドナーT細胞キメリズムは、501日目に34%であった。患者は、移植の後無症状のままであり、優勢に正常RBCを生成している(図8B)。患者SCD#4についての網状赤血球数は、0.5%から1%の間であり、これは正常範囲である(図8C)。
まとめ
この項では、同胞ドナーからのHLA同一骨髄を用いた2名の重度輸血SCD患者の移植の成功について記載した。これらの患者はともに、強さを低減した非骨髄破壊的前処置を用いてうまく移植を受け、2年を超えて疾患なしのままである。参加の時点で、彼らは輸血に依存しており、疼痛発作およびその他の合併症の危険性が非常に高かった。これらの患者はともに、免疫抑制から離脱することに成功した。
サラセミアからの罹患性および死亡についての危険性が高い5名の個体が、以下の参加および除外基準に従うプロトコールにより参加した。
参加基準
以下の基準を確立して、高い罹患性が予測され、早期の死亡の危険性にあるサラセミアの個体を同定した:アルファまたはベータ重症型サラセミアの患者;または他の複合型および輸血依存性ヘモグロビン異常症の患者。個体は、以下の包括的参加基準も全て満たさなければならない:個体は、関連ドナー(同一または1、2もしくは3のHLA-A、-Bもしくは-DR遺伝子座のミスマッチ)を有さなければならない;個体は、心エコー図もしくは放射性核種スキャンにより文書化された適切な心肺機能を有さなければならない(短縮率>26%または駆出率>40%もしくは年齢についての正常値の>80%);個体は、年齢について予測される≧50%のFEV1および/もしくは10歳を超える年齢の患者について、年齢について予測される≧50%のDLCO(ヘモグロビンについて補正)により文書化された適切な肺機能を有さなければならない(患者がPFTを行うことができない場合、小児もしくは成人の呼吸器科医による室内空気もしくはクリアランスについての安静時パルスオキシメータ>85%が必要である);個体は、血清アルブミン>3.0mg/dL、およびSGPTもしくはSGOT<正常な上限値の5倍により証明される適切な肝機能を有さなければならない;そして、個体は、血清クレアチニン<2mg/dLにより証明される適切な腎機能を有さなければならない。血清クレアチニンが>2mg/dLであるならば、クレアチニンクリアランス試験または核医学GFRが、≧50ml/分/1.73m2のGFRを文書化すべきである。このプロトコールについて、年齢制限はない。
除外基準
個体は、以下の基準のいずれかを満たすとこの試験から除外される:個体は、関連ドナーを欠く;個体は、管理不能な感染または重度の併発疾患を有し、低下した強さの移植に耐容性でない可能性がある;個体は、<70%のKarnofsky(16歳を超える患者)またはLansky(16歳未満の小児)スコアにより証明されるように機能性能に重度の障害を示す;個体は、腎機能不全を示す(GFR<50ml/分/1.73m2);個体は、陽性のヒト免疫不全ウイルス(HIV)抗体試験結果を有する;個体は、陽性の血清HCG試験により示されるように妊娠している;個体の唯一のドナーは、移植を意図する時点で妊娠している;個体は、妊娠可能であり、適切な避妊を行っていない;個体は、TBIを妨げ得る放射線療法に以前に曝露された;個体は、エホバの証人である;個体は、管理されない脾機能亢進症を有する;または個体は、重度の同種免疫を示し、適切なPRBCドナーの供給を保証できない。
レシピエント評価
個体の完全な履歴および身体の検査を行う。プレHSCT LanskyまたはKarnofsky状態を推定する。履歴は、診断時の年齢、全体的な成長および発育、輸血の頻度および回数、いずれかの再生不良性発作、以前の処置(例えばヒドロキシ尿素)、ベースラインHbFとA2レベル、同種免疫状態、処置および日時、いずれかのMRIスキャン、輸血療法、感染、無菌性壊死、肝炎の履歴、鉄過剰症、以前の肝生検および病理的知見を含む。
ドナー評価および選択
HLA同一のドナーとレシピエントを用いるか、またはドナーおよびレシピエントのミスマッチの対(例えばハプロタイプ一致まで(親、おば、おじ、いとこ、または同胞))を用いる。骨髄の提供を望む家族メンバーは、HLAの型を決定する。最良の利用可能なマッチを選択する。参加する全てのドナーは、幹細胞採集の前にドナースクリーニングについてのFDAの規則に従って評価する。全ての評価は、移植の30日以内に完了する。小児のドナーは、動員のために考慮する。ドナーがアフェレーシスの良好な候補でないならば、骨髄は腸骨稜から採集する。1名より多くの関連ドナーが利用可能であるならば、より近いマッチング、より若いものおよび/またはCMV陰性ドナーを選択する。全てのドナーを、鉄補充療法に供する。フェレーシスを行ったドナーには、ビタミンKおよび/またはカルシウムを補充できる。
ドナーの移植前の処置
ドナーについて、合計で560ccの血液を、免疫能試験のためのリンパ球をアーカイブに保管するために回収する。これは、移植前の単回の血液提供(450cc)として得ることができる。残りの11の10cc黄色キャップ回収チューブは、1回目の提供の8週間後に得る。小児の骨髄ドナーについて、小児の研究採血についてのNIHガイドラインに従って、1回で3ml/kgより多くを採血せず、6週間にわたって7ml/kgより多くを採血しない。
レシピエント前処置
放射線療法士が個体を検査して、TBIのための線量測定を決定する。中心静脈アクセスを、前処置の開始前に全ての患者において確立する。Campath-1Hを、第1セッションにおいて-53、-52、-51および-50日目に30mgの最大用量で投与し、第2セッションにおいて20mgの最大用量で-24、-23、-22および-21日目に投与する。Campathの小児用量は、サイクル1において10mg/日およびサイクル2において7mg/日である。より小さいレシピエントおよび1歳未満のレシピエントについて、Campath-1Hは、第1計画について0.4mg/kgの切り上げ用量および第2計画について0.3mg/kgの切り上げ用量である。皮下または静脈内のいずれかでのCampathの投与経路は、担当医師の判断による。Campath投与の開始日時は、予定の衝突に合わせて1〜3日間前後できる。フルダラビンは、-5、-4および-3日目に投与する。個体は、TBIを受け、シクロスポリン免疫抑制を-1日目に開始する。ミコフェノール酸モフェチルでの2回目の免疫抑制薬物投与は、HSC+hFC輸血の日の夜に開始する(0日目)。前処置計画を、以下の表に示す。
HSC+hFC細胞処理
動員末梢血幹細胞を、アルファベータTCR T細胞およびB細胞に特異的なモノクローナル抗体とインキュベートし、次いで、免疫磁性分離により枯渇させる。注入細胞の組成物を、CD34 HSC;CD8+/TCR-/CD56dim/neg hFC;γδT細胞およびαβ-TCR+ T細胞について染色する免疫蛍光により評価する。細胞枯渇の妥当性をフローサイトメトリー分析により決定し、臨床医に、注入の前に予備的な細胞用量を知らせる。細胞生成物は、細菌、真菌および内毒素についても分析する。HSC+hFC生成物を、施設のガイドラインに従って、モニタリングされた状況において中心静脈ラインにより注入する。
細胞投与アルゴリズム
できるだけ多くのHSC、hFCおよび前駆細胞を、最大限に可能なT細胞用量の関係において投与して、GVHDを回避する。現在、最大限の投与量は、レシピエントの体重1kgあたり3.0×106〜4.2×106のアルファベータT細胞である(好ましい開始点は、レシピエントの体重1kgあたり3.8×106のアルファベータT細胞)。レシピエントを最短で28日間追跡する。生着が観察されないならば、最大限に可能なアルファベータTCR用量を1単位増加する(レシピエントの体重1kgあたり4×105)。最大限に可能なアルファベータTCR用量は、著しいGVHDなしで安定な生着が達成されるまで増加する。HLA一致移植片について、最大限のT細胞キャップはなく、細胞用量は、これらの一致移植片の転帰に基づいて増加しない。ミスマッチである患者について、最大限に可能なアルファベータTCR用量を決定する。
移植を受けたさらなる鎌状赤血球患者
対象#5は、移植の時点で9歳であった(2006年3月)。彼は、複数の疼痛発作、2回の急性胸部症候群のエピソードを経験した後に移植を受け、交換輸血で7年間処置された。対象は、HLA一致形質の同胞ドナーの腸骨稜骨髄を受け、上記の項Cに記載したことと本質的に同じ計画で前処置された。移植片は、体重1kgあたり5.24×106のCD34+細胞、体重1kgあたり0.55×106のアルファベータ-TCR+細胞および体重1kgあたり0.35×106のFC細胞を含有した。対象は、幹細胞移植後に輸血に依存せず、移植後1525日を超えて100%のドナーRBC生成およびFISHにより20〜30%のドナーキメリズムレベルであった。免疫抑制は、移植後23ヶ月で中止した。対象は、移植の後、移植片対宿主病(GVHD)、移植関連毒性または鎌状赤血球合併症を示していない。
(実施例1)
患者の採用
プロトコールのための候補者を、腎移植を待っている患者または移植について評価されている患者のリストから選択した。この選択プロセスは、University of LouisvilleのInstitute of Cellular Therapeutics(「施設」)の移植外科医および移植看護師コーディネーターが行った。
参加基準
候補患者は、18歳から65歳の間であり、終末器官不全のための腎移植について施設の基準を満たさなければならない。候補患者は、1回目の腎移植を受けなければならない。候補患者は、腎移植だけを受けなければならない。交差適合は、ドナーとレシピエントとの間で陰性でなければならない。妊娠の可能性がある女性は、TBIの開始前48時間以内に妊娠試験(尿検査が許容される)が陰性でなければならず、移植後1年間、信頼できる避妊を用いることに同意しなければならない。候補患者は、現在または過去にドナー特異的抗体の証拠を示してはならない。
除外基準
患者が臨床的に活性な細菌、真菌、ウイルスもしくは寄生体感染であるか、または妊娠しているならば、その患者は候補者でない。患者が、レシピエントが腎移植片を受けることを妨げるウイルス感染の臨床的または血清学的証拠を示すならば、その患者は適格でない。患者が、TBIを妨げる用量の放射線療法を以前に受けているか、ドナーとレシピエントとの間に陽性の交差適合があるか、またはドナーに対向する免疫記憶についての証拠があるならば、その患者は候補者でない。患者の肥満度指数(BMI)が18未満または35を超えても、その患者は除外される。
ドナー選択基準
このプロトコールについてのドナーは、腎臓および幹細胞移植についての施設の基準を全て満たさなければならない。
プロトコール
全ての操作についてのタイミングは、0日目のレシピエントのTBI前処置に関する。-3日目に開始して4日間まで、10μg/kgのG-CSFをb.i.d.投与した。回収は、0日目に開始した。0日目に、CD34計数を行った後に、G-CSFの最終用量を投与した。HSC+hFC移植は、腎臓採集の所望の日時の4〜6週間前に予定した。腎臓の提供および移植は、ドナーの血小板数がベースラインおよび腎臓提供のための安全レベル(例えば100,000/μl全血を超える)に戻るまで予定しない。
移植前の前処置
細胞用量(HSC+hFC)、ならびにレシピエントの前処置の程度および型は、生着に影響する独立した変数であった。今回のプロトコールにおいて、細胞用量および前処置は、>1%ドナーキメリズムが確立されるまで最適化した。HSC+hFCについての初期の標的細胞用量は、≧1×108 CD34+/kgであった。最初の患者は、200cGyのTBI、フルダラビン(-3日目〜-1日目に30mg/m2)、ならびに6ヶ月間または臨床上の必要性に応じてのMMF(0日目に開始して15mg/kg q 12h)およびFK506(-1日目に開始して0.02mg/kg q 12h)を用いる移植後免疫抑制を受けた。患者によって薬物に対する耐容能力が異なるので、FK506(タクロリムス)またはシクロスポリンのいずれを用いるかの決定は、医師に任せた。骨髄は、+1日目に注入した。予定をTable 8(表8)に示す。
転帰
最小限でHSC+hFC注入後約2週間、またはレシピエントが幹細胞移植手順から完全に回復するために必要なだけ長く経過した後に、腎移植を同じドナーから行った。以下のアルゴリズムは、HSC+hFC移植の転帰に基づいて用いた。
細胞投与アルゴリズム
本研究の目的は、GVHDを回避しながら、同種生着のための適当なHSC、hFCおよび前駆細胞を有する移植片を工学的に作製することであった。最大限に可能なアルファベータT細胞用量と結びついた細胞投与アルゴリズムを確立した。例えば、毒性(GVHD)が生じないが生着に耐久性がないならば、最大限に可能なT細胞用量を1単位増加させた(以下を参照されたい)。多くのHSC、hFCおよび前駆細胞を含有する最大限に可能なT細胞用量を投与した。用いたアルゴリズムをTable 9(表9)に示す。
HSC+hFC
ドナーPBMCを処理して、hFCおよびHSCを濃縮した。強磁性アプローチを用いて、GVHD生成T細胞およびB細胞を含む全骨髄組成物のおよそ85%を除去した。得られた生成物は、hFC、HSCおよび前駆細胞が濃縮されていた。処理の妥当性をフローサイトメトリーにより確認した後に、HSC+hFC移植片を注入のために承認した。ドナー細胞採集の72時間後までの骨髄移植の遅延を許容して、骨髄処理および移植を可能にした。用量を調整したBactrimとValcyte(CMV+であるならば)またはValtrex(CMV-であるならば)での予防を開始した。患者は、例えば呼吸、血圧または過敏症を示し得る血管浮腫におけるいずれの変化についても検出するために、骨髄の注入中および注入後に注意深くモニタリングした。
移植後免疫抑制
このプロトコールに参加した対象は、担当医師の判断で、そして施設のプロトコールに従って、標準的な免疫抑制を受けた。1ヶ月目でドナーキメリズムの証拠がない死亡ドナーの腎臓/HSC+hFCのレシピエントおよび生存ドナーのHSC+hFCのレシピエントについて、これは、全般的に、ALGまたはCampathでのリンパ球枯渇導入療法の後のPrografとMMFとを含んだ。Prografのレベルは、8〜12ng/mlの間に維持した。0日目に開始して、MMFを、全般的に、1〜1.5gm b.i.d.で投与した。1回用量のCampathを30mg IVで、手術室において場合によって与えた。SoluMedrolは、Campath用量の1時間前に手術室において500mg IVの用量で、次いで手術後の1日目に250mg IVの用量で、手術後の2日目に125mgの用量で与えた。FK506およびMMFは、キメラである患者において少なくとも6ヶ月間継続して、生着および寛容誘導を助長した。
予備的実質臓器移植プロトコール
予備的実験は、腎移植レシピエントにおけるHSC+hFCおよび免疫に基づく骨髄非破壊的前処置の成功と安全性を証明した。HSC+hFCの投与を行って、安全性を最大限にし、HSCおよびhFC含量を最適化した。全体的な目的は、GVHDを完全に回避することであった。実質臓器寛容プロトコールを、レシピエントの体重1kgあたり最大で0.2×106のT細胞を用いて開始して、用量漸増を行った。GVHDについての他のエフェクター細胞(NK、B細胞およびAPC)が、全アルファベータT細胞に対して比例する量で全て存在するため、全アルファベータT細胞を用量漸増実験において用いた。CD34およびhFC用量を、最大限に可能なT細胞用量内で最適化した。著しい免疫学的事象(すなわち拒絶エピソードまたは抗体生成)は、いずれの患者においても観察されなかった。GVHDを発症した患者はいなかったが、一過性のキメリズムだけが観察された。
予備的プロトコールの結果
初期において、観察されたキメリズムは低く(<0.2%)、一過性だけであった。しかし、全細胞用量を増加すると、耐久性のある混合キメリズムが達成された。移植片に対する免疫応答は、混合リンパ球反応(MLR)アッセイにおける一過性のドナー特異的寛容(より最近移植された患者において観察された)および臨床的または組織学的拒絶エピソードが存在しないことにより証明されるように、骨髄注入により調節された。
実質臓器移植についての改変プロトコール
フルダラビン前処置を加え、腎臓移植片を配置する1ヶ月前にHSC+hFCを投与して生体移植を逐次的に行うために、腎臓/HSC+hFC移植プロトコールを改変した。
(実施例1)
ドナーおよびレシピエントの適格性
全てのプロトコールは、Northwestern Institutional Review Board、FDA IND 13881により承認され、全てのドナーおよびレシピエントからインフォームドコンセントを得た。ドナーおよびレシピエントは、適切な生体移植ドナーおよびレシピエントとして施設の基準を満たさなければならなかった。参加者は、研究への参加を考慮されるために、移植前ドナーおよびレシピエント評価の全ての段階を完了しなければならなかった。移植レシピエントについての参加基準は、18歳から65歳の間の年齢、フローPRA分析により評価されるドナー特異的抗体の非存在、および生体腎臓移植片だけを受けることを含んだ。妊娠可能年齢の女性は、TBIを受ける前48時間以内に妊娠試験(尿検査が許容される)が陰性でなければならず、移植後1年間、信頼できる避妊を用いることに同意しなければならない。除外基準は、臨床的に活性な細菌、真菌、ウイルスまたは寄生体感染、妊娠、TBIを妨げる用量の以前の放射線療法、ドナーとレシピエントとの間の陽性のフローサイトメトリー交差適合、ドナー特異的抗体の存在、肥満度指数(BMI)>35または<18、ならびにHBV、HCVおよびHIVについての陽性の血清学を含んだ。
前処置およびドナー生成物調製
前処置は、図12に示すように、腎移植に関して-4、-3、-2日目の3回の用量のフルダラビン(30/mg/kg/用量);-3および+3日目の2回の用量のシクロホスファミド(Cytoxan;50mg/kg/用量);ならびに-1日目の200cGyのTBIからなった。血液透析を、フルダラビンおよびCytoxanの投与後6〜8時間行った。タクロリムス(標的トラフ濃度8〜12ng/ml)およびミコフェノール酸モフェチル(MMF)(Cellcept;レシピエントの体重が<80kgであるならば1gmを経口で1日2回、レシピエントの体重が>80kgであるならば1.25gmを1日2回)を-3日目に開始し、全体を通して継続した。HSC+hFCは、レシピエントに、0日目(すなわち移植と同日)または+1日目に投与できる。
造血幹細胞回収
腎移植の少なくとも2週間前に、ドナーを、10mcg/kg b.i.d.の顆粒球コロニー刺激因子(G-CSF)を用いて動員し、アフェレーシスを+4日目に行った。生成物を配達業者によりInstitute for Cellular Therapeutics (ICT)に輸送し、成熟移植片対宿主病(GVHD)生成細胞を除去しながら造血幹細胞(HSC)、促進細胞(FC)および前駆細胞を保持するために処理した。生成物を、次いで、新鮮な生成物または低温保存されたもののいずれかとして、注入のためにNorthwestern Universityに送り返した。
免疫学的モニタリング
PHA、カンジダ、破傷風トキソイド、ドナーおよび第三者同種抗原に対するレシピエントの応答を毎月試験した(例えば、Patelら、2008、J. Allergy Clin. Immunol.、122:1185〜93頁を参照されたい)。ドナー抗体を検出するためのフロー交差適合アッセイを1および6ヶ月目に行った。キメリズム試験を、ショートタンデムリピートを用いる分子アッセイにより行った(Akpinarら、2005、Transplant.、79:236〜9頁)。調査監視生検を1年目に行った。選択された時点にて、末梢血の免疫表現型分析をT細胞、B細胞、NK細胞、単球、CD4+/CD25+ Fox P3+制御性T細胞(Treg)およびTエフェクター細胞(Teff)の回復について行った。
キメリズム試験
キメリズムを、ショートタンデムリピート(STR)をコードする単純配列長多型の遺伝子型決定により決定した。系列キメリズム試験のために、CD19+(B細胞)、CD3+(T細胞)およびCD66B+骨髄系細胞を全血から選別し、次いで、分子STRタイピングにより分析した。
免疫抑制の離脱
PrografおよびMMFを、医療の標準に応じて、移植後6ヶ月まで継続した。この時点で、キメリズムまたはドナー特異的寛容が存在するならば、MMFをまず中止し、次いで、Prografを、その後数ヶ月間で治療量未満に漸減させた(例えば9ヶ月間で≦3.0ng/ml)。Prografは、キメリズムおよび/またはin vitroドナー特異的反応性低下の証拠が存在するならば、12ヶ月目に中止した。
結果
対象#1〜対象#9のまとめを以下のTable 12(表12)に示し、Table 13(表13)は、細胞投与計画を示す。数人の対象について、以下により詳細に論じる。
hFCおよび生体腎移植のまとめ
移植された9名の対象のうち、非骨髄破壊的前処置は耐容性がよかった。さらに、全ての対象についての移植後最下点期間は、外来患者として管理が容易であった。
(実施例1)
遺伝性代謝障害の処置
対象#1は、異染性白質ジストロフィーの7歳の小児であった。彼は、6つのうち3つのHLAが一致した移植片を、異染性白質ジストロフィーの形質を有する父親から受けた。対象は、上の項C、実施例2に記載したことと本質的に同様にして前処置を受けた。彼は、外来患者として前処置および注入に対して耐容性が非常によかった。彼は、体重1kgあたり14.4×106のCD34+細胞、体重1kgあたり3.8×106のアルファベータTCR+細胞および体重1kgあたり4.1×106のFCを受けた。彼の最下点は短く、輸血療法を必要としなかった。分子STRによる彼のキメリズムは、80%〜98%の間の範囲である。移植後14ヶ月目に、レシピエントはGVHDを示さなかった。この対象についてのMLRの結果は、ドナーに対する寛容を示し、耐久性のある長期の生着の可能性が確認された。移植前に、対象のアリールスルファターゼA酵素レベルは3であったが、これと比較してドナーのレベルは移植後に50であった。対象のレベルは移植後3および6ヶ月でおよそ50であり、移植後1年目で88.6であった。これは、表現型が正常な患者の酵素レベルを示す。

造血幹細胞(HSC)のドナーソースを準備するステップと、
前記ドナーソースからアルファベータTCR+ T細胞を枯渇させて枯渇ドナーソースを生成するステップと、
前記枯渇ドナーソース中のアルファベータTCR+ T細胞の数を、レシピエントの体重1kgあたりアルファベータTCR+ T細胞1×105より多い数に調整し、
それによりレシピエントへの送達用の治療用細胞組成物を生成するステップと
を含む方法。 - 前記HSCのソースが、骨髄、胸腺、末梢血、胎児肝または胚性卵黄嚢である、請求項14に記載の方法。
- 前記HSCのソースが、骨髄である、請求項14に記載の方法。
- 細胞が、1または複数の抗体を用いて枯渇される、請求項14に記載の方法。
- 1または複数の抗体が、磁性ビーズとコンジュゲートしている、請求項17に記載の方法。
- アルファベータTCR+ T細胞の数が、レシピエントの体重1kgあたりアルファベータTCR+ T細胞約2.0×106から約5.0×106の間に調整される、請求項14に記載の方法。
- アルファベータTCR+ T細胞の数が、レシピエントの体重1kgあたりアルファベータTCR+ T細胞約3.0×106から約4.2×106の間に調整される、請求項14に記載の方法。
- hFCが、前記hFCの非存在下で生着したHSCと比較して、前記HSCの生着能力を改善する、請求項14に記載の方法。
- ドナーの免疫系とキメラであるレシピエントの免疫系を作製する方法であって、
請求項14に記載の治療用細胞組成物を、前処置されているレシピエントに投与するステップ
を含む方法。 - レシピエントの前処置が、300cGyを超えない線量の全身放射線照射(TBI)を含む、請求項22に記載の方法。
- 治療用細胞組成物が、レシピエントに静脈内投与される、請求項22に記載の方法。
- レシピエントの免疫系が少なくとも約1%ドナー起源である場合に、レシピエントの免疫系が、ドナーの免疫系とキメラであるとみなされる、請求項22に記載の方法。
- レシピエントが疾患を有する、請求項22に記載の方法。
