Abstract
Description
本発明は、特に、骨髄炎の予防または治療のため、および整形外科用インプラントまたはグラフトの移植のための、黄色ブドウ球菌(Staphylococcus aureus)感染に対する受動免疫化に関する。黄色ブドウ球菌グルコサミニダーゼに特異的に結合する抗体、およびそれを含有する薬学的組成物は、これらの目的のために使用することができる。
一件あたり概算で$15,000〜70,000の病院費で、米国内で年間約112,000件の整形外科用デバイスに関連する感染が発生しており、慢性骨髄炎(OM)への新規介入治療法に対する大きなニーズが存在する(Darouiche,"Treatment of Infections Associated With Surgical Implants,"N.Engl.J.Med.350(14):1422−9(2004))。外科技術の改善および積極的な抗生物質による予防は整形外科用インプラント手術後の感染率を1〜5%に減少させたが、骨髄炎(OM)は依然として深刻な問題であり、また低侵襲手術によるものは増加しているようである(Mahomed et al.,"Rates and Outcomes of Primary and Revision Total Hip Replacement in the United States Medicare Population,"J.Bone Joint Surg.Am.85(A−1):27−32(2003)、WHO Global Strategy for Containment of Antimicrobial Resistance,2001)。この再燃は、80%が黄色ブドウ球菌によるものであるが、臨床分離株のうち約50%までもが、メチシリン耐性黄色ブドウ球菌(MRSA)であるという事実により、その重要性が増幅される。ここ十年間における人工関節および骨折固定デバイスに関する感染率は、各々ほんの0.3〜11%および5〜15%の症例ではあるが(Lew and Waldvogel,"Osteomyelitis,"Lancet 364(9431):369−79 (2004)、Toms et al.,"The Management of Peri−Prosthetic Infection in Total Joint Arthroplasty,"J.Bone Joint Surg.Br.88(2):149−55(2006))、この結末は、切断または死亡につながり得る。さらに、選択的関節全置換術(TJR)における「低侵襲手術」が普及しているが、この手術は移植の間に人工関節が皮膚に接触することにより、非常に小さい切開創がしばしば合併症につながることがあり、結果としてOMの発生を著しく増加させることとなった(Mahomed et al.,"Rates and Outcomes of Primary and Revision Total Hip Replacement in the United States Medicare Population,"J.Bone Joint Surg.Am.85(A−1):27−32(2003)、WHO Global Strategy for Containment of Antimicrobial Resistance,2001)。これらの感染は、非常に高価な二段階再置換手術を必要とし、最近の報告では成功率が50%と低い可能性があることが示唆されている(Azzam et al.,"Outcome of a Second Two−stage Reimplantation for Periprosthetic Knee Infection,"Clin.Orthop.Relat.Res.467(7):1706−14(2009))。しかしながら、最大の懸念は薬剤耐性株、とりわけMRSAの発生であり、これは、北米における最大の致死性病原体としてHIVをしのぎ、慢性OMの管理をより困難なものにし続け、新規介入治療法を大いに要することとなっている。特に、TJRの主要なレシピエントである免疫無防備状態の高齢者に対する代替的な介入治療戦略に対する大きなニーズが存在する。
一態様において、本発明は、黄色ブドウ球菌グルコサミニダーゼに特異的に結合し、かつ黄色ブドウ球菌のインビボ増殖を阻害する、モノクローナル抗体に関する。本発明のモノクローナル抗体は、メチシリン耐性の黄色ブドウ球菌を標的とするようなものであってもよい。

マウスモデルにおいてOMを定量化するRTQ−PCR方法は、非常に有用であるにも関わらず、このアプローチには、3つの主要な制限が存在する。第1に、この分析は、DNAを採取するためにマウスの殺処理が必要であるため、長期的ではない。第2には、DNA単離、PCR、およびデータ分析の際に、非常に大きな労働力と高度の注意を要する。第3には、黄色ブドウ球菌ゲノムDNA(nuc遺伝子)の検出は、活動的な増殖期にある細菌と、バイオフィルム中に堅く詰まった休眠中の細菌とを区別することができない。このため、RTQ−PCRは、インビボの細菌増殖へのmAb効果を評価するために容易に使用することができない。
OMに対する抗自己分解酵素受動免疫化の可能性を評価するために、初期の活性組み換えGmdワクチン研究を実施し、マウス(n=20)を次のように免疫化した:群1(アジュバント中のPBS(陰性対照))、群2(20μg黄色ブドウ球菌Xen29全プロテオーム抽出物をアジュバントと同容量で1:1に乳化(陽性対照))、群3(アジュバント中の20μg His−グルコサミニダーゼ)。攻撃誘発の28日前に、各ワクチンの150μl乳化液を筋肉注射(i.m.)した。ブースター免疫(筋肉注射、フロイント不完全アジュバント中20μg タンパク質)を、攻撃誘発の14日前に実施した。
実施例6に記載のHis−Gmd免疫化の成功に基づき、このプロトコールを使用して、マウス抗Gmd mAbを作製した。標準手順を使用して、mAbを得た。調製したハイブリドーマの初期プールから、第1のサブセットを選択し、抗Gmd活性のためのELISAによってスクリーニングし、高いアフィニティーを有する第2のサブセットを選択して、ウェスタンドットブロットアッセイ法を行った。
各細胞株(1C11、1E12、2D11、3A8、および3H6)の凍結アリコートを、本発明者らの依頼により調製した供給元から直接入手した(Precision Antibodies,Inc.,Columbia,MD)。凍結細胞を解凍し、次いで、50μg/mLのゲンタマイシンおよび10%ウシ胎児血清(FBS)を補ったダルベッコ改変イーグル培地(DMEM)で洗浄した。採取した培養上清を、遠心分離(10分、1000×g)によって浄化し、凍結させた。

Gmd酵素活性の測定方法は、本質的に、Maniらの方法(Mani et al.,"Isolation and Characterization of Autolysis−Defective Mutants of Staphylococcus aureus Created by Tn917−lacZ Mutagenesis"J.Bacteriol.175(5):1493−1499(1993))である。凍結乾燥および再懸濁した単球菌はGmd活性によって分解され、その結果、490nmでの光散乱が減少した。Gmdを含有する100μLのサンプルを、0.05% Tween20(PBST)を加えたリン酸緩衝食塩水中に希釈し、96ウェルマイクロタイタープレートのウェルに添加した。100μLの0.15%(w/v)の単球菌懸濁液を、各ウェルに添加し、光散乱を即座に測定して、開始A490を概して約0.8と定めた。次いで、プレートを37℃でインキュベートし、光散乱を30分および60分で再測定した。60分でのA490における低減を、Gmd活性の尺度としてとらえた。わずかなバックグラウンドレート(Gmdなし)を控除した。この方法は、Gmd活性をリゾチーム活性と区別しない。
下腿ピンを有するOMモデル(実施例1および6を参照)を使用して、対象mAb 1C11および3A8の、黄色ブドウ球菌のインビボ増殖を阻害する能力を評価した。簡単にいうと、5週齢の雌のBALB/cJマウスに、手術の3日前に、生理食塩水(n=10)、または0.25mlの生理食塩水中の1mgの精製3A8抗Gmd抗体(n=5)、または0.25mlの生理食塩水中の31C11 A8抗Gmd抗体(n=5)の腹腔内投与を行った。手術時に、マウスに、500,000CFUのXen29黄色ブドウ球菌を含有する下腿インプラントを移植した。0、3、5、7、10または11、および14日目において、マウスを撮像してバイオルミネセンスを評価し、各群の代表的な動物からのBLIヒートマップを伴う画像を、図21Aおよび22Aに示す。注目すべきは、抗体力価が有効濃度未満に減少したと推定される時の、11日目までの抗Gmd3A8動物、および5日目までの1C11動物におけるBLI信号の非存在である。本研究における各マウスに対する3日目のBLI値を、各群に対する平均とともに示す(図21B、p=0.02、図22B)。興味深いことには、1C11については、この治療が、3日目で、50%の動物を治療したことである。14日目に取得した各群における代表的な動物からのX線写真により、抗Gmd処理動物には存在しなかった、プラセボマウスにおける骨溶解領域(矢印)を例示する(図21C、22C)。

Claims (50)
- 黄色ブドウ球菌(Staphylococcus aureus)グルコサミニダーゼに特異的に結合し、かつ黄色ブドウ球菌のインビボ増殖を阻害する、モノクローナル抗体またはその結合部分。
- 前記黄色ブドウ球菌がメチシリン耐性である、請求項1に記載のモノクローナル抗体。
- 前記抗体または前記結合部分が、10−9Mを上回るアフィニティーでグルコサミニダーゼの保存されたエピトープに結合する、請求項1に記載のモノクローナル抗体。
- 前記黄色ブドウ球菌グルコサミニダーゼのR3ドメイン内に全体的にまたは部分的に位置するエピトープに、前記抗体または前記結合部分が結合する、請求項1に記載のモノクローナル抗体。
- SEQ ID NO:36に示される、自己分解酵素のアミノ酸776〜873位、776〜931位、776〜1016位または842〜948位にわたるグルコサミニダーゼポリペプチドフラグメントに結合する、請求項1に記載のモノクローナル抗体。
- 以下のアミノ酸配列:

- Gmdの活性を約70〜約80%阻害する、請求項1に記載のモノクローナル抗体。
- 前記インビボ増殖阻害が、500,000CFUの生物発光性黄色ブドウ球菌に感染させた下腿インプラントを移植された動物モデルを使用して測定される、請求項1に記載のモノクローナル抗体。
- ヒト化された、請求項1〜10のいずれか1項に記載のモノクローナル抗体。
- 請求項1に記載のモノクローナル抗体結合部分。
- Fabフラグメント、Fvフラグメント、一本鎖抗体、VHドメイン、またはVLドメインを含む、請求項12に記載のモノクローナル抗体結合部分。
- 請求項1〜11のいずれか1項に記載のモノクローナル抗体または請求項12もしくは13に記載の結合部分を発現する、細胞株。
- ハイブリドーマ1C11、1E12、2D11、3A8または3H6である、請求項14に記載の細胞株。
- 担体と、1つ以上の請求項1〜11のいずれか1項に記載のモノクローナル抗体、または1つ以上の請求項12に記載のモノクローナル抗体結合部分とを含む、薬学的組成物。
- 前記担体が水溶液である、請求項16に記載の薬学的組成物。
- 抗生物質または免疫療法剤をさらに含む、請求項16に記載の薬学的組成物。

- 前記免疫療法剤が、テフィバズマブ、BSYX−A110、またはAurexis(商標)である、請求項18に記載の薬学的組成物。
- 患者に整形外科用インプラントを導入する方法であって、
整形外科用インプラントを必要とする患者に、請求項1〜11のいずれか1項に記載のモノクローナル抗体、請求項12に記載のモノクローナル抗体結合部分または請求項16に記載の薬学的組成物の有効量を投与する段階、
該患者に該整形外科用インプラントを導入する段階
を含む、方法。 - 前記導入する段階の前に、前記投与を繰り返す段階をさらに含む、請求項21に記載の方法。
- 前記導入する段階の後に、前記投与を繰り返す段階をさらに含む、請求項21に記載の方法。
- 前記投与が全身に実施される、請求項21に記載の方法。
- 前記投与が移植部位に直接実施される、請求項21〜24のいずれか1項に記載の方法。
- 前記整形外科用インプラントが、人工関節、グラフト、または合成インプラントである、請求項21に記載の方法。
- 前記人工関節が、人工膝関節、人工股関節、人工指関節、人工肘関節、人工肩関節、人工側頭下顎関節、または人工足関節である、請求項26に記載の方法。
- 前記グラフトまたは前記合成インプラントが、人工椎間板、半月板インプラント、または、合成もしくは同種移植片の前十字靱帯、内側側副靱帯、外側側副靱帯、後十字靱帯、アキレス腱もしくは回旋筋腱板である、請求項26に記載の方法。
- 前記患者に第2の治療剤を投与する段階をさらに含み、該第2の治療剤が抗生物質または免疫療法剤である、請求項21〜28のいずれか1項に記載の方法。
- 前記抗生物質が、バンコマイシン、トブラマイシン、セファゾリン、エリスロマイシン、クリンダマイシン、リファンピン、ゲンタマイシン、フシジン酸、ミノサイクリン、コトリモキサゾール、クリンダマイシン、リネゾリド、キヌプリスチン−ダルフォプリスチン、ダプトマイシン、チゲサイクリン、ダルババンシン、テラバンシン、オリタバンシン、セフトビプロール、セフタロリン、イクラプリム、およびカルバペネムCS−023/RO−4908463から成る群より選択される、請求項29に記載の方法。
- 前記免疫療法剤が、テフィバズマブ、BSYX−A110、またはAurexis(商標)である、請求項29に記載の方法。
- 前記整形外科用インプラントが全関節再置換術のための人工関節であり、前記方法が、
前記患者に該整形外科用インプラントを導入する前記段階の前に、感染した人工関節を該患者から取り出し、かつ該感染に関して該患者を治療する段階
をさらに含む、請求項21に記載の方法。 - 前記治療する段階が、抗生物質、請求項1〜11のいずれか1項に記載のモノクローナル抗体、請求項12に記載のモノクローナル抗体結合部分、請求項16に記載の薬学的組成物またはそれらの組み合わせの有効量を投与することを含む、請求項32に記載の方法。
- 前記全関節再置換術の間の感染または再感染を予防するのに効果的である、請求項32または33に記載の方法。
- 黄色ブドウ球菌感染を治療する方法であって、
黄色ブドウ球菌感染を有する患者に、請求項1〜11のいずれか1項に記載のモノクローナル抗体、請求項12に記載のモノクローナル抗体結合部分または請求項16に記載の薬学的組成物の有効量を投与する段階
を含む、方法。 - 前記投与を繰り返す段階をさらに含む、請求項35に記載の方法。

- 前記患者に第2の治療剤を投与する段階をさらに含み、該第2の治療剤が抗生物質または免疫療法剤である、請求項35〜39のいずれか1項に記載の方法。
- 前記抗生物質が、バンコマイシン、トブラマイシン、セファゾリン、エリスロマイシン、クリンダマイシン、リファンピン、ゲンタマイシン、フシジン酸、ミノサイクリン、コトリモキサゾール、クリンダマイシン、リネゾリド、キヌプリスチン−ダルフォプリスチン、ダプトマイシン、チゲサイクリン、ダルババンシン、テラバンシン、オリタバンシン、セフトビプロール、セフタロリン、イクラプリム、およびカルバペネムCS−023/RO−4908463から成る群より選択される、請求項40に記載の方法。

- 骨髄炎を治療する方法であって、
黄色ブドウ球菌の骨感染または関節感染を有する患者に、請求項1〜11のいずれか1項に記載のモノクローナル抗体、請求項12に記載のモノクローナル抗体結合部分または請求項16に記載の薬学的組成物の有効量を投与する段階
を含む、方法。 - 前記投与を繰り返す段階をさらに含む、請求項43に記載の方法。
- 前記投与が全身に実施される、請求項43に記載の方法。
- 前記投与が、前記黄色ブドウ球菌の骨感染または関節感染の部位に直接実施される、請求項43に記載の方法。
