・トラマドールと、それ自体に有益な薬理学的効果を有する別の活性剤との形態を提供し、最終活性成分について極めて効率的な投与量/体重の関係を可能にすること、又は
・トラマドール及び別の活性剤であるNSAID‐コキシブ‐のいずれか又は両方を、より低い治療投与量で使用できること;
・トラマドールと別の活性剤であるNSAID‐コキシブ‐との組み合わせにより、同一の新しい薬物になり得る形態中で相乗効果を有すること;又は

(rac)−トラマドール・HCl − セレコキシブ (1:1) 共結晶を得るための方法:
例1a: (溶媒支援摩砕(solvent−assisted grinding)による調製)
5mLのステンレス鋼ボールミル反応器に7mmの鋼球2個, (rac)−トラマドール塩酸塩 (48mg, 0.16mmol), セレコキシブ (61mg, 0.16mmol, 1当量) 及びメチルイソブチルケトン1滴を仕込んだ。該反応器を30Hzで45分間揺り動かした。溶媒の痕跡を真空中で除去し、(rac)−トラマドール・HCl − セレコキシブ (1:1) 共結晶を白色固体として得た (109 mg, 定量的収量(quantitative yield))。
機械的スターラー, 添加漏斗(addition funnel)及び冷却器を備え、トラマドール・HCl (26.54 g, 88.5 mmol) 及びセレコキシブ (33.74 g, 88.5 mmol, 1当量)が入った1 L の三つ口フラスコに 122 mL のエタノールを添加した。得られた懸濁液を加熱して還流させた (完全な溶解)。還流を続行しながらシクロヘキサン (203 mL) を徐々に該溶液に添加し (添加時間 20 分)、その後、該溶液を撹拌しながら室温まで徐々に冷却した。55℃において該溶液に例1aで得たもの(form)を接種し、結晶化を始めた。該混合物を0℃で2時間冷却した。
白色固体を焼結漏斗(sintered funnel)no 3で濾過し、0−5℃で溶媒混合物により洗った (1 体積, 60 mL, (0.6 : 1) EtOH / シクロヘキサン)。真空下、室温にて2日乾燥させた後、(rac)−トラマドール・HCl − セレコキシブ (1:1) 共結晶を白色固体として得た (54.6 g, 91 % の収量)。
例1に従って得た(rac)−トラマドール・HCl − セレコキシブ (1:1) 共結晶について、1H−NMR, FTIR, 粉末X線回折, DSC及びTGによって十分に特性決定を行った(図1〜3参照)。
PXRD分析は、Cu Kα放射線をブラッグ−ブレンターノジオメトリー(Bragg−Brentano geometry)で使用するフィリップス X'Pert 回折計で行った。該系は、単一次元リアルタイム複数ストリップ検出器(monodimensional, real time multiple strip detector)を備える。測定パラメータは以下のとおりである: 2θの範囲は毎分8.8°のスキャン速度にて3°〜40°であった (図1参照)。角度2θ及びd−値で表されるピークを表1に詳しく示す。
プロトン核磁気共鳴分析を、5 mmのブロードバンドプローブ(broadband probe)ATB 1H/19F/Xを備えたVarian Mercury 400 分光計にてメタノール−d4 中で記録した。スペクトルは、サンプル5〜10mgを重水素化溶媒0.6 mLに溶かして得た。
FTIR スペクトルは、ビームスプリッター KBr 系, 励起源としての 35 mW He−Ne レーザー及び DTGS KBr 検出器を備えたThermo Nicolet Nexus 870 FT−IRを用いて記録した。スペクトルは4cm−1の解像度にて32回のスキャンで得た。
DSC 分析は、Mettler DSC822eで記録した。1.6230 mg のサンプルを、ピンホール蓋を有する40μLアルミニウムるつぼ中に秤量し、窒素下(50mL/分)、10℃/分で30℃から200℃まで加熱した。
熱重量分析を熱重量分析器(thermogravimetric analyzer)Mettler TGA/SDTA851eにて記録した。3.0560 mg のサンプルをピンホール蓋を有する70μLアルミナるつぼ中に秤量し、窒素下(50mL/分)、10℃/分で30℃から200℃まで加熱した。
結晶構造を単結晶X線回折データから決定した。用いた無色の結晶(prism)(0.33 × 0.16 × 0.11 mm)は、等モル量の (rac)−トラマドール塩酸塩及びセレコキシブをヘプタン及びIPAに溶解させ、種結晶を入れた(seeded)溶液の結晶化から得た。
目的は、本発明の (rac)−トラマドール・HCl − セレコキシブ (1:1) の共結晶のAUCを決定し、該共結晶の各活性成分及び該2種の活性成分の一定の組み合わせと比較することにより、(rac)−トラマドール・HCl 及びセレコキシブの犬における血漿暴露(plasma exposure)を測定することである。
本研究の目的は、足切開後の術後疼痛のラットモデルにおける(rac)−トラマドール・HCl−セレコキシブ(1:1)共結晶、トラマドール及びセレコキシブの鎮痛効果と効能を評価することであった。足底切開の後、ラットは、温度(熱過敏)及び段階的なフォンフライ(von Frey)式フィラメント(機械的過敏)への反応閾値の減少を示す(Brennan et al., Pain 1996, 64, 493)。
動物
オスのウィスターラット(120〜160g、ハーラン、イタリア)を、テスト前に少なくとも5日間、室温調節された部屋に収容した。食糧と水をテスト時間まで適宜与えた。
全てのラットに、(rac)−トラマドール・HCl‐セレコキシブ(1:1)の共結晶又は各薬剤を別々に、蒸留水中0.5%ヒドロキシプロピルメチルセルロース懸濁液へ溶解させて、腹腔内に投与した。投与量は10ml/kgであった。次に、動物の抗痛覚過敏 (antihyperalgesic)又は抗異痛(antiallodynic)反応を薬物投与から60分後に評価した。
オメダ気化器(Ohmeda vaporizer)及び麻酔容器(anaesthesia chamber)を用いて、ラットを獣医用3%イソフルランで麻酔した。外科手術の間、イソフルラン蒸気を動物の鼻に送る管で麻酔を維持した。麻酔後、ラットをうつ向けに寝かせ、右後足をアルコールで清浄した。その後、かかとの端から0.5cmの付近からつま先にかけて、足底面の皮膚を貫き筋膜に至る長さ1cmの切開を23番外科用メスで施した。従って、表面組織(皮膚)及び深部組織(筋肉)の両方と神経を負傷させた。最後に、絹撚の縫合糸(3.0)で足の皮膚を縫い合わし、傷をポビドンで清浄した。
外科手術(足底切開)の4時間後に薬物をテストした;製造物の投与60分後、2つの行動エンドポイント(behavioural endpoint)を診断した:熱過敏又は痛覚過敏及び機械的過敏又は異痛。
過敏又は痛覚過敏は、個々の足の温度を選択的に上昇させるハーグリーブス(Hargreaves)装置(Ugo Basile plantar test)を用いて熱刺激に対する反応測定により評価した(Dirig, et al., J Neurosci Methods, 1997, 76, 183)。動物を、クリスタル製の床を備えた該装置のメタクリル樹脂製ケージ内に入れた。ケージ内の順応期間はおよそ10分であった。熱刺激は、クリスタル製の床の下を移動するランプから生じ、両足に加えられ、学習行動を避けるために2つの刺激の間に最小限1分の間隔をもたせた。ラットは、ランプからくる熱により生じる不快(疼痛)を感じると、自由に足を引き込むことができる;次に、ランプのスイッチを切り、引き込み反応の潜伏時間を秒単位で記録する。動物の足を痛めつけるのを避けるため、ランプのスイッチを32秒後に自動的に切った。痛覚過敏は、賦形剤による対照実験動物の潜伏時間と比較して、反応に対して減少した潜伏時間と定義され、テスト化合物の鎮痛効果は、正常値に向かう潜伏時間の(部分的)回復として観察される (Dirig, et al., J. Pharmacol Expt Therap. 1998, 285, 1031)。
フォンフライ式フィラメントを使用して、機械的異痛をテストした。動物を、メタクリル樹脂シリンダー内の、フィラメントを利用するための穴が開けられた金網を有する台の上に置いた。シリンダー内において、30分の順応期間の後、両方の後ろ足を刺激した(負傷及び非負傷の足、コントロールとして後者を提供)、最も力の弱いフィラメント(0.4 g)から始め15 gフィラメントへ達した。疼痛に対する動物の反応は、フィラメントによって引き起こされる疼痛を伴う刺激の結果として足の引き込みにより明白に示された。足の引き込みを誘発する圧力(グラムあたりの力)の閾値を記録した。テスト化合物の鎮痛効果は、正常値に向かう閾値の(部分的な)回復として観察される。
トラマドールとセレコキシブの相乗相互作用は、R. J. Tallarida, et al., Life Sci., 1989, 45, 947により開示されたイソボログラム分析(isobologram analysis)によって決定した。この方法には、50%用量レベルにおいて所定の相乗的な抗痛覚過敏効果を生じさせるために必要とされる混合物の総量(すなわち、ED50 又はZmix)の決定と、これに対応する単純な加成性により予想される総量(ED50 add 又は Zadd)の決定とが含まれる。特定の固定比率においてZmix < Zaddが成立する場合、その組成物は相乗的な抗痛覚過敏効果を有する。ED50mixとED50addの値は両方とも任意変数である。ED50mixは、各化合物の特定の固定比率の用量‐反応曲線から決定する;ED50addは、個々の薬剤のED50の値から算出する。そして、スチューデントのt検定によりZmixをZaddと比較する。
この研究で、(rac)−トラマドール・HCl−セレコキシブ(1:1)共結晶、トラマドール及びセレコキシブの用量反応(腹腔内経路)を得た。機械的異痛及び熱過敏を行動エンドポイントとして使用した。機械的異痛を評価したとき、全ての薬物は完全な効果を誘発した。
腹腔内に投与された(rac)−トラマドール・HCl−セレコキシブ(1:1)共結晶は、相乗的に作用して、足切開術後疼痛モデルにおける機械的異痛及び熱痛覚過敏の両方を抑制した。
本研究では、ラットの急性単関節炎における機械的異痛及び運動により引き起こされた疼痛への(rac)−トラマドール・HCl−セレコキシブ(1:1)共結晶、トラマドール及びセレコキシブの効果を評価した。コンピューターを備えた行動モニタリングシステム(CBMS)により、運動により引き起こされた疼痛を評価した。この疼痛誘発歩行適応を評価する方法を、誘発された疼痛を評価するフォンフライ(von Frey)法と共に用いることにより、動物が感じる痛みのより良くまたより信頼性の高い全体像が得られた。
ラットのカラゲニンモデル(the rat carrageenan model)は、膝関節の注射に続いて起こる炎症に伴う疼痛を利用する(単関節炎モデル)。本研究の目的は、右膝関節にカラゲニン300μgを注射して誘発した単関節炎ラットの疼痛緩和挙動において、トラマドール、セレコキシブ及び(rac)−トラマドール・HCl−セレコキシブ(1:1)共結晶の鎮痛効果と効能を評価することであった。カラゲニン注射5時間後及び薬剤投与30分後の歩行に関係のある変化を評価するためにCBMSを使用した。歩行障害は、静的(跡面積、跡長さ、跡幅)、動的(停止、弾み)、及び協調運動(位相分散)に分類された異なるCBMSパラメーターで観察した。CBMS歩行分析の15分後にフォンフライ式フィラメントで機械的異痛を測定した。本研究では、診療での負傷又は炎症に関連する中程度から重度の疼痛による共通の処方に起因して、(rac)−トラマドール・HCl−セレコキシブ(1:1)共結晶、トラマドール及びセレコキシブの効果を評価した。
動物
オスのウィスターラット(225−250 g、Charles River Laboratories)を、室温調節された部屋に収容した。食糧と水をテスト時間まで適宜与えた。
全てのラットに、(rac)−トラマドール・HCl‐セレコキシブ(1:1)の共結晶又はトラマドール塩酸塩及びセレコキシブ各薬剤を別々に、蒸留水中0.5%ヒドロキシプロピルメチルセルロース懸濁液へ溶解させて、腹腔内に投与した。投与量は2ml/kgであった。次に、動物の薬剤反応を薬物投与から30分及び45分後に評価した(CBMS及びフォンフライそれぞれ)。
動物を短時間のイソフルラン(IsoFlo(登録商標), Abbott−Esteve, バルセロナ, スペイン)麻酔(3%)において、膝蓋下靭帯を通して右膝関節腔に、カラゲニン(Sigma Chemical, St. Louis, MO)を30−ゲージ針を用いて経皮的に注射し(300μg, 40μl)、関節炎症を誘発した。
CBMS手段を使用して、歩くラットについて詳しい歩行分析を行った。簡潔にいうと、蛍光灯の光をガラス板に透過させた。光線は、内部で完全に反射される。何かが、例えばラットの足が、ガラス面に接するとすぐに、光は下に反射する。これにより、明るい足跡の鮮明な画像が生じる。全動作(run)を、ガラス板の下に置かれたカメラを介して記録した。
本研究において、各足単独に関するパラメーターを分析した:
・跡面積(mm2で示す):このパラメーターは、立脚期において足が接触した床の総面積を表す。
・最大接触面積(mm2で示す):最大接触面積は、立脚時、足が最も大きく床に接触した瞬間の足面積を表す。
・跡幅(mmで示す):これは、跡面積の幅の指標である。
・跡長さ(mmで示す):これは、跡面積の長さの指標である。
・停止(sで示す):これは、ガラス板に足が接触している秒単位の期間である。
・弾み(sで示す):これは、ガラス板に足が接触していない秒単位の期間である。
・弾み速度(m/sで示す):これは、足が弾む時の速度(距離単位/秒)である。このパラメーターは、歩長と弾み期間から計算される。
・使用率(%):これは、停止を歩調周期の百分率として示す。
・停止指数:足がガラス板との接触を失うときの速度の指標である。
・最大接触(秒数で示す):動作を開始してから、足がガラス板に最も大きな面積で接触するまでの秒数である。これは、停止中に制動フェーズから駆動フェーズに変わる時点とみなすことができる。
フォンフライ式フィラメントを使用して、機械的異痛をテストした:動物を、メタクリル樹脂シリンダー内の、フィラメントを利用するための穴が開けられた金網を有する台の上に置いた。シリンダー内において、15分の順応期間の後、両方の後ろ足を刺激した(負傷及び非負傷の足、コントロールとして後者を提供)、最も力の弱いフィラメント(0.4 g)から始め15 gフィラメントへ達した。疼痛に対する動物の反応は、フィラメントによって引き起こされる疼痛を伴う刺激の結果として足の引き込みにより明白に示された。足の引き込みを誘発する圧力(グラムあたりの力)の閾値を記録した。テスト化合物の鎮痛効果は、正常値に向かう閾値の(部分的な)回復として観察される。
足関節へのカラゲニン(CAR)注射によって引き起こされた関節炎は、ラットの歩き方を描写するためのパラメーターのいくつかに変化を生じさせ、注射された足を使いたがらなかった。CARが誘発する歩行変化は、セレコキシブ、トラマドール及び(rac)−トラマドール・HCl‐セレコキシブ(1:1)共結晶により抑制された(図9)。(rac)−トラマドール・HCl‐セレコキシブ(1:1)共結晶の投与は、跡面積、跡長さ、最大接触面積、停止指数及び位相分散を含む多様なCBMS関連パラメーターについて大きな有益な効果(トラマドール及びセレコキシブの個々の投与と比較して)を生じる結果を示した。

(rac)−トラマドール・HCl‐セレコキシブ(1:1)共結晶は、急性単関節炎疼痛のラットモデルのさまざまな誘発疼痛の歩行変化のテストにおいて、単独薬物に対し優れた利益を生じた。

- 請求項1に記載の共結晶であって、前記コキシブが、トラマドール単独又はトラマドールと活性剤としての対応するコキシブとの混合物のいずれかと比べた場合に、
・該共結晶の溶解度が増加し、及び/又は
・該共結晶の用量応答が増加し、及び/又は
・該共結晶の効能が増加し、及び/又は
・該共結晶の溶解性が増加し、及び/又は
・該共結晶の生物学的利用能が増加し、及び/又は
・該共結晶の安定性が増加し、及び/又は
・該共結晶の吸湿性が減少し、及び/又は
・該共結晶の形態多様性が減少し、及び/又は
・該共結晶のモルホロジーが調節される
ように選ばれる前記共結晶。 - 前記NSAIDが
- セレコキシブ,
- エトリコキシブ,
- ルミラコキシブ,
- パレコキシブ,
- ロフェコキシブ,
- バルデコキシブ, 又は
- シミコキシブ
から選択される請求項1又は2記載の共結晶。 - 前記トラマドールが(-)-トラマドール、(+)-トラマドール、(rac)-トラマドール、又はそれらの塩である請求項1〜3のいずれか1項に記載の共結晶。
- 前記コキシブがセレコキシブ又はその塩である請求項1〜4のいずれか1項に記載の共結晶。
- ・遊離塩基としての又は生理学的許容塩としての(rac)-トラマドールと、セレコキシブとを含む共結晶;
・遊離塩基としての又は生理学的許容塩としての(+)-トラマドールと、セレコキシブとを含む共結晶;
・遊離塩基としての又は生理学的許容塩としての(-)-トラマドールと、セレコキシブとを含む共結晶;
から選択される、あるいは好ましくは
・(rac)-トラマドール・HClとセレコキシブとを含む共結晶
を含む、請求項5に記載の共結晶。 - (rac)-トラマドール・HClとセレコキシブとを含む請求項1〜6いずれか1項に記載の共結晶。
- (rac)-トラマドール・HClとセレコキシブとの分子比が1:1である請求項7に記載の共結晶。
- 分子比が1:1の(rac)-トラマドール・HClとセレコキシブを含む請求項8に記載の共結晶であって、ピーク[2θ]を7.1, 9.3, 10.2, 10.7, 13.6, 13.9, 14.1, 15.5, 16.1, 16.2, 16.8, 17.5, 18.0, 19.0, 19.5, 19.9, 20.5, 21.2, 21.3, 21.4, 21.8, 22.1, 22.6, 22.7, 23.6, 24.1, 24.4, 25.2, 26.1, 26.6, 26.8, 27.4, 27.9, 28.1, 29.1, 29.9, 30.1, 31.1, 31.3, 31.7, 32.5, 32.8, 34.4, 35.0, 35.8, 36.2 及び 37.2 [°]に有する粉末X線回折パターンを示す、但し、該2θ値は銅放射線 (CuKα1 1.54060Å)を用いて得られるものである、ことを特徴とする上記共結晶。
- 分子比が1:1の(rac)-トラマドール・HClとセレコキシブを含む請求項8に記載の共結晶であって、3481.6 (m), 3133.5 (m), 2923.0 (m), 2667.7 (m), 1596.0 (m), 1472.4 (m), 1458.0 (m), 1335.1 (m), 1288.7 (m), 1271.8 (m), 1168.7 (s), 1237.3 (m), 1168.7 (s), 1122.6 (s), 1100.9 (m), 1042.2 (m), 976.8 (m), 844.6 (m), 820.1 (m), 786.5 (m) 625.9 (m) cm-1に吸収バンドを有するフーリエ変換赤外パターンを示すことを特徴とする上記共結晶。
- 分子比が1:1の(rac)-トラマドール・HClとセレコキシブを含む請求項8に記載の共結晶であって、下記の寸法の斜方晶系単位格子、
a = 11.0323(7) Å
b = 18.1095(12) Å
c = 17.3206(12) Å
を有することを特徴とする上記共結晶。 - 分子比が1:1の(rac)-トラマドール・HClとセレコキシブを含む請求項8に記載の共結晶であって、融点に対応するシャープな吸熱ピークは164℃に立ち上がりを有することを特徴とする上記共結晶。
- (a) 前記コキシブを溶媒中に溶解又は懸濁させる工程; 場合により該溶液又は分散液を周囲温度よりも高く、かつ、該溶液又は分散液の沸点よりも低い温度まで加熱する工程;
(b) 工程(a)と同時に、又は工程(a)の後に、又は工程(a)の前に、遊離塩基としての又は塩としてのトラマドールを溶媒中に溶解させる工程であって、場合により工程(a)と合体されて、工程(a)において既に前記コキシブと共にトラマドールを溶解させる工程
(c) 場合により(b)の溶液を(a)の溶液に添加しそれらを混合する工程;
(d) 場合により溶媒を(a), (b) 又は (c)の溶液に添加しそれらを混合する工程;
(e) 工程(a), (b), (c) 又は (d)の混合溶液/分散液を周囲温度以下に冷却する工程;
(f) 場合により前記溶媒の一部又は全部を蒸発させる工程; 及び
(g) 生じた共結晶を濾取する工程
を含む請求項1に記載の共結晶を製造する方法。 - 治療有効量の請求項1〜12のいずれか1項に記載の共結晶を生理学的に許容される媒体中に含むことを特徴とする医薬組成物。
- 請求項1〜12のいずれか1項に記載の共結晶であって、糖尿病性神経障害又は糖尿病性末梢神経障害及び変形性関節症、線維筋痛症;リウマチ様関節炎、強直性脊椎炎、凍結肩又は坐骨神経痛を含む、疼痛、好ましくは急性疼痛、慢性疼痛、神経障害性疼痛、侵害受容性疼痛、軽度及び重度から中程度の疼痛、痛覚過敏、中枢性感作に関する疼痛、異痛又は癌性疼痛を治療するための上記共結晶。
