Description

多くの病気の状態は、遺伝子DNAのコピー数の変化を通して、あるいは特定の遺伝子の転写レベルの変化を通して(たとえば、開始、RNA前駆体の提供、RNAのプロセシングなどの制御を通して)、様々な遺伝子の発現レベルの違いによって特徴づけられる。

インターフェロンβ(IFNβ)は、抗ウイルス、抗増殖、または免疫調節として分類されている広い範囲の応答を誘導する。この機能的な多重性は、MSでのその有効性の原因となる作用機構を理解する挑戦に貢献している。インビトロウイルス保護分析(CPE分析)と、タイプI(IFNγ)及びタイプII(IFNβ)との両方のインターフェロンによって誘導される単一の抗ウイルス遺伝子MxAの発現を測定する最近導入された分析の2つの基本的なアプローチが、現在IFNβ生物活性を測定するために使用される。その性質上、これらのウイルスベースの分析のどちらも、IFNβの抗増殖または免疫調節作用に直接的な相関関係を与えるということはできない。この直接的な相関関係を与えるインビトロのIFNβ生物活性を測定するための新規のアプローチが提供される。
健常者、処置を受けていない再発寛解型多発性硬化症(RRMS)患者、およびIFNβ-1B(Betaseronなど)で処理した患者の間の末梢血単核細胞(PBMC)において観察された異なる遺伝子発現パターンに基づいて、臨床的に関連のあるMS-関連遺伝子セットが同定された。図1には、RRMS患者におけるIFNβ-1Bの単回投与に対する応答の遺伝子発現プロファイリングの例が示されている。既述のように、MSと病気の進行に関連付けられることが知られている生物学的調節への関与に基づく病気セット内で、関連遺伝子が特別に選定された。この固有のセットには、IFNβ-1Bにより異常調節され、かつ反対調節される遺伝子のほか、Th1-Th2応答、接着、走化性、インターフェロンシグナル伝達、および細胞周期応答に関連した遺伝子が含まれている。
IRIS遺伝子は、RT-PCRを含む任意の最先端の遺伝子プロファイリング法を使用して、あるいは、遺伝子オリゴヌクレオチドアレイなどのアレイまたはRT-PCRフォーマットマイクロ流体カードによって、直接測定することができる。IRIS遺伝子のフィンガープリントは、選択数の遺伝子だけを含んでいるので、ある実施形態では、低密度アレイを使用することもできる。アレイの作成では、IRIS遺伝子に選択的にハイブリダイズするプローブが、遺伝子発現解析のためのアレイ上に配置される。このアレイは、被験者でのMS処置の有効性を評価するのに有用である。

(c)その中和抗体状態が、現行のウイルス阻害アッセイによって測定して、処置に対する臨床応答の不存在に相関する患者を同定する、および
(d)その中和抗体状態が、IFNβ有効性に対して影響を有しない患者を同定する。これらのそれぞれの分析において、前のセクションに記載された遺伝子の特定のセットの発現が、測定される。以下は、これらの方法のさまざまな側面についての説明である。
遺伝子発現を評価することができるさまざまな方法がある。これらの方法は、蛋白質、またはmRNAレベルのいずれかを見る。mRNAを見る方法は、すべて基本的には、基本的なレベルでは、核酸ハイブリダイゼーションに依拠する。ハイブリダイゼーションは、DNAおよび/またはRNAの相補的なストレッチとで選択的に二重分子を形成する核酸の能力と定義される。想定される適用に応じて、プローブまたはプライマーの標的配列についての選択性の度合いを変化させるためには、ハイブリダイゼーションの条件の変化が用いられるであろう。
多くの核酸、特に、mRNAは、低濃度であるので、核酸増幅は、発現を評価する能力を大幅に向上させる。一般的なコンセプトは、核酸は、関心のある領域に隣接する一対のプライマーを用いて増幅することができるということである。本明細書中で使用される用語「プライマー」は、テンプレートに依存するプロセスで初期の核酸の合成をプライミングすることができる任意の核酸を包含することが意図されている。典型的には、プライマーは、長さ10から20および/または30塩基対のオリゴヌクレオチドであるが、より長い配列を用いることができる。プライマーは、一本鎖の形態が好ましいが、一本鎖、および/または二本鎖の形態で提供されうる。
任意の増幅に続いて、テンプレートおよび/または過剰なプライマーから増幅産物を分離することが望ましい。一実施形態では、増幅産物は、標準的な方法を使用してアガロース、アガロース-アクリルアミドまたはポリアクリルアミドゲル電気泳動泳動によって分離される(Sambrookら、1989)。分離された増幅産物は、さらなる操作のため、ゲルから切り出し、溶出することができる。低融点アガロースゲルを使用して、分離されたバンドは、ゲルを加熱し、続いて、核酸を抽出することによって取り出すことができる。
マイクロアレイは、実質的に平面な基盤、たとえば、バイオチップの表面に、空間的に分布し安定的に結合した複数のポリマー分子を含む。ポリヌクレオチドのマイクロアレイは、開発されてきており、スクリーニングやDNA配列決定などのさまざまな応用の用途が見出される。マイクロアレイが特に用途を見出す1つの領域は、遺伝子発現解析である。
本発明の別の局面において、いずれかの蛋白質ベースの診断のアプローチを採用することができる。タンパク質同定の最も一般的な形態は、抗体を使用することである。本明細書においては、用語「抗体」はIgG、IgM、IgA、IgDおよびIgEなどの免疫学的結合剤を広く意味するものとする。生理的な状況の中で最も一般的な抗体であるため、そして、実験環境で最も簡単に作られるため、一般的には、IgGおよび/またはIgMが好ましい。用語「抗体」は、抗原結合領域を有し、たとえば、Fab'、Fab、F(ab').sub.2、単一ドメイン抗体(DABs)、Fv、 scFv(単鎖Fv)などの抗体断片を含む任意の抗体様分子をも意味する。様々な抗体ベースの構造物および断片を調製し使用するための技術が、当技術分野でよく知られている。ポリクローナルおよびモノクローナルの両方の抗体を調製し特徴づける手段は、当該分野でよく知られている(たとえば、抗体:研究所マニュアル、コールドスプリングハーバー研究所、1988などを参照。参照により本明細書に組み込まれる)。
Claims (19)
- 被験者の多発性硬化症(MS)処置の有効性を評価するためのアレイであって、
1または複数の異常調節遺伝子、および1または複数の逆調節遺伝子に特異的な複数のプローブを含み、
ここにで、前記異常調節および逆調節遺伝子は、インターフェロンβ-1Bの導入に対する応答を示し、
それにより、有効性が、前記処置の前における遺伝子発現と比較した場合の前記処置の後の前記異常調節または逆調節遺伝子の遺伝子発現の変化によって評価されることを特徴とするアレイ。 - 1または複数の異常調節遺伝子が、ギャップ比率分析によって選択される、請求項1記載のアレイ。
- 1または複数の異常調節遺伝子が、表3にリストされている遺伝子から選択される、請求項1または2記載のアレイ。
- 1または複数の逆調節遺伝子が、表2にリストされている遺伝子から選択される、請求項1〜3のいずれか1記載のアレイ。
- さらに、表1にリストされた遺伝子から選択される標準インターフェロンマーカーを含む、請求項1〜4のいずれか1記載のアレイ。
- さらに、分析コントロールマーカーを含む、請求項1〜5のいずれか1記載のアレイ。
- 分析コントロールマーカーが、内因性遺伝子および細胞系譜遺伝子を含む、請求項6記載のアレイ。
- 内因性遺伝子が、GAPDHおよびHPRT1からなる群から選択される、請求項7記載のアレイ。
- 細胞系譜遺伝子がCD3e、CD14、CD19、ITGAX、NCAM、およびCD16からなる群から選択される、請求項7記載のアレイ。
- アレイが、低密度マイクロ流体分析プレートである、請求項1〜9のいずれか1記載のアレイ。
- 請求項1〜10のいずれか1記載のアレイを使用して、被験者における多発性硬化症の処置の有効性を評価する方法。
- 多発性硬化症の処置の有効性を評価する方法であって、
(a)抗MS剤で処置する前に患者から取られた第一の生物サンプル中における、1または複数の異常調節遺伝子および1または複数の逆調節遺伝子の発現レベルを測定し、
ここに、前記異常調節および逆調節遺伝子は、インターフェロンβ-1Bの導入に対する応答を示す、
(b)抗MS剤での初期処置後に患者から取られた少なくとも第二の生物サンプル中の異常調節遺伝子および逆調節遺伝子の発現レベルを測定し、次いで
(c)第二の生物学的サンプル中の異常調節および逆調節遺伝子の発現レベルを、第一の生物学的サンプル中の異常調節および逆調節遺伝子の発現レベルと比較することを含み、
ここに、第一の生物サンプル中の異常調節または逆調節遺伝子の発現レベルと比較した第二の生物サンプル中の異常調節または逆調節遺伝子の発現レベルの変化が、処置の有効性を示すことを特徴とする方法。 - 異常調節および逆調節遺伝子の発現レベルの変化が、MRI、再発率、病気の進行、および障害スコア(EDSS)などの測定可能な臨床応答と関連するパターンを作出し、該パターンが統計的手法を用いて測定される、請求項12記載の方法。
- 異常調節遺伝子が、表3にリストされている遺伝子から選択される、請求項12記載の方法。
- 逆調節遺伝子が、表2にリストされている遺伝子から選択される、請求項12記載の方法。
- 前記生物サンプルが、血液、尿、骨髄、および生検サンプルからなる群から選択される、請求項12記載の方法。
- 多発性硬化症の処置に有用な化合物を同定する方法であって、
(a)1または複数の異常調節遺伝子および1または複数の逆調節遺伝子の発現レベルを分析し、ここに、前記異常調節および逆調節遺伝子は、化合物の処置前の細胞または組織サンプルにおいて、インターフェロンβ-1Bの導入に対する応答を示し、次いで
(b)化合物での処置後の細胞または組織サンプルにおける異常調節および逆調節遺伝子の発現レベルを分析することを含み、
ここに、異常調節および逆調節遺伝子の発現レベルの変動が薬物の有効性を示すことを特徴とする方法。 - インターフェロンβ-1Bの導入に対する患者応答における中和抗体を検出する方法であって、
(a)1または複数の異常調節遺伝子と1または複数の逆調節遺伝子の発現レベルを測定し、
ここに、前記異常調節および逆調節遺伝子が、抗MS剤で処置する前の患者から取られた第一の生物サンプル中において、インターフェロンβ-1Bの導入に対する応答を示し、
(b)少なくとも抗MS剤での初期処置の後の患者から取られた第二の生物サンプル中の異常調節遺伝子および逆調節遺伝子の発現レベルを測定し、次いで
(c)第二の生物学的サンプル中の異常調節および逆調節遺伝子の発現レベルを、第一の生物サンプル中の異常調節および逆調節遺伝子の発現レベルと比較すること、
を含み、
それにより、中和抗体活性が、インターフェロンβ-1Bの有効性を減少させたか、または薬物の有効性に影響を与えなかったかを決定することができることを特徴とする方法。 - インターフェロンβ-1Bの導入に際して差別的に発現する特定の遺伝子のセットの発現プロファイルを含み、MRI、再発率、病気の進行および障害スコア(EDSS)などの測定可能な患者の臨床反応との相互の関連づけに有用であることを特徴とする遺伝子発現フィンガープリント。
