ケース3は、前板部3a、両側板部3b、背板部3c、底板部3d、および、天板部3eによって構成された縦長の直方体類似の形状になっている。前板部3aは背板部3cよりも僅かに低くなっていて、これにより、天板部3eは前板部3aに向かって下り傾斜している。下り傾斜させることは必ずしも必要なことではないが、下り傾斜させることによる特有の利点がある。この利点については、次項で述べる。

図1ないし3が示すように、天板部3eには、ケース3の外部と内部(落下領域7、図4参照)とを連通する挿入口3hを形成してある。本実施形態では矩形としたが挿入口3hの形状は丸でもそれ以外の形状でも構わないが、図3に示すように、持針器101の把持部103を、縫合針Nを把持したまま、手術の際に使用者が簡単に挿入するために充分な大きさであることが必要である。挿入口3hは、後述する仕切壁11のスリット11aの上端と繋がっていて、その中に挿入した把持部103(縫合針Nを把持している)を、そのままスリット11a内に導けるようになっている。前項で述べたように天板部3eを傾斜させたのは、傾斜させることにより使用者に対し正対する(使用者が天板部3eの真上にいることはほとんどない)ようになり、挿入口3hを正面から見ることが可能になる。斜めに見た場合に比べ正面から見れば挿入口3hが最も大きい状態で見ることができるので、挿入口3hに把持部103を挿入しやすくなる。これが、天板部3eを傾斜させた利点の一つである。

前板部3aの幅方向中央には、その上端から縦方向半分ほどまで延びる開口空間が形成されている。この開口空間は、図2が両脇を開口側壁部5a,5bにより、背面を仕切壁11により、さらに底面を開口底板部5cにより、それぞれ囲まれ、仕切壁11から見た前方(図3の上から下方向)と上方は閉鎖されていない。この開口空間のことを、計数領域5とする。計数領域5は、仕切壁11を挟んで落下領域7と対向している。落下領域7は、縫合針Nを落下させるための領域であり(図4参照)、ケース3の内部空間のほとんどがその領域となる。仕切壁11は、落下領域7にある縫合針Nや把持部103に付着した血液等が、計数領域5(その延長線上の使用者)へ飛散しないようにする役目も担っている。開口底板部5cは、後述するように計数

仕切壁11の幅方向中央には、計数領域5の上端から下端まで延びるスリット11aを形成してある。スリット11aは、縫合針Nを把持した持針器101の把持部103を、把持したままの状態で長手方向(図4では上下方向)へ通過させるための溝であって、縫合針Nの厚み方向(同図の左右方向)通過を許さない幅寸法に形成してある。仕切壁11は、シリコンゴムのような可撓性部材によって構成してあるため、仕切壁11に外力を加えると変形して(撓んで)スリット11aの幅が広くなるが、変形後の仕切壁11は、外力を取り去ることにより元の形状に弾性復帰する。このため、実際には、スリット11aの幅寸法は、変形がなくては把持部103を通過させられないが、変形により通過できるように設定してある。把持部103と仕切壁11との間に、できるだけ隙間ができないようにすることにより、計数領域5側への血液等の飛散防止がより有効なものとなる。なお、縫合針Nはスリット11aを通過できないように構成したのは、計数領域5の外へ退去させようとする持針器101の把持部103縫合針Nが付着していたときにその縫合針Nをしごき落とすためである。スリット11aの中を通過する持針器101は、仕切壁11を扉のように押し広げながら、把持部103が縫合針Nとともに落下領域7を、同じく握り部105が計数領域5を、それぞれ通過することになる。

縫合針Nの計数は、計数機構13によって行われる。計数機構13は、マイクロスイッチ13aと、マイクロスイッチ13aを駆動させるレバー13bと、マイクロスイッチ13aの計数結果を表示する表示器13cと、から構成してある。マイクロスイッチ13aは、ケース3の前板部3aの計数領域5の側方内面に取り付けてあり、レバー13bは開口側壁部5aに形成した側壁開口5dから計数領域5内に突出している(図2、4参照)。これに加え、レバー13bの先端部は、側壁開口5dを貫通して開口側壁部5bの裏側に突出していて、結果としてレバー13bは計数領域5を横断する形になっている。表示器13cは、内蔵電池(図示を省略)を電源とした液晶表示を行う。液晶の代わりに、たとえば、より見やすくするためにティルト機構付きのLED(発光ダイオード)を採用してもよい。また、計数機構13の全体を電池不要のメカニカル式のものとすれば、電池を不要としたことによって、より環境に配慮した廃棄が可能になる。

ここで、持針器101が計数領域5を通過しこれによりレバー13bが押し下げられるとマイクロスイッチ13aが駆動し、駆動回数は表示器13cが積算表示する。押し下げられたレバー13bは、持針器101の負荷から解放されると、マイクロスイッチ13aの内部にある付勢部材(図示を省略)の作用により定常位置に復帰する。図1の表示器13cは、持針器101が計数領域5を12回通過したことを示している。定常位置にあるレバー13bの先端部は、側壁開口5dの周縁上端に当接して、その位置よりも上に行けないようになっている。これは、一度通過した持針器101が、使用者が誤って持ちあげようとしても、レバー13bがストッパとして機能してそれより上に行けないようにするためである。上に行くことを許すと、上に行った持針器101が再度計数されてしまい、計数誤差が生じてしまうのでその防止のためである。以上が、計数機構13の構成と作用効果である。なお、レバー13bのストッパ機能と併用もしくはこれの代わりに、持針器101の逆戻りを防ぐためのストッパ構造(図示せず)を設けることを妨げない。

上述したように、持針器101が計数領域5を通過する通過回数は計数機構13によって計数されが、この計数は、縫合針Nを対象とするものではない。しかし、把持部103によって把持された状態で落下領域7を通過する縫合針Nは、持針器101の計数後に把持解除されるので、持針器101の通過回数と縫合針Nの個数は数の上で一致する。これにより、持針器101の通過回数を縫合針Nの個数とみなすことができるので、問題は生じない。縫合針Nの個数を直接計数し、計数後に把持解除しようとする従来の技術に比べ正確な計数ができる。

計数領域5の下端は、開口底板部5cによって仕切られている。この点は既に述べた。この開口底板部5cは、スリット11aの終点を定める部位でもある(図2参照)。使用者は、計数のために持針器101を押し下げることになるが、この押し下げをスリット11aの途中で止めることは、よほど意図的でないとできない。したがって、スリット11aを通過した持針器101は、やがて開口底板部5cに衝突する。この衝突は、使用者をして持針器101(縫合針N)の計数修了を手を介した触覚(衝撃)で感じ取らせることになる。計数修了のタイミングを確実に知ることは持針器の解除操作のタイミングを使用者に正しく知らせることになり、持針器101の操作面で計数ミスを有効に防いでくれる。上記衝突する部位にセンサー(図示せず)を配しておき、そのセンサーの検知信号を受け、たとえば、「ピッ」や「ピンポーン」というチャイムを鳴らせるにしておけば、触覚に併せ聴覚によっても計数修了を知らせることができる。

ケース3内の落下領域7下方には収納容器21を配してある(図2、4参照)。収納容器21は、上面開口21aを備えている。収納容器21は、たとえば、ケース3の背板部3cを取り外し可能もしくは開閉可能に構成しておき(図示省略)、取り外しもしくは開いたときにケース3から出し入れできるように(分離収納可能に)構成する。上面開口21aは、落下領域7全体を水平方向にカバーしており、落下してくる縫合針Nを収容可能に、かつ、さらに、落下してきた縫合針Nが底面に当たって跳ね返っても外部に飛びださない程度の深さを持っている。縫合針Nの跳ね返り防止と付着した血液等の吸収などの目的で、収納容器21の底面にガーゼなどの吸収材を敷いておくことは有効である。収納容器21は、その上面開口21aを蓋部23によって水密閉鎖できるように構成してある。ケース3から取り出した収納容器21に蓋部23を被せ(中には縫合針Nが収容されている)、水密状態を確保してから医療廃棄物として容器ごと廃棄するようにしてもよい。したがって、収納容器21や蓋部23は、これらを合成樹脂によって構成することが好ましい。透明な合成樹脂によって構成すれば、中にある縫合針Nの存在を目視することができ、縫合針Nの存在や個数を再確認するのに便利である。

図1ないし4を参照しながら、前述した縫合針計数装置1を用いて実施する縫合針Nの計数方法について説明する。まず、使用済みの縫合針Nをその把持部103で把持した持針器101を挿入口3hから挿入し(図4の矢印(1))、仕切壁11のスリット11aを通過させる(図4の矢印(2))。このとき持針器101は、計数領域5内を通過するときに計数機構13のレバー13bを押し下げてマイクロスイッチ13aを駆動させる。マイクロスイッチ13aの駆動により持針器101の通過が計数される。使用者は、持針器101が計数された後、握り部105を操作して縫合針Nの把持を解除する。把持解除された縫合針は、収納容器21内に落下する(図4の矢印(3))。最後に、持針器101を計数領域5内であるケース3内から退去させて縫合針計数を完了する。計数結果は、表示器13cに表示される。

図5を参照しながら、本実施形態の変形例について説明する。計数装置51は、合成樹脂製のケース53を備えている。他の素材を利用してもよいが合成樹脂製としたのは、コスト面で有利であるとともに、医療廃棄物として廃棄しやすいからである。中身が見えるように透明なものとすると、収容された縫合針を目視確認するために便利である。ケース53は、側板部53aと天板部53bを含む横長の直方体形状を有する。天板部53bには、持針器101の把持部103を、縫合針Nを把持した状態で挿入するための天板挿入口55と、挿入口55に連続するスリット57とが形成してある。スリットは天板部53bに形成した天板スリット57aと、側板部53aに形成した側板スリット57bに分かれ、両者によってスリット57はL字状に構成される。

スリット57は、縫合針Nを把持したまま天板挿入口55を通過した(図5の矢印(1))把持部103の長手方向の通過を許し(図5の矢印(2))、かつ、縫合針Nの厚み方向通過を許さない(縫合針Nの長さより狭い幅)ように形成してある。スリット57の途中には、計数機構61がある。計数機構61は、天板部53bの裏面に固定したマイクロスイッチ61aと、マイクロスイッチ61aを駆動するためのレバー61bと、計数結果を表示する表示器61cとから構成してある。レバー61bはスリット57を横切るように配してあり、通過する持針器101によって押し動かされるようになっている。スリット57を通過し計数されたら、使用者は握り部105を操作して縫合針Nの把持を解除し、持針器101全体をケース53の外部へ引き抜く(図5の矢印(3))。これで、縫合針Nの計数が完了する。解除により縫合針Nは収納容器の機能を兼ねるケース53の底部に落下する。なお、計数機構61を電池不要のメカニカル式とすることが可能であることは、本実施形態における計数機構13の場合と同じである。
