Description
本発明の第一の態様において、5−FUアッセイで使用するために(例えば、5−FUまたは5−FUのプロドラッグで治療した患者から)5−FUを含む患者サンプルを処理する方法が提供されている。方法は、サンプル中の5−フルオロウラシルの安定性の向上、および、5−FUの測定精度の大幅な改善をもたらす。従って、別の態様において、本発明は、また、処理方法に従って処理した患者サンプルを用いて5−FUをアッセイする方法を提供する。一般に、処理方法は、(例えば、5−FUまたは5−FUのプロドラッグを投与した患者から)5−FUを含むサンプルを得る工程;サンプルをジヒドロピリミジン−デヒドロゲナーゼの不活性化剤と混合して混合物を形成する工程;および、混合物中の5−FUをアッセイするために研究室に混合物を送る工程;を含む。
本発明は、また、患者血液サンプルにDPD不活性化剤を転送し、患者血液サンプルとDPD不活性化剤を混合させるための転送装置を提供する。本発明は、その中のDPD不活性化剤の有無にかかわらず、後述の転送シリンジアセンブリを包含する。図6を参照すると、転送シリンジアセンブリ600および600’は、DPD不活性化剤の溶液をクリニックまたは試験ラボに保管および出荷するために、および、保管されたDPD不活性化剤の溶液を患者サンプルに転送するために、特に有益である。転送シリンジアセンブリ600(600および600’)は、DPD不活性剤の溶液620(例えば、DMSOまたはメタノール溶媒中のギメラシルまたはエニウラシルの溶液)が事前に充填される、転送シリンジのチャンバーを画定するバレル610を含んでいる。バレル610は、遠位端から延びる先端部630を有している。先端部630は、使用時に注射針と作動可能に接続される。さらに、転送シリンジアセンブリは、先端に取り付けられたあるいはバレル610の遠位端から延びる先端シールド640を含む。先端シールド640は、先端部630を超えて縦方向に延びている。さらに、転送シリンジアセンブリ600は、操作可能に先端630または先端シールド640に接続する、先端キャップ650を含んでいる。好ましい実施形態において、先端シールド640および先端キャップ650は、嵌合溝を持ち、一緒に連結している。例えば、先端シールド640は内側に向く溝を有し、先端キャップ650は外側に向く溝を有し、あるいはその逆の溝を有し、先端キャップ650は、先端シールド640としっかりとねじ込まれて固定され、バレルチャンバーの先端部630を封止する。プランジャ660は、バレル610のチャンバー内に摺動自在に挿入される。
1.安定剤としてのエニルウラシルの使用
血液の6つのチューブ(スプレーオンK2EDTAチューブ、6mL)が6人のドナーのそれぞれから引き出され、血液チューブが穏やかに抗凝固剤を混合するために数回反転された。血液はプールされ、ドナーから分離された。各血液サンプルの等分量は新しいコニカルチューブに転送され、在庫の5−FUの計算量(200ng/μLの)が、500ng/mLの5−FUの最終濃度のために各血液サンプルに添加された。各血液サンプルは、以下の条件でエニルウラシルを含む6本のチューブ(チューブ当たりの血液5mL)のセットに小分けされた:
高濃度(最終的に4000ng/mLの血液)、DMSO溶媒;
低濃度(最終的に1000ng/mLの血液)、DMSO溶媒。
血液の3つのチューブ(スプレーオンK2EDTAチューブ、10mL)が6人のドナーのそれぞれから引き出され、血液チューブが穏やかに抗凝固剤を混合するために数回反転された。血液はプールされ、ドナーから分離された。各血液サンプルの等分量は新しいコニカルチューブに転送され、在庫の5−FUの計算量(200ng/μLの)が、500ng/mLの5−FUの最終濃度のために各血液サンプルに添加された。各血液サンプルは、4本のチューブ(チューブ当たり5mLの血液)のセットに小分けされた。チューブの各セットは異なっていたが、下記の条件の組み合わせを含んでいた:
5−ヨードウラシルの低濃度(血液中で最終的に10μg/mL);
5−ヨードウラシルの高濃度(血液中で最終的に40μg/mL);
5−ビニルウラシルの低濃度(血液中で最終的に5μg/mL);
5−ビニルウラシルの高濃度(血液中で最終的に20μg/mL);
ギメラシルの低濃度(血液中で最終的に300ng/mL);
ギメラシルの高濃度(血液中で最終的に1,200ng/mL)。
血液の4つのチューブ(スプレーオンK2EDTAチューブ、10mL)が6人のドナーのそれぞれから引き出され、穏やかに抗凝固剤を混合するために数回反転された。すべてのドナーの血液は、1つの大きなチューブにプールされ、そのプールされた血液の2つの等分量が2つの新しいチューブに転送された。残りの血液は、白血球(WBC)/バフィーコートを分離するために使用された。WBC/バフィーコートは、残りの血液チューブを遠心分離し、各チューブからWBC層を抽出してプールし、WBC/バフィーコートを濃縮するために再び遠心分離し、クリーンなチューブに濃縮したWBC/バフィーコート層を分離する、ことにより単離した。血液の1つのボリュームは、500ng/mLの最終濃度の在庫5−FUの計算量(200ng/μLの)に添加され、血液の他のボリュームは、1500ng/mLの最終濃度の在庫5−FUの計算量(200ng/μLの)に添加された。8つのチューブごとに3セットが、分注した血液を等分にするために準備された。さらに、ギメラシルの代わりに300μLの30%メタノール/水を接種されたネガティブコントロールが用いられた。セットごとの8つのチューブの4つに、低濃度の5−FUの血液が、各チューブに6mL分注された。高濃度の5−FUの血液が、6mLごとのネガティブコントロールチューブに対してと同様に、セットごと残りの4つのチューブに分注された。8つのチューブは、その後、30%のMeOH/水中0.12mg/mLギメラシルの溶液300μLまたは0.04mg/Lのギメラシルの300μLいずれかを接種された。血液チューブはギメラシルを混合させるために反転され、45分間室温で置かれた。血液チューブは10分間室温で2000xgで遠心分離された。血漿は、遠心分離した直後に吸引され、クリーンなクリオバイアルに分注された。WBC/バフィーコートは、プラズマの指定されたチューブに添加され、均一に混合するためにボルテックスされた。血漿サンプルは、AU680アナライザ(200μL、12,500rpmで15分間スピン)の標準的な条件の下で実行された。測定は、WBC/バフィーコートを添加した後24時間(「24時間」)とともにWBC/バフィーコートを添加した直後(「初期」)に行われた。24時間の測定では、血漿は、一晩環境での出荷をシミュレートするため、実験の最初の24時間室温で保存された。下記の表1はその結果をまとめたものである。
血液の6つのチューブ(スプレーオンK2EDTAチューブ、6mL)が6人のドナーのそれぞれから引き出され、血液チューブは抗凝固剤を血液中に混合するために数回反転された。血液チューブはプールされドナーによって分離された。各ドナーの血液の等分量は新しいチューブに転送され、在庫の5−FUの計算量(200ng/μLの)が、500ng/mLの5−FUの最終濃度を達成するために各血液サンプルに添加された。混合された血液サンプルは、ドナーあたり8チューブであってチューブあたり血液4mLとなるように、クリーンなチューブ中に等分に分注された。メタノール溶液中のギメラシルが、1200ng/mLの最終濃度に達するように血液チューブに添加された。ネガティブコントロールのために、メタノールのみがコントロールされる血液チューブに添加された。血液チューブは、血漿分離後45分間、24時間または48時間室温に置かれた。適切なタイミングで、血液チューブは、血漿を分離するために、室温で10分間2000xgで遠心分離された。血漿は遠心分離した直後に吸引され、クリーンなクリオバイアルに分注された。5−FUのアッセイのために、血漿サンプルは、上述したオリンパスAU680アナライザのための標準条件(200μL、12,500rpmで15分間スピン)を用いて実行した。5−FU濃度の測定は、血漿の収集後および最初の測定から2日の間実施された。血漿は、測定間隔の間室温で保存された。この実施例の結果は以下の表3と図5にまとめられる。
サンプルの収集は、定常状態のレベルを(5−FUの平均t1/2は〜13分)を確認するため、5−FU治療の開始後および薬剤注入の終了前の少なくとも2時間の間に臨床医学者の現場で取り出された静脈血で構成される。オンドーズサンプルスタビライザーが、採血に続きすぐに血液サンプルに注入され混合されて、血液サンプルは最大4時間まで室温で残ることができる。サンプルは、その後遠心分離される(1500−2000g、10分間)。血漿の約半分は、バーコードで標識された血漿輸送チューブに転送され、分析のために、Myriad Genetic Laboratories,Inc.に周囲温度で一晩で出荷される。

- 5−FUおよびそのプロドラッグを投与された患者からのサンプルにおいて5−フルオロウラシルをアッセイする方法であって、前記方法が:
前記患者からサンプルを取得する工程であって、前記サンプルはジヒドロピリミジン−デヒドロゲナーゼの一またはそれ以上の不活性化剤と体外で混合される工程;および、
前記サンプル中の5−フルオロウラシルの量を決定する工程;
を備えることを特徴とする方法。 - 5−FUおよびそのプロドラッグを投与された患者からのサンプルにおいて5−フルオロウラシルをアッセイする方法であって、前記方法が:
5−フルオロウラシルまたはそのプロドラッグを投与された前記患者から得られたサンプル中の5−フルオロウラシルを測定する工程であって、前記サンプルが前記患者から得られた後ジヒドロピリミジン−デヒドロゲナーゼの一またはそれ以上の不活性化剤と体外で混合される工程;
を備えることを特徴とする方法。 - 5−フルオロウラシルまたはそのプロドラッグで患者を治療する方法であって:

前記サンプルを10℃以下の温度に冷却するための手段;
を備えることを特徴とするキット。 - 前記不活性化剤がピリミジンまたはピリジンの誘導体である、請求項22に記載のキット。
- 前記不活性化剤が少なくとも850ng/mlの濃度であり、前記サンプルが血液または血漿または血清である、請求項22または23に記載のキット。
- コンパートメントに分けられたコンテナ内に、(1)5−FUと選択的に免疫反応性を有する抗体、および(2)DPD不活性化剤、を備えることを特徴とするアッセイキット。
- ポリマーまたは基板と結合された複数の5−FU分子をさらに備え、前記抗体が固体支持体に取り付けられた、請求項25に記載のキット。
- 別のコンパートメントに、5−フルオロウラシルをさらに備える、請求項25または26に記載のキット。
- 5−FU抗体およびDPD不活性化剤がキット中の別のコンパートメントに存在する、請求項25、26または27に記載のキット。
- クレーム30乃至34のいずれか1項に記載の転送シリンジアセンブリをさらに備える、クレーム22乃至28のいずれか1項に記載のキット。
- 転送シリンジアセンブリであって:
転送シリンジのチャンバーを規定するバレルであって、前記バレルの終端から延びる端部を有するバレル;
前記バレルのチャンバー内に摺動自在に挿入されたプランジャ;
端部に装着され、前記端部に沿って縦方向に前記バレルの終端から延びる端部シールド;および、
前記端部または端部シールドと駆動可能に接続され、前記端部シールドと結合することで前記端部をシールする端部キャップ;
を備えることを特徴とする転送シリンジアセンブリ。 - 前記チャンバー中に、ジヒドロピリミジン−デヒドロゲナーゼの不活性化剤の溶液をさらに備える、請求項30に記載の転送シリンジアセンブリ。
- 転送シリンジアセンブリであって:
転送シリンジのチャンバーを規定するバレルであって、前記バレルの終端から延びる端部を有するバレル;
前記バレルのチャンバー内に摺動自在に挿入されたプランジャ;
前記端部と駆動可能に接続した針状部を有する針状部カバーであって、前記針状部カバーが、針状部の周りに同軸状に配置された通常チューブ状の部材と、針状部に沿って針状部の尖った先端部を超えて延びる針状部の壁部と、を有する針状部カバー;
を備えることを特徴とする転送シリンジアセンブリ。 - 患者サンプルを含む、穴明け可能なキャップを有するサンプル収集チューブであって、前記サンプル収集チューブが針状部カバー内に挿入され、針状部が穴明け可能なキャップを介してサンプル収集チューブに挿入されるサンプル収集チューブをさらに備える、請求項32に記載の転送シリンジアセンブリ。
- 前記チャンバー中に、ジヒドロピリミジン−デヒドロゲナーゼの不活性化剤の溶液をさらに備える、請求項32または33に記載の転送シリンジアセンブリ。
