Description

試料確認および診断基準
研究は、4,957人のAD症例および9,682人の対照の段階1「発見試料」ならびに2,023人のAD症例および2,340人の対照の段階2「追跡調査試料」を含んだ。この研究に含まれた個人は、欧州および米国から得た。個人はみなコーカサス人の家系であった。すべてのAD症例は、高可能性(NINCDS−ADRDA、DSM−IV)または確定(CERAD)の基準を満たした。

遺伝子型同定は、Sanger Institute、UKで実行した。精度管理(QC)を通過した、標準化された試料はすべて、96ウェルプレートフォーマットの中にBiomek(登録商標) FX Laboratory Automation Workstation(Beckman Coulter(登録商標), Inc.、Fullerton、CA)を使用して、GWASのために再配置した。試料当たり200ngの入力DNAを使用し、Illumina Infinium(商標)システム(Illumina(登録商標) Inc.、San Diego、CA、USA)を使用して遺伝子型を同定するために調製した。初めから終わりまでメーカーのプロトコールに従った。手短かに言えば、DNAは、一晩、等温で増幅し、その後、酵素により断片化し、アルコール沈殿し、再懸濁した。この研究において、発明者らは、キャピラリーフロースルーチャンバーにおいてハイブリダイゼーションのために調製したIllumina Human 610−Quad BeadChips (Illumina(登録商標) Inc.、San Diego、CA、USA)を使用した。増幅し、断片化したDNA試料は、Tecanロボットを使用してビーズチップにハイブリダイズし、酵素による塩基伸長を対立遺伝子特異性を付与するために使用した。チップは、続いて染色し、それぞれのビーズで蛍光を検出するためにiSCANリーダー(Illumina(登録商標) Inc.、San Diego、CA、USA)を使用してスキャンした。データは、Beadstudioにロードし、X、Y、X−Raw、およびY−Rawを含有する最終コールレポートが出力された。クラスター分析のためのIlluminusアルゴリズムは、遺伝子型コールのために使用した15。

SNPは、加法モデルを仮定して、ロジスティック回帰を使用し、ADとの関連について試験した。共変数は、地理的地域およびチップを考慮に入れるために、つまり、1)イギリス諸島からの個人、2)ドイツからの個人、3)610または550チップ上で分類された米国からの個人、4)300チップ上に分類された米国からの個人を識別するために、ロジスティック回帰分析中に含めた。対照のみを550チップ上で遺伝子型を同定したので、それぞれのチップの共変数を含むことは可能でなかった。同様に、2つのみが症例および対照の両方を含んだので(610およびMayo群)、8つの群のそれぞれについて共変数を含むことは可能でなかった。EIGENSTRATから抽出した最初の4つのPCもまた、先に記載されるように、共変数として含めた。分析の後に、130のクラスタープロットを、p値≦1×10−4を有するSNPについて視覚的に調査した。不十分なクラスターの形成を示す13のSNPは排除した。したがって、発明者らの分析は、529,205のSNPに基づくものであり、0.05/529205=9.4×10−8の控え目のゲノムワイド有意性閾値を使用した。試験結果のQ−Qプロットを図5に示す。全体的なゲノムの対照インフレーション因子、λは、1.037であると計算された。結果は、表S9において、p値≦1×10−4を有するSNPについて示す。症例および対照におけるマイナー対立遺伝子頻度の内訳を表S10においてゲノムワイド有意SNPについて示す。
発明者らは、5つのヨーロッパ人のコホートからの症例および対照におけるSNPの遺伝子型を同定した(表S2およびS3に記載される)。遺伝子型同定は、メーカーの推奨に従って、MassARRAYおよびiPlexGOLDシステム(Sequenom、San Diego、CA)を使用して、Cardiffで実行した。分析はすべて、30の参照Centre d’Etude du Polymorphisme Humain (CEPH) 親−子供トリオにおける最適化に基づいて使用に先立って検証した。試料プレートは、症例、対照、およびブランク試料を含有した。精度管理手段は、試料同一性に対して盲検のおよび他の評価者に盲検の独立二重遺伝子型同定ならびに入手可能な場合、HapMap(www.hapmap.org)におけるものに対する発明者らのCEPH遺伝子型の比較を含んだ。さらに、GWASに含まれた231人の個人もまた、Sequenomプラットフォーム上で遺伝子型を同定した。発明者らは、両方のプラットフォーム上で分類された7つのSNPについて平均一致率を99%と計算した。すべての遺伝子型を同定したSNPは、追跡調査試料において遺伝子型コール度数率>90%を有し、どのSNPも、症例または対照においてHWE P値≦0.05を有していなかった。SNPは、加法モデルを仮定して、ロジスティック回帰を使用し、ADとの関連について試験した。共変数は、それぞれのコホート、つまり、1)ベルギー、2)MRC、3)ART、4)ボン、5)ギリシアを考慮に入れるためにロジスティック回帰分析に含めた。
発明者らは、CLUおよびPICALM座のSNPについてのメタ分析における段階1および2からの遺伝子型データを含めた。さらに、発明者らは、TGEN研究、公的に入手可能なAD GWASデータセットからの遺伝子型データを使用した。この試料は、Affymetrix 500Kチップ上で遺伝子型を同定した861人のAD症例および550人の対照から構成される。対象とするSNPが発明者らのGWASまたはTGENデータセットにおいて遺伝子型を同定されなかった場合、PLINKにおいて遺伝子型を帰属させるための試みが、参照パネルとして60のHapMap CEU初代を使用してなされた。0.8を超える情報量計量値を有する帰属されたSNPのみを分析に含めた(PLINKウェブサイトを参照されたい)。SNPは、加法モデルを仮定して、ロジスティック回帰を使用し、ADとの関連について試験した。共変数は、段階1における地理的地域およびチップならびに段階2におけるコホートについて考慮するためにロジスティック回帰分析に含めた。TGEN試料に含めた共変数は、オランダBrain Bankからの試料および米国からの試料を識別した。メタ分析の結果は、表1および表S11に示す。
発見メタ分析
発明者らは、ABCA7およびMS4A座のSNPについてのメタ分析における段階1および全段階2試料からの遺伝子型データを含んだ。さらに、発明者らは、加法モデルを仮定し、TGEN studyについて出身国共変数を含めて、ロジスティック回帰を使用して、ADに対する関連について社内で分析した、共に公的に入手可能なAD GWASデータセットであるTGEN(Translational Genomics Research Institute)22およびADNI(Alzheimer’s Disease Neuroimaging Initiative)23研究からの遺伝子型データを使用した10。発明者らは、最初に、GERAD1、ADNI、およびTGENデータセットの分散逆数重みづけ固定効果メタ分析を実行した。その後、このメタ分析からのP値は、フィッシャーの組み合わせ確率検定を使用して、EADI1(European Alzheimer’s Disease Initiative Stage 1)24研究からの公的に入手可能なP値と組み合わせた(EADI1研究において遺伝子型を同定したすべてのSNPについて、オッズ比および分散は公的に入手可能でなく、したがって、すべての4つのGWASの分散逆数重みづけメタ分析を不可能にしたことに注目されたい)。組み合わせ分析は、496763の常染色体SNPを試験した。これらのSNPは、発明者らの段階1試料ならびにADNIおよびEADI1 GWA研究のそれぞれにおいてQCを通過し、52391のこれらのSNPもまた、遺伝子型を同定し、TGEN GWAS(他の研究と異なりAffymetrix500Kアレイを使用した)においてQCを通過した。組み合わせ分析において、61のSNPがP≦1×10−5でADと関連した(表S14を参照されたい)。
発明者らの段階2試料において関連について証拠を示した5つのSNPについて(P<0.05)、概略のデータ(オッズ比および分散を含む)は、CHARGE(Cohorts for Heart and Aging Research in Genomic Epidemiology)25およびEADI1研究から得、すべてのデータ(つまり発明者らの段階1&2、EADI1、ADNI、TGEN、およびCHARGE)は、分散逆数重みづけ固定効果メタ分析において組み合わせた(11607人までの症例および31871人の対照の全試料)。コクランのQ−検定を実行し、I2は異質性を評価するために計算した。同じ方向の効果が、CR1 SNP rs3818361について分析したすべての研究において観察されたが、異質性のいくらかの証拠があった(コクランのQ検定 P=0.02、I2=65%)。このSNPのランダム効果メタ分析もまた、比較のために実行した(表S15を参照されたい)。
発明者らはまた、発症の年齢(AAO)との関係についてGWS SNPをも試験した。この目的のために、発症の年齢(年)は、線形回帰分析における従属変数として使用し、加法モデルを仮定した。AAOデータは、2,856人のAD症例について入手可能であった。共変数は、地理的地域およびチップを考慮するために、つまり、1)イギリス諸島からの症例、2)ドイツからの症例、3)610チップ上で分類された米国からの症例、4)300チップ上で分類された米国からの症例を識別するために、ロジスティック回帰分析に含めた。結果は、表S12に示す。
発明者らは、発明者らが、偶然期待されるよりも段階1でより有意なSNPを観察したかどうかを決定するために発明者らの結果を評価した。発明者らは、最初、危険性SNPの両側500kb以内のSNP、つまりrs429358(APOEε4 SNP)、rs11136000(CLU)、およびrs3851179(PICALM)を除去した。発明者らは、したがって、170の「APOE」SNP、290の「CLU」SNP、および257の「PICALM」SNPを排除した。528,448の残りのSNPのうち、発明者らは、発明者らが(15)において記載したアルゴリズムを使用して、397,224.7の「独立」検定を評価した。有意性レベルα=10−5の16の有意なSNPのうちで(APOE、CLU、およびPICALM SNPを排除)、発明者らは、12.6「独立」検定を評価した。発明者らは、二項分布を使用して、α=10−5レベルで、有意差検定の期待数の平均値(N*α=397224.7*10−5≒4.0)および分散(N*α*(1−α)=3.97)を計算した。したがって、12.6の有意差検定を観察する可能性はP=7.5×10−6である。
新規なGWS座の予備的な分析:2つの関連する遺伝子領域(CLUおよびPICALM)内の関係をさらに調査するために、発明者らは、それぞれからさらなるSNPを選択し、これらは、どちらも段階1で関連についていくらかの証拠を示し(p≦0.001)、新規なGWSヒットと連鎖不平衡にある、推定上の機能的なSNPであった(D’>0.3)(表S11を参照されたい)。予測された機能的SNPは、PupaSuite(http://bioinfo.cipf.es/pupasuite/www/index.jsp)を使用して同定した26。これらは、独立した試料においてADとの関連について試験し、さらに、TGEN GWASにおいて関連についてチェックした。4つのさらなるSNPをCLU座から選択し、1つは段階1でいくらかの証拠を示し(rs7012010;p=8×10−4)、3つは推定上の機能的な有意性を有したが、段階1において分類されなかった(rs3087554、rs9331888、およびrs7982、表S11を参照されたい)。rs7012010は、本来の段階2試料において関連について証拠を示さず、TGENデータセット(p=0.033)においていくらかの証拠を示したが、GWSのメタ分析において有意レベルに達しなかった(p=1×10−4)。残りのSNPの2つは、関連について証拠を示さなかったのに対して、第3のrs7982、クラスタリンにおいてGWS SNPとの強力なLDにおける同義のSNP(HapMap CEU個人においてr2=1;拡張試料においてr2=0.95)は、同様の大きさの効果で本来の段階2試料において関連を示した(メタp=8×10−10;段階1遺伝子型が帰属した)。
