Abstract
Description
この開示は、全般的に移植可能医療デバイスに関し、より具体的には、とりわけ、移植可能医療デバイスにおいて感知された信号におけるノイズを管理するシステムおよび方法に関する。
移植可能医療デバイス(IMD)は、患者に治療を提供するために一般的に用いられる。移植可能医療デバイスは、とりわけ、心調律管理デバイスおよび神経学的刺激デバイスを含み得る。いくつかのタイプの移植可能医療デバイスは、電気刺激を目標組織へ、目標組織に配置されるか、または目標組織の周りに配置された1つ以上の電極を有するリードワイヤー(刺激リード)またはカテーテルを介して搬送する。
発明の実施形態は、とりわけ、移植可能医療デバイスの感知された信号におけるノイズを管理することに関する。実施形態において、発明は、患者から得られた電気信号を処理する方法を含む。方法は、患者内の第一のセットの電気信号を移植可能医療デバイスを用いて集めることを含み得る。方法は、また、第一のセットの電気信号から第二のセットの電気信号を提供するためにフィルタリングすることを含み得、第二のセットの電気信号は、カットオフ周波数より上の周波数を含む。方法は、また、第一のセットの電気信号に存在するノイズの量を第二のセットの電気信号の大きさに基づいて推定することを含み得る。
感知された信号のノイズは、デバイス機能性のさまざまな局面を制御するために用いられるアルゴリズムの正確性を抑制し得る。例として、ノイズは、所与の心室を収縮させるのに必要な電気刺激の最小振幅を決定する自動閾値テストの正確性に悪影響を与え得る。
Claims (20)
- 患者から得られた電気信号を処理する方法であって、
該方法は、
該患者内の第一のセットの電気信号を移植可能医療デバイスを用いて集めることであって、該第一のセットの電気信号は、ペーシング事象にわたる別個の試料時間ウィンドウからの電気活動の試料のセットを含む、ことと、
第二のセットの電気信号を提供するために該第一のセットの信号をフィルタリングすることであって、該第二のセットの電気信号は、カットオフ周波数より上の周波数を含む、ことと、
該第一のセットの電気信号に存在するノイズの量を該第二のセットの電気信号の大きさに基づいて推定することと
を含む、方法。 - 前記第二のセットの電気信号を構成する最初にフィルタリングされた結果のうちの1つ以上は、前記ノイズの量を推定することにおいて考慮されない、請求項1または請求項3〜10のうちのいずれか一項に記載の方法。
- ペースアーチファクト閾値を前記推定されたノイズの量に基づいて調節することをさらに含む、請求項1〜2または請求項4〜10のうちのいずれか一項に記載の方法。
- 前記推定されたノイズの量がノイズ閾値を超えた場合に、前記第一のセットの電気信号を破棄することをさらに含む、請求項1〜3または請求項5〜10のうちのいずれか一項に記載の方法。
- 前記推定されたノイズの量が閾値を超えた場合に、前記第一のセットの電気信号をノイズを示すものとして識別することをさらに含む、請求項1〜4または請求項6〜10のうちのいずれか一項に記載の方法。
- 前記第一のセットの電気信号がノイズを示すものとして識別された場合に、ノイズカウンターを増やすことをさらに含む、請求項1〜5または請求項7〜10のうちのいずれか一項に記載の方法。
- 前記ノイズカウンターが所定の値を超えた場合、自動閾値テスト手続きを終了させることをさらに含む、請求項1〜6または請求項8〜10のうちのいずれか一項に記載の方法。
- 前記第一のセットの信号をフィルタリングすることは、68Hzにおけるチェビシェフ高域フィルタによって実行される、請求項1〜7または請求項9〜10のうちのいずれか一項に記載の方法。
- 前記第一のセットの電気信号は、100ms時間ウィンドウに対して取られた心房心筋電気活動の試料のセットを含む、請求項1〜8または請求項10のうちのいずれか一項に記載の方法。
- 前記第一のセットの電気信号に存在するノイズの量を前記第二のセットの電気信号の大きさに基づいて推定することは、該第二のセットの電気信号の大きさに対する整流平均値を計算することを含む、請求項1〜9のうちのいずれか一項に記載の方法。

第二のセットの電気信号を提供するために該第一のセットの電気信号をフィルタリングすることであって、該第二のセットの電気信号は、カットオフ周波数より上の周波数を含む、ことと、
該第一のセットの電気信号に存在するノイズの量を該第二のセットの電気信号の大きさに基づいて推定することであって、該第二のセットの電気信号を構成する最初にフィルタリングされた結果のうちの1つ以上は、ノイズ推定から除外される、ことと
を行うように構成されている、医療デバイス。 - 前記第二のセットの電気信号を構成する最初にフィルタリングされた結果のうちの1つ以上は、ノイズ推定から除外される、請求項11または請求項13〜20のうちのいずれか一項に記載の医療デバイス。
- ペースアーチファクト閾値を推定されたノイズの量に基づいて調節することをさらに含む、請求項11〜12または請求項14〜20のうちのいずれか一項に記載の医療デバイス。
- 推定されたノイズの量がノイズ閾値を超えた場合に、前記第一のセットの電気信号を破棄するようにさらに構成されている、請求項11〜13または請求項15〜20のうちのいずれか一項に記載の医療デバイス。
- 推定されたノイズの量が閾値を超えた場合に、前記第一のセットの電気信号をノイズを示すものとして識別するようにさらに構成されている、請求項11〜14または請求項16〜20のうちのいずれか一項に記載の医療デバイス。
- 前記第一のセットの電気信号がノイズを示すものとして識別された場合に、ノイズカウンターを増やすようにさらに構成されている、請求項11〜15または請求項17〜20のうちのいずれか一項に記載の医療デバイス。
- 前記ノイズカウンターが閾値を超えた場合に、自動閾値テスト手続きを終了させるようにさらに構成されている、請求項11〜16または請求項18〜20のうちのいずれか一項に記載の医療デバイス。
- 68Hzにおける高域チェビシェフフィルタをさらに含む、請求項11〜17または請求項19〜20のうちのいずれか一項に記載の医療デバイス。
- 前記第一のセットの電気信号は、100ms時間ウィンドウに対して取られた心房電気活動の試料のセットを含む、請求項11〜18または請求項20のうちのいずれか一項に記載の医療デバイス。
