Abstract
Description
本発明は、精巣癌の分野に関する。さらに、前癌期の精巣癌を含む精巣障害の検出、ならびに精巣癌患者に対する予後の確立に関する。
癌
癌は、西欧諸国において最も一般的な疾患の1つ、および死の主な原因である。一般的に、発生率は、癌の多数の形態に対して年齢とともに増加する。一般健康状態の増加によってヒト集団が長生きし続けるため、癌はより多くの個体に影響し得る。多数の一般的な癌型の原因は、未だほとんど知られていないが、環境因子(食生活、タバコの煙、UV放射など)ならびに遺伝因子(p53、APC、BRCA1、XPなどのような「癌遺伝子」の生殖細胞株変異)と癌の発症の危険性の関連を提供する知識体系が増加している。
疑わしい腫瘍由来の生検材料の顕微鏡的評価は、癌診断に対するすばらしい標準を維持している。確定診断を得るために、腫瘍組織をホルマリンに固定し、組織処理し(histo-processed)、パラフィン包埋する。得られるパラフィンブロックから、組織切片を生産し、組織化学的な、すなわち、ヘマトキシリン−エオシン染色、および免疫組織化学的(IHC)方法の両方を使用して染色することができる。次に、外科的標本を肉眼的および顕微鏡分析を含む病理学的技術で評価する。この分析は、しばしば、特定の診断への割り当て、すなわち、腫瘍型の分類および悪性腫瘍、腫瘍の程度の等級付けの基礎を形成する。
精巣癌は、全男性の癌の約1%を占めるだけであるが、20から40歳の男性の最も一般的な癌である。発生率は地域間で変化する。アジアおよびアフリカにおいて、該疾患は約1/100,000の年齢標準化発生率とあまり一般的でないが、北欧諸国において、発生率は世界の残りと比較して比較的高く、デンマーク、ノルウェーおよびスウェーデンは全て世界の発生率のトップテンである。デンマークは、約10/100,000の最も高い発生率を有し、ノルウェーは、少し低い約9/100,000であり、スウェーデンにおいて対応する率は約5/100,000である。この30年間に、西欧諸国における発症率の実質的な増加があり、いくつかの国において多数の場合で2倍である。
日常的に測定される予後診断腫瘍マーカーには、ヒト絨毛性ゴナドトロピン(HCG)、α−フェトプロテイン(AFP)および乳酸デヒドロゲナーゼ(LD)がある。
精巣癌は、通常、超音波診断で診断される。癌を排除することができないとき、睾丸摘出(精巣の外科的除去)が通常、実施され、生検材料を分析する。生検は、また、対側精巣から得ることができる。しばしば、腫瘍マーカーのレベルも測定される。
患者は、国際胚細胞腫瘍予後分類(IGCCCG)により開発された分類体系に基づく3つの予後診断グループ、低い危険性、中程度の危険性および高い危険性に分類することができる。NSGCTにおいて、良い予後グループに対して5年全生存は世界の一部において90%以上であり、中程度の予後グループに対してそれは約75%であり、悪い予後グループに対してそれは約48%である。精上皮腫において、悪い予後グループが存在せず、良い予後グループは約90%の5年全生存を有する患者の90%を含む。精上皮腫の中程度の予後グループは、72%の5年全生存を有する。
第I病期の精上皮腫において、放射線療法をアジュバントとして与えられ得る。さらに進行した工程において、化学療法が通常、適用される。
本発明の局面の目的は、精巣障害、例えば、前工程または初期工程の精巣癌の診断を提供することである。
本明細書の第1の局面として、哺乳動物対象が第1または第2のグループのいずれに属するかを決定する方法であって、第1のグループの対象が第2のグループの対象よりも精巣障害を有する危険性が高く、

単一特異的ポリクローナル抗体
1.抗原の産生
a)材料および方法
EnsEMBL遺伝子ID ENS000000102317によってコードされる標的タンパク質の適当なフラグメントを、生物情報学的ツールを使用して鋳型としてヒトゲノム配列で選択した(Lindskog M et al (2005) Biotechniques 38:723-727, EnsEMBL、www.ensembl.org)。該フラグメントを、RBM3タンパク質(配列番号:2;EnsEMBLエントリー番号ENSP00000365946)のアミノ酸18−151(配列番号:1)に対応する134アミノ酸長フラグメントの生産のために鋳型として使用した。
RBM3の全長転写産物(配列番号:3)のヌクレオチド281−682に対応する遺伝子フラグメントは、特異的プライマーを使用してヒトRNAプールからRT−PCRにより成功裏に単離した。フラグメントは、標的タンパク質RBM3(配列番号:2)のアミノ酸18から151をコードする。標的タンパク質(配列番号:2)の134個のアミノ酸フラグメント(配列番号:1)は、成熟型タンパク質において開裂されるため、大腸菌において効率的な発現を保証するために膜貫通領域を欠くように、任意のシグナルペプチドを欠くように設計された。加えて、タンパク質フラグメントは、他のヒトタンパク質と低い相同性を有するユニークな配列からなり、産生されたアフィニティー試薬との交差反応性を最小限にし、立体構造エピトープの形成やさらには細菌系における効率的なクローニングおよび発現を可能にするのに適当なサイズであるように設計された。
a)材料および方法
上記のとおりに得られた精製されたRBM3フラグメントを抗原として使用して、国の指針にしたがってウサギを免疫化した(スウェーデン許認可番号A84−02)。ウサギを、一次免疫化としてフロイント完全アジュバント中の200μgの抗原で筋肉内投与して免疫化し、4週間の間隔をおいてフロイント不完全アジュバント中の100μgの抗原で3回追加免役した。
ポリクローナル抗体調製物の質は、抗体精製におけるストリンジェンシーの程度に依存することが証明されており、標的タンパク質由来でないエピトープに対する抗体の枯渇が、他のタンパク質との交差反応性およびバックグラウンド結合を回避するために必要であることが以前に示されている(Agaton C et al (2004) J. Chromatogr. A 1043:33-40)。したがって、タンパク質マイクロアレイ分析によって、高い特異性の単一特異的ポリクローナル抗体がHis6−タグに対する抗体ならびにABP−タグに対する抗体の枯渇により産生されていることが確認された。
3.モノクローナル抗体の産生
a)材料および方法
部1において得られた精製されたフラグメント(配列番号:1)を、モノクローナル抗体の生産のための抗原として使用した。抗原をAbSea Biotechnology Ltd (Beijing, China)に送り、簡潔には、抗原を3週間隔でBALB/cマウス(4−6週齢、メス)に皮下的に注射した。抗原を第1の注射のために完全フロイントアジュバントおよび後の注射のために不完全フロイントアジュバントと混合した。融合の3日前に、最後にマウスに静脈内的に抗原投与した。ハイブリドーマをSp2/0骨髄腫細胞株とマウス脾細胞の融合により産生した。ELISAを使用するいくつかの細胞株のスクリーニングにより、抗原(配列番号:1)に対して特異的な抗体を分泌する細胞を同定し、さらなる特性化のためにAtlas Antibodies ABに送達した。ELISA、ウエスタンブロット(WB)および免疫組織化学(IHC)において陽性結果を示す細胞株を、AbSea Biotechnology Ltdにより実施されるサブクローニングのために選択した。
抗原(配列番号:1)を認識するが、アフィニティータグHis−ABPを認識しないモノクローナル抗体(mAb)を生産する系を同定するために、細胞株をELISA(AbSeaで)によりスクリーニングした。8つの細胞株は、ELISAにおいて配列番号:1の抗原への特異的結合を示し、さらなる試験のために選択した。選択された8つのクローンのそれぞれに対して、150−300μlの上清を回収し、アジドを加え、上清を湿性氷上でAtlas Antibodies ABに送達した。上清を、AbSeaの指示にしたがって到着時に+4℃で保存した。細胞株のさらなる試験は、ウエスタンブロットおよびIHC分析の両方において陽性結果を与える3つの興味ある細胞株、クローン1B5、6F11および7G3の同定をもたらした。これらのクローンを、AbSea Biotechnology Ltdにより実施されるサブクローニングおよび拡大のために選択した。
4.細菌ディスプレイIを使用するエピトープマッピング
配列番号:1(すなわち、ENSP00000365946のaa18−151またはENST00000376755のbp261−682)に対応するRBM3 DNAを、鋳型としてベクターpAff8cを使用してPCRにより増幅した。増幅したDNAを、超音波処理により種々の長さ(約50−150bp)に破砕し、次にブドウ球菌ディスプレイベクター(pSCEM2)にライゲーションし、S. Carnosusを形質転換し、約100000個の形質転換体を得た。インフレームのDNAフラグメントがブドウ球菌表面上にペプチドとして示した。抗体(上記部2において得られる配列番号:1に対して選択的)および蛍光的に標識された二次試薬とインキュベーション後、陽性および陰性細胞をフローサイトメトリーを使用して別々に分類し、エピトープおよび非エピトープ提示細胞を単離した。単離された細胞をパイロシーケンシングによりシーケンシングし、最後にエピトープの同定のために配列をRBM3抗原とアラインした。
配列番号:1内の2つのエピトープ領域、配列番号:4(RGFGFITFTNPEHASVAMRAMNGESLDGR)および配列番号:5(RSYSRGGGDQGYGSGRYYDSRPGG)を同定した。
a)合成ペプチド調製物
RBM3におけるPrEST HPRR232631(配列番号:1)に対応する25個のビオチン化ペプチドからなるPEPスクリーンライブラリーをSigma−Genosys(Sigma−Aldrich)により合成した。これらのペプチドは、10個のアミノ酸重複を有する15アミノ酸長であり、共に全PrEST配列を含む。ペプチドを10mg/mlの最終濃度に80%のDMSOに溶解させた。
ニュートラアビジン(Pierce, Rockford, IL)を、製造業者のプロトコールにしたがって、カルボキシル化ビーズ(COON Microspheres, Luminex-Corp., Austin, TX)上に固定した。106個のビーズの結合を、以前に記載されているとおりに(Larsson et al (2009) J Immunol Methods 15;34(1-2):20-32、Schwenk et al (2007) Mol Cell Proteomics 6(1) 125:32)、フィルター膜底マイクロタイタープレート(MultiScreen-HTS, Millipore, Billerica, MA)を使用して実施した。異なる色コードIDを有する25個の異なるグループのビーズを、1−エチル−3−(3−ジメチルアミノ−プロピル)カルボジイミドおよびN−ヒドロキシスクシンイミドを使用して活性化させた。ニュートラアビジン(MES中で100μg/ml)をビーズに加え、振とう器で120分間インキュベートした。最後に、ビーズを洗浄し、再懸濁し、NaN3を補ったタンパク質含有バッファー(BRE, Blocking Reagent for ELISA, Roche, Basel, Switzerland)中で4℃で保存のために微小遠心管に移した。すべての結合したビーズ集団を5分間、超音波洗浄器(Branson Ultrasonic Corporation, Danbury, CT)において超音波処理で処理した。ビオチン化ペプチドを20μMの濃度にBREで希釈し、100μlのそれぞれのペプチドを結合反応に使用し、これを室温で60分間振とうしながら行った。最後に、ビーズを3×100μlのBREバッファーで洗浄し、さらなる使用まで4℃で保存した。
すべての25個のビーズIDを含むビーズ混合物を調製し、PBSで50ng/mlに希釈した45μlのそれぞれの抗体を5μのビーズ混合物と混合し、室温で60分間インキュベートした。フィルター底マイクロタイタープレート(Millipore)を洗浄および次のそれぞれのインキュベーションのために利用し、すべてのウェルを3×100μlのPBSTで洗浄した。50μlのR−フィコエリトリン標識抗ウサギIgG抗体(0.5μg/ml、Jackson ImmunoResearch)または50μlのAlexa Fluor 555 ヤギ抗マウスIgGを室温で60分の最終インキュベーションのために加えた(0.4ug/ml)。
モノ特異的ポリクローナル抗体(抗−RBM3、HPA003624)およびモノクローナル抗体6F11の特異性を、合成ビオチン化ペプチドと結合したビーズを使用して、アッセイで試験した。抗RBM3は、3つの異なる領域、PrEST配列、コンセンサス配列、配列番号:6、7、8および9に対応する8個のペプチド、すなわち6、7、8、14、15、16、24および25に強い結合を示した。特に、配列番号:9に対応するペプチド24および25は強いシグナルを生じた。モノクローナル抗体6F11は、PrEST配列、コンセンサス配列、配列番号:8において1つの異なる領域に対応する2つのペプチド:15および16と反応した。抗−RBM3および6F11の両方がペプチド15および16に結合するため、これは、これらの抗体がこの領域内の1つ以上のエピトープを共有していることを示す。配列番号:6は配列番号:4内であり、配列番号:8は配列番号:5内とある程度重複していることに注目すべきである。

ポリクローナル抗体(抗RBM3)および2つのモノクローナル抗体(6F11および1B5)の特異性をウエスタンブロットにより分析した。還元条件下で17%のSDS−PAGEゲル上で選択されたヒト細胞株由来の全タンパク質抽出物を単離し、次に製造業者の推奨にしたがって、PVDF膜(Bio-Rad Laboratories)に電子移動することにより、ウエスタンブロットを実施した。膜を室温で1時間ブロックし(0.1%のTween20を有する1×PBS中の5%のBSA)、アフィニティー精製された一次抗体(ブロッキングバッファーで1:1000に希釈された)とインキュベートし、PBSTで洗浄した。HRPと接合された二次抗体(ヒツジ抗マウス免疫グロブリン/HRP、GE)をブロッキングバッファーにおいて1:10000に希釈し、化学発光検出を、製造業者のプロトコールにしたがって、ChemidocTMCCDカメラ(Bio-Rad Laboratories)およびWestern Blotting Luminol Reagent(Santa Cruz Biotechnologies, Inc)を使用して実施した。
ウエスタンブロット分析の結果は、抗体が細胞株において約16kDaのバンドを特異的に検出することを示す。RBM3の理論分子量は、16kDa(配列番号:2のRBM3アミノ酸配列から計算される)であり、得られた結果に十分に対応している。さらなるバンドが抗−RBM3および6F11に対して観察された。概して、結果は、モノクローナル抗体がポリクローナル抗体よりもより特異的であり、1B5抗体が6F11抗体よりもさらにより特異的であったことを示す(図1参照)。
a)材料および方法
腫瘍材料を、Department of Pathology、UMAS、1995から2008年で精巣胚細胞腫瘍(TGCT)と診断された30人の患者から回収した。5個のサンプルは純粋な精上皮腫であり、25個は非精上皮腫性TGCTであった。非精上皮腫性TGCTは、国際胚細胞腫瘍予後分類(IGCCC)にしたがって分類した。25個のサンプルのうちの4個はIGCCCの中度の予後として、5個のサンプルはIGCCCの不良な予後として分類した。残りの16個の非精上皮腫性TGCTは、IGCCCの良好な予後として分類した。この試験のための許可は、Ethics Committee at Lund University; ref nr 447-07 and 493-09から得た。5人の患者はそれらの疾患で死に、その内4人はIGCCCの不良、1人はIGCCCの中度であった。すべての患者にシスプラチンを含む標準処置を施した。
RBM3発現を、3が最も高い染色スコア(SS)を示す0、1、2または3として注釈した。図2において見ることができるとおり、RBM3タンパク質発現のレベルは、確立されている予後診断カテゴリーと相関関係である。それらの疾患で死んだ患者は、一貫して低いRBM3発現のレベルを示す。これは不良な予後カテゴリーに属する患者にも当てはまるが、高いRBM3発現を有する全ての患者は良好な予後カテゴリーにおいて見出すことができる。一般的に特に良好な予後を有する精上皮腫を有する全ての患者は、高いRBM3発現を有した。
a)材料および方法
組織材料を、Department of Pathology、UMAS、between 1995から2008年で精巣胚細胞腫瘍(TGCT)と診断された30人の患者から回収した。この材料から、15個のサンプルを未分類管内胚細胞腫瘍の領域(ITGCN)から回収し、22個のサンプルを正常精巣組織から回収した。この試験のための許可は、Ethics Committee at Lund University; ref nr 447-07 and 493-09から得た。
RBM3発現を、3が最も高い染色スコア(SS)を示す0、1、2または3として注釈した。すべての未分類ITGCNサンプルは、強いRBM3タンパク質発現(SS=3)を示したが、正常精巣組織において、セルトリ細胞および精原細胞のみがRBM3タンパク質を発現した。ITGCNは精巣胚細胞腫瘍(TGCT)の前駆体であり、また、in situの癌(CIS)と称される。全てのITGCN細胞におけるRBM3の強い発現は、CISにおけるRBM3に対する役割を示唆する。
