Abstract
【解決手段】 ベラプロストナトリウムを、金属イオンにより疎水化し、これをポリL−乳酸又はポリ(L−乳酸/グリコール酸)共重合体、及びポリDL−又はL−乳酸−ポリエチレングリコールブロック共重合体又はポリ(DL−又はL−乳酸/グリコール酸)−ポリエチレングリコールブロック共重合体と作用させることにより得られるベラプロストナトリウム含有ナノ粒子であり、有効効成分の徐放性に優れ、副作用を軽減し、さらにまた血中滞留性にも優れており、特に薬効の持続性において、極めて顕著なものである。

Description
その治療にあたっては、肺血管拡張剤の投与により、血液の流れを確保し、肺動脈圧を下げ、これにより拡張した心臓や太くなった肺血管への負担を軽減させることが行われており、各種プロスタグランジンI2(プロスタサイクリン)誘導体が臨床的に使用されている。
また本薬剤は、抗血小板作用、平滑筋増殖抑制作用も併せ持つとされており、これまでの検討では、未治療の原発性肺高血圧症の3年生存率が約40%であるのに対し、エポプロステノール治療群では約70%と、生命予後の著明な改善が証明されており、本薬剤の治療効果はほぼ確立しているといってよい。
このベラプロストナトリウムは、日本で開発されたプロスタサイクリン誘導体であって、生体内半減期も1.1時間程度、また化学的半減期も10日程度と長く、経口投与が可能な点で、他のプロスタサイクリン誘導体と比較して、大きな長所を持っているものである。
ベラプロストナトリウムは、当初閉塞性動脈硬化症の治療薬として認可されていたが、その後、原発性肺高血圧症と膠原病に合併する肺高血圧症を対象とした臨床試験が行われ、ベラプロストナトリウムの3ヶ月間の投与・治療で、肺血管抵抗は有意に低下を来し、その結果、原発性肺高血圧症に対する治療薬として認可されるに至り、現在では、ベラプロストナトリウムが肺高血圧症に対する第一次選択薬として広く用いられている。
従来の経口投与によるベラプロスト製剤の100μg投与後の最高血中濃度到達時間(Tmax)は1.42時間、最高血中濃度(Cmax)は440pg/mLと発表されており、このためベラプロストナトリウム服薬後は、比較的急速に本薬剤の血中濃度は増加し、血管拡張作用を発揮するものの、血中濃度半減期は、1.1時間でしかなく、急速にその薬効が消失してしまう問題点があった。また本薬剤の血中濃度の増加は、血圧低下などの副作用を導くため、有効性が期待される静脈内投与は可能ではなかった。
そのため、長時間にわたる、十分なベラプロストナトリウムの持続的な血中濃度を維持できる製剤の開発が急務である。
さらに本発明者等は、この技術を更に改良し、陰荷電基を持つ低分子薬物を患部に効率的にターゲッティングし、薬物徐放性に優れ、かつ肝臓集積を低減することにより副作用を軽減させた、徐放性に優れた陰荷電基を持つ低分子薬物含有ナノ粒子を提供している(特許文献2)。
(1)すなわち、その基本的態様は、次式(I):
(2)さらに、塩基性低分子化合物を混合することを特徴とする上記(1)に記載のベラプロストナトリウム含有ナノ粒子;
(3)粒子の直径が20〜300nm、好ましくは50〜200nmである上記(1)または(2)記載のベラプロストナトリウム含有ナノ粒子;
(4)金属イオンが、鉄イオン、亜鉛イオン、銅イオン、マグネシウムイオン、カルシウムイオン、ニッケルイオン、ベリリウムイオン、マンガンイオン又はコバルトイオンの1種又は2種以上である上記(1)または(2)に記載のベラプロストナトリウム含有ナノ粒子ナノ粒子;
(5)ポリDL−又はL−乳酸−ポリエチレングリコールブロック共重合体又はポリ(DL−又はL−乳酸/グリコール酸)−ポリエチレングリコールブロック共重合体の重量平均分子量が3,000〜30,000である上記(1)または(2)に記載のベラプロストナトリウム含有ナノ粒子;
(6) 塩基性低分子化合物が、(ジメチルアミノ)ピリジン、ピリジン、ピペリジン、ピリミジン、ピラジン、ピリダジン、キノリン、キヌクリジン、イソキノリン、ビス(ジメチルアミノ)ナフタレン、ナフチルアミン、モルホリン、アマンタジン、アニリン、スペルミン、スペルミジン、ヘキサメチレンジアミン、プトレシン、カダベリン、フェネチルアミン、ヒスタミン、ジアザビシクロオクタン、ジイソプロピルエチルアミン、モノエタノールアミン、ジエタノールアミン、トリエタノールアミン、エチルアミン、ジエチルアミン、トリエチルアミン、メチルアミン、ジメチルアミン、トリメチルアミン、トリエチレンジアミン、ジエチレントリアミン、エチレンジアミン、トリメチレンジアミンから選択される1種又は2種以上のものである上記(2)記載のベラプロストナトリウム含有ナノ粒子;
(6) 上記(1)〜(5)に記載のベラプロストナトリウム含有ナノ粒子を有効成分とする、静脈注射用製剤または局所注射用製剤の形態にある非経口投与用製剤;

したがって、半減期の比較的短いベラプロストナトリウムについて、ナノ粒子化することにより、その薬効の持続性に優れると共に、血中滞留性が良好なものであり、患者のQOLに配慮した肺高血圧症の治療薬を提供できるものであり、その産業上の利用可能性は多大なものである。
ポリ−L−乳酸はポリ−DL−乳酸に比し、有機溶媒への溶解性が異なり、また結晶性が高いことが知られている。本発明では、ポリ−L−乳酸を、ポリDL−又はL−乳酸−ポリエチレングリコールブロック共重合体又はポリ(DL−又はL−乳酸/グリコール酸)−ポリエチレングリコールブロック共重合体と共に混合しナノ粒子を形成させることにより、ポリ−L−乳酸の水相での結晶化を抑止し、安定に分散可能なナノ粒子を調製することができる。
ポリ−L−乳酸は、アセトンに不溶性であるため、その溶解性を上げるために、アセトン/ジオキサンあるいはアセトン/テトラヒドロフランの混合液を使用して、ナノ粒子を調製した。
特に、静脈内投与が可能なものであり、これまでのベラプロストナトリウムが経口投与製剤でしかなく、その持続的な投与ができなかった点を改良するものとして、極めて特異的なものである。
また、本発明においては有効成分であるベラプロストナトリウムは、ナノ粒子化するにあたって金属イオン、特に鉄イオンの存在が必須であり、鉄イオンの存在によって、不溶性コンプレックスを調製することができ、その結果ナノ粒子化が可能となった。
かかる点で、本発明は極めて特異的なものである。
すなわち、ベラプロストナトリウムと金属イオン、好ましくは鉄イオンを、有機溶媒又は含水有機溶媒の溶媒中で混合して疎水性薬物とし、この混合液中にポリL−乳酸又はポリ(L−乳酸/グリコール酸)共重合体、さらにポリDL−又はL−乳酸−ポリエチレングリコールブロック共重合体又はポリ(DL−又はL−乳酸/グリコール酸)−ポリエチレングリコールブロック共重合体を加えて攪拌し、この溶液を水中に添加、拡散することにより調製することができる。
特に、塩化鉄によって、ベラプロストナトリウムが初めて不溶性コンプレックス(沈殿物)が形成されることが判明した。
このような塩基性低分子化合物としては(ジメチルアミノ)ピリジン、ピリジン、ピペリジン、ピリミジン、ピラジン、ピリダジン、キノリン、キヌクリジン、イソキノリン、ビス(ジメチルアミノ)ナフタレン、ナフチルアミン、モルホリン、アマンタジン、アニリン、スペルミン、スペルミジン、ヘキサメチレンジアミン、プトレシン、カダベリン、フェネチルアミン、ヒスタミン、ジアザビシクロオクタン、ジイソプロピルエチルアミン、モノエタノールアミン、ジエタノールアミン、トリエタノールアミン、エチルアミン、ジエチルアミン、トリエチルアミン、メチルアミン、ジメチルアミン、トリメチルアミン、トリエチレンジアミン、ジエチレントリアミン、エチレンジアミン、トリメチレンジアミン等を挙げることができ、好ましくは、2級又は3級アミン類であり、ジエタノールアミンが特に好ましい。
この粒径は、PDLLA−PEG若しくはPLLA−PEG又はPDLLGA−PEG若しくはPLLGA−PEGを溶解する溶媒、好ましくはアセトン又はジオキサンの量を調節することにより調整することができ、アセトン又はジオキサンの量を多くすることにより小さな粒径のナノ粒子が得られる。また、ナノ粒子の粒径が大きくなるほど薬物の封入率が高くなる傾向にある。
メトキシ−PEG40g(Mw5200、日本油脂社製)、L−ラクチド40g(プラーク社製)、オクチル酸スズ(400mg)を二口丸底フラスコに入れ十分に混合した。油圧ポンプにて脱気後、オイルバスにて110℃で加熱し溶解した。溶解後155℃に昇温し、4時間反応させた。反応物(固形物)を冷却後、250mL程度のジクロロメタンに溶解し、氷冷したイソプロパノール2.5Lに徐添加で再沈精製し、凍結乾燥することでポリL−乳酸−ポリエチレングリコールブロック共重合体(PLLA−PEG)を合成した。合成物は、ゲルろ過クロマトグラフィー(GPC)あるいはプロトンNMRにて評価した。
PLA(多木化学社製)の26mgを300μLのジオキサンに溶解した。実施例1で合成したPLLA−PEGの24mgをアセトン500μLに溶解し、前記のジオキサン溶液とともに混合した。
この混合液に、2.5mgのベラプロストナトリウムを溶解したジオキサン/メタノール混液700μLを添加し、つづいて9.5mgのジエタノールアミンを溶解した200μLのアセトン溶液を添加した。直ちに、2.4mgの無水塩化第二鉄のアセトン200μL溶液を加えて混和し、室温で10分間放置した。
50mLのサンプル瓶に25mLの水を入れ2cmのスターラーバーで攪拌しながら、そこに26G注射針をつけた3mLシリンジで上記の反応液を徐々に滴下した(スターラー回転数:1000rpm、注射針:26G、シリンジ:ニプロ3mL、滴下速度:48L/hr)。得られた懸濁液に500mMのEDTA水溶液(pH7)を2.5mL及び200mg/mLのTween80[ポリオキシエチレン(20)ソルビタンモノオレート]水溶液を12μL添加した。限外濾過(YM−50、アミコン社製)にて濃縮後、50mMのEDTA水溶液(pH7)を添加し、再び濃縮を行った(これを2回繰返した)。得られた濃縮懸濁液を超音波照射30秒後、遠心(1000rpm、5分)にて凝集塊を除去し、動的光散乱測定装置で粒径測定及び粒子内のベラプロストナトリウムの封入量をHPLCにて定量した。
得られたベラプロストナトリウム含有ナノ粒子における粒子径の分布、封入率、回収率を以下に示した。
図1に、ベラプロストナトリウム含有ナノ粒子における平均粒子径の分布を、下記表2に、ベラプロストナトリウム含有ナノ粒子におけるベラプロストナトリウムの封入率、回収率を示した。
本発明のベラプロストナトリウム含有ナノ粒子においては、ベラプロストナトリウムは塩化第二鉄の存在下で初めて不溶性のコンプレックスを形成し、その結果、効率よくナノ粒子として作成されることが判明した。
すなわち、ベラプロストナトリウムは、塩化第二鉄との相互作用によってナノ粒子中に封入されているものであった。
その点を確認するため、塩化第二鉄の存在/非存在下において、ジエタノールアミン(DEA)の添加の変化による溶液のpHの変動と、その上清中におけるベラプロストナトリウムの残存率を検討した。
具体的には、一定濃度の鉄イオン(455mM)とベラプロストナトリウム(32.5mM)の水溶液中に、ジエタノールアミンを添加することで溶液のpHを種々のpHになるように調整した。その溶液(懸濁液)を16,500gで10分遠心し、上清に溶解しているベラプロストナトリウムの量を、HPLCで定量した。また、その溶液のpHも測定した。
一方、一定濃度の鉄イオンの代わりに塩酸を用いて、ベラプロストナトリウム(32.5mM)の水溶液中にジエタノールアミンを添加することで、溶液のpHを種々のpHになるように調整した。その溶液(懸濁液)を16,500gで10分遠心し、上清に溶解しているベラプロストナトリウムの量を、HPLCで定量した。また、その溶液のpHも測定した。
図2は、塩化第二鉄の存在下において、ジエタノールアミンの添加の変化による溶液のpHの変動と、その上清中におけるベラプロストナトリウムの残存率を示した結果であるが、不溶性コンプレックスが形成されている点が確認され、本発明のナノ粒子の調製には、塩化第二鉄の存在が重要なポイントであることが理解される。
一方、図3は塩化第二鉄の非存在下において、ジエタノールアミンの添加の変化による溶液のpHの変動と、その上清中におけるベラプロストナトリウムの残存率を示した結果であるが、不溶性コンプレックスは全く形成されていなかった。
ベラプロストナトリウムナノ粒子の懸濁液45μLに、ウシ胎児血清(FBS)を50μL、ペニシリン・ストレプトマイシン溶液を1μL、およびリン酸緩衝生理食塩水5μL加え、よく混合した後100μLをマイクロチューブに分注した。その後サンプル溶液を37℃インキュベーター内に静置し、各日サンプリングを行った。
なお、FBSを用いたのは、生体内の環境を模倣するためである。
サンプリングは、サンプル溶液に50mMリン酸緩衝液(pH7)を900μL加え、超遠心(30,000rpm、4℃、30分間)にかけた。超遠心後、上清を除去し、沈殿物に超純水1mLを加え、同様に超遠心を行った。上清除去後の沈殿物をベラプロストナトリウム定量用のサンプルとした。
ベラプロストナトリウムの定量は、封入率測定法の項で述べた操作方法で行った。
図中からの結果からも判明するように、約2週間にわたってナノ粒子中からベラプロストナトリウムが放出されていることが確認された。
ベラプロストナトリウムの生体内半減期1.1時間程度、また化学的半減期も10日と比較すれば、本発明のナノ粒子とすることにより、封入されたベラプロストナトリウムが長期間にわたって安定的に放出され点で、本発明は極めて特異的なものである。
Wister系雄性ラット(5週齢:n=3)の尾静脈からベラプロストナトリウムナノ粒子、ベラプロストナトリウム(比較例)をそれぞれ静脈内投与した。投与後、各時間において投与部位とは異なる尾静脈をメスで切開し、採血管(ヘパリン処理済み)を用いて採血した。1,4−ジオキサン(150μL)入りのマイクロチューブに血液を50μL添加し、サンプル溶液とした。サンプル溶液を遠心分離(13,200rpm、4℃、10分間)した後、上清中に含まれるベラプロストナトリウムを、HPLCを用いて定量した。
図中に示した結果からも判明するように、本発明のベラプロストナトリウム含有ナノ粒子は、静脈内投与後24時間後でも血中に存在していることが判明するが、ベラプロストナトリウムは、投与後3時間後には血中には殆ど存在していないものであった。
このことから、本発明のベラプロストナトリウム含有ナノ粒子とすることにより、ベラプロストナトリウムの血中滞留性が著しく向上する点が理解される。
プロスタサイクリンは血管平滑筋のプロスタサイクリン受容体を介してアデニルサイクラーゼを活性化し、cAMP濃度を増加させることによって、血管平滑筋の弛緩をもたらし、肺血管の拡張作用を発揮する。
そこで、本発明のベラプロストナトリウム含有ナノ粒子を投与し、持続的なcAMPの上昇を検討した。
Wister系雄性ラット(5週齢:n=2)の尾静脈からベラプロストナトリウム含有ナノ粒子、ベラプロストナトリウム(比較例)をそれぞれ静脈内投与した。投与後、各時間において投与部位とは異なる尾静脈をメスで切開し、採血管を用いて採血した。遠心分離(2,000g、4℃、10分間)により血清(上清)を採取し、血清サンプルとした。これを適宜希釈し、cAMP ELISAキットを用いて、血清中に含まれるcAMPを定量した。測定方法は、assay design社のプロトコールに従った。
図中に示した結果からも判明するように、本発明のベラプロストナトリウム含有ナノ粒子は、24時間後でも血漿中のcAMP上昇が維持されている。
これに対して、ベラプロストナトリウムの静脈内投与ではcAMP上昇の持続が観察されず、ベラプロストナトリウムは、ナノ粒子化することによって持続的な薬理効果が得られることが理解される。
MCT(モノクロタニン)により、肺高血圧症誘発モデル動物を用いて、本発明のベラプロストナトリウム含有ナノ粒子の薬効を評価した。
MCTを、まず1M−塩酸水溶液に溶解させ、その後1M−水酸化ナトリウム水溶液で滴定し、中性としたものを用いた。
5週齢Wister系雄性ラット(体重:128〜150g、n=8〜12)にペントバルビタール(50mg/kg;腹腔内投与)で麻酔をかけ、調整したMCT溶液を頸部に皮下注射した。MCT投与後4週間の間、生存率の変化を検討した。
図7は、ベラプロストナトリウムの経口投与並びにVehicleの結果を示した図であり、図8は、本発明のベラプロストナトリウム含有ナノ粒子の投与の結果を示した図である。
両図の対比から明らかなように、本発明のベラプロストナトリウム含有ナノ粒子を3日に1回静脈内投与することにより、生存率の顕著な改善が認められた。
MCT(モノクロタニン)により、肺高血圧症誘発モデル動物を用いて、本発明のベラプロストナトリウム含有ナノ粒子の薬効を評価した。
実施例7において、MCT投与4週後(28日後)に生存しているラットをサンプリングし、右心室重量、肺血管肥厚を定量した。
すなわち、MCT投与から4週間後において生存しているラットに関して、体重を測定し、脱血屠殺後、心臓をおよび肺を摘出した。
摘出した心臓は、まず右心室を切り出し、左心室は中隔部分を除いて左室自由壁のみとし、それぞれの重量を測定した。右心室肥大の定量は、体重に対する右心室重量の割合で算出した。また、摘出した肺を用いて肺血管肥厚の定量を行った。肺摘出後10%ホルマリンで浸漬固定した。パラフィン包埋し、約4μm厚の切片を作製し、ヘマトキシリン・エオシン染色(HE染色)した後、鏡検にて肺血管肥厚を評価した。Kayらの方法に従い、肺細動脈の中膜の厚さを評価した。評価対象とした血管は直径20〜200μmの筋性動脈で、短軸断面で切断されているもののみを計測した。
各標本につき10個の血管を計測し、血管壁厚/血管径を%で算出して中膜肥厚(%wall thickness)とした。
その結果を、図9及び10に示した。
図9は、右心室重量(対全体重)の結果を示した図であり、図10は、肺血管肥厚を示した図である。
本発明のベラプロストナトリウム含有ナノ粒子を3日に1回静脈内投与することにより、顕著な病態改善効果が認められた。
Claims (7)
- さらに、塩基性低分子化合物を混合することを特徴とする請求項1に記載のベラプロストナトリウム含有ナノ粒子。
- 粒子の直径が20〜300nm、好ましくは50〜200nmである請求項1または2に記載のベラプロストナトリウム含有ナノ粒子。
- 金属イオンが、鉄イオン、亜鉛イオン、銅イオン、マグネシウムイオン、カルシウムイオン、ニッケルイオン、ベリリウムイオン、マンガンイオン又はコバルトイオンの1種又は2種以上である請求項1又は2に記載のベラプロストナトリウム含有ナノ粒子。
- ポリDL−又はL−乳酸−ポリエチレングリコールブロック共重合体又はポリ(DL−又はL−乳酸/グリコール酸)−ポリエチレングリコールブロック共重合体の重量平均分子量が3,000〜30,000である請求項1又は2に記載のベラプロストナトリウム含有ナノ粒子。
- 塩基性低分子化合物が、(ジメチルアミノ)ピリジン、ピリジン、ピペリジン、ピリミジン、ピラジン、ピリダジン、キノリン、キヌクリジン、イソキノリン、ビス(ジメチルアミノ)ナフタレン、ナフチルアミン、モルホリン、アマンタジン、アニリン、スペルミン、スペルミジン、ヘキサメチレンジアミン、プトレシン、カダベリン、フェネチルアミン、ヒスタミン、ジアザビシクロオクタン、ジイソプロピルエチルアミン、モノエタノールアミン、ジエタノールアミン、トリエタノールアミン、エチルアミン、ジエチルアミン、トリエチルアミン、メチルアミン、ジメチルアミン、トリメチルアミン、トリエチレンジアミン、ジエチレントリアミン、エチレンジアミン、トリメチレンジアミンから選択される1種又は2種以上のものである請求項2に記載のベラプロストナトリウム含有ナノ粒子。
- 請求項1〜6に記載のベラプロストナトリウム含有ナノ粒子を有効成分とする、静脈注射用製剤または局所注射用製剤の形態にある非経口投与用製剤。
