睡眠時に、いびきや閉塞性呼吸障害が起きる機序としては、顔面を上にして眠る仰向け寝の状態においては、個々には程度の違いはあるとしても、一般には咽喉頭部周辺の骨格筋の弛緩と軟口蓋や舌根部の垂下が起こることによって、上気道における空気の流入を妨げ、あるいは気道を閉塞することによって、いびきや一時的な換気停止(呼吸停止)を生ずるものであって、呼吸停止は10秒から30秒を超えるなどその病態は多彩である。
睡眠時のそうした呼吸障害は成人男性では約25%、女性は約その半数に発生があるといわれており、ことに60歳以上の男性では30%という調査も見られ、そのような状況は国内外ともにかわりがない状況である。
そして、睡眠時の呼吸障害は、次のような多くの病気の遠引をなしていることが知られている。習慣的低酸素血症による代償性高血圧、心血管障害、不整脈、などの循環器障害、糖尿病、脂質異常症などの代謝障害、日中の過眠、疲労感、焦燥、夜間の睡眠障害(途中覚醒)、記憶障害、注意集中力の低下、認知機能の低下、抑うつなどの精神症状など多岐にわたっている。
一方、仰向け寝がもたらす睡眠時の呼吸障害それ自体に対する医学的療法は極めて少ない。外科的に、口蓋垂軟口蓋咽頭形成術による処置。内科的に、経鼻持続陽圧呼吸装置による強制呼吸処置。および、歯科における、下顎前方固定装置の装着処置などがあるが、いずれも重症者にたいする処置方法で一般的なものではない。
