Description
本発明は、血液内皮細胞および循環する血液細胞によって発現される FSH 受容体を特異的に標的とする組成物を用いる画像診断(diagnostic imaging)および治療に関し、特に多くのタイプの癌の画像診断および治療に関する。
癌の診断
腫瘍から採取された組織切片の顕微鏡による評価は、依然として癌の診断を決定するための最も確立された基準(golden standard)である。ゲノム DNA、転写された遺伝子および発現したタンパク質の解析はすべて、顕微鏡画像において検出された組織学的特徴に重要な情報を追加する。将来の診断、予後情報および処置の選択は、ほぼ確実に核酸およびタンパク質の解析と併用される形態の総観的(synoptic)評価に基づくものとなるであろう。
前立腺癌(PCa)は、先進国の男性において最もよく見られる癌である[Stenman et al.、2005; Wilson、2005]。PCa は、フランスの男性の癌による死亡原因の第2位であり(癌による死亡の 11%)[www.doctissimo.fr/html/ dossiers/cancer_prostate.htm]、米国の男性人口の17%を侵し[Wilson、2005; Jemal et al.、2008]、イタリアにおいては最も頻発する癌の部位として肺を凌ぐ[AIRT working group、2006]。前立腺癌の発生率および該疾患による死亡率は、年齢と共に指数関数的に増大する[Scardino、2003]。高齢人口のパーセンテージの増大のため、米国においては前立腺癌の症例数が2006年における234,000件から、2025年には380,000件にまで増加すると予測されている[Scardino、2003]。
本発明は、先行技術において全く言及されていない、腫瘍における内皮細胞または腫瘍患者の循環細胞における FSHR の存在の同定に基づく。
定義
本明細書において用いる場合、“FSHR”の用語は、本発明の方法を適用する受容体を意味する。卵胞刺激ホルモン受容体または FSH-受容体(FSHR)は、卵胞刺激ホルモン(FSH)と相互作用する膜貫通型受容体であり、G タンパク質共役受容体(GPCR)に相当する。
本発明者らによって示される通り、FSHR が内皮細胞上に位置することは、FHSR が単に腫瘍細胞において発現するのみであるなら達成することができない画像化様式を可能にする。

本発明者らはまた、FSHR を発現する細胞が、腫瘍状態を有する患者の循環する血液細胞において蓄積することを発見した。したがって、FSHR の血中レベルは癌のマーカーとして用いることができる。
FSHR が内皮細胞上に位置することは、本発明者らによって示される通り、腫瘍の血管を攻撃することによる治療を可能にする。
1. 抗体標的化(antibody targeted)放射線治療: シード照射またはビーム照射の代わりとしての、ごく近傍において照射によって腫瘍細胞のみを殺し、健康な組織を残す放射性同位体と結合した抗体(Parry et al、2006 を参照);
2. 標的とされた細胞を破壊するペプチド(Arap et al 2002; Leuschner and Hansel、2005 を参照);
3. 新たに形成される血管を優先的に殺すタンパク質をコードする遺伝子 (Hood et al 2002 を参照);
4) FSHR リガンドと結合した場合に癌細胞を破壊することができる抗癌剤 (即ち、化学療法剤)。“化学療法”の用語は通常、一般に迅速に分裂する細胞に影響を与える細胞毒性薬剤をいう。化学療法剤は、様々な可能な方法で細胞分裂を妨げ、例えば、DNA の複製または新たに形成される染色体の分離を妨げる。多くの癌細胞が DNA の損傷を修復できない一方、正常な細胞は通常修復できるため、ある程度の特異性は生じ得るが、ほとんどの形態の化学療法は全ての迅速に分裂する細胞を標的とし、癌細胞に対し特異的ではない。したがって、化学療法は、健康な組織、特に高い交換速度を有する組織(例えば腸管の内層)を害する可能性を有する。これらの細胞は通常、化学療法の後に自身を修復する。
5) 癌細胞の調節が解除された(deregulated)タンパク質に特異的な小分子
かかる小分子は通常、癌細胞内の突然変異した、過剰発現する、あるいは重大な意味を持つタンパク質上の酵素ドメインの阻害剤である。顕著な例は、チロシンキナーゼ阻害剤であるイマチニブおよびゲフィチニブである。
画像化組成物(imaging composition)または医薬組成物は通常、哺乳類への投与に適する形態において、生体適合性の担体と組み合わせて形成される。
図 1、FSH 受容体は前立腺腫瘍において発現するが、正常な前立腺においては発現しない。癌性の(PCa)および正常な(N)組織からの等しい量(1mg の全タンパク質)の Triton X-100 抽出物を、FSHR323 モノクローナル抗体を用いて免疫沈降させ、還元 SDS-PAGE 上で分離し、ニトロセルロース膜へ転写した。該サンプルを、FSHR323 抗体を用いて探索した。分子量マーカーの位置は、左側に示される。他の4人の患者から得られた組織においても、同様の状況が見出された。かすかなバンドは、免疫沈降に用いたマウス IgG に相当する。

組織検体
ドナーは、いかなる薬剤投与も受けなかった。腫瘍検体を 10% ホルマリン中で 48 時間固定し、5 mm 厚の組織切片に切断し、PBS を用いて十分に洗浄し、段階的な(graded)エタノールおよびキシレン中で脱水し、パラフィン中に包埋した。1.5-2.5 cm の大きな 5 μm 切片をパラフィンブロックから切り出した。いくつかの未固定の前立腺サンプルをドライアイス上で凍結させ、免疫蛍光顕微鏡法および/または免疫沈降およびイムノブロット実験のためにクライオスタットを用いて切片作成をするときまで液体窒素中に保存した。
免疫組織化学のために、標準的な間接免疫ペルオキシダーゼ法を用いた。組織切片を、SuperFrost スライドに付着させ、トルエンを用いて脱パラフィンし、エタノール中において徐々に脱水させ、流れる水道水を用いて 60 分間洗浄した。スライドを pH 6 の 10 mM クエン酸バッファーと共に 90℃で 40 分間インキュベートすることによって、抗体付着のための組織抗原部位への接近を増強した。室温において 20 分間冷却した後、およびその後の各工程の後に、PBS を用いてスライドをリンスした。内在性のペルオキシダーゼ活性を遮断するため、切片を 6% 過酸化水素と共にインキュベートした(室温において 15 分)。水素化ホウ素ナトリウム(50 mg/ml)を用いて遊離アルデヒド基をクエンチした(20 分)。スライドを、PBS 中の 2% ヤギ血清(ブロッキングバッファー)と共に室温で 2 時間インキュベートすることによって、抗体の非特異的結合を遮断した。スライドを、ブロッキングバッファー中の 5 μg/ml の抗 FSHR モノクローナル抗体 323 [Vannier et al.、1996] と共にインキュベート(終夜、4℃)することによって、FSHR を検出した。セイヨウワサビペルオキシダーゼと結合したヤギ抗マウス IgG (Fc 特異的)(Sigma、1:200 希釈) を、二次抗体として用いた。色素原(chromogen)として、3-アミノ-9-エチル-カルバゾール(AEC; Sigma)を用いた。光学顕微鏡を介して 15 分間、発色をモニターした。切片を蒸留水中で洗浄し、弱いマイヤー(Mayer)のヘマトキシリンを用いて対比染色した。スライドを、Dako Glycergel 封入剤(mounting medium)中において封入した。
EDTA 上で回収された血液を、塩化アンモニウムで処理して赤血球を溶解させ、55 g での遠心分離によって多形核細胞を濃縮した(Eggleton et al.、1989)。細胞を、4% ホルムアルデヒドを用いて 4℃において終夜固定し、ポリリジンでコートされた顕微鏡スライドに付着させた。上記の抗 FSHR モノクローナル抗体 323 を用いて FSHR を検出した。
Tissue-Tek を用いて包埋された未固定の凍結した前立腺の連続的クライオスタット切片(7 μm)を SuperFrost スライド上で回収し、空気乾燥させた。切片を、PBS 中の 3% パラホルムアルデヒドを用いて 15 分間室温において固定した。PBS 中の 50 mM NH4Cl を用いて切片を 15 分間処理することにより、遊離のアルデヒド基をクエンチした。抗体の非特異的結合を遮断するため、スライドを PBS 中の 2% ヤギ血清と共に室温で 1 時間インキュベートした。切片を、GS-PBS 中の抗 FSHR323 抗体(3 μg/ml)と共に室温にて 2 時間インキュベートし、続けてヤギ抗マウス IgG-Alexa 555 と共にインキュベート(Molecular Probes; ブロッキングバッファーにおける 1:750 希釈; 1 時間)することにより、FSHR を検出した。抗 FSHR323 抗体(ブロッキングバッファーにおける 3 μg/ml)および内皮細胞の特異的マーカーであるウサギポリクローナル抗フォン・ヴィルブランド因子(Sigma; 希釈 1:1000)の混合物と共にインキュベートされた前立腺切片を用いて、二重標識実験を行った。ヤギ-抗マウス IgG-Alexa 555 およびヤギ-抗ウサギ Ig-Alexa 488 の混合物(Molecular Probes; 希釈 1:750)を、二次抗体として用いた。いくつかの実験において、スライドを TOTO-3 (Molecular Probes; 希釈 1:1000 in PBS)と共に 10 分間インキュベートすることにより、核を検出した。スライドを 15 mM のアジ化ナトリウムを含有する Dako(登録商標)蛍光封入剤中において封入し、Zeiss 510 共焦点レーザースキャニング顕微鏡(Confocal Laser Scanning Microscope)を用いて調査した。陰性対照は、蛍光二次抗体混合物のみと共にインキュベートした前立腺サンプルからなった。FSHR323 抗体の濃度が 5 μg/ml であることを除いて同様である方法を用いて、パラフィン切片についても免疫蛍光共焦点顕微鏡観察を行った。
簡潔には、各血管のプロファイルとして、管腔を除く血管の領域、およびこの領域にわたる総 FSHR シグナルをコンピュータにより算出した。次いで、各血管にわたる FSHR シグナルの平均強度を、総 FSHR シグナルを該領域に分割することによってコンピュータにより算出した。上記の値は、Adobe Photoshop を用いて、以下に記載する通りに、各血管プロファイルのデジタル画像から抽出した。“投げ縄(Lasso)”ツールを用いて血管の外側輪郭を描写し、選択された領域をコピーして新しいファイルにペーストした(本明細書において、ファイル A と称する)。このファイル中において“選択(Select)”および“色域(Colour range)”のコマンドを用い[Tolivia et al.、2006]、曖昧さ(fuzziness) 30 を用いて、染色された領域を描写した。(代表的な画像から、ペルオキシダーゼ反応の産物に特異的な赤褐色の色を最初に選択し、“色域”メニューから保存(Save)コマンドを用いてファイルに保存した。その後、このファイルを各画像の解析のためにロードした。) 選択された領域を、新しいファイル(B)中にコピーした。同様の操作を血管の内径についても行い、ファイル C および D を生成した。(ファイル A-C のバックグラウンドは黒であった)。全てのファイルに対し“レイヤーを統合する(Merge Layers)”コマンドを適用し、ファイル B および D をグレースケールに変換した。解析の第二段階において、NIH ImageJ ソフトウェアを用いてファイル A-C から数値を抽出した。ファイル A および C から、“Freehand”ツールならびにその後の“Analyze”および“Measure”コマンドを用いて、各血管プロファイルの内側および外側の領域を測定した。次いで、画像 B および D の全領域を“Rectangle”ツールを用いて選択し、全ピクセルの合計を“Analyze”および“Measure”を用いて導き出した。(血管の外側の領域は黒色であるため、全く寄与しない。) 値を Excel のファイルへ導入した。次いで、全血管プロファイルについての値から管腔についての値を差し引くことによって、血管壁に対応する値を得た。血管壁の染色の平均強度を、血管壁にわたる総シグナルを血管の輪郭の領域に分割することによってコンピュータで算出した。
1. FSH 受容体は前立腺腫瘍において発現する
FSHR323 抗体を用いる免疫沈降によって濃縮された前立腺組織からの受容体調製物を、FSHR18 抗体を用いるイムノブロッティングによって解析した。前立腺癌組織において、およそ 87 kDa のバンドが検出された(図 1、レーン PCa)。前立腺癌患者の前立腺中の正常に見える組織からの抽出物においては、FSHR は観察されなかった(レーン N)。検出されたバンドのサイズは、ヒトの卵巣組織においてこれまでに検出された 87 kDa の成熟したグリコシル化された形態[Vannier et al.、1996]と同じであった。
次に、FSH323 抗体を用いる免疫組織化学によって FSHR の局在化を調査した。大きな 矩形の(rectangular)切片(1.5-2.5 cm)を用いた。これらの切片のほとんどは、腫瘍組織および正常に見える領域の両方を含んでいた。一般に知られている通り、ほとんどの前立腺腫瘍は前立腺の辺縁に局在し、中央域において生じるものはほとんど無い[De Marzo et al.、2007]。5以上のグリーソンスコアを有する患者の辺縁の腫瘍領域から得られた画像において、上皮細胞の中にかすかなシグナルが見られた(図 2 a および b 中の矢じり)。対照的に、辺縁の正常に見える領域における上皮細胞の圧倒的多数は、FSHR 染色を全く示さなかった(図 2c)。[正常に見える辺縁組織において、分泌細胞に関連する基底部の FSHR 染色の小さな局所的領域がまれに見られる(図示せず)]。同じ前立腺の中央領域における正常に見える組織中の上皮細胞においては、FSHR シグナルは検出できない。腫瘍組織における血管の大部分は豊富に染色され(図 2、a および b 中の矢印)、一方、正常に見える辺縁組織における血管のほとんどは染色されない(図 2c 中の矢印)。腫瘍の血管における豊富な FSHR の発現は、5以上のグリーソンスコアを有すると診断された 50 人の患者全てにおいて例外なく見られた。(解析のために利用できる5未満のグリーソンスコアを有する患者からの組織は無かった)。中央領域における正常な組織の血管プロファイルにおいて、FSHR の発現は存在しなかった(図示せず)。例外として、辺縁部および中央部の両方において正常な領域中に見出される炎症性の領域においては、血管が染色される(以下を参照)。前立腺の良性の過形成は、いくつかの炎症性領域を除いては、上皮細胞においても、血管壁においても、FSHR を発現しない(以下を参照)。
FSHR について染色された血管プロファイルのサイズおよび外観(appearance)は、それらが毛細血管、細静脈および細動脈を表すという結論と一致する。これらの血管のいくつかは、内皮細胞の他に、周皮細胞(いくつかの毛細血管および後毛細管(周皮細胞性(pericytic))細静脈)および平滑筋細胞(細動脈および筋性細静脈)により構成される [Simionescu and Simionescu、1988]。したがって、腫瘍においてこれらの細胞のうちいずれが FSHR を発現するのかを明らかにすることは価値がある。内皮細胞に特異的なマーカーであるフォン・ヴィルブランド因子は、毛細血管および細静脈において FSHR が内皮細胞によって発現され、血管壁を構成する他の細胞の型によっては発現されないことを示した。細動脈においては、内皮の他に、解析した全てのグリーソンスコア(5 から 10)において平滑筋細胞が染色される(図 3)。これに比較して、筋性細静脈における平滑筋細胞はいかなるグリーソンスコアにおいても染色されない(図示せず)。毛細血管および細静脈の内皮が解析した全てのグリーソンスコア(5 から 10)において FSHR について染色される一方、細動脈の内皮は 6 以上のグリーソンスコアにおいてのみ染色される(図 3 b、c)。この内皮細胞および平滑筋細胞の染色のパターンは、解析した 50 人全ての患者からの組織について妥当(valid)であった。
個々の血管の内皮細胞にわたる平均シグナル強度を、図 2 におけるものと同様のデジタル画像を用いて決定した。毛細血管および後毛細管細静脈のみ測定した。細動脈については、染色された内皮細胞の他に染色された平滑筋細胞が存在することによって導入される複雑さのため、測定を行わなかった。高いグリーソンスコアにおいて FSHR の発現が増大するはっきりとした傾向がある(スコア 9 および 10 については平均して 118±8(SEM) 単位であるのに対し、グリーソンスコア 5 および 8 については 64±7、p<0.0005)(図 4、黒棒)。FSHR シグナルの強度を、血管を除く組織の領域についても測定した(図 4、白棒)。この場合においても、最も高いスコアにおいて発現が高い(グリーソンスコア 10 において 18.4±1.1 単位であるのに対し、グリーソンスコア 5 から 9 については 3.8±0.3 単位、p<1.7 x 10-11)。FSHR シグナルは組織の残りの部分よりも内皮細胞についての方がずっと高いことに注目すべきである(およそ 6 から 10 倍高い)。
50人の患者の各々に関し、大きなパラフィン切片は、腫瘍または正常な領域との明確な関連を伴わず、高密度の炎症性細胞の緻密な領域を少なくとも1つ含んでいた(図 5)。たとえ周囲の組織が正常な、非腫瘍性の様相を有していても、白血球浸潤の内部または近傍の血管の大多数が FSHR を強く発現することが一貫して観察された。この効果は、炎症性領域の近位の縁(proximal edge)から 3.5 mm (平均 2±0.75 (標準偏差)、n=34) までの距離にある血管について認められた。
前立腺は、大いに(richly)神経支配された器官である。2つの神経血管束が腺の直腸面に沿って走り、前立腺実質へ枝を広げる前に他の神経が被膜(capsule)の内表面に沿って走る[Powell、2005]。全ての神経には血管(神経脈管)が付随しており、そのうちいくつかは神経鞘(epineurial sheath)に貫入している。本発明者らは、全てのグリーソンスコアにおいて、癌に侵された末梢領域中の全ての神経線維におけるシュワン細胞の強い FSHR 染色を認めた。これらの神経に関連する血管もまた、FSHR を発現する(図 6)。神経および炎症性浸潤の両方を含む領域において、より強い血管の染色が見られた。
他のヒトの腫瘍の型について、同様の免疫組織化学実験を行った(表 I 参照)。全ての腫瘍についての対照は、腫瘍の除去のために行われた手術によって得られた組織における、腫瘍から 10 mm よりも外側に位置する正常に見える組織であった。解析した腫瘍の大部分(およそ 70%)はグレード I および II のものであり、およそ 25% がグレード III であり、残りが最も進行したグレード IV であった。腫瘍の各々の型および対応する正常な対照についての代表的な画像を 図 8 に示す。前立腺におけるのと同様に、乳房腫瘍および膵臓外分泌腫瘍において腫瘍細胞は時折、かすかに染色される(示さず)。本発明者らはさらに、脳腫瘍に関連する血管において FSHR の発現を認めた(示さず)。
前立腺癌患者から単離された多形核細胞は、FSHR を発現する(図 10 C)。調製物中においても見られる単核細胞は、いかなる FSHR シグナルも示さない。FSHR 陽性の血管内の細胞もまた、前立腺腫瘍からの切片(図 10 A および B)ならびに結腸、腎臓、膀胱、膵臓、乳房、卵巣、精巣、肺、胃および肝臓からの腫瘍からの切片において見られる。高倍率の共焦点顕微鏡観察は、シグナルを生成するタンパク質が、膜貫通型受容体である FSHR について予測される通り、細胞膜に局在することを明らかにしている(図 10 C、挿入図)。
腫瘍の血管における FSHR の発現の理由について、以下の仮説を提案することができる。初期の発生の間、前立腺が非常に速い速度で成長する際には、成長を維持するために多量の FSH が必要とされると考えられる。循環する FSH は自力では容易に内皮障壁を通過せず[Vu Hai et al.、2004]、トランスサイトーシスを介する高い輸送速度を促進するために、内皮表面上における多量の FSHR が必要とされる可能性がある。この受容体に媒介される FSH 輸送は、前立腺が成熟した大きさに達するとおそらく下方制御されるが、腫瘍において上皮が再び増殖性の高い状態に入ると、再活性化される可能性がある。

- 癌の重症度の評価または抗腫瘍治療の有効性のモニターのための、請求項1から7のいずれかの FSH 受容体(FSHR)リガンド。
- 静脈内投与のための画像化組成物の形態である、請求項1から8のいずれかの FSH 受容体(FSHR)リガンド。
- 該腫瘍に関連する血管における内皮性 FSHR の発現を検出することによる腫瘍のインビボの診断または画像化において画像化剤として使用するための、請求項1から9のいずれかの FSH 受容体(FHSR)リガンド。
- ガス充填微小胞によって標識されている、請求項1から10のいずれかの FSH 受容体(FHSR)リガンド。
- コントラスト増強超音波画像法において使用するための、請求項11の FSH 受容体(FHSR)リガンド。
- 以下からなる工程を含む、画像化データを収集する方法:
a) 検出可能に標識された FSH 受容体リガンドを含む画像化剤を提供する工程;
b) 該画像化剤を患者に投与する工程;

- 静脈内投与のための形態である、請求項23の医薬組成物。
- 腫瘍に関連する状態の処置のための、抗腫瘍剤と結合した FSH 受容体(FSHR)リガンド。
- 該腫瘍に関連する状態が、前立腺、精巣、腎臓、卵巣、肺、肝臓、膵臓、胃、乳房、結腸、脳および膀胱の癌からなる群から選択される、請求項25に記載の抗腫瘍剤と結合した FSH 受容体(FSHR)リガンド。
- 試験対象の血液サンプル中における FSHR を検出することを含み、循環する血液細胞中における FSHR の存在が癌を示す、癌の診断または予後または治療のモニタリングのためのインビトロの方法。
- 血液サンプルが全血または末梢血多形核細胞(PMN)の調製物である、請求項27に記載の方法。
- 癌の処置において使用するための、FSHR を発現する循環する血液細胞の存在量または循環する血液細胞による FSHR の発現もしくはシグナル伝達を調節する FSH 受容体(FSHR)リガンド。
- 静脈内投与のための形態である、請求項29の FSH 受容体(FSHR)リガンド。
