a)粒径xが45≦x≦300μmである粒子、20〜98重量%、および
b)粒径x>300μmである粒子、2〜80重量%
を含んでいる。
− 第I期褥瘡:負荷を除去しても頑固で限局性の赤発が皮膚に残ったままである。赤発はシャープな輪郭を持ち、硬化または発熱することがある。皮膚はまだ無損傷である。
− 第II期褥瘡:当期では水疱形成および皮膚落屑が発生し、延いては皮膚の部分損失に到る。表皮から真皮領域に到るまで損傷を受けている。当期では表面創傷または扁平な潰瘍が存在する。
− 第III期褥瘡:病状の進んだ当期では既にあらゆる皮膚層の損失が観察できる。それに加え、皮下組織の損傷および場合によっては、下方の筋肉組織にまで達し得る壊疽も観察できる。経験によれば、組織損傷の範囲全体が認識可能になるまでに、先ず壊疽性組織の領域を限定しなければならない。第III期褥瘡は臨床的には開口した深い潰瘍として現れる。
− 第IV期褥瘡:極めて危険な当期では、全皮膚層の破壊域拡大を伴う損失、および筋肉、骨または支持構造(腱、関節包)の組織壊疽または損傷が認められる。第IV期褥瘡は、臨床的には広い領域に亘って開口した深い潰瘍として現れる。
a)粒径x≦300μmである粒子、20〜100重量%、および
b)粒径x>300μmである粒子、0〜80重量%
を含む、ポリアクリレート粒子から構成される粒子混合物のプロテアーゼ、特にメタロプロテアーゼの抑制剤としての使用、褥瘡、下腿静脈潰瘍、下腿動脈潰瘍または糖尿病性足部症候群の治療のための使用、およびこれらの治療薬の製造のための使用にも関する。この場合組織の再生は、創傷治癒を妨害するメカニズムの遮断によって促進されるが、それは臨床的には創傷症状の回復および創傷の縮小に認められる。
− 3.3mlのゼラチン溶液(豚の皮膚からのゼラチン(CASナンバー 9000−70−8)、3mg/mlH2O)、
− 0.85mlの蒸留水、
− 2.5mlの1.5Mトリス(トリス(ヒドロキシメチル)アミノメタン)/HCl/0.4%のSDS溶液(ドデシル硫酸ソーダ溶液)、pH8.8、
− 3.35mlの30%アクリルアミド/0.8%ビスアクリルアミド溶液、
− 50μl(10%、w/v)の過硫酸アンモニウムおよび
− 5μlのTEMED(N、N、N’、N’−テトラメチレンジアミン)
上層ゲルは次のとおりである。
− 3.075mlの蒸留水、
− 0.625mlの0.5Mトリス(トリス(ヒドロキシメチル)アミノメタン)/HCl/0.4%のSDS溶液(ドデシル硫酸ソーダ溶液)、pH6.8、
− 0.75mlの30%アクリルアミド/0.8%ビスアクリルアミド溶液、
− 50μl(10%、w/v)の過硫酸アンモニウムおよび
− 5μlのTEMED(N、N、N’、N’−テトラメチレンジアミン)
0=被膜なし。
1=創傷表面の20%がフィブリン被膜で覆われていた。
2=創傷表面の40%がフィブリン被膜で覆われていた。
3=創傷表面の60%がフィブリン被膜で覆われていた。
4=創傷表面の80%がフィブリン被膜で覆われていた。
5=創傷表面の100%がフィブリン被膜で覆われていた。
0=肉芽組織認められず。
1=特徴として、創傷表面の20%に目を引く赤い肉芽組織があった。
2=特徴として、創傷表面の40%に目を引く赤い肉芽組織があった。
3=特徴として、創傷表面の60%に目を引く赤い肉芽組織があった。
4=特徴として、創傷表面の80%に目を引く赤い肉芽組織があった。
5=特徴として、創傷表面の100%に目を引く赤い肉芽組織があった。
Claims (15)
- 様々なサイズのポリアクリレート粒子(15、16、25、26、45、46)を含む粒子混合物および前記粒子混合物用の担体材(17、27、47)を含む、傷内プロテアーゼの抑制および/またはハイドロ活性による創傷治療のための傷当て材(10、20、40)であって、前記粒子混合物が
a)傷内プロテアーゼ抑制剤としての粒径x≦300μmの粒子(16、26、46)、5〜100重量%、および
b)水溶液の吸収および/または排出剤としての粒径x>300μmの粒子(15、25、46)、0〜95重量%
を含み、かつ、
前記傷当て材が担体材の総重量に対して少なくとも10重量%のポリアクリレート粒子(15、16、25、26、45、46)を有する、傷当て材。 - 前記粒子混合物が
a)傷内プロテアーゼ抑制剤としての、粒径xが45<x≦300μmである粒子(16、26、46)、5〜100重量%、および
b)水溶液の吸収および/または排出剤としての、粒径xが300<x≦1500μmである粒子(15、25、46)、0〜95重量%
を含む、請求項1に記載の傷当て材(10、20、40)。 - 前記粒子混合物が
a)傷内プロテアーゼ抑制剤としての、粒径xが150<x≦300μmである粒子、20〜98重量%、および
b)水溶液の吸収および/または排出剤としての、粒径xが600<x≦850μmである粒子、2〜80重量%
を含む、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。 - 前記粒子混合物が
a)傷内プロテアーゼ抑制剤としての粒径x≦300μmの粒子、50〜100重量%、および
b)水溶液の吸収および/または排出剤としての粒径x>300μmの粒子、0〜50重量%
を含む、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。 - 前記粒子混合物が
a)傷内プロテアーゼ抑制剤としての粒径x≦300μmの粒子、5〜50重量%、および
b)水溶液の吸収および/または排出剤としての粒径x>300μmの粒子、50〜95重量%
を含む、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。 - 前記ポリアクリレート粒子(15、16、25、26、45、46)が、前記ポリアクリレート粒子の総重量に比較して最大で10重量%の水を含む、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。
- 前記ポリアクリレート粒子(15、16、25、26、45、46)が、架橋結合および/または交叉結合および/または表面架橋結合したポリアクリレートを含む、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。
- 多層傷当て材であることを特徴とする、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。
- さらに、前記粒子混合物と前記粒子混合物用担体材(47)とを取り囲むための被覆部(49)、または前記粒子混合物と前記担体材(17、27)とを傷から離すための創傷接触層(11、21)を有する、前記請求項の少なくとも一項に記載の傷当て材(40)。
- 前記粒子用担体材(17、27、47)が、第1繊維として合成ポリマーおよび/または天然ポリマーから構成されるステープルファイバを含む繊維素材である、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。
- 粒子含有第1層A(23)および少なくとも1つの粒子含有第2層B(24)を有し、前記第1層A(23)が前記粒子混合物の粒子a)(26)を、前記第2層B(24)が前記粒子混合物の粒子b)(25)を含む、前記請求項の少なくとも一項に記載の傷当て材(20)。
- 前記被覆部(49)が不織布、編物、メリヤス地または織物である、前記請求項の少なくとも一項に記載の傷当て材(10、20、40)。
- 前記のほか、さらに、粒子を含まない傷接触層(11、21)を有している、前記請求項の少なくとも一項に記載の傷当て材(10、20)。
- サイズの異なる下記ポリアクリレート粒子、すなわち、
a)粒径x≦300μmの粒子、20〜100重量%、および
b)粒径x>300μmの粒子、0〜80重量%
を含む粒子混合物の傷内プロテアーゼ抑制のための使用。 - 前記粒子混合物が、
a)粒径xが45≦x≦300μmである粒子、20〜98重量%、および
b)粒径x>300μmである粒子、2〜80重量%
を含んでいることを特徴とする、請求項14に記載の粒子混合物の使用。
