Abstract
Description
本願は、米国仮特許出願第61/000,632号(名称「Molecular Targeted Microbubbles for Enhanced Blood Vessel Imaging and Therapeutic Treatment of Neointimal Hyperplasia」、2007年10月26日出願)および米国仮特許出願第61/099,025号(名称「Molecular Targeted Microbubbles for Enhanced Blood Vessel Imaging and Therapeutic Treatment of Neointimal Hyperplasia」、2008年9月22日出願)の優先権の利益を主張し、これらの出願の開示は、その全体が本明細書に参考として援用される。

薬剤溶出ステント(DES)の臨床上の使用は、ベアメタル・ステント(BMS:Bare Metal Stent)に関して、およそ18カ月間に、米国におけるおよそ0%の使用から、米国における冠状動脈ステント処置のおよそ80%に用いられる処まで進展した[2(特許文献2)、3(特許文献3)]。
成獣における血管SMCの主要機能は収縮であり、SMCは、SMC分化マーカー遺伝子と呼ばれる、SMα−アクチン、平滑筋ミオシン重鎖(SMMHC:smooth muscle myosin heavy chain)、SM22α、カルポニン、デスミン、スムーセリンの遺伝子を含めて、この特殊な収縮形態を可能にする固有の遺伝子レパートリーを発現する[5−8(特許文献5−8)]。この遺伝子レパートリーは、一般に「収縮」表現型または成熟SMCを記述するために用いられる。
損傷によって誘発される表現型の調節に伴う、SMC遺伝子発現プロファイルの変化は、過渡的である。すなわち、SMCは、損傷血管を修復する自然な反応として、表現型の調節を受け収縮表現型から合成表現型へと移行するが、障害が消散するにつれて元の収縮表現型に戻る。それ故に、この一連の調節を受けたSMC遺伝子発現プロファイルを用いれば、分子標的化を利用して、新内膜の発達に費やされる表現型改変SMCを標的にすることができる。VCAM−1(血管細胞接着分子1)は、増殖SMC中で発現され[9、10(特許文献9、10)]、急性血管損傷後のSMC、およびアテローム動脈硬化性病変中で過渡的に上方制御される[11(特許文献11)]。VCAM−1の機能は、SMC遊走、および病変中への他の細胞タイプの動員または誘引に必要な細胞−細胞相互作用、例えば、SCM上のVCAM−1と、白血球、単球またはマクロファージ(すべて炎症細胞)上のインテグリンとの相互作用を促進することである[9(特許文献9)]。VCAM−1の発現レベルは、不活性で収縮性のSMC表現型ではずっと低いが、増殖性のSMC中では増加する。従って、増殖性SMCを標的にするためにVCAM−1を用いることができる。
細胞周期は、5つの基本ステップからなる:休止(G0)或いは収縮性のSMC表現型、ギャップ期1(G1)、合成(S)、有糸分裂前またはギャップ期2(G2)および有糸分裂(M)。急性血管損傷への反応として、SMCは、G0を離れてG1に入り、M期の増殖および分裂へと細胞過程を開始する;これは、合成・遊走性または増殖性のSMC表現型である。SMCの増殖、および細胞周期への移行を防ぐための戦略は、細胞が損傷または何らかの急性増殖刺激に反応してG0を離れた時点で、細胞周期の様々なフェーズを阻止することであった。シロリムスまたはラパマイシン、およびその類似体、ABT578(Abbot Pharmaceuticals社)並びにエベロリムスは、抗炎症および抗増殖の両特性をもち、細胞のG1からS期への移行を抑制することによって早期に細胞周期に介入する免疫抑制薬である。G1期に細胞周期を抑制する薬物は、細胞増殖抑制性であると考えられ、細胞周期の後の方で作用する薬物に比べて毒性を低めることができる[12、13(特許文献12、13)]。ラパマイシンは、この群のなかで最も詳細に調べられた薬剤であり、冠状動脈再狭窄を予防するためのベンチマーク薬になっている[14(特許文献14)]。したがって、ラパマイシンは「細胞増殖抑制性」であると考えられ、ラパマイシン処置されたSMCは、死なずに増殖停止状態で生存能力を維持する。
血管内疾患の徴候を検出する手段として、標的化超音波コントラスト・マイクロバブルの実現可能性が最近の研究で詳細に調べられてきた。病変は、しばしば病的血管の内皮細胞内層における変化を伴う。この機能障害は、微小循環に発生することがあり、血管内皮表面上でのいくつかの分子の選択的発現または上方制御によって確認される。アテローム性動脈硬化症[15(特許文献15)]、移植拒絶反応[16]、炎症および虚血再灌流障害[17]のような病状に対応する内皮機能障害の分子マーカーの多くは、よく特徴付けられている。しかしながら、現在、かかる血管病変の範囲および部位を評価する非侵襲で臨床的に認可された技術はない。所期の標的に対する特異的表面結合リガンドを含む標的化マイクロバブルの実験的な製剤が、血管内に注入され、短い循環時間の後に標的部位に蓄積することが観察されている。これに続く超音波イメージングによって、標的とする疾患状態の位置および広がりの測定が可能である[18]。この技術は、「標的化コントラスト強調超音波」として知られており、高空間分解能、リアルタイム・イメージングに加えて、付着マイクロバブル・受信信号間の測定可能な線形などの相関を達成することができる。
空間的に局在化した集束的な非侵襲/低侵襲性の処置には、選択された標的部位を周囲の生体構造の環境の中で治療(焦点)領域の局在化を誘導するための、適切な非侵襲性のリアルタイム・イメージングが必要である。この点は簡単に見えるかもしれないが、非侵襲性処置に関する深遠な意味をもつ。このパラダイムは、集束的な処置ゾーンの正確で信頼性の高い位置合わせが、いかなる非侵襲性のイメージングを用いてでも確実に行なわれるよう注意すべきことをさらに示唆する。理想的なモデルは、像面が、治療の点、線または面と一致することであろう。しばしば、小さいイメージング用アレイが、より大きい治療用アレイから「切り取られた」アパーチャ内の中心に配置される。Rosenschein[21]は、7.5MHzの環状アレイが同心円的に中に配置された、直径94mmの治療用アレイについて記載している。このシステムは、牛動脈切片におけるインビトロの血栓溶解に首尾よく使用された。Unger[22]は、共通の前方平面(front face plane)をもつ治療用およびイメージング用アレイ素子を組み込んだトランスデューサ設計について(少なくとも概念的に)記載している。この例では、治療用アレイが、イメージング用アレイ中のホール内に置かれる。アレイ・アパーチャ中の大きい中心「ホール」は、近接場の盲点、および−アパーチャ中の「ホール」により潜在する、不十分な空間サンプリングに起因する典型的な回折ローブ関連の−歪んだサイドローブ・パターンを生じさせる。現在まで、治療用集束トランスデューサ/アレイに対してイージング用アレイを固定する多くの研究が行われてきた[23〜25]。イメージングおよび治療用集積化アレイが、例えば、ワシントン大学によって記載されている[26]。しかしながら、かかる「集積化」アレイが、本明細書で提案されるような厳密に一致した「治療」およびイメージング用アレイを備えると信じるべき理由はない。「集積化」の要求レベルを規定する明確な必要性は、特定の応用に依存するのである。
ラパマイシンの化学的および生物学的特性、並びにラパマイシンが血管損傷関連のSMC増殖をインビボで防ぐベンチマーク試薬である理由は、上述の通りである。超音波誘起によるマイクロバブル・キャリアからの放出にラパマイシンを選ぶ根本的な理由は、これから明らかである。培養SMCをラパマイシンで処理すると、SMC増殖が減速することを複数のグループが示している[12、30]。しかしながら、超音波誘起によるマイクロバブル・キャリアからの放出を利用したラパマイシンの送達は、実施されてこなかった。
シェル中にラパマイシンを含む改質された超音波マイクロバブルと併せて、超音波をラットの平滑筋細胞に適用した。マイクロバブルは、共同発明者A.L.KlibanovによってUVAで調製された。マイクロバブルは、以前に記載された手順[31]と同様に、ホスファチジルコリン(2mg/ml)およびポリエテレングリコール(PEG:Polyethylene Glycol)ステアラート(2mg/ml)と、ラパマイシン(0.2mg/ml)および/または微量の螢光色素DiI(Molecular Probes社、ユージーン、オレゴン州)とからなる水性ミセル混合物中に、デカフルオロブタン・ガスを超音波分散させる間に、脂質単分子層の自己組織化によって形成された。マイクロバブル媒体だけでは細胞に影響を及ぼさないことを確実にするために、蛍光標識されたDiIマイクロバブルを対照として用いた。ラパマイシン・マイクロバブルの効果を比較するための対照として、100%エタノールに溶解したラパマイシン薬物も用いた。細胞を蒔く前に24時間、フィブロネクチンを蒔くことによって、OptiCell(Biocrystal,ウェスタービル,オハイオ州)フラスコに細胞が確実に強く付着するようにした。12個の各OptiCell内のDF10培地にラットのSMCを低濃度で蒔き、48時間にわたって成長させた。ベースラインの状態を確認するために、処理の5時間前に、デジタル位相差顕微鏡法により細胞の光学像を撮影した。すべての像は倍率4×で撮影した。蒔いた24時間後に、この培地をDiIマイクロバブル(媒体対照)、ラパマイシン薬物(薬物対照)、またはラパマイシン・マイクロバブルのいずれかを含んだ新鮮な培地に交換した。マイクロバブル(DiIまたはラパマイシン)を10×106バブル/mlの濃度でOptiCellに加え、ラパマイシンを10ng/mlの濃度で加えた。マイクロバブルの濃度は、加えたマイクロバブルの数が、同量のラパマイシン〜10ng/mlを含むように選んだ。OptiCellフラスコの半数を選んで2時間だけ処理することによって、曝露が長引かなくても、薬物が効果を生じることを確認した。2時間後に、薬物/バブルを含んだ培地を新鮮な培地に交換した。細胞は、OptiCellフラスコで次の6つの処理のうちの1つを受けた:DiIバブルで48時間、ラパマイシン薬物で48時間、ラパマイシン・バブルで48時間、DiIバブルで2時間、ラパマイシン薬物で2時間、ラパマイシン・バブルで2時間。すべての条件でテストを2回繰り返した。
図5Aおよび5Bは、超音波誘起による放出形態マイクロバブル・キャリアからの〜10ng/mlのラパマイシンの送達が、同数のマイクロバブル・キャリアからの螢光膜色素DiI(Invitrogen)の放出と比較してSMCの増殖を妨げることを、細胞数の変化として示す。さらに、図5Dの定量的な解析は、超音波誘起によるマイクロバブル・キャリアからの放出を用いたラパマイシン(10ng/ml)の送達が、細胞培地において自由なラパマイシン薬物(10ng/ml)で処理された細胞と違わなかったことを示す。同様の結果は、超音波後に2時間だけ処理した後、48時間成長させた6つのOptiCellのセットでも観察された(図5Cおよび5E)。したがって、これらの結果は(とりわけ)、ラパマイシンおよび不活性な細胞標識色素(DiI、図5C)が、超音波誘起によるマイクロバブル・キャリアの放出を用いてSMCに送達できることを示した。
二重機能のイメージング治療のための例となるトランスデューサの解決法は、トランスデューサ素子に十分な多用途性があり、両方のタスク−高周波(HF)イメージングおよび低周波(LF)バブル操作/破壊−を達成できることにある。これによって、イメージング平面と治療平面とが一致した設計が可能になる。これらの設計にはこれまで欠陥があることから、より優れた解決法が必要なことが示唆される。
入手できる様々なオプションのうちでも、SonixRP(Ultrasonix社、リッチモンド、ブリティッシュ・コロンビア州、カナダ)は、本発明を実施する基盤となる機器化に利用すべき多用途プラットホームである。もちろん、当業者によく知られるように、他のスキャナー・プラットホームを入手または設計/構築することもできる。RPおよびその研究能力(高レベルのソフトウェア/ハードウェア・アーキテクチャを含む)が、最近の公表文献に詳しく記載されている[32][32]。
血管内注入される標的化造影剤を用いる際に遭遇する問題は、十分に接近して(<1μm)所望のリガンド・受容体間分子結合を形成する機会をたとえ僅かでももつことになるのは、非常に細い血管を除けば、注入物質のごく一部に過ぎないことである。生理的流れの条件下では、灌流マイクロバブルのごく一部だけが明確に維持されるに過ぎないことを、標的化マイクロバブルのP−セレクチン基質上への付着に関するインビトロ研究が報告している[例えば、Klibanov[33]]。単一マイクロバブルの検出が可能であるとはいえ[34]、マイクロバブル標的化が低効率なために、さもなければ必要であろう量に比べて、より多量のマイクロバブル投与が必要となっている。マイクロバブルは、赤血球と同様の流動学的挙動を示し[35]、血管の中心に向かって遊走する傾向がある。ほとんどの内皮タンパク質は、内皮からナノメーターの範囲にあるに過ぎないので[36]、標的脈管構造を通って流れるマイクロバブルの多くは、所望の分子標的と接触しそうにない。マイクロバブルの付着効率は、循環しているマイクロバブルが血管壁に接触するように移動させ、したがって、マイクロバブル:標的接着事象の頻度を高めることによって高めることができる。Dayton[37]およびその他[38]は、超音波放射圧を用いて自由流動マイクロバブルを血管壁に推し付けることによって、血管内皮に対するマイクロバブルの付着性が向上することを以前に提唱した。音圧印加下のアビジン:ビオチンのモデルシステムにおいて、マイクロバブル付着[39]、および音響活性なリポソーム[40]が調べられ、培養内皮細胞に標的化マイクロバブルが付着することが報告されている[39]。
1.ラパマイシン搭載マイクロバブル+超音波は、実証済で選択的な抗増殖効果をラットSMC上で有する。
(単一素子トランスデューサ(一般にイメージングが不可能))
簡単な、軸対称集束、単一素子のトランスデューサを用いた我々の予備データは、有望な初期結果を示した。必要なのは、二重機能の(低周波バブル「破壊」+高周波イメージング)トランスデューサおよび関連機器化である。
例となる設計は、一方を他方の上に積み重ねた、1:3コンポジット圧電セラミック−エポキシ活性層を備えるとよい。(「1:3」コンポジットは、ポリマーマトリックス中に埋め込まれた圧電セラミック・ポストを備える−すなわち、2つの構成部分は、電気的および機械的に「並列」である。1:3配置は支配的な複合配置であり、商業的に広く使用されている)。このコンポジット材料は、セラミックの体積分率がおよそ50%であり、およそ15MRaylの音響インピーダンスを有する。タングステン粒子を充填した高密度のバッキング・ブロックが使われる。12MHz動作のために、およそ4分の1波長に整合した薄い整合層が上面に使われる。高さ方向の焦点を得るために、従来の充填剤入りシリコンゴムレンズが使われることになろう。高さ方向の焦点深度は、およそ15mmである。特に、我々はおよそ12MHzのB−モード・イメージングを用いており、方位分解能および距離分解能は、その結果として<200μmになる。この周波数におけるλは125μmである。従って、実用的なf#(すなわち、1〜2)に対して200μmの分解能が可能である。アレイシステムは、十分過ぎるほどの走査フレームレート(小さい選択視野−例えば、15mm×15mmに対して>100フレーム/s)を提供する。集束超音波送達は、1〜2MHzで送達される。我々は、治療効果が得られる領域をおよそ3λ−すなわち、およそ2mmのスポットサイズに制御することができる。
目的は、解剖学的B−モード・イメージング能力、バブル固有イメージング、およびバブル破壊パルス印加をユーザー制御の下で提供することである。解剖学的B−モード・イメージングは、標準B−モード・イメージ形成技術−すなわち、最適化されたアパーチャ・アポダイゼイション、焦点が固定された送信、動的な受信焦点合わせ、信号検出および走査変換−を用いて達成される。バブル固有イメージングは、「パルス反転」(PI:Pulse Inversion)[45]−すなわち、1、−1送信極性/振幅;続いて、線形成分除去のための「1」+「−1」処理)−を用いることによって提供されることになる。必要ならば、他のバブル固有技術、例えば振幅スケーリング(すなわち、1、2送信極性/振幅;続いて(2×「1」)−「2」処理[46])、およびPIと振幅スケーリングとの組み合わせ(例えば、−1、2、−1送信極性/振幅;続いて「−1」+「2」+「−1」処理[47])。
これらのモードには、アレイの低周波素子を用いる。トランスデューサの実施形態の設計は、合計が128より少ない素子(96個の12MHz素子、および24個の2MHz素子)を備える。このように、利用できる128個のトランスデューサ・コネクタチャンネルから選択されるトランスデューサ・アパーチャを単に再プログラムすることによって、イメージングおよび治療動作モード間の切り替えを行うことができる。
(バブル生成およびラパマイシンの取り込み)
マイクロバブル形成は、以前に記載された手順[48]と同様であり、ホスファチジルコリンおよびPEGステアラート(2mg/ml)と、ラパマイシン(0.2mg/ml)および/または微量の螢光色素DiI(Molecular Probes、ユージーン、オレゴン州)との水性ミセル混合物中に、デカフルオロブタン・ガスを超音波分散させる間に、脂質単分子層の自己組織化によって形成される。いくらかの事例では、グリセロールトリオレアートを添加することによって、膜の肥厚化が達成される(マイクロバブル・シェルを厚くすると、ラパマイシン含有量が増加する)[49]。バブルシェルに取り込まれていない自由な脂質、色素およびラパマイシンは、順次(3×)遠心浮遊法(100×g、5分)によって除去し、最初の洗浄液は試薬を節約するために再利用された。
ロバストで高感度の高速液体クロマトグラフィー(HPLC)の手順は、臨床検査に関する文献に記載されている。我々の研究室ではHPLCが利用でき、かかる手順[50]を実施することになるであろう。簡潔に言えば、テストサンプル(マイクロバブルまたは媒体)を凍結乾燥し、クロロブタノール中に再び溶かして、遠心分離により沈降物を除去する;サンプルを自動サンプラーのバイアル中に置いてHPLCを実行し、既知量のラパマイシンの較正曲線に照らしてUV検出を行う。
ラパマイシン含有マイクロバブルを分散させた生理食塩水(粒子濃度106〜107/ml)を、容積10mlのOptiCell(USA Scientific社、オカラ、フロリダ州)中に置く。細胞培養研究に記載された条件でバブルを超音波破壊して、OptiCellからマイクロバブル粒子を除去し、残りのマイクロバブルが、(もし存在すれば)サンプルから除去されるであろうことを証明するために、遠心浮遊法を適用する。次に、上述のようにHPLC技術を用いて、バブルのないインフラナタント(infranatant)中のラパマイシンの定量化を行なうことになる。
抗VCAM−1抗体のマイクロバブル表面への連結は、[44]に記載されるようにストレプトアビジン連結技術を用いて行なわれる。簡潔に言えば、マイクロバブルを調製する間に、2mol%のビオチン−PEG3400−ホスファチジルエタノールアミンを脂質混合物に加える。他の抗体について以前に記載されたように[33、51]、ビオチン標識された抗VCAM−1抗体をマイクロバブル表面に連結するためには、ストレプトアビジン架橋技術を適用する。抗体分子のビオチン標識化は、ビオチンN−ヒドロキシスクシンイミドエステル試薬を用いて、pH7.5のDPBS緩衝液中で行なう。以前に記載されたように[例えば、Klibanov[33]]、抗体のビオチン標識化の度合いは、HABA試験法を用いて試験する。抗体/ビオチン−NHSの調整により、インキュベーションによる連結の比率として抗体当たり〜1ビオチンが達成されるであろう。ビオチン標識化が抗体を不活性化しないことを、VCAM−1抗原のELISAテストを用いて確認する。ストレプトアビジン−バブル(109/ml)をビオチン標識された抗体とともに氷上で30分間インキュベーションする;脱気したDPBS緩衝液を用いて、バケット・ロータ型遠心機(100×g、5分)中で遠心浮遊洗浄を3回行い、自由な抗体をバブルから除去する。浮選を繰り返した後、抗体被覆バブルの平均サイズは、標準的に〜2.5μmであり、>99%の粒子は8μmより小さい(粒子のサイズおよび濃度は、Coulter Multisizer IIe機器(Beckman Coulter社、マイアミ、フロリダ州)を用いて評価する。バブル当たりの付着抗体量は、先述のように、蛍光分光学的な標識法によって試験する;典型的に、この技術によってマイクロバブル当たり〜105の抗体分子が付着する[51]。
当然のことながら、前述のカテーテル1218、マイクロバブル貯蔵器1232、マイクロバブル・ポートまたはチャンネル1233、超音波源(単数または複数)1212、およびコントローラは、対象1211の適切な位置の完全に内部に、対象の位置の外部に、或いは対象の位置の内部または外部に組み合わせて配置することができる。対象の1つ以上の位置1211は、臓器とすることができる。臓器は、中空臓器、中実臓器、実質組織、間質組織、および/または管を含むことができる。対象の1つ以上の位置1211は、管状の解剖学的構造とすることができる。管状の解剖学的構造は、血管とすることができる。さらにまた、例えば、処置部位1210は、次の少なくとも1つ:狭窄領域、或いは血管疾患を示す任意の領域、を備える脈管構造の処置部位とすることができる。さらにまた、例えば、処置部位1210は、脈管構造の処置部位および/または診断部位とすることができる。

1.多用途性の向上−シェル組成を(すなわち、可能性として薬物/遺伝子の負荷量および濃度を「飛行中」に)変えることができる;
2.別の方法では不可能なバブルが可能になる。先端におけるバブル生成は、安定度の問題の軽減を意味する。バブルは治療用送達前の数秒存続すればよいだけである。これによって、より不安定な化学製剤、或いはより不安定なバブル(すなわち、シェル/気体)置換が可能になる。現在、気体は非常に低拡散速度(すなわち、高分子量)のものに限られている。新しい設計では、新しい気体または低分子気体の使用が少なくとも可能になる。我々の意見では、この分野はまだ正当に探究されていない。
本発明二重使用IVUSの実施形態または手法は、イメージングおよび治療を提供する。
