Abstract

Description
本発明は、血管閉塞を処置するための装置および方法に関し、そしてより具体的には、閉塞内に流体を注入しそしてその閉塞を押し出すように構成された、1つの無外傷性先端および少なくとも1つの送達ポートを有する注入カテーテルを提供することによって閉塞を処置するための、装置および方法に関する。
介入を必要とする血管内閉塞は、種々の公知の医療技術を使用して処置され得る。これらの技術の各々において、処置部位で逆向性の流動能力を提供することが望ましくあり得る。このことは、処置の間に自由にされた塞栓を近位方向に流動させ、その結果、この塞栓は血管床内により深くには移動せず、そして梗塞形成および/または壊死を生じる、さらなる閉塞性事象を引き起こさない。

Claims (31)
- 血管壁に接着した血管閉塞を破壊するのに適した装置であって、該装置は、以下:
近位末端および遠位末端ならびにそれらの間に延びる内腔、該遠位末端に配置された無外傷性先端、ならびに該内腔と流体連絡される該無外傷性先端の近位に配置される少なくとも1つの送達ポート、を有する注入カテーテルを備え、
ここで、該注入カテーテルは、該血管壁の脈管内膜への該閉塞の接着を流体が低減するように、該閉塞内に該流体を注入するよう構成される、装置。 - 前記無外傷性先端が白金コイルを備える、請求項1に記載の装置。
- 少なくとも1つの送達ポートが、前記注入カテーテルの長手軸に対して斜角を形成する流体射出を送達する、請求項1に記載の装置。
- 前記送達ポートが流体を近位方向で排出させる、請求項3に記載の装置。
- 前記送達ポートが環状構造を備える、請求項1に記載の装置。
- 前記注入カテーテルがさらに、前記無外傷性先端の近位に配置された、少なくとも1つの放射線不透過性マーカーバンドを備える、請求項1に記載の装置。
- 前記送達ポートの近位に第1放射線不透過性マーカーバンドが配置され、そして該送達ポートの遠位に第2放射線不透過性マーカーバンドが配置される、請求項6に記載の装置。
- 前記流体が生理食塩水である、請求項1に記載の装置。
- 前記注入カテーテルの内腔内に長手軸方向に進められるように構成される、取り外し可能なスタイレットをさらに備える、請求項1に記載の装置。
- 前記注入カテーテルの内腔中に注入される流体の圧力を制御するように構成される注入ポンプをさらに備える、請求項1に記載の装置。
- 請求項1に記載の装置であって、近位末端および遠位末端ならびにそれらの間に延びる内腔を有する塞栓除去カテーテルをさらに備え、そして該遠位末端に配置される閉塞要素をさらに備え、ここで、該塞栓除去カテーテルの内腔が該注入カテーテルの長手軸方向の進行を可能にするようにサイズ決めされる、装置。
- 前記塞栓除去カテーテルの内腔が患者の静脈管系と流体連絡される、請求項11に記載の装置。
- 請求項11に記載の装置であって、近位末端および遠位末端およびその間に延びる内腔を有する静脈戻りシースをさらに備え、ここで、該静脈戻りシースが、前記塞栓除去カテーテルの内腔と流体連絡されそして患者の血管系中に配置されるように適合される、装置。

ここで、該注入カテーテルは、該血管壁の脈管内膜への該閉塞の接着を流体が低減するように、該閉塞内に該流体を注入するように構成される、装置。 - 請求項16に記載の装置であって、近位末端および遠位末端ならびにそれらの間に延びる内腔を有する外側シースをさらに備え、該外側シースが、該外側シース内に収縮した状態で展開可能なストラットを拘束するように構成される、装置。
- 前記展開可能なストラットが展開した状態で前記血管壁に嵌合するようにサイズ決めされる、請求項16に記載の装置。
- 前記展開可能なストラットがニッケル−チタン合金を含む、請求項16に記載の装置。
- 少なくとも1つの送達ポートが、前記注入カテーテルの長手軸に対して斜角を形成する流体射出を送達する、請求項16に記載の装置。
- 前記送達ポートが流体を近位方向で排出させる、請求項20に記載の装置。
- 前記カテーテルが、無外傷性先端の近位に配置された、少なくとも1つの放射線不透過性マーカーバンドをさらに備える、請求項16に記載の装置。

無外傷性先端の近位位置で注入カテーテル上に配置された、少なくとも1つの放射線不透過性マーカーバンドを提供する工程;および
該閉塞内に該送達ポートの位置を決定するために、該放射線不透過性マーカーバンドを使用する工程、を包含する、方法。 - 注入ポンプを使用して制御された圧力で前記閉塞内に流体が注入される、請求項25に記載の方法。
- 請求項25に記載の方法であって、さらに以下:
