Abstract
Description
本願は、2007年10月5日に出願された米国仮特許出願第60/997,856及び第60/997,873に基づく優先権を主張する。本願は2007年2月20日に出願された米国仮特許出願第60/890,831号(発明の名称:傾斜骨穿孔術及び治療方法(Directional Bone Drilling and Methods of Treatment))及び2007年2月22日に出願された米国仮特許出願第60/891,183(発明の名称:傾斜骨穿孔術及び治療方法(Directional Bone Drilling and Methods of Treatment))に関する。
米国整形外科学会によれば、「米国において毎年、120万人超の人々が新たに癌と診断され、そのうち約50%の腫瘍が骨格に広がる、即ち、転移可能である」とされている。そのため、米国内だけでも転移性骨癌を罹患する患者が50万人超いる。骨は、3番目に転移性疾患が起こりやすい場所である。骨に最も転移しやすい癌として、乳癌、肺癌、前立腺癌、甲状腺癌及び腎臓癌が挙げられる。多くの場合、複数の骨転移部位があり、治療をより困難なものとしている。骨転移に関連した病的状態の主なものとして、疼痛、病的骨折及び高カルシウム血症がある。疼痛が最も一般的な症状であり、患者の70%に見られる。
治療領域に外部放射線ビームを向ける外部ビーム放射線療法(例えば、上述した骨癌について)とは異なり、近接照射療法は治療を必要とする領域中又はその隣接に放射性源を入れる放射線治療の一種である。また、近接照射療法は、密封線源治療又はエンドキュリー療法(Endocurietherapy)としても知られており、通常、局在化した前立腺癌及び頭や首の癌を治療するのに用いられる。放射性源を皮膚の近くに入れることにより、表在腫瘍を治療することができる。間隙近接照射療法では、放射性源が組織に挿入される。腔内近接照射療法では、放射性源を既存の体腔に入れる。脈管内近接照射療法では、カテーテルと共に放射性源を血管内に入れる。
特に記載がない限り、全てのパーセンテージは重量/重量(W/W)である。
MURRとはUniversity of Missouri Research Reactor (Columbia, MO)のことであり、放射性同位体を提供するサービスを有する
多くの場合、放射性同位体が非放射性キャリア同位体を混合物として含んでいることが理解されよう。
2.0μLの50%w/wNaOHを10μLのLu−177溶液(MURRから得たもの。0.05MのHCL中1.09Ci/mL)に加え、その後8.0μLの水を加えて、高pHなLu−177溶液を調製した。当該混合物を注入前に30分間放置した。混合物のpHは約10超であった。
0.05MのHCL中Lu−177溶液は約1.09Ci/mLを含有し、MURRから得た。同量のLu−177溶液及び0.05MのHCLを混合することにより、注入液を調製した。pHは約2未満であった。
HT−29異種移植片を有する無胸腺マウスに麻酔をかけ、実施例1の2〜3μLの混合物を約20μLの水で希釈し、腫瘍に直接投与した。腫瘍周囲の幾つかの異なる部位において複数の注入を行い、また腫瘍塊にも直接行った。注入した放射能量が0.924mCiのLu−177であることを決定した。治療から13日後のガンマカメラ画像から、注入部位での多くの放射能が残っていることが分かった。注入13日後のいかなるマウスの尿又は糞において、Lu−177は1μCi未満であった。腫瘍の大きさを測定し、生理食塩水を注入した同様のコントロールマウスと比較した。生理食塩水コントロールマウスの腫瘍は大きくなり、この実施例2のマウスの腫瘍は小さくなった。これらの結果を以下の表1に示す。
HT−29(ヒト大腸癌)異種移植片を有する無胸腺マウスに麻酔をかけ、実施例Aの2〜3μLの溶液を腫瘍に直接投与した。ドーズキャリブレータを用いて注入した放射能量が1.08mCiのLu−177であることを決定した。
実施例1の約6〜8μLの溶液量を、麻酔下の正常のスプラーグドーリ−ラットの前立腺の左葉に投与した。0.924mCiのLu−177をラットが受け取った。
MURRから得たLu−177は、到着時に0.1MのHCL中で0.71mCi/μLであった。CapintecTM CRC−15ドーズキャリブレータを用いて放射能量を測定した。3.0 μLのこの溶液に3.0μLの1.0N NaOH (Fisher)を加えた。水を加えて最終量を10.0μL とした。
麻酔下の364gのスプラーグダウリーラット(オス)の肺に、インスリンシリンジを用いて実施例4の3〜5μL(〜1.0mCi)の調製物を直接注入した。針を挿入して皮膚を貫通させることによって投与量が肺の左葉に堆積した。
ホルミウム−166(Ho−166)をMURRから得た。この溶液は350μL中52.4mCiであって、比活性は0.1MのHCl中0.15mCi/μLであった。Ho−166溶液(10μL)をバイアルに入れ、0.5μLの0.1MのNaOHを加えた。pH紙でpHを測定し、約10のpHであった。
0.1MのHCl中のHo−166をMURRから得た。pHをpH紙で測定し、約1のpHであった。実施例6に記載の上記ミニチュアドリルを用いて、麻酔したスプラーグダーリーラットの大腿骨に孔を形成した。上記ミニチュアポンプを用いて、ドリルで形成された孔に3μLのHo−166溶液を運搬した。放射線量を注入した2時間後に、ラットを屠殺し解剖した。注入部位における放射能量は、注入した量の5%であった。しかし、52%の量は肝臓にあり、23%の投与量は残りの骨にあった。全骨格量は、反対側の大腿骨の投与量(%)に25を乗じることにより決定した。非標的領域における高投与量は、この形態のHo−166は患者に投薬する有効な方法でないことを示す。
ヒドロキシアパタイト(6mg)をバイアルに入れ、600μLの水を加えた。混合物を振とうしてスラリーを形成し、15μLのスラリーをとって他のチューブに入れた。これに0.1MのHCl中の3μLのHo−166を加えた。実施例6に記載されたミニチュアドリルを用いて、麻酔したスプラーグダーリーラットの大腿骨に孔を形成した。実施例6に記載されたミニチュアポンプを用いて、ドリルで形成した孔に3μLのHo−166で標識したヒドロキシアパタイト溶液を運搬した。投与量を注入した3時間後に、ラットを屠殺して解剖した。注入部位又はその直接的周辺の放射放射能量は、注入した投与量の94%であった。肝臓又は他の骨では投与量の1%未満であった。全骨格量は、反対側大腿骨の投与量(%)に25を乗じることにより決定した。
0.1M HCl中のSm−153をMURRから得た。5.6μLの13mg/mLのDOTMP(先にpH7〜8に調製した)を含有する溶液及び4μLの水と5μLのSm−153を合わせることにより、Sm−153及びDOTMPの間に形成した複合体を調製した。更に5μLのDOTMP溶液を加えて、高複合体収率を得た。複合体としてのSm量は、イオン交換クロマトグラフィーにより99%であった。既知の合成技術でDOTMPを調製及び精製した。キレート剤は99%超の精製度であった。実施例6に記載されたミニチュアドリルを用いて、麻酔したスプラーグダーリーラットの大腿骨に孔を形成した。実施例6に記載されたミニチュアポンプを用いて、2μLのSm−153−DOTMP溶液を、ドリルで形成した孔に運搬した。投与量を注入してから2時間後に、ラットを屠殺して解剖した。注入部位の放射能量は、注入した投与量の9%であった。肝臓において放射能はなく、残りの骨において20%であった。全骨格量は、反対側大腿骨の投与量(%)に25を乗じることによって決定した。尿においては、平均で注入した投与量の65%であった。
上記実施例は本発明を説明するものである。高pHな放射性同位体の組成物溶液が少量で投与されると、放射能の顕著な量が投与部位から移動してしまう低pHな放射性同位体の同様の投与(例えば、実施例B)と比較すると、注入後13日を経過しても大部分の同位体が投与部位に残留する(例えば、実施例2)。骨腫瘍に約0.1%の放射能しか吸収されない骨親和性放射性医薬品のI.V.投与と比較すると、骨に対して同位体の直接注入を直接行うと顕著に高いパーセントの放射能が骨に運ばれる。これにより、かなり低量の放射能を注入して、標的組織にはるかに大量の放射線量を運搬することができる。
Claims (21)
- 治療が必要な動物又はヒトにおける所望でない組織塊の当該治療のための、約7超のpHを有する治療有効量の放射性同位体製剤と、医薬として許容される液体キャリアとを含む組成物であって、当該組成物が当該組織塊に直接又は近傍に投与され、放射性崩壊の2半減期以内に約15%未満の当該放射性同位体が投与部位から移動する組成物。
- pHが約8超である請求項1記載の組成物。
- 他の非放射性金属が加えられると前記放射性同位体と共沈する、請求項1又は2記載の組成物。
- 前記所望でない組織塊が癌である請求項1記載の組成物。
- 投与する製剤の量が治療する組織塊の約50μL/cm3未満である請求項1記載の組成物。
- 投与する製剤の量が治療する組織塊の約20μL/cm3未満である請求項5記載の組成物。
- 前記癌が骨癌であり、骨に孔を形成するためにミニチュアドリルを用い、当該孔を通して針又はカテーテルを挿入することができ、少量の流体を送達することができる装置を用いて前記製剤を送達する請求項4又は5記載の組成物。
- ポンプ又はシリンジを介して前記量が送達される請求項1又は5記載の組成物。
- 前記放射性同位体がSm−153、Ho−166、Y−90、Pm−149、Gd−159、Lu−177、Yb−175、Pb−212、Bi−212、Bi−213、Ac−225である請求項1記載の組成物。
- 前記放射性同位体がSm−153、Ho−166、Y−90、Bi−213、Ac−225又はLu−177である請求項9記載の組成物。
- 治療有効量の放射性同位体製剤を直接投与するための組成物であって、治療が必要な動物又はヒトの所望でない組織塊の当該治療のために、当該放射性同位体がバインダーに結合し、放射性崩壊の2半減期以内に約5%超の当該放射性同位体が投与部位に残留する組成物。
- 前記バインダーがホスホン酸キレート剤である請求項11記載の組成物。
- 前記放射性同位体がDOTMPにキレート化され、前記放射性同位体がSm−153、Lu−177、Ho−166又はAc−225である請求項12記載の組成物。
- 前記バインダーが固体担体であり、放射性崩壊の2半減期以内に約75%超の前記放射性同位体が投与部位に残留する請求項11記載の組成物。
- 前記固体担体がヒドロキシアパタイトである請求項14記載の組成物。
- 前記製剤が、溶液、懸濁液、スラリー又は乳濁液の形態の液体である請求項1又は11記載の組成物。
- 全身的になる放射能を捕捉及び除去するために、アミノメチレンカルボン酸の静脈投与が用いられる請求項1又は11記載の組成物。
- 前記アミノメチレンカルボン酸がEDTA又はDTPAである請求項17記載の組成物。
- 前記製剤をマイクロリットルの量で正確に送れるマイクロポンプによって、前記放射性同位体を投与する請求項1又は11記載の組成物。
- 治療が必要な動物又はヒトの骨癌又は軟組織腫瘍の所望でない組織塊に、請求項1記載の放射性同位体製剤を治療有効量で直接投与するための方法であって、当該組成物が、放射性崩壊の2半減期以内に約15%未満の当該放射性同位体が投与部位から移動するようになっている方法。
- 治療が必要な動物又はヒトの骨癌又は軟組織腫瘍の所望でない組織塊に、請求項11記載の放射性同位体製剤を治療有効量で直接投与するための方法であって、当該放射性同位体がバインダーに結合し、放射性崩壊の2半減期以内に約5%超の当該放射性同位体が投与部位に残留する方法。
