Abstract
Description
本願は、「BIASED ACOUSTIC SWITCH FOR IMPLANTABLE MEDICAL DEVICE」という名称の2007年3月26日に出願された米国仮出願第60/908,171号に対して、米国特許法第119条の下に優先権を主張し、この仮出願は、本明細書においてその全体が参照により援用される。
本開示は、埋込型医療デバイスに関する。より具体的には、本開示は、埋込型医療デバイスを選択的に作動させるためのデバイス、システム、および方法に関する。
ペースメーカーおよび埋込型心臓除細動器等の埋込型医療デバイス(IMD)は、体内の種々の状態を監視および調節する際に利用される。埋込型心臓除細動器は、例えば、心拍数および心律動を監視するための心調律管理用途で、ならびに心臓ペーシング、心臓細動除去、および/または心臓治療等の種々の治療を送達するために、利用されてもよい。埋込型医療デバイスは、体内で発生する種々の生理的パラメータを感知する責任がある、体内に埋め込まれる他の医療デバイスと通信することができる。例えば、温度、圧力、ひずみ、流量、化学的性質、電気的性質、磁気的性質、および均等物等のパラメータを監視するために、1つ以上の遠隔センサが体内の深部に位置してもよい。次いで、遠隔センサから受信される測定値に基づいて、埋込型医療デバイスは、患者に適切な処置を送達してもよい。
本開示は、埋込型医療デバイスを選択的に作動させるためのデバイスおよび方法に関する。例証的実施形態による医療デバイスは、エネルギー貯蔵デバイスと、音響信号を電気信号に変換するように構成される、音響トランスデューサと、電気信号がバイアス要素によって確立された特定の閾値を超えるとトリガ信号を生成するように構成される、信号検出器と、制御回路と、医療デバイスを非動作状態と動作状態との間で切り替えるように構成される、作動/動作停止スイッチとを含む。
図1は、患者の体内で1つ以上の遠隔デバイスと通信する、および/またはそれに電力供給するための例証的システム10を示す、概略図である。図1の実施形態では、システム10は、患者の皮膚下の場所で体内に埋め込まれるパルス発生器12と、図示されるように、患者の心臓16の動脈または心室のうちの1つの中、または主肺動脈18等の肺動脈のうちの1つの中等、患者の体内の深部に埋め込まれる、遠隔センサ14とを含む、心調律管理システムである。心臓16は、右心房20、右心室22、左心房24、および左心室26を含む。右心室22は、主肺動脈18につながる流出路28を含む。図1の例証的実施形態が、遠隔センサ14と通信しているパルス発生器12を含む一方で、他の実施形態では、システム10は、センサ14と通信するために、患者の身体の内側または外側に位置する他の医療デバイスを採用してもよい。
Claims (24)
- エネルギー貯蔵デバイスと、
音響信号を交流(AC)電気信号に変換するように構成される、少なくとも1つの音響トランスデューサと、
該少なくとも1つの音響トランスデューサに連結され、該AC電気信号が特定の閾値を超えるときに、作動トリガ信号を生成するように構成される少なくとも1つの信号検出器であって、該信号検出器は、該特定の閾値を設定するバイアス要素を含む、信号検出器と、
制御回路と、
該トリガ信号に応答して医療デバイスを非動作状態と動作状態との間で切り替えるように構成される、動作停止/作動スイッチと
を備える、医療デバイス。 - 前記少なくとも1つの音響トランスデューサは複数の音響トランスデューサを含む、請求項1に記載の医療デバイス。
- 前記信号検出器は低閾値トランジスタを含む、請求項1に記載の医療デバイス。
- 前記バイアス要素は電圧バイアス要素であり、前記特定の閾値は電圧閾値である、請求項1に記載の医療デバイス。
- 前記信号検出器はフローティングゲート電界効果トランジスタ(FGFET)を含み、前記バイアス要素は該FGFET内のフローティングゲートを含む、請求項1に記載の医療デバイス。
- 前記FGFETは前記フローティングゲートに電気的に連結される複数の制御ゲートを含み、該複数の制御ゲートのうちの1つは前記少なくとも1つの音響トランスデューサから前記AC電気信号を受信するように構成され、該複数の制御ゲートのうちのもう1つは直流(DC)電源に連結される、請求項5に記載の医療デバイス。
- 前記信号検出器は強誘電体電界効果トランジスタ(FEFET)を含み、前記バイアス要素は該FEFET内の強誘電体層を含む、請求項1に記載の医療デバイス。
- 前記信号検出器は分圧器回路を含む、請求項1に記載の医療デバイス。
- 前記バイアス要素は少なくとも1つのプルアップインピーダンスデバイスを含む、請求項1に記載の医療デバイス。
- 前記バイアス要素は、少なくとも1つの逆バイアスダイオードを含む、請求項1に記載の医療デバイス。
- 前記少なくとも1つの音響トランスデューサと直列に連結される容量性要素をさらに含む、請求項1に記載の医療デバイス。
- 前記バイアス要素は前記エネルギー貯蔵デバイスとは別のバッテリを含み、該バッテリは前記容量性要素を充電するために前記少なくとも1つの音響トランスデューサと並列に連結される、請求項11に記載の医療デバイス。

- 前記バイアス要素は前記少なくとも1つのトランスデューサと並列に連結されるコンデンサを含み、前記制御回路は前記容量性要素を充電するように該コンデンサに動作可能に連結される、請求項11に記載の医療デバイス。
- 前記バイアス要素は前記少なくとも1つの音響トランスデューサと並列に連結されるショットキーダイオードを含み、該ショットキーダイオードは、前記容量性要素を充電するために前記AC電気信号の一部を整流するように構成される、請求項11に記載の医療デバイス。
- 前記バイアス要素は、回路の不安定性の閾値未満で前記容量性要素を充電するように前記少なくとも1つの音響トランスデューサに連結される回路を含み、該回路は、前記AC電気信号に応答して該不安定性閾値を超えて駆動されるように構成される、請求項11に記載の医療デバイス。
- 前記制御回路は、前記バイアス要素を変動させて前記特定の閾値を調整するように構成される、請求項1に記載の医療デバイス。
- 前記制御回路は、作動トリガ信号が生成された後の所定の期間内に確認信号が受信されない場合に、前記動作停止/作動スイッチの状態を変更するように構成される、請求項1に記載の医療デバイス。
- 前記少なくとも1つの信号検出器はそれぞれ異なる閾値を有する複数の信号検出器を含み、前記制御回路は作動トリガ信号を生成するように該複数の信号検出器のうちの1つを選択することによって、前記特定の閾値を動的に調整するように構成される、請求項1に記載の医療デバイス。
- 前記制御回路は、環境条件を感知するためのセンサを含み、該制御回路は、該感知された環境条件に基づいて前記特定の閾値を調整するように構成される、請求項1に記載の医療デバイス。
- 前記動作停止/作動スイッチは、埋込型医療デバイスを、前記エネルギー貯蔵デバイスから前記制御回路への電流のフローを実質的に制限する第1の状態から、該エネルギー貯蔵デバイスから該制御回路への電流フローを許容する第2の状態に切り替えるように構成される、請求項1に記載の医療デバイス。
- 前記医療デバイスは、1つ以上の医療機能を行うように構成される稼働要素をさらに含む、請求項1に記載の医療デバイス。
- 医療デバイスであって、
エネルギー貯蔵デバイスと、
音響信号を交流(AC)電気信号に変換するように構成される少なくとも1つの音響トランスデューサと、

該特定の閾値を動的に調整するために構成される制御回路と、

該エネルギー貯蔵デバイスおよび該制御回路に連結される動作停止/作動スイッチであって、該動作停止/作動スイッチは、該作動トリガ信号に応答して、該エネルギー貯蔵デバイスから該制御回路への電流フローを防ぐ第1の状態から、該エネルギー貯蔵デバイスから該制御回路への電流フローを許容する第2の状態に変更する、動作停止/作動スイッチと
を備える、医療デバイス。 - 音響信号を伝送するように構成される第1のデバイスと、

エネルギー貯蔵デバイスと、
該第1のデバイスから受信される音響信号を交流(AC)電気信号に変換するように構成される少なくとも1つの音響トランスデューサと、
該少なくとも1つの音響トランスデューサに連結され、該AC電気信号が特定の閾値を超えると作動トリガ信号を生成するように構成される少なくとも1つの信号検出器であって、該特定の閾値を設定するバイアス要素を含む、信号検出器と、
制御回路と、
該トリガ信号に応答して該第2のデバイスを非動作状態と動作状態との間で切り替えるように構成される動作停止/作動スイッチと
を備える、第2のデバイスと
を備える、デバイスを音響的に作動させるためのシステム。
