Abstract
Description
The present invention relates to a blood purification apparatus for purifying a patient's blood while circulating it extracorporeally, such as dialysis treatment using a dialyzer.
Generally, at the time of dialysis treatment, a blood circuit for circulating the collected patient's blood extracorporeally and returning it to the body is used. Such a blood circuit is, for example, a dialyzer (blood purification means) having a hollow fiber membrane. It is mainly composed of an arterial blood circuit and a venous blood circuit that can be connected to each other. An arterial puncture needle and a venous puncture needle are attached to the tips of the arterial blood circuit and the venous blood circuit, respectively, and are punctured by the patient to perform extracorporeal circulation of blood in dialysis treatment.
Among these, the arterial blood circuit is provided with a squeezed blood pump, and by driving the blood pump, blood is sent from the patient's body to the dialyzer side, while the arterial blood circuit and the venous blood are supplied. An arterial drip chamber and a venous drip chamber are connected to the circuit so that blood is returned to the patient's body after defoaming.
In addition, a priming solution supply line (saline solution line) for supplying physiological saline at the time of priming or blood return is provided upstream of the blood pump in the arterial blood circuit (that is, the arterial puncture needle side). It is connected via a T-shaped tube or the like, so that prior to dialysis treatment, priming can be performed by flowing and filling physiological saline (priming solution) into the blood circuit and components such as the drip chamber connected to the blood circuit. It is configured.
However, in the above conventional blood purification apparatus, it was impossible to know in advance how long it would take for blood removal, so blood removal would occur within a predetermined time unless blood removal was actually started. There was a problem of not knowing whether to end. That is, in the blood removal method in which the ultrafiltration pump is driven and the priming solution is filtered through the blood purification membrane of the dialyzer, the processing speed at which blood removal can be performed differs depending on the ultrafiltration rate of the dialyzer. However, in the past, the ultrafiltration rate was not grasped on the device side, and it was not known in advance how long it would take for blood removal.
Thus, if it is determined that the blood removal will not be completed within a predetermined time, the blood removal method may be replaced with another blood removal method by driving the ultrafiltration pump and filtering the priming liquid through the blood purification membrane of the dialyzer. It is necessary to switch to a method (for example, a method of puncturing a patient with an arterial puncture needle at the tip of an arterial blood circuit, while opening a tip of a venous blood circuit, and discharging physiological saline from the open end) There is a problem that the switching work takes time and the blood removal process takes a long time.
The present invention has been made in view of such circumstances, a blood purification apparatus that can determine in advance the time required for blood removal and can select in advance an appropriate blood removal process for each patient.
Hereinafter, embodiments of the present invention will be specifically described with reference to the drawings.
The blood purification apparatus according to this embodiment includes a dialysis apparatus for performing dialysis treatment. As shown in FIG. 1, a blood circuit including an
An arterial puncture needle a is connected to the distal end of the
The
A plurality of hollow fibers (not shown) are accommodated in the
The compound pump 6 (reciprocating pump) is disposed across the dialysate introduction line La and the dialysate discharge line Lb in the dialyzer body, and the dialyzer body includes a patient flowing through the
Further, the
Furthermore, one end of the dialysate introduction line La is connected to the dialyzer 3 (
The storage means 7 (so-called “saline bag”) is made of a flexible transparent container and can store a predetermined volume of physiological saline (priming solution). It is attached to the tip of a pole (not shown). The priming fluid supply line Lc is connected to a portion (connecting portion P) between the arterial puncture needle a and the
As shown in FIG. 2, the dialysis apparatus (blood purification apparatus) as described above connects connectors c and d to form a closed circuit as shown in FIG. 2, and also uses a priming fluid supply line Lc to connect the blood circuit (artery). A priming solution filling step is performed in which physiological saline (priming solution) is supplied into the
Following the priming solution filling step as described above, as shown in FIG. 3, the connectors c and d are disconnected, and the arterial puncture needle a and the venous puncture needle b are attached to the connectors c and d, respectively. By puncturing the patient's shunt with puncture needle a and venous side puncture needle b,
In this blood removal step, the
That is, in the blood removal process, the blood circuit and the blood flow path of the
When the physiological saline (priming solution) is completely replaced with the patient's blood, the blood removal process is completed, and the tip of the arterial blood circuit 1 (arterial puncture needle a) is passed through the dialyzer 3 (blood channel). Dialysis treatment such as purification by the blood purification membrane of the
Here, the dialysis apparatus (blood purification apparatus) according to the present embodiment includes a
The calculation means 9 is capable of calculating the time required for the blood removal step as described above, and the blood purification membrane when the physiological saline (priming solution) in the blood channel of the
Specifically, the calculation means 9 multiplies the value of the ultrafiltration rate (UFR) by the maximum assumed transmembrane pressure difference (TMP) value (ultrafiltration rate (UFR) × transmembrane). pressure difference (TMP)) ultrafiltration pump is determined by 8 (ultrafiltration driving speed of the filtering pump) and (V UFP), priming volume value of the drive speed of the sought ultrafiltration pump (V UFP) Of the
As described above, the required time (t) in the blood removal step determined by the calculation means 9 is an allowable time (a time that can be spent in the blood removal step determined by the medical facility in the entire treatment time. For example, blood removal is rapidly performed. Doing so will burden the patient, and taking too much time for blood removal will shorten the blood purification time, which is the original purpose, so medical facilities (such as doctors) will judge from the patient's condition The blood removal method in the actual blood removal step can be arbitrarily changed depending on whether or not it is within the range. That is, as shown in FIG. 3, the distal end of the
Thus, if it is understood from the calculation result by the calculation means 9 that the blood removal process is completed within the allowable time, the first blood removal process described above is performed under the control of the control means 12, When it is determined that the process does not end within the allowable time, another form of blood removal step can be performed under the control of the
More specifically, in the second blood removal step, the water removal pump 8 (ultrafiltration pump) and the
Also, the third blood removal step is different from the first blood removal step and the second blood removal step in that the substituted physiological saline (priming solution) is positively filtered through the blood purification membrane of the
In this third blood removal step, as shown in FIG. 5, almost all of the physiological saline filled in the priming solution filling step is made by having the patient puncture the tip of the vein side blood circuit 2 (vein side puncture needle b). And the tip of the venous blood circuit 2 (venous puncture needle b) was opened without puncturing the patient and filled in the priming solution filling step, as shown in FIG. And a method in which blood removal is terminated after almost all of the physiological saline is discharged from the venous puncture needle b.
However, if it is determined that neither the first blood removal step nor the second blood removal step is preferable, one of the third blood removal steps (any of FIGS. 5 and 6) is performed. Become. In this case, in the third blood removal step shown in FIG. 5, most of the priming solution is introduced into the patient. Alternatively, in the third blood removal step shown in FIG. It will not be introduced.
The display means 10 displays the required time (t) in the blood removal step (first blood removal step) obtained by the calculation means 9, the driving speed (V UFP ) of the ultrafiltration pump, and the
Furthermore, in this embodiment, based on the required time (t) in the blood removal step (first blood removal step) obtained by the calculation means 9, the blood removal method should be the first blood removal step or the second blood removal. A blood removal step is performed by automatically selecting whether to be a step. That is, when the required time (t) in the first blood removal step does not exceed the allowable time, the first blood removal step is automatically performed under the control of the control means 12, while the required time (t) is When the allowable time is exceeded, the second blood removal step is automatically performed under the control of the control means 12.
According to the blood purification apparatus according to the above-described embodiment, the ultrafiltration rate of the blood purification membrane when the physiological saline (priming solution) in the blood flow path of the
In addition, according to the present embodiment, the driving speed (V UFP ) of the water removal pump 8 (ultrafiltration pump), the driving speed (V BP ) of the
Still further, a first blood removal step in which no physiological saline (priming solution) is introduced into the patient, and a second blood removal step in which the blood removal is completed within a predetermined time although physiological saline (priming solution) is somewhat introduced into the patient. In addition, since it was possible to select a blood removal step from the third blood removal step in which a lot of physiological saline (priming solution) was introduced into the patient or no physiological saline (priming solution) was introduced into the patient while taking time to work, A blood removal process more suitable for an individual patient can be selected in advance.
However, in the first blood removal step or the second blood removal step, when the blood circuit side pressure detector or the dialysate side pressure detector detects an excessive negative pressure, the water removal pump 8 (ultrafiltration pump) or the blood pump Preferably, the speed of 4 is temporarily reduced or stopped. Examples of the case where negative pressure is generated on the blood circuit side or dialysate side are as follows.
If there is an abnormality (clogging or breakage) in the puncture needle (arterial puncture needle a, vein puncture needle b) or the blood circuit in the vicinity thereof, negative blood pressure is generated in the blood circuit because blood cannot be normally removed. Further, due to erroneous input by the input means 11, the flow rate due to the driving speed of the water removal pump 8 (ultrafiltration pump) is higher than the flow rate at which physiological saline (priming solution) is filtered from the blood side to the dialysate side. In early cases, negative pressure is generated on the dialysate side.
Thus, excessive negative pressure must be avoided because it not only damages the blood circuit and dialyzer, but also can cause blood to hemolyze (destroy red blood cells). The negative pressure detection level may be set at a level that does not hemolyze the blood, but is generally in the range of −100 to −300 mmHg. Therefore, if excessive negative pressure is detected, the water removal pump 8 (ultrafiltration pump) and the
The installation position of the pressure detector is as shown in P1 to P4 of FIG. P1 and P2 are installed on the arterial drip chamber and the venous drip chamber, respectively, but the installation location is not limited as long as the blood pressure can be detected. However, between the arterial puncture needle a and the
Furthermore, you may comprise so that it may transfer to a dialysis treatment automatically with the completion | finish of a blood removal process in a 1st blood removal process or a 2nd blood removal process. The first blood removal step and the second blood removal step are executed in a state where the artery side puncture needle a and the vein side puncture needle b are connected to the patient. Since the first blood removal step and the second blood removal step are completed in a predetermined amount of liquid feeding and a predetermined time, respectively, it is possible to automatically shift to dialysis treatment.
The
Although the present embodiment has been described above, the present invention is not limited to these embodiments. For example, the
In the present embodiment, the water removal pump 8 functions as an ultrafiltration pump, but a separate ultrafiltration pump may be provided in the middle of the dialysate discharge line Lb.
