Claims (24)
- 呼吸用気体の加圧流を発生する加圧流モジュールと、
加圧流モジュールに連結されかつ患者の気道に呼吸用気体流を伝達する患者回路(56)と、
患者の呼吸パターン特性を監視するセンサ(61,62)と、
センサに接続される制御装置(64)とを備え、
制御装置(64)は、呼吸パターン特性が上限閾値を超えるとき、吸気中の二酸化炭素割合を増加し、呼吸パターン特性が下限閾値未満のとき、自動制御換気を実施し、監視される患者の呼吸パターン特性に基づき、上限閾値と下限閾値とを変更することを特徴とする患者の気道に呼吸用気体流を供給する装置(50)。 - 制御装置は、呼吸パターン特性が下限閾値未満のとき、自動制御換気の実施に加えて、吸気中の二酸化炭素割合を増加させる請求項1に記載の装置。
- (a)再呼吸を実現する、(b)二酸化炭素の補充供給源を設ける、(c)吐出された呼吸用気体を取り込む貯蔵部から二酸化炭素を供給する、(d)(a)〜(c)の何れかの組み合わせにより、制御装置は、吸気中の二酸化炭素割合を増加させる請求項1に記載の装置。
- 送風機(52)、弁(60)又はその両方を加圧流モジュールに設けた請求項1に記載の装置。
- 上限閾値と下限閾値は、同一の単一値である請求項1に記載の装置。
- 呼吸パターン特性は、患者のピーク流量又は患者の周期的換気量である請求項1に記載の装置。
- 制御装置は、呼気圧力に対する吸気圧力の増加比率を供給して、自動制御換気を実施する請求項1に記載の装置。
- 呼吸用気体流を発生する加圧流モジュールと、
加圧流モジュールに連結されかつ患者の気道に呼吸用気体流を搬送する搬送通路となる患者回路(56)と、
患者の呼吸パターン特性を監視するセンサ(61,62)と、
患者回路に接続される二酸化炭素補充装置(46)と、
加圧流モジュールと二酸化炭素補充装置とに作動接続される制御装置(64)とを備え、
制御装置は、センサに接続され、呼吸パターン特性が上限閾値を超えるとき、患者回路内の患者吸気中の二酸化炭素量を増加し、
制御装置は、呼吸パターン特性が下限閾値未満のとき、気体流発生モジュールを制御して、患者の気道に供給される呼気圧力に対する吸気圧力の比率を増加させることを特徴とする患者の気道に呼吸用気体流を供給する装置(10)。 - 送風機(52)、弁(60)又はその両方を加圧流モジュールに設けた請求項8に記載の装置。
- (a)呼気中の二酸化炭素を再呼吸する患者に帰還させる弁(43)、(b)患者回路に連絡される外部の二酸化炭素供給源、(c)呼気される呼吸用気体を捕捉する二酸化炭素貯蔵部(44)又は(d)(a)〜(c)の何れかの組み合わせを二酸化炭素補充装置に設けた請求項8に記載の装置。
- 上限閾値と下限閾値は、同一の単一値である請求項8に記載の装置。
- 特性は、患者のピーク流量又は患者の周期的換気量である請求項8に記載の装置。
- 呼吸パターン特性が下限閾値未満に低下すると、呼気圧力に対する吸気圧力の比率を増大させる請求項8に記載の装置。
- 患者回路(56)を通じて呼吸用気体の供給源から患者の気道に気体流を供給する過程と、
患者の呼吸パターン特性を監視する過程と、
一定時間の経過に伴う呼吸パターン特性の平均値に基づき、呼吸パターン特性の下限閾値を設定する過程と、
呼吸パターン特性の初期値を増加又は減少して、呼吸パターン特性の上限閾値を設定し、呼吸パターン特性の平均値の変化が所定の限界値を超えるとき、上限閾値を低下させ、患者に適用する特定の治療の回数が所定回数を超えるとき、上限閾値を上昇させる過程とを含むことを特徴とする呼吸療法を受ける患者の換気状態を決定する方法。 - 患者に適用される特定の治療は、二酸化炭素の再呼吸である請求項14に記載の方法。
- 呼吸パターン特性の時間経過に伴う平均値を所定の係数だけ減少して、下限閾値を得る請求項14に記載の方法。
- 患者回路を通じて呼吸用気体の供給源から患者の気道に気体流を供給する過程と、
患者の呼吸パターン特性を監視する過程と、
一定時間の経過に伴う呼吸パターン特性の変化が正値でありかつ所定の限界値を超えるとき、高呼吸状態を予測する過程と、
一定時間の経過に伴う呼吸パターン特性の変化が負値でありかつ所定の限界値を超える絶対値を有するとき、低呼吸状態を予測する過程と、
予測された状態に基づき、患者に適切な治療を適用する過程とを含むことを特徴とする患者を治療する方法。 - 呼吸パターン特性は、ピーク流量又は周期的換気量である請求項17に記載の方法。
- 一定期間は、多数の呼吸周期に依存する請求項17に記載の方法。
- 患者回路を通じて呼吸用気体の供給源から患者の気道に気体流を供給する過程と、
患者の呼吸パターン特性を監視する過程と、
患者の呼吸パターンに基づき、呼吸パターン特性の上限閾値を決定する過程と、
患者の呼吸パターンに基づき、呼吸パターン特性の下限閾値を決定する過程と、
呼吸パターン特性値が上限閾値を超えるとき、吸気中の二酸化炭素割合を増加する過程とを含むことを特徴とする患者の気道に加圧呼吸用気体を供給する方法。 - 呼吸の予測状態又は現行状態に基づき、患者に適切な投与量の治療を施す過程と、
所与の投与量に応じて、患者の不安定性指数を測定する過程と、
一連の投与量の各々に対する不安定性指数を示す一連の測定点を記録する過程と、
測定した不安定性指数が許容値を超えるとき、測定点未満の不安定性指数に所与の投与量を適合させ、測定した不安定性指数が許容値未満であるとき、測定点を超える不安定性指数に所与の投与量を適合させる治療窓を設定する過程とを含むことを特徴とする患者の治療の理想的な治療窓を決定する方法。 - (a)最小ピーク流量と最大ピーク流量との比率、(b)最小周期的換気量と最大周期的換気量との比率、(c)移動窓平均ピーク流量又は移動窓中央ピーク流量に適合する接続線の傾斜、(d)移動窓平均周期的換気量若しくは移動窓中央周期的換気量に適合する接続線の傾斜又は(e)患者の呼吸動作系のループ利得の測定値又は予測値を不安定性指数とする請求項21に記載の方法。
- 呼吸の予測状態又は現行状態に基づき、患者に適切な投与量の治療を施す過程と、
所与の投与量に応じて患者の不安定性指数を測定する過程と、
一連の投与量の各々に対する不安定性指数を示す一連の測定点を記録する過程と、
測定した不安定性指数が許容値を超えるとき、測定点未満の不安定性指数に所与の投与量を適合させ、測定した不安定性指数が許容値未満のとき、測定点を超える不安定性指数に所与の投与量を適合させる治療窓を設定する過程と、
治療窓の範囲内で治療の次の投与量を与える過程とを含むことを特徴とする理想的な治療窓の範囲内で患者を治療する方法。 - (a)最小ピーク流量と最大ピーク流量との比率、(b)最小周期的換気量と最大周期的換気量との比率、(c)移動窓平均ピーク流量又は移動窓中央ピーク流量に適合する接続線の勾配、(d)移動窓平均周期的換気量又は移動窓中央周期的換気量に適合する接続線の勾配又は(e)患者の呼吸動作系のループ利得の測定値又は予測値を不安定性指数とする請求項23に記載の方法。
