Abstract
脳アミロイドアンギオパチーは、学習及び記憶に必要とされるタンパク質合成を障害し、及び虚血損傷の閾値を低下させることができる動脈血流の減少を介してアルツハイマー認知症に関与する。アルツハイマー病(AD)に苦しめられている又は発生のリスクを有する対象において上昇した血清応答因子(SRF)又はミオカルディン(MYOCD)活性は、脳動脈の「血管平滑筋細胞」(VSMC)過収縮表現型を促進し、及び血管壁におけるAβの蓄積を増強する。順に、このことはADにおいて普通に見られる脳動脈低灌流及び神経血管脱共役を起こし得る脳動脈での疾患プロセスを開始し得る。それ故、SRF及びMYOCDは、ADにおける認知機能低下に関連する動脈機能障害を治療するための新たな標的となっている。
Description

米国保健福祉省からのNIH助成金AG023084又はAG023993 の条件により規定されているように、米国政府は本発明に特定の権利を有する。
本発明は、病因論に取り組みことによるアルツハイマー病(AD)及びその病変形成に関し、それによりその過収縮(hypercontractile)表現型を寛解させる又は逆行させる。本明細書で使用される生成物及びプロセスが提供される。
一つの目的は、対象の血管系の少なくとも一つの細胞における血清応答因子(SFR)及び/又はミオカルディン(MYOCD)調節遺伝子発現を減少させることにより、アルツハイマー病に付随する過収縮表現型に取り組む(例えば、逆戻りさせる)ことである。SRF/MYOCD調節遺伝子発現の減少は、例えば、アンチセンス阻害、RNA干渉、トランスドミナント干渉、及びSRF−MYOCD転写経路中の遺伝子活性化又は調節の、他の阻害剤のような技術により達成することができる。こうした治療は、細胞中の一つ又はそれより多くの収縮タンパク質の減少した発現、及び/又は血管中の増加した血流も起こすことができる。対象の治療は、自家又は異種(即ち、同種間又は異種間)源からの移植可能な細胞(単数又は複数)を用い、インビボ又はエキソビボで一又はそれ以上の回数で実施することができる。
転写調節因子、血清応答因子(SRF)及びその補助因子ミオカルディン(MYOCD)の過剰発現は、アルツハイマー病様過収縮表現型を引き起こす。それは、少なくとも血管、具体的には脳又は動脈、及びより特別には大脳動脈細胞(例えば、平滑筋細胞)におけるSRF及び/又はMYOCD調節遺伝子発現を妨げることにより寛解させることができる。血管系において、血管拡張を減弱させることができ、及び血管収縮薬への応答を増強することができる。Aβリポタンパククリアランスレセプター(LRP)の増加した(正常又は非病的状態と比較して)活性及び/又はアミロイドβ−ペプチド(Aβ)のクリアランスはそれらにより得ることができる。もしくは、アミロイドアンギオパチーのようなアルツハイマー病(AD)に関連する病的状態、及びその結果生じた血流の減少は、SRF及び/又はMYOCD調節遺伝子発現を妨げることにより寛解させることができる。一つ又はそれより多くの収縮タンパク質の発現を減少させることができ、又はそれにより血管系において血流を増加させることができる。
参加者及び神経病理学的診断
VSMCは、18個体からの小さな皮質軟膜動脈(領域9/10)から、迅速な脳剖検で単離された。AD患者及び同年齢の対照は臨床的に評価し、University of Southern California のAD Research Centers 及びUniversity of Rochester Medical Center, New Yorkでの剖検に従った。AD及び対照個体のCDRスコアは、それぞれ3〜5及び0であった。AD症例はブラーク病期V−VI31であり、及びCERAD32は高頻度から中程度であった。対照はブラーク0又は0〜1、及びCERADは陰性又は低密度であった。臨床的及び神経病理学的特性については表1を参照されたい。血管リスクファクター(例えば、高血圧、アテローム性動脈硬化症その他)の発生率、性比、死因及び死後間隔は、AD及び同年齢対照間で同程度であった。若年対照(平均年齢31.2歳)からのVSMCは、Monroe Medical Examiner Center, Rochesterでの自動車事故後に剖検された、血管リスクファクターを有していない神経学的に正常な若年個体の迅速な脳剖検で単離された。
軟膜動脈VSMCが単離され、以前に記載されているように特徴付けられた51。死後ヒト脳からの軟膜動脈血管を切り裂き、次ぎに15mMヘペス及び抗生物質を含有するダルベッコ変法イーグル培地(DMEM)中の0.1%ディスパーゼ及び0.1%コラゲナーゼで消化した。細かく切られた血管は最初に4℃で2時間保ち、及び次ぎに37℃で1.5時間インキュベートした後に摩砕した。遠心分離により細胞を集め、10%ウシ胎児血清、1mMピルビン酸ナトリウム、0.1mM非必須アミノ酸、100単位/mlペニシリン及び100μg/mlストレプトマイシンを含有するDMEM中で培養した。培養されたVSMCは、血管平滑筋細胞α−アクチン、血管平滑筋ミオシン重鎖及びSM22αを強く発現することが示された。
VSMCを冷リン酸緩衝食塩水で洗浄し、次ぎに「クラック(crack)」緩衝液(50mMトリス−HCl、pH、6.8、100mM DTT、1mMオルトバナジン酸ナトリウム、100μg/ml PMSF、2%SDS、10%グリセロール及び各々1μg/mlのペプスタチンA、ロイペプチン及びアプロチニン)で溶解した。溶解液は、23g針を10回通して剪断し、10分煮沸し、次ぎに14,000gで10分、4℃にて分離した。上清を集め、タンパク質アッセイキット(Pierce)で定量し、完全性及び相対的添加量についてクーマシー染色ポリアクリルアミドゲルで分析した。典型的には、変性10%ポリアクリルアミドゲル(BioRad MiniProtean)に、50〜100μg/レーンのタンパク質を添加し、続いて150Vで1〜2時間電気泳動した。ゲルをニトロセルロースに移し、確立された方法によりイムノブロッティングのために処理した。一次抗血清及びそれらの希釈液は、SRF(1:1000, Santa Cruz, sc-335)、SM−カルポニン (1 :10,000, hCP,Sigma )、平滑筋ミオシン重鎖(SM-MHC, 1:500, Santa Cruz, sc-6956)、SM α−アクチン(1:1000、Sigma A-2547)、SM22α(1:2000, Univ. of Chicago のJulian Solway 博士からの寄贈)、MYOCD(1:2000,Univ. of Texas South-western Antisera Coreからの寄贈)及びβ−チューブリン(1:1000, Pharmingen 556321)を含む。適切な二次抗血清とのインキュベーション後、免疫反応性生成物を化学発光キット(Pierce)で検出する。免疫反応性生成物の相対的レベルはレーザーデンシトメーター(Molecular Dynamics)で測定し、次ぎにβ−チューブリン対照抗体のレベルへの規格化により計算する。
mRNAは、蛍光標識化オリゴヌクレオチドプローブ52を用い、TAQMAN(商標)増幅アッセイ(Applied Biosystems)を使用して定量した。
VSMCを、24ウェルプレート中、10%ウシ胎児血清、1mMピルビン酸ナトリウム、0.1mM非必須アミノ酸、100単位/mlペニシリン及び100μg/mlストレプトマイシンを含有するDMEMに4x104細胞/ウェルで蒔き、50%〜60%コンフルエントになるまでに増殖させた。収縮性活性測定のため、DMEMをO2及びCO2(それぞれ95%及び5%)で通気した生理学的塩(クレブス)溶液で置き換えた。クレブス溶液中で5分インキュベーション後、細胞を75mM KClクレブス溶液に2分暴露して収縮を誘導し、続いてKClフリーのクレブス溶液中でインキュベートした。時間経過研究のためには、VSMCを倒立顕微鏡(Nikon TE2000-S)の試料台上のインキュベーションチャンバー中、37℃に保ち、SimplePCIソフトウェアーパッケージ(Compix)により駆動されたデジタルカメラ(Spot)を使用し、20xの拡大率で画像を捕捉した。細胞長及び領域は、Image-Pro Plusソフトウェアーを使用して異なった時点で決定した。典型的には、KCl刺激の5から10分以内に収縮活性が観察された。カルシウムがVSMC収縮に必要とされるかどうかを決定するため、KClフリー及びKCl含有クレブス溶液の2.5mM CaCl2を除外し、上記のようにアッセイを実施した。アデノウイルス仲介MYOCD遺伝子導入及びSRF遺伝子サイレンシングの効果を評価するため、同じアッセイを使用した。最大細胞短縮(収縮)は、群当たり3〜8の異なったVSMC培養物を用い、培養物当たり100の細胞から三重に決定した。
KCl刺激によるVSMCの細胞内カルシウムレベルを、記載されたように53、カルシウム感受性蛍光色素、Fura−2AM(Teflabs)を使用して画像化した。簡単には、カバースリップ上で培養したVSMCを、4μM Fura−2AMのDMEM溶液と40分インキュベートした。カバースリップを、倒立Nikon Diaphot 300 顕微鏡の試料台に固定した灌流チャンバーへ移し、通常クレブス溶液で15分灌流した後、75mM KClを含むクレブス溶液で刺激した。[Ca2+]は、バージョン4.0ソフトウェアー(T.I.L.L. Photonics)を使用し、デジタル像蛍光顕微鏡法(対物レンズ, Fluor 40/1.3; Nikon)により測定した。蛍光像は、電荷結合素子(CCD)カメラ(T.I.L.L. Photonics)で集めた。較正したデータをプールし、[Ca2+]の平均±s.e.m としてプロットした。
AD患者及び同年齢対照からの脳組織についての免疫組織化学的分析のため、脳表面及び軟膜血管に隣接する前頭皮質(領域9/10)のパラフィン切片(6μm)を使用した。キシレンで洗浄することにより切片からパラフィンを除去し;次ぎに組織切片はエタノールの濃度を連続的に減少させて再水和した。抗原回収は、Retrievagen B(BD PharMingen)で組織を処理することにより実施した。以下の一次抗体を免疫組織化学的分析のために使用した:ヒトSRFに対するモノクローナルマウス抗体(1:500, 0.2 mg/ml, Santa Cruz Biotechnology)、ヒトMYOCDに対するヤギ抗体(1:1000, 0.2 mg/ml, Santa Cruz Biotechnology 及びUniv. of Texas Southwestern Antisera Core からの寄贈);ヒト平滑筋特異的アクチンに対するモノクローナルマウス抗体(1:200, 0.2 mg/ml, Oncogene)、ヒトカルポニンに対するマウス抗体(1:500, 86 /μg/ml, DAKO);ヒトAβに対するポリクローナルウサギ抗体(1:500, 0.66 mg/ml, Biosource)。一次抗体はフルオレセイン又はローダミンコンジュゲート二次抗体で検出された。画像分析は、SPOTデジタルカメラを備えたNikon 蛍光顕微鏡で実施した。微小血管サイズは以下のように決定した:毛細血管から小さな細動脈(<20μm)、中程度の細動脈及び小さな動脈(それぞれ20μm〜40μm及び40μm〜100μm)又はより大きな血管(>100μm)。シグナルの強度はImage-ProPlus を使用して測定した。10の切片からの各領域中の、少なくとも10のランダムに選択された視野を分析した。
簡単には、U6駆動SRF RNAiカセット42又は指示された対照を含有するシャトルベクター(pENTRクラスベクター、Invitrogen)を、LRクロナーゼ(Invitrogen)を使用してpAd/pl−DEST(Invitrogen)と組換えてアデノウイルス構築物を作製した。PacI(New England Biolabs)での直線化に続いて、各アデノウイルス構築物を、リポフェクタミン2000(Invitrogen)を用いて別々にHEK−293A細胞内にトランスフェクトした。細胞溶解が95%を越えて完了したと判断されるまでウイルス産生を進行させ、その時点で上清を集めた。粗ウイルス溶解物は、3回の凍結融解サイクルによりこの上清から調製し、試験して機能を確認した。引き続いて、アデノウイルスを増幅し、次ぎにVirapur からのAdenoMini キットを使用し、製造元の指示に従って精製した。感染単位(IFU)で測定されるウイルス力価は、Adeno-X Rapid Titer キット(BD Clontech)を使用し、製造元の指示に従って決定した。大規模アデノウイルス調製物は、親切にもUniv. of Pittsburgh's National Heart Lung and Blood Institute-funded Vector Core Facility から提供された。収縮タンパク質のウエスタンブロット分析のため、60mmディッシュに蒔かれた2x105のAD VSMCを、100の感染効率(MOI)でのAd.shSRF又はAd.shGFPと、DMEM/2%FBS中、揺らしながら室温で2時間インキュベートした。ウイルス除去後、形質導入されたAD VSMCをDMEMでさらに4日間培養した。インビトロ収縮性アッセイのためには、24ウェルプレートに蒔かれた1x104のAD VSMCに、上記のごとく、100のMOIでAd.shSRF又はAd.shGFPを形質導入した。
アデノウイルス構築は、本質的には記述されているように42実施した。簡単には、CMV駆動ヒトMYOCD(Dr.Michael Parmacek から親切にも寄贈された)又は指示された対照を、LRクロナーゼ(Invitrogen)を使用してpAd/pl−DEST(Invitrogen)と組換えてアデノウイルス構築物を作製した。組換えに先立って、FLAGエピトープをコードする短い配列をインフレームのMYOCDのN末端に挿入した。PacI(New England Biolabs)での直線化、HEK−293A細胞のトランスフェクション、ウイルス産生、粗ウイルス溶解物の調製、アデノウイルスの増幅及び精製は上に記載した。収縮タンパク質プロフィールのウエスタンブロット分析のためには、同年齢対照VSMCを100のMOIでAd.MYOCD又はAd.GFPとインキュベートした。ウイルス除去後、形質導入対照VSMCを48時間培養した。インビトロ収縮性アッセイのためには、24ウェルプレートに蒔かれた1x104の対照VSMCに、上記のごとく、100のMOIでAd.MYOCD又はAd.GFPを形質導入した。
National Institutes of Health ガイドラインに従って承認された研究所プロトコールを使用し、麻酔した(0.5mg/kgケタミン及び5mg/kgキシラジンi.p.)野生型マウスから、結合組織を含まない胸部大動脈を単離し及び除去した。3〜4mmの切片を使用し、10mlのRadnoti器官浴システム及びGrassミオグラフ(Grass-Telefactor Instruments)を使用して収縮及び弛緩を決定した。組織をクレブス溶液に浸し、pH7.4及び37℃±0.5℃にて95%O2及び5%CO2を連続的に通気した。静止張力は0.5gに維持した。フェニレフリンに対する収縮、及び0.25μMフェニレフリンによる前収縮に続くアセチルコリンに対する弛緩についての累積用量応答曲線はAd.MYOCD又はAd.GFPを形質導入した大動脈環で決定した。
胸部大動脈を、上記のように麻酔した3〜4ヶ月齢のC57BI/6Jマウスから単離した。MYOCD遺伝子の形質導入は、エキソビボ動脈調製について記載したように実施した54,55。簡単には、96ウェルプレートを使用し、2つの4mmセグメントを、5xインスリン/トランスフェリン/セレン(Sigma)及びペニシリン/ストレプトマイシン補給ヒト内皮−SFM(Life Technologies)中、Ad.MYOCD又はAdt.GFPの2x108pfuを含有する50μlのウイルス懸濁液と一緒に、95%O2及び5%CO2下、37℃で2時間インキュベートした。100μlの内皮増殖培地(10%ウシ胎児血清、10%Nuserum、30μg/ml内皮細胞増殖サプルメント(Sigma)、5U/mlヘパリン、1mMピルビン酸ナトリウム、1%非必須アミノ酸、1%ビタミン、25mMヘペス、100単位/mlペニシリン及び100μg/mlストレプトマイシンを補給したRPMI1640)を加えた後、インキュベーションを一夜続けた(20〜24時間)。β−ガラクトシダーゼの検出は、記載されているように実施した56。染色後、動脈セグメントをOCTに包埋し、クリオスタットで10μmの薄片とし、4xの拡大率で撮影した。GFP発現は、倒立蛍光顕微鏡(Nikon TE2000-S)で可視化し、及び10xの拡大率で撮影した。ウエスタンブロット分析については、大動脈環を氷冷PBSで2回すすぎ、次ぎに各環を25μlの1xSDS試料緩衝液中で溶解した。マウスモノクローナル抗ヒト抗体(SM-MHC, 1:2,000, Upstate)を有するミオシン重鎖を検出するため、溶解液(レーン当たり10μl)を6%ポリアクリルアミドゲルで分離した。β−チューブリンをタンパク質添加についての内部対照として使用した。
Tg2576APPsw+/−マウス21を18〜22ヶ月齢で使用した。National Institutes of Health ガイドラインに従って承認された研究所プロトコールを使用し、麻酔した(0.5mg/kgケタミン及び5mg/kgキシラジンi.p.)マウスから脳を除去した。SRF及びAβについての免疫染色分析は、ヒトSRFに対するポリクローナルウサギ抗体(1:1000, 0.2mg/ml, Santa Cruz Biotechnology)及びヒトAβ特異的モノクローナル抗体66.1(1:500、SUNY Stonybrook のvan Nostrand 博士から入手した)を使用し、6μm厚パラフィン切片で実施した。
このことは報告されているように実施した48,50。マルチスポットガラススライドを、細胞無しで、500の対照脳VSMCと、500のAD VSMCと、又は500のAd.shSRF又は Ad.shGFPを形質導入したAD VSMCとともに、Cy3標識Aβ42(5μg/スポット)で被覆した。細胞を72時間インキュベートし、残余蛍光Cy3強度を、倒立顕微鏡(Nikon TE2000-S)を使用して決定した。核をヘキスト染色により可視化した。Cy3−Aβ42とのVSMCインキュベーションに先立って、5A6抗体(1:1000; Calbiochem)を使用し、記載されているように23、細胞中のLRPの相対レベルを決定した。
ANOVAが統計学的有意差を決定するために使用された。p<0.05が統計学的に有意であると考えられた。
アルツハイマー動脈症の分子又は細胞基盤はあまり理解されていない。ここでは、脳活性化間に、血流に対して最も大きな抵抗を与え及び脳血流(CBF)調節において主たる役割を果たす小皮質性軟膜及び内大脳動脈に由来する血管平滑筋細胞(VSMC)1,2を分析した。VSMCは、重度の病態を有する8人の後期アルツハイマー病(AD)患者[ブラーク(Braak)−V〜VI31、CERAD(Consortium to Establish a Registry for Alzheimer's Disease プロトコール)−高頻度又は中程度32、臨床認知症評価尺度(CDR)スコア−4、CAA存在、年齢−79歳]、病態がないか又はわずかである(ブラーク−0又は0〜1、CERAD−陰性又は低密度、認知症スコア−0、非CAA、年齢−77歳)5人の神経学的に正常で認知症ではない同年齢対照、及び5人の病態を有しない若年対照(年齢−32歳)から得た。AD及び同年齢対照間には、性、死因、死後間隔(<4時間)及び血管リスクファクターの発生率に相違はなかった(表1)。第一に、対照と比較してADにおいて、VSMC限定タンパク質をコードする遺伝子の下位集団が大量に提示されることがマイクロアレイスクリーニングにおいて注目された(データは示されていない)。いくつかのこうしたマーカーについてのウエスタンブロッティング13は、SMC収縮タンパク質、即ち、SMミオシン重鎖、SM−カルポニン、SMα−アクチン及びSM22αのレベルが、同年齢VSMCと比較してAD VSMCにおいて、それぞれ10、7、2.5及び1.7倍上昇していることを示した(図1A−1B)。同年齢及び若年対照間では、収縮タンパク質の発現に有意な相違はなかった(示されていない)。多数のSMC限定遺伝子が、CArGボックスとして知られている1,216倍縮重シスエレメントに結合する転写因子、SRFにより調節されている13。完全長SRFのレベルは、対照と比較したAD VSMCにおいて23倍高かった(図1Aの上側の矢印、図1Cのアイソフォーム1)。対照的に、SRFの天然ドミナントネガティブアイソフォームをコードする低分子量SRFスプライス変異体33,34は、AD VSMCにおいてほとんど検出不能であったが、対照VSMCにおいては大量に発現されていた(図1Aの下側の矢印、図1Cのアイソフォーム4)。

Claims (17)
- アルツハイマー病の過収縮表現型を治療する方法であって、患者の血管系の少なくとも一つの細胞における血清応答因子(SRF)及び/又はミオカルディン(MYOCD)調節遺伝子発現を減少させることを含んでなる、前記方法。
- アンチセンス阻害が該減少を起こす、請求項1の方法。
- 該アンチセンス阻害が、オリゴヌクレオチド又は該オリゴヌクレオチドを産生する発現構築物、及び生理学的に許容できるビヒクルを含んでなる、パイロジェンフリー組成物により仲介される、請求項2の方法。
- RNA干渉が該減少を起こす、請求項1の方法。
- 該RNA干渉が、siRNA又はsiRNA前駆体を産生する発現構築物、及び生理学的に許容できるビヒクルを含んでなるパイロジェンフリー組成物により仲介される、請求項4の方法。
- SRF及び/又はMYOCDトランスドミナント干渉が該減少を起こす、請求項1の方法。
- 該トランスドミナント干渉が、ドミナントネガティブSRF及び/又はMYOCD突然変異体を産生する発現構築物、及び生理学的に許容できるビヒクルを含んでなるパイロジェンフリー組成物により仲介される、請求項6の方法。
- 一つ又はそれより多くの収縮タンパク質の細胞中の発現が減少する、請求項1〜7のいずれか一項の方法。
- 血流が増加する、請求項1〜7のいずれか一項の方法。
- 治療がインビボで実施される、請求項1〜7のいずれか一項の方法。
- 治療がエキソビボで実施され、そして該細胞が次ぎに移植される、請求項1〜7のいずれか一項の方法。
- 該細胞が平滑筋細胞である、請求項1〜7のいずれか一項の方法。
- 対象におけるアルツハイマー病を診断する方法であって:
(a)該対象から体液又は組織のサンプルを提供すること、
(b)転写、翻訳又はタンパク質活性のレベルでSRF又はMYOCD発現を決定すること、及び
(c)アルツハイマー病の存在又は発生についてのリスクファクターとして増加したSRF又はMYOCD発現を同定すること、
を含んでなる、前記方法。 - 追加のリスクファクターとして、血管過収縮性を同定することをさらに含んでなる、請求項13の方法。
- 追加のリスクファクターとして、減弱された血管拡張又は血管収縮薬への増強された応答を同定することをさらに含んでなる、請求項13又は14の方法。
- 追加のリスクファクターとして、アミロイドアンギオパチー又は減少した血流を同定することをさらに含んでなる、請求項13〜15のいずれか一項の方法。
- アルツハイマー病を治療する医薬の製造のための、平滑筋細胞中のSRF及び/又はMYOCD調節遺伝子発現の阻害剤の使用。
