Abstract
Description
本出願は、米国特許出願第60/734,528号(2005年11月7日出願)、米国特許出願第60/753,754号(2005年12月23日出願)、および米国特許出願第60/773,087号(2006年2月14日出願)に対する優先権を主張し、これら各々の全内容は、本明細書中で参考として援用される。
本発明は、グルコース関連疾患に関する。
1つの局面では、本発明は、被験体、好ましくは、2型糖尿病もしくは本明細書中に記載の他の病気にかかる危険性があるかまたは2型糖尿病もしくは当該病気を患った被験体に治療または予防的治療を行う方法を特徴とする。(本明細書中で用いる治療とは、当該病気または疾患の兆候または望ましくない特徴を低減することを意味する。本明細書中で用いる予防的とは、被験体の疾患を予防すること、例えば、その発症を遅延することを意味する。)上記方法は、被験体に少なくとも1日に2回(好ましくは、1日に2回以下の頻度で)メグリチニド(meglitinide)(例えば、ミチグリニド(mitiglinide)、レパグリニド(repaglinide)、またはナテグリニド(nateglinide)、好ましくは、ミチグリニド)を投与する工程であって、被験体は、少なくとも1回の食事を、メグリチニドを投与せずに摂取する工程と、好ましくは、少なくとも1日1回(例えば、1日1回または1日2回)ビグアニド(例えば、メトホルミン)を投与(好ましくは、経口投与)することにより被験体を治療する工程を含む。
2型糖尿病、前糖尿病および関連代謝性疾患の治療法、予防的治療法および予防法を説明する。また、そのような疾患を治療および予防するための組成物および服用量についても説明する。
本明細書中に開示する化合物および組成物は、2型糖尿病、前糖尿病、または本明細書中に記載の他の症状の治療または予防に用いることができる。
上記方法は、ビグアニド(例えば、有効量のメトホルミン)を、ミチグリニド、レパグリニド、またはナテグリニド等のメグリチニド(例えば、有効量のミチグリニド)と組み合わせて投与する工程を含むようにすることができる。例えば、上記方法を用いて、代謝性疾患、例えば、代謝性症候群(例えば、X症候群)、糖尿病、食後高血糖症等を治療または予防することができる。「代謝性疾患」とは、当業者であれば被験体における少なくとも1つの物質(例えば、糖質または脂質)の代謝作用を変化させる生理学的変化を見つけられるような疾患を指す。
代謝性症候群(例えば、X症候群および症候群関連インスリン耐性)が代謝性危険因子群を呈する患者において現れる。これらの因子は、中心性肥満(腹部内および周辺の脂肪過多な組織)、アテローム発生性脂質異常(動脈壁内におけるプラークの蓄積を助長する血中脂質疾患(主に、高トリグリセリド、低HDLコレステロール));インスリン耐性または耐糖能異常(例えば、身体がインスリンまたは血糖を適切に用いることができない);前血栓状態(prothrombotic state)(例えば、血中の高フィブリノゲンまたはプラスミノゲン活性化因子阻害剤[−1]);血圧上昇(130/85mmHg以上);および前炎症状態(例えば、血中の高感度C反応性タンパク質の増加)を含む。過体重/肥満、運動不足および遺伝因子が上記症候群に関与する可能性がある。代謝性症候群を患った人は、冠状動脈性心臓病、動脈壁内のプラークの蓄積に関連する他の疾患(例えば、脳梗塞、および末梢血管障害)、および2型糖尿病にかかる危険性が高い。代謝性症候群は、インスリン耐性と密接に関連する場合がある。
1つの実施形態では、上記代謝性疾患は、糖尿病(例えば、2型糖尿病)である。例えば、耐糖能異常(IGT)であるいわゆる前糖尿病被験体または糖尿病被験体である患者は、(例えば、血糖値に関しては)正常とみなされることがある。それらの患者は空腹時高血糖症を患っている場合があり、例えば、それ以外の場合には、患者は糖尿病の特徴を有しておらず、空腹時のグルコースレベルが100〜125mg/dLである。糖尿病の例としては、インスリン依存糖尿病および非インスリン依存糖尿病が挙げられる。例えば、上記方法は、糖尿病患者または糖尿病にかかる危険性がある患者に、本明細書中に記載の化合物を組み合わせて投与する工程を含む。患者が耐糖能異常(IGT)または空腹時高血糖症であることで、糖尿病を発症する危険性があると特定できる場合がある。
他の実施形態では、本発明は、本明細書中に記載の治療レジームを用いて、被験体の高血圧症を治療または予防的治療を行う方法、高血圧症にかかる危険性を低減する方法、または高血圧症を予防する方法を含む。高血圧症(または高血圧)とは、動脈(すなわち、拍出運動を行う心臓から体内の組織および器官全てに血液を運ぶ血管)の圧力(張力)が高いことを意味する。正常な血圧は、一般に、120/80未満であり;血圧が120/80〜139/89までは、一般に、「高血圧前症」とみなされ、血圧が140/90以上の場合には、一般に、高血圧とみなされる。最高値である収縮期血圧は、心臓が収縮し血液を動脈内に送出する際の動脈の圧力を表す。最低値である拡張期血圧は、収縮後に心臓が弛緩する際の動脈の圧力を示す。それゆえ、拡張期血圧は、動脈がさらされる最低血圧を反映する。
他の実施形態では、本発明は、本明細書中に記載の治療レジームを用いて、被験体の心筋梗塞の治療または予防的治療を行う方法、心筋梗塞にかかる危険性を低減する方法、または心筋梗塞を予防する方法(例えば、第一次または第二次予防)を含む。いくつかの実施形態では、上記方法は、有効量のメグリチニド(例えば、ミチグリニド、レパグリニド、またはナテグリニド)を被験体に投与する工程を含む。いくつかの好適な実施形態では、上記メグリチニドは、ビグアニド(例えば、メトホルミン)等、他の薬剤と組み合わせて投与される。いくつかの好適な実施形態では、上記方法は、心筋梗塞の第二次予防、例えば、心筋梗塞にかかったことがある被験体をミチグリニドを用いて治療し、さらなる心筋梗塞にかかる危険性を低減することを含む。第二次予防がミチグリニドと他の薬剤(例えば、メトホルミン)を用いた複合療法を含む場合もある。
第3の薬剤(例えば、抗糖尿病薬)を用いて被験体をさらに治療できる場合もある。抗糖尿病薬の例としては、グルコシダーゼ阻害剤、チアゾリジンジオン(例えば、TZD)、インスリン感作物質、グルカゴン様ペプチド−1(GLP−1)、インスリン、PPARα/γ二重アゴニスト、aP2阻害剤、および/またはDPP4阻害剤が挙げられる。
被験体に対する投与量を調節することにより、例えば、各化合物を準最適または無影響用量で用いて治療を開始し、用量を増加していくことによって、メグリチニド(例えば、ミチグリニド、レパグリニド、またはナテグリニド)、またはビグアニド(例えば、メトホルミン)のいずれかの最適用量を判定できるように当該被験体の治療を最適化することができる。各薬物は、マトリックス方式で同時に投与量を調節して、個人に応じた投与法を得ることができる。
Claims (30)
- 被験体の糖尿病の治療または予防的治療を行う方法であって、該方法は、
被験体に少なくとも1日に2回メグリチニドを投与することにより該被験体を治療する工程であって、該被験体は、少なくとも1回の食事を、メグリチニドを投与せずに摂取する、工程を包含し、
少なくとも1日に1回ビグアニドを投与する工程をさらに包含する、方法。 - 前記メグリチニドはミチグリニドである、請求項1に記載の方法。
- 前記メグリチニドはレパグリニドである、請求項1に記載の方法。
- 前記メグリチニドはナテグリニドである、請求項1に記載の方法。
- 前記ビグアニドはメトホルミンである、請求項1に記載の方法。
- 前記ビグアニドは、1日に2回以下の頻度で投与される、請求項1に記載の方法。
- 少なくとも1回の食事とともにメグリチニドを投与するが、1日の全ての食事とともに投与することはない、請求項1に記載の方法。
- 第1の投与を第1の食事とともに行い、第2の投与を食事とは別に行う、請求項7に記載の方法。
- 第1の投与を第1の食事とともに行う工程と;第2の投与を第2の食事とともに行う工程を包含し、前記被験体は、第3の食事を、メグリチニドを投与せずに摂取し、第1、第2および第3という用語は、単に前記食事および投与を特定するために用いられ、食事または投与の順序を示すものではない、請求項7に記載の方法。
- 朝食またはその日の最初の食事とともに投与を行う工程と;最後の食事または夕食とともに投与を行う工程を包含し、前記被験体は、第3の食事または昼食も摂取する、請求項9に記載の方法。
- メグリチニドとビグアニドの組み合わせを、朝食またはその日の第1の食事とともに投与する工程と;メグリチニドとビグアニドの組み合わせを、最後の食事または夕食とともに投与する工程を包含し、前記患者は、第3の食事または昼食も摂取するが、メグリチニドを該第3の食事または昼食と一緒には摂取しない、請求項7に記載の方法。
- 前記メグリチニドはミチグリニドである、請求項11に記載の方法。
- 前記メグリチニドはレパグリニドである、請求項11に記載の方法。
- 前記ビグアニドはメトホルミンである、請求項11に記載の方法。
- 前記被験体に1日に2回以下の頻度でメグリチニドを投与する、請求項1に記載の方法。
- 前記被験体に1日に2回以下の頻度でビグアニドを投与する、請求項1に記載の方法。
- 前記被験体に1日に2回以下の頻度でメグリチニドを投与する、請求項16に記載の方法。
- 少なくとも1回のメグリチニドの投与をビグアニドの投与とともに行う、請求項1に記載の方法。
- 前記メグリチニドの投与およびビグアニドの投与は複合投与剤形で行う、請求項18に記載の方法。
- ミチグリニドおよびビグアニドを含む医薬組成物。
- 前記ビグアニドはメトホルミンである、請求項20に記載の医薬組成物。
- メトホルミン:ミチグリニドの複合塩を含む、請求項21に記載の医薬組成物。
- 被験体に対するメグリチニドの投与量を調節する方法であって、
該メグリチニドを漸増用量で該被験体に投与する工程と、
該用量を増加させながら該メグリチニドに対する該被験体の耐性を評価する工程と、
を包含する、方法。 - 前記メグリチニドはミチグリニドである、請求項23に記載の方法。
- 前記メグリチニドはレパグリニドである、請求項23に記載の方法。
- 前記被験体にビグアニドを投与する工程をさらに包含する、請求項23に記載の方法。
- 前記ビグアニドはメトホルミンである、請求項26に記載の方法。
- 前記患者はメグリチニドの投与を受けたことがない患者ではない、請求項23に記載の方法。
- 前記患者は、第1のメグリチニドから第2の異なるメグリチニドに切り替え中である、請求項28に記載の方法。
