Abstract
Description
(目標とする脚長)=ΔLL x-ray + 調整値
ここで、(ΔLL x-ray)は、先に説明した術前X線写真により初期に取得した下肢長不一致である。この調整値は、初期に取得した下肢長不一致を考慮して、目標の脚長を補正するオペレータが選択する何れかの値である。
(現在のΔLL)=(GTintraop)−(GTpreop)−(目標とする脚長)
ここで(GTintraop)は、再アライメント手順後の大転子点の術中Y値であり、(GTpreop)は、再アライメント手順後の大転子点の術前Y値であり、(目標とする脚長)は、先に計算済みである。現在の脚長不一致は、全脚長として、または値ゼロで脚長が等しいことを表す両脚の相対値として、CASシステム50により表示することができる。
(現在のΔMLO)=(GTintraop)−(GTpreop)
ここで(GTintraop)は、再アライメント手順(ステップ116)後の大転子点の術中X値であり、(GTpreop)は、再アライメント手順後の大転子点の術前X値である。
Claims (19)
- 大腿骨に股関節インプラントを挿入する際にオペレータを誘導するために、コンピュータ支援外科手術における外科的パラメータを測定する方法であって:
i)骨盤の基準系をデジタル化するステップであって、前記骨盤基準系の位置および方向は離隔して追跡可能であり;
ii)前記骨盤基準系の関数として前記大腿骨の第1基準系をデジタル化するステップ;
iii)前記骨盤基準系に対する、前記大腿骨基準系の基準方向を取得するステップ;
iv)インプラント整復開始後、前記骨盤基準系の関数として、前記基準方向に対する前記大腿骨の第2基準系をデジタル化するステップ;を含み、
よって、前記大腿骨の前記第1基準系と前記第2基準系との間の差として、前記大腿骨を前記骨盤に関係付ける外科的パラメータを測定する測定方法。 - 前記基準方向は、前記骨盤基準系の前額面内で、患者の縦軸と所定角度をなす、前記大腿骨基準系の機械的な軸を有する請求項1の方法。
- 前記所定角度が3度である請求項2の方法。
- 前記骨盤基準系の前記前額面は、腸骨稜上の2点と、恥骨結節の内の一つの点とをデジタル化することにより取得される請求項2の方法。
- 前記大腿骨基準系の前記機械的な軸は、前記大腿骨の回転中心を通過し、前記基準方向は、前記大腿骨と臼蓋窩との一致する回転中心を有する請求項2の方法。
- 前記ステップiv)は、前記インプラントの整復後に続いて、臼蓋窩の回転中心をデジタル化することにより、前記骨盤基準系を更新するステップを含む請求項1の方法。
- 前記外科的パラメータは、下肢長不一致および内外側オフセットの内の少なくとも一方である請求項1の方法。
- 前記ステップi)およびii)は、前記骨盤へ固定される単一の追跡可能基準に対して実行される請求項1の方法。
- 前記方法が、骨モデルまたは解剖用死体で実行される請求項1の方法。
- 股関節置換術を実施中、大腿骨へ股関節インプラントを挿入する際に外科的パラメータを測定して、オペレータを誘導するためのCASシステムであって:
骨盤との関係を固定した状態での少なくとも第1追跡可能基準であって、前記第1追跡可能基準は骨盤基準系を形成するように追跡可能であり;
追跡可能なレジストレーションツール;
少なくとも前記第1追跡可能基準および前記レジストレーションツールを追跡するためのセンサ装置;および、
少なくとも前記第1追跡可能基準および前記レジストレーションツールの追跡データを受信するために、前記センサ装置へ接続されるコントローラユニット;を備え、
前記コントローラユニットは:
順次に実行される二つのステップで、少なくとも前記骨盤基準系を追跡するための骨盤追跡可能基準と、大腿骨基準系を生成するための前記レジストレーションツールとの位置および方向を前記追跡データから計算する位置および方向計算器;
前記骨盤基準系と少なくとも前記第1追跡可能基準に関係付けられる前記大腿骨基準系との追跡データを受信して、前記骨盤基準系に対して基準方向となるように前記大腿骨基準系の方向を向けるために、前記位置および方向計算器へ接続されて基準方向の関数として基準調整値を生成する基準方向調整器;および
前記レジストレーションツールから追跡データを受信して、前記基準調整値の関数として外科的パラメータを計算する外科的パラメータ計算器;を有し、前記外科的パラメータは、前記順次に実行される二つのステップで前記基準方向により関連付けられるCASシステム。 - 前記順次に実行される二つのステップの内の第1ステップが術前に実行され、前記順次に実行される二つのステップの内の第2ステップが術中に実行される請求項10のCASシステム。
- 前記基準方向は、前記骨盤基準系の前額面内で、患者の縦軸と所定角度をなす前記大腿骨基準系の機械的な軸を有する請求項10のCASシステム。
- 前記所定角度が3度である請求項12のCASシステム。
- 前記骨盤基準系の前記前額面は、腸骨稜上の2点と、恥骨結節の内の一つの上の点とをデジタル化することにより取得される請求項12のCASシステム。
- 前記大腿骨基準系の前記機械的な軸は、前記大腿骨の回転中心を通過し、前記基準方向は、前記大腿骨回転と、臼蓋窩との一致する回転中心を有する請求項12のCASシステム。
- 前記二つの順次ステップの内の前記第2ステップでは、インプラント整復に続いて、臼蓋窩の回転中心をデジタル化することにより、前記骨盤基準系を更新するステップを含む請求項10のCASシステム。
- 前記外科的パラメータは、下肢長不一致および内外側オフセットの内の少なくとも一方である請求項10のCASシステム。
- 前記骨盤へ固定される追跡可能基準の内の単一のものを備える請求項10のCASシステム。
- 二つの前記追跡可能基準を備え、前記追跡可能基準の内の第2のものは前記大腿骨へ取付けられる請求項10のCASシステム。
