CADEX(商標)知識ベースの重要な適用は、Salus(商標)薬物製剤の上市である。本発明者らは今回、第1のアルブミンに関連した薬物動態をモジュレートする、特別に選択された第2の化合物を共投与することによって、少なくともいくつかの問題のある薬物の治療プロフィールを劇的に改善することができることを実証した。この治療的な薬物置換手法は、正確な薬物相互作用の特定および適切な属性を有する置換用薬剤の慎重な選択に基づく。(このような薬物相互作用は、何年にも及ぶ何百万ドルもかけた原子構造の調査の後、NCP科学者によって以前に独自に決定されている。それらのIB結合性薬物は、本明細書の表1に特定する)。NCP Salus(商標)(Salus:「安全性」、「完全」、「救済」を表すラテン語)薬物製剤の基礎である、特定した薬物の組合せは、以下の利点を提供する。
−正確な結合モジュレーション/置換
−より少ない必要投与量で高い有効性
−FDAによって認可された医薬品を利用している
−多くの毒性のある治療薬では投与必要量を少なくすることによって許容性および安全性

−血清アルブミンへの結合性が高度な多くの新しい医薬品および現存する医薬品の効果を
向上させる
−多くの場合、迅速な市場への参入および承認が可能である
IB結合性治療薬を再調査した後、本発明者らは、薬物動態を改善するために、1)IBに対する高親和性、2)低い毒性および医薬品としての安全な経歴、3)適用の妨げとなりそうにないSalusの治療適応、および4)実現可能、好都合、かつFDA認可された投薬という属性を有する適切なSalus候補を選択した。このような判定基準から、本発明者らは、ターゲット治療薬の薬物動態を改善しそうなSalus候補として脂質低下薬のクロフィブラートを選択する。プロジェクトの初期の歴史は、カンプトテシンファミリーの抗癌剤の化学を使用するものであったので、以下で最初の試験について述べる。
植物由来のアルカロイド化合物であるカンプトテシンは、1960年代に、免疫不全(ヌード)マウス(2、3)をキャリアとする13種を超えるヒト癌異種移植片系統において有効な抗癌活性を有することが発見された。癌細胞に対するその強力な活性の結果として、その発見後直ちに臨床試験に突入した。しかし、異種移植片(4〜6)での腫瘍細胞に対する優れた活性にもかかわらず、ヒトでの非常に失望させる結果のために、すべての試験はすぐに終了した。最近、カンプトテシンが、DNAおよびトポイソメラーゼとの共有結合複合体を形成することによる、トポイソメラーゼIの阻害剤であることが確認された後、カンプトテシンに対する関心は復活した。この部類の化合物は、癌細胞が正常細胞よりもはるかに頻繁に経る過程であるDNA複製が行われている細胞に対するその特異的な毒性のために、魅力的なのである。
治療係数は、半致死量(LD50)対半有効量(ED50)の比であると定義される。最近、Bjorn Ekwall博士および同僚らは、スウェーデンのウプサラを拠点とするIn−Vitro細胞傷害性試験の多施設評価において、動物でないヒト細胞系統試験がin vivo(動物)データよりもヒト毒性を予測し得ることを実証した。動物試験では、ヒト急性毒性がせいぜい約65パーセントしか予測されないが、4種の細胞試験を組み合わせると、物質がヒトにとって危険であることを80パーセントより優れた正確さで判定することができる。

カンプトテシン(CPT)は、様々な動物およびヒト腫瘍の増殖を抑制することがわかっている。カンプトテシンおよびその関連した同族体は、様々な腫瘍系統で過剰発現される核内酵素であるトポイソメラーゼI(topoI)酵素のDNAへの共有結合を安定化するという、独特の作用機序を示す。この薬物/酵素/DNA複合体は、フォーク衝突モデルによれば複製の際に不可逆的で致死的な二本鎖DNAの破損に変換される可逆的な一本鎖切断をもたらす。したがって、その細胞傷害性の機序のためにCPTはS期に特異的であり、DNA合成の最中にある細胞に対してのみ毒性であることが示唆される。癌性細胞のような急速に複製する細胞は、健常な組織と比較してS期により多くの時間を費やす。したがって、topoIの過剰発現は、より速い速度の細胞複製とあいまって、カンプトテシンが、健常な宿主組織よりも癌性細胞の細胞傷害性に選択的に影響を及ぼすことのできる基礎を提供する。カンプトテシンはS期特異性であるため、topoIの最適な阻害には、カンプトテシン薬剤への継続的な曝露が必要であることを留意されたい。
付録1:以下は、細胞傷害性研究のための試験プロトコルである。
薬物の乳癌細胞(MDA−MB−435S)に対する細胞傷害性
最初の段取り:
1.フラスコ中の細胞数をカウントする。各ウェル中の細胞の最終濃度は、5000〜40000細胞/ウェル(100000〜800000細胞/ml)の間にすべきである。10%のFBSおよび0.1%のインスリンを補充したLeibovitzのL−15を使用して、細胞を適切な濃度に希釈する。反復操作用ピペットを使用して、マイクロプレートに100μlの細胞を加える。プレートに加える前に必ず試験管をボルテックスにかける。プレートを37℃で24時間インキュベートする。(注:ウェルA1−H1、A11−H11は、蒸発させる目的の培地しか含まない。ウェルA2は、培地のみで細胞を含まず、実験で使用する。)
培地:LeibovitzのL−15培地。
2.アルブミン:
●HSAを60mg/ml(900mgのHSA+13.3mlの培地)で作る。SigmaHSA A−8763を使用する。
●2mMの301を含む60mg/mlのHSA(5.4mlのHSAに対して2.33mgの301)を5ml作る。
●培地中2mMの301(1.23mlの培地中に0.53mg)を1ml作る。200μlを200μlの培地に希釈して1mMの301を作る。
●DMSO中に薬物ストックを作る。ストックモル濃度=最も高い試験サンプルの1000倍濃度(例.2μMが必要ならば、ストックは2mMにする必要がある。)これは、薬物の投与量に基づき各薬物によって様々である。癌細胞では、最高濃度は、薬物の投与量の1〜2倍にすべきである。
3.試験に先立って(試験する5〜24時間前に)試験濃度を作る:試験濃度は、その濃度の2倍で作り、細胞100μlに100μlを加え、最終濃度をマイクロプレートチャートに示されたものと同じにする。試験濃度を加える前に、(10%のFBSおよび0.1%のインスリンを補充したL−15を使用して)プレート中の培地を交換する。
●100%細胞:100μlの培地
●培地:100μlの培地
●0.5mMの301:培地中に作った1mMの301を100μl加える
●HSAを含む0.5mMの301:30mg/mlのHSAを含む1mM 301を100μl加える
●1mMの301:培地中に作った2mMの301を100μl加える
●HSAを含む1mMの301:30mg/mlのHSAを含む2mMの301を100μl加える
●30mg/mlのHSA:培地中に作った60mg/mlのHSAを100μl加える(2ウェル)
●薬物の投与量:培地を希釈液として使用して1:500希釈物を作る。300μlの培地に加えて2倍の段階希釈物を作る(総体積600μl)。希釈物は必ずウェルに加える前に作る。
●30倍HSAおよび投与量薬物:薬物を用いた上記と同じ段階希釈手順に従うが、希釈液は培地+60mg/mlのHSAとする
●30倍HSA、0.5mMの301、および薬物:薬物を用いた上記と同じ段階希釈手順に従うが、希釈液は、培地+1mMの301を含む60mg/mlのHSAとする
●30倍HSA、1mMの301、および2μMの薬物:薬物を用いた上記と同じ段階希釈手順に従うが、希釈液は、培地+2mMの301を含む60mg/mlのHSAとする。
48時間後:
4.無菌フード下で、XTTバイアルを5mlの培地で再構成する。
5.すべての液体をウェルから除去する。培地で洗浄し、除去し、次いでウェルを200μlの新鮮な培地で満たす。
6.すべてのウェルに40μlのXTTを加える。
7.ウェルを2時間インキュベートする。
8.655nmの読みを基準波長として使用してマイクロプレートリーダーでプレートを読み取る。450nmの読み。450nmの読みから655nmの読みを引き算する。
Claims (28)
- その必要のある患者において急速に分裂する細胞のレベルを低減し、ヒト血清アルブミンのIB部位で結合する薬物の治療有効性を最大にする方法であって、ヒト血清アルブミンのIB部位で前記薬物と競合的に結合する化合物を、患者において前記薬物の治療有効性を最大にするのに有効な量で共投与することを含む方法。
- ヒト血清アルブミンのIB部位で結合する競合的な化合物が、クロフィブラート、クロフィブリン酸、トルメチン、フェノプロフェン、ジフルニサル、エトドラク、ナプロキセン、ナムブトン、イブプロフェン、クロロチアジド、ゲムフィブロジル、ナリジクス酸、メチルドペート、アンピシリン、セファマンドールナフェート、N−(2−ニトロフェニル)−アントラニル酸、N−フェニルアントラニル酸、およびグルコン酸キニジンからなる群から選択される、請求項1に記載の方法。
- 競合的な化合物の患者における血漿濃度が約0.1mM〜25.0mMの範囲となる、請求項1に記載の方法。
- ヒト血清アルブミンのIB部位で結合する競合的な化合物を、静脈内に、腹腔内注射もしくは皮下注射によってまたは経口的に投与する、請求項1に記載の方法。
- ヒト血清アルブミンのIB部位で結合する競合的な化合物を、アルブミン結合性薬物の投与の前、それと同時、またはその後に投与する、請求項1に記載の方法。
- 血流中で、競合的な化合物のヒト血清アルブミンIB部位でのヒト血清アルブミンに対する親和性が、アルブミン結合性薬物よりも高い、請求項1に記載の方法。
- 血流中で、競合的な化合物が、アルブミン結合性薬物のヒト血清アルブミンIB部位での結合をブロックし得る、請求項1に記載の方法。
- 血流中で、競合的な化合物が、ヒト血清アルブミンのIB部位でアルブミン結合性薬物と置換し得る、請求項1に記載の方法。
- IB結合性薬物が、カンプトテシン、10−ヒドロキシカンプトテシン、9−アミノカンプトテシン、トポテカン、およびイリノテカンを含むがこれらに限定されないカンプトテシンファミリーの薬物、ドキソルビシンおよびエピルビシンを含むがこれらに限定されないアントラサイクリンファミリーの薬物、パクリタキセルを含むがこれに限定されないタキソールファミリーの薬物、エトポシドファミリーの薬物、ならびにテニポシドファミリーの薬物からなる群から選択される、請求項1に記載の方法。
- 競合的な化合物が、患者において急速に分裂する細胞のレベルを低減する薬剤である、請求項1に記載の方法。
- 競合的な化合物の投与が、患者において急速に分裂する細胞のレベルを低減する薬物の治療係数を最大にする、請求項1に記載の方法。
- その必要のある患者において急速に分裂する細胞のレベルを低減し、ヒト血清アルブミンのIB部位で結合する薬物の遊離濃度を増大させる方法であって、患者の血流中の前記薬物の遊離濃度を増大させるのに有効な量の、ヒト血清アルブミンIB部位で前記薬物と競合的に結合する化合物の存在下でその薬物を投与することを含む方法。
- ヒト血清アルブミンのIB部位で結合する競合的な化合物が、クロフィブラート、クロフィブリン酸、トルメチン、フェノプロフェン、ジフルニサル、エトドラク、ナプロキセン、ナムブトン、イブプロフェン、クロロチアジド、ゲムフィブロジル、ナリジクス酸、メチルドペート、アンピシリン、セファマンドールナフェート、N−(2−ニトロフェニル)−アントラニル酸、N−フェニルアントラニル酸、およびグルコン酸キニジンからなる群から選択される、請求項12に記載の方法。
- ヒト血清アルブミンのIB部位で結合する競合的な化合物を、静脈内に、腹腔内注射もしくは皮下注射によってまたは経口的に投与する、請求項12に記載の方法。
- 競合的な化合物を、IB結合性薬物の投与の前、それと同時、またはその後に投与する、請求項12に記載の方法。
- IB結合性薬物が、カンプトテシン、10−ヒドロキシカンプトテシン、9−アミノカンプトテシン、トポテカン、およびイリノテカンを含むがこれらに限定されないカンプトテシンファミリーの薬物、ドキソルビシンおよびエピルビシンを含むがこれらに限定されないアントラサイクリンファミリーの薬物、パクリタキセルを含むがこれに限定されないタキソールファミリーの薬物、エトポシドファミリーの薬物、ならびにテニポシドファミリーの薬物からなる群から選択される、請求項12に記載の方法。
- 競合的な薬物が、ヒト血清アルブミン上の他の結合部位に対するアルブミン結合性薬物の親和性を低減する、請求項12に記載の方法。
- 患者において急速に分裂する細胞のレベルを低減し、ヒト血清アルブミンにIB部位で結合する薬物と、必要とする患者の血流中の薬物の遊離濃度を増大させまたはモジュレートするのに有効な量の、ヒト血清アルブミンに競合的に結合する化合物とを含む、その必要のある患者において急速な細胞のレベルを低減する治療用組成物。
- ヒト血清アルブミンのIB部位で結合する競合的な化合物が、クロフィブラート、クロフィブリン酸、トルメチン、フェノプロフェン、ジフルニサル、エトドラク、ナプロキセン、ナムブトン、イブプロフェン、クロロチアジド、ゲムフィブロジル、ナリジクス酸、メチルドペート、アンピシリン、セファマンドールナフェート、N−(2−ニトロフェニル)−アントラニル酸、N−フェニルアントラニル酸、およびグルコン酸キニジンからなる群から選択される、請求項18に記載の組成物。
- IB結合性薬物が、カンプトテシン、10−ヒドロキシカンプトテシン、9−アミノカンプトテシン、トポテカン、およびイリノテカンを含むがこれらに限定されないカンプトテシンファミリーの薬物、ドキソルビシンおよびエピルビシンを含むがこれらに限定されないアントラサイクリンファミリーの薬物、パクリタキセルを含むがこれに限定されないタキソールファミリーの薬物、エトポシドファミリーの薬物、ならびにテニポシドファミリーの薬物からなる群から選択される、請求項18に記載の組成物。
- 薬学的に許容できる媒体、担体、または賦形剤をさらに含む、請求項18に記載の組成物。
- 高血圧を軽減する薬物の治療有効性を最大にする方法であって、ヒト血清アルブミンにIB部位で結合する抗高血圧薬を、ヒト血清アルブミンのIB部位で前記薬物と競合的に結合する化合物と共に、患者において高血圧の軽減を成し遂げるのに有効な量で投与することを含む方法。
- ヒト血清アルブミンのIB部位で競合的に結合する競合的な化合物が、クロフィブラート、クロフィブリン酸、トルメチン、フェノプロフェン、ジフルニサル、エトドラク、ナプロキセン、ナムブトン、イブプロフェン、クロロチアジド、ゲムフィブロジル、ナリジクス酸、メチルドペート、アンピシリン、セファマンドールナフェート、N−(2−ニトロフェニル)−アントラニル酸、N−フェニルアントラニル酸、およびグルコン酸キニジンからなる群から選択される、請求項22に記載の方法。
- 薬物が、プラゾシン、ラミプリル(Ramapril)、キナプリル、テラゾシン、ヒドララジン、メチルドペート、バルサルタン、イルベサルタン、アルプレノロール、クロロチアジド、およびドキサゾシンからなる群から選択される、請求項22に記載の方法。
- 競合的な化合物が、患者において抗高血圧薬の遊離濃度を増大させる、請求項22に記載の方法。
- ヒト血清アルブミンのIB部位に結合する抗感染症薬の治療有効性を最大にする方法であって、前記薬物と共に、ヒト血清アルブミンのIB部位で前記薬物と競合的に結合する化合物を抗感染症薬の有効性を最大にするのに有効な量で投与することを含む方法。
- ヒト血清アルブミンのIB部位で競合的に結合する化合物が、クロフィブラート、クロフィブリン酸、トルメチン、フェノプロフェン、ジフルニサル、エトドラク、ナプロキセン、ナムブトン、イブプロフェン、クロロチアジド、ゲムフィブロジル、ナリジクス酸、メチルドペート、アンピシリン、セファマンドールナフェート、N−(2−ニトロフェニル)−アントラニル酸、N−フェニルアントラニル酸、およびグルコン酸キニジンからなる群から選択される、請求項26に記載の方法。
- 抗感染症薬が、アンピシリン、メタンピシリン(Metampicillin)、スルフイソキサゾール、ナリジクス酸、およびセファマンドールナフェートからなる群から選択される、請求項26に記載の方法。
