Abstract
Description
1969年に、アール・ギルマン(R Guillemin)およびエー・シャリー(A Schally)の率いるグループが独立して、脳下垂体前葉に甲状腺刺激ホルモンを放出させている視床下部物質がL−ピログルタミル−L−ヒスチジル−L−プロリンアミド(pGlu−His−ProNH2)であることを発表した。この生物学的活性に基づいて、このトリペプチドは甲状腺刺激ホルモン放出ホルモン(TRH)として知られている。
時折、臨床医は容易に利用できる治療(たとえば貧血症に関連した疲労のためのエリスロポイエチン)がある、疲労の改善可能な原因を同定することがある。貧血症、甲状腺機能低下症、電解質異常(低マグネシウム血症、低カリウム血症)は、CRFの医学的に改善可能な原因の一部である(モック(Mock)、2001)。CRFのこれらの主な医学的に改善可能な原因は、CRFを経験しているほぼ50%の患者で確認され得る(デメトリら(Demetri et al)、1998)。しかしながら、患者の残りの50%において、包括的なワークアップにもかかわらず病因論が不明のままであるという事実に変わりはない。以下で「特発性CRF」と称する、かかる症例における病因論および/または病態生理学は、おそらくより複雑で、未確認の中枢主導のメカニズムに関連している。
本発明は、患者に、治療的に有効な癌関連疲労を緩和する量のTRHもしくはTRH類似体、またはTRHもしくはTRH類似体の医薬的に許容し得る塩、水和物もしくはプロドラッグを投与する工程を備える、CRFを治療する方法を提供する。より具体的には、好ましい観点において、本発明は、重篤かつ持続的な疲労を呈する患者において、該患者に、疲労を緩和する有効量のTRHもしくは代謝的に安定なTRH類似体、またはTRHもしくはTRH類似体の医薬的に許容し得る塩、水和物もしくはプロドラッグを投与する工程を含む、特発性癌関連疲労を治療する方法を提供する。
患者の選択。本研究において、癌、乳癌または前立腺癌の診断を受けた患者を、腫瘍臨床設定の簡易疲労一覧表(BFI)を用いてCRFに関してスクリーニングする。BFIは単純な9項目の自己記入尺度であり、完了するのに約5〜10分を要する。重篤なレベルの疲労(BFI≧7)を経験している患者を、このスクリーニングに基づいて同定する。これらの患者は、その後、医学的に改善可能なまたは他の治療可能な疲労の原因(すなわち貧血症、甲状腺機能低下症、電解質異常、CRFの症状より優勢な顕著な痛み)を有する患者を同定および除外するために、総合的な面接および臨床検査(ヘモグロビン、ヘマトクリット、甲状腺刺激ホルモン濃度および電解質)によるさらなるスクリーニングを受ける。重篤なCRFを有し、かつ医学的に改善可能なまたは他の治療可能なCRFの原因を有しない合計20人の患者が、初期スクリーニング過程に基づいて同定される。医学的に改善可能なまたは他の治療可能なCRFの原因は、50%もの癌患者および/または癌治療を受けている患者に存在するとみられる。(デメトリら(Demetri et al)、1998)。したがって、特発性CRFを有する20人の患者を同定するために、乳癌または前立腺癌の診断を受けた約40人の患者をスクリーニングする。
初期スクリーニング相に入るための採用基準:
1.全ての研究関連の手順の開始の前に、同意書を提出すること。
3.研究に必要な事項を理解し、これに応じられること。
初期スクリーニング相に登録するための除外基準:
1.DSM−IV基準により定義される、評価前6ヶ月以内の物質濫用または依存症の特定可能な診断を受けた患者(完全な寛解状態にある場合、およびカフェインまたはニコチン依存症を除く)。
4.複数の向精神薬を投薬されている患者。
6.定期的に免疫抑制薬を使用している患者。
8.調査者の見解において、患者の登録が不適切となるか、または研究への患者の参加を妨げる可能性のある任意の他の状態。
以下の全てを、本研究の、第2の総合的な疲労評価相(初期スクリーニングおよび臨床検査に基づく)に入ることから除外する基準とする:
1.貧血症、甲状腺機能低下症および電解質異常などの、CRFの医学的に改善可能な原因を有する患者。
中断のための基準:
患者は、いつでも本研究の治療および評価を中断することができる。患者が本研究を中断するための具体的な理由は、以下のとおりである:
1.患者による自発的な中断(患者は、いつでも自由に本研究への参加を中断することができる)。
3.誤った登録。
4.研究期間中の、任意の状態に起因する患者の入院。
本研究には、二重盲検プラセボ対照クロスオーバーデザインを用いる。4週期間にわたり、1週間間隔で、患者は、ジョンデンプシー病院(John Dempsey Hospital)の一般臨床研究ユニット(General Clinical Research Unit)(GCRC)で評価され、ABBAクロスオーバーデザインに従った無作為な順番で、TRH(第1の活性薬物試行については0.5mg、第2の活性薬物試行については1.5mg)または生理食塩水の静脈内注入を受ける。
0.5mgまたは1.5mgでの甲状腺刺激ホルモン放出ホルモンの投与は、4週間の治療介入相の間の各比較点におけるBFIおよびMFIにより評価した平均疲労スコアのベースラインからの変化により評価されるとおり、癌関連疲労の改善においてプラセボよりも優れている。
安全性と耐容性は、本研究中に採取した以下のデータの統計学的分析および臨床的検討によって評価する:有害事象(Ae’s)、AE’sによる中断の理由、臨床検査数値、ECG、身体検査およびバイタルサイン。
有効性分析に用いた基礎研究対象集団(primary study population)は、最大の解析対象集団(FAS)である。FASは、被験薬物を受け、かつ少なくとも1つの無作為化後の評価を受けた全ての患者を含む。任意の中断例に対応するためにlast observation carried forward(LOCF)分析を行う。有効性の1次解析を共分散分析によって行う。

研究デザイン:
本研究は、標準的な無作為化二重盲検プラセボ対照デザイン形式で行う。これは4相からなる:初期スクリーニング相、1週間続く単純盲検プラセボランイン相、6週間のタルチレリンによる二重盲検プラセボ対照治療相、および1週間の漸減および終了相。
10mgのTRH類似体は、6週間の治療後にBFIおよびMFIにより評価した平均疲労スコアのベースラインからの変化により評価されるとおり、癌関連疲労の改善においてプラセボよりも優れている。

重量部で表示した以下の成分からなる錠剤を調製する。
タルチレリン水和物 10部
ラクトース一水和物 64部
トウモロコシ澱粉 20部
ポリビニルピロリドン(ポリビドン K 30) 5部
ステアリン酸マグネシウム 1部
活性化合物、ラクトース一水和物およびトウモロコシ澱粉を、0.63mmのふるいによって篩別し、キューブブレンダーで10分間混合し、水中のポリビニルピロリドン水溶液(200mlの水に50g)で造粒して乾燥させ、ステアリン酸マグネシウムと共に0.8mmのふるいで大きさをそろえて混合、コルシュ(Korsch)EK 0偏心プレスなどの慣用の錠剤成形機を使用して、直径6mm、平均重量100mgの錠剤にプレスする。
重量部で表示した以下の成分からなる経口投与用液体製剤を調製する。

溶液1リットルあたりタルチレリン水和物10gを含有する溶液体積に十分な水。固形成分を全て水に溶解し、0.23ミクロンの膜を通してろ過してボトルに充填した。得られた溶液の1mlは、10mgのタルチレリンを含んだ。個々の投薬は、患者に、溶液の個々の体積を投与することによって達成することができる。

Claims (19)
- 腫瘍性疾患を患う患者における癌関連疲労の治療のための方法であって、前記患者に、癌関連疲労を緩和する量のTRHもしくはTRH類似体またはその医薬的に許容し得る塩またはその水和物またはそのプロドラッグを投与する工程を備える方法。
- 前記患者が、休息によって軽減されない、重篤で持続的な疲労を患っている請求項1に記載の方法。
- 治療的に有効な癌関連疲労を緩和する量のTRHまたは甲状腺刺激ホルモン放出ホルモンのペプチド模倣類似体を投与する工程を備える、腫瘍性疾患を患い、そのための治療を受けた対象における癌関連疲労の緩和のための方法。
- 前記患者が、手術、放射線治療を受け、かつ/または化学療法剤および/または免疫療法剤で治療されている請求項1に記載の方法。
- 前記対象がHPA軸機能不全および前炎症活性を示す請求項3に記載の方法。
- 約0.01mg/kgから約2mg/kgの前記ペプチド模倣薬が1日1回から3回投与される請求項3に記載の方法。
- 約5mgから約30mgの前記ペプチド模倣薬が1日1回から3回投与される請求項3に記載の方法。
- TRHまたは前記ペプチド模倣薬が、経口的に、経皮的に、吸入により、注射により、経鼻的に、経直腸的に、または徐放製剤により投与される請求項3に記載の方法。
- TRHまたは前記TRH類似体が、前記患者に前記癌関連疲労を緩和する期間投与され、その後の期間は薬物なしのホメオスタシスを得るために漸減用量で治療される請求項3に記載の方法。

- 薬物がTRHまたはTRHのペプチド模倣類似体である請求項11または請求項12に記載の使用方法。
- 手術、放射線治療、化学療法剤による治療および免疫療法剤による治療の少なくとも1つを受けた患者の治療に適した医薬の製造への請求項11から請求項13のいずれか一項に記載の使用方法。
