先進諸国では、人口の高齢化により認知症に罹患している患者の数が増加している。これらの患者の多くは、アルツハイマー病(AD)を患っている。非AD性認知症の中でも、レビー小体型認知症(DLB)は罹患率(prevelance)が高い。DLBとは、皮質および皮質下にレビー小体が様々な度合いで存在することと関連した認知症の臨床病理症候群(びまん性レビー小体病、レビー小体型老年性認知症、さらにアルツハイマー病のレビー小体変異など)を表す。レビー小体型認知症(DLB)と呼ばれるこの機能障害の臨床病理分類(McKeith IG, Galasko D, Kosaka K, et al. レビー小体型認知症(DLB)の臨床病理診断の総合指針(Consensus guidelines for the clinical and pathological diagnosis of Dementia with Lewy bodies (DLB)):DLB国際ワークショップコンソーシアム(Consortium on DLB International Workshop)での報告 Neurology 1996; 47: 1113-24. 2000; 54: 1050-8)では、ADやパーキンソン病(PD)の特徴との共通項も考慮に入れられる。全体として、疾患の臨床データによりびまん性レビー小体病と診断されるが、この疾患についての確定的な診断学的検査はない。神経病理学的検査では、特徴、局在性および疾患に固有の分布により特徴的な変化が示されることから、この神経病理学的検査が、現在のところ、唯一の診断形式である。
最近の研究では、DLBの臨床発生率が年間100,000件に26件であり、80〜84年に及んでは最高率の年間100,000件に68.6件であると示している。この割合は、これまでの研究で年間100,000件に93件確認されたADより低いが、年間100,000件に14件と推定される前頭側頭認知症(FTD)の割合よりも高い。同研究で、文献に記載されているDLBが男性優位であることを検出した。DLBとADを有する男性の割合は63.2%と23.1%であった。この研究は患者の臨床サンプルに関する研究であり、一般集団のものではないことから、この研究ではいくつかの方法論的制限を考慮しなくてはならない。さらに、現行の臨床的判定基準の感度が足りないために実際の割合を過小評価する可能性があるということも考慮しなければならない。一般集団における発生率はわずかに高いかもしれない(S. Lopez-Pousa, J. Garre-Olmo, A. Turon-Estrada, E. Gelada-Batlle, M. Lozano-Gallego, M. Hernandez-Ferrandiz, V. Morante-Munoz, J. Peralta-Rodriguez, M. M. Cruz-Reina. Incidencia clinica de la demencia por cuerpos de Lewy. REV NEUROL 2003; 36 (8): 715-720)。
本発明は、認知症の発症が疑われる患者におけるレビー小体型認知症(DLB)の診断法であって、患者から得られたサンプルを解析して、ユビキチンカルボキシ末端ヒドロラーゼ L1(UCH−L1)遺伝子の発現レベルまたはこの遺伝子によってコードされるタンパク質の酵素活性を決定することを含む診断法に関する。
場合により、検出を、UCH−L1リガーゼ活性による産物、すなわち、二量体または多量体型のポリユビキチンの存在についての直接または間接解析によって実施することができる。
一方で、本発明のさらなる目的は、このような種類の認知症の進行を停止または逆行させるためにUCH−L1レベルを増加させる化合物の能力に基づいて治療薬を同定するための化合物の解析方法を提供することである。化合物は、UCH−L1遺伝子転写の特異的プロモーターまたはそのUCH−L1遺伝子によってコードされるタンパク質の分解の特異的阻害剤であり得る。
以下、好ましい実施形態について記載するが、本発明を限定するものではない。
下表では、本シリーズで研究する症例を、対応する神経病理学的データとともにまとめる:パーキンソン病(PD)、レビー小体型認知症純粋型(DLBp)および通常型(DLBc)、ならびに対照。M:男性、F:女性。NFT:神経原線維変化。ブラーク病期は、Braak および Braak (Braak H, Braak E (1999) Temporal sequence of Alzheimer's disease related pathology. In: Cerebral cortex, vol 14, Neurodegenerative and age-related changes in structure and function of cerebral cortex (Peters A, Morrison JH, eds), pp 475-512. New York, Boston, Dordrecht, London, Moscow: Kluwer Academic/Plenum Publishers.)が記載している、DDLBを伴うアルツハイマー病(AD)の変化を示している。
種々の患者および対照に由来する皮質領域8のサンプルからのmRNAの単離
mRNAを二工程で単離した。TRizol試薬(Life Technologies)を用いて全RNAを得、次に、RNeasy Protect Mini Kit (Qiagen)を用い、トランスファーRNAと5Sおよび5.8SリボソームRNAを含まないmRNAを得た。冷凍ヒト脳組織をそのまま、組織100mg当たり1mlのTRizol中でホモジナイズし、製造業者のプロトコールに従って全RNAを抽出した。精製した全RNAを100μlのRNアーゼフリー水に再懸濁し、RNeasy Protect Mini Kitに示されている明細に軽微な変更を加え(ゲノムDNAはTRizolによる抽出の際に除去されたことから、DNアーゼI処理は行わなかった)、mRNAの精製を行った。各サンプルの濃度を260nmでの吸光度により測定し、RNAの完全性をアガロース−ホルムアルデヒドゲル電気泳動により確認した。
トランスクリプトミクス分析のため、RNAサンプルを、断片サイズおよびサンプル濃度ならびに16Sと28SRNAの間のシグナル比を算出するAgilent 2100 BioAnalyzerを用いてさらに分析した。総てのサンプルをKit MessageAmp RNA (Ambion)で標識した。第一鎖のcDNA合成は、オリゴ−dTプライマーを用いてプライミングした。標識RNAのサンプルを、特に着目する627個の遺伝子を表す(各々はDNA−チップの表面に無作為に分布した5つのオリゴ2組で表される)オリゴDNA−チップで分析した。これらのオリゴは、1つの機器内に総ての機能を組み込んだマイクロアレイプラットフォームである、FEBIT製のGeniom IベンチトップDNA−チップ合成装置を用いて合成した(Baum M, Bielau S, Rittner N, Schmid K, Eggelbusch K, Dahms M, Schlauersbach A, Tahedl H, Beier M, Guimil R, Scheffler M, Hermann C, Funk JM, Wixmerten A, Rebscher H, Honig M, Andreae C, Buchner D, Moschel E, Glathe A, Jager E, Thom M, Greil A, Bestvater F, Obermeier F, Burgmaier J, Thome K, Weichert S, Hein S, Binnewies T, Foitzik V, Muller M, Stahler CF, Stahler PF. Validation of a novel, fully integrated and flexible microarray benchtop facility for gene expression profiling. Nucleic Acids Res 2003;31:e151)。生データをLowessの非線形法を用いてノーマライズした(Workman C, Jensen LJ, Jarmer H, Berka R, Gautier L, Nielser HB, Saxild HH, Nielsen C, Brunak S, Knudsen S. A new non-linear normalization method for reducing variability in DNA microarray experiments. Genome Biol 2002;3:9)。
各10μlの逆転写反応液中、200ngのヒトRNA(水3.85μl中)と、2.5μMのオリゴ−dTプライマー、1×RT TaqManバッファー、5.5mM MgCl2、各500μMのdATP、dTTP、dCTPおよびdGTP、0.08UのRNアーゼ阻害剤ならびに0.31Uの逆転写酵素MultiScribe (Applied Biosystems)とを混合した。プライマー−RNA鋳型の結合を最大とするために25℃で10分間反応を行った後、48℃で30分間インキュベーションし、95℃で5分間逆転写酵素を不活化した。ゲノムDNAの混入がないことを確認するために、逆転写物酵素MultiScribeの不在下で並行反応を行った。TaqManプローブ(Applied Biosystems)は、フォワードPCRプライマーとリバースPCRプライマーの間の鋳型DNA鎖の1つにアニーリングする。TaqManプローブは、Taqポリメラーゼがハイブリダイズする蛍光団を含み、遊離した蛍光団の量が、そのPCR反応において生じた産物の量に比例する。プライマーとヒトUCH−L1およびβ−アクチン遺伝子に特異的なプローブは、Applied Biosystemsからオンデマンドアッセイとして入手した。ヒトβ−アクチンを内部標準として用いた。
UCH−L1タンパク質レベルが低下したかどうかを確認するため、冷凍前頭皮質(100mg)のサンプルをそのまま、1mlの溶解バッファー(20mM Hepes、10mM KCl、1.5mM MgCl2、1mM EDTA、1mM EGTA、1mM DDT、2mM PMSF、1μg/mlアプロチニン、ロイペプチンおよびペプスタチン)中でホモジナイズし、音波処理を施した。溶解液を4℃にて5,000rpmで10分遠心分離し、タンパク質濃度をBCA法(Pierce)によって測定した。50μgの全タンパク質を95℃で3分加熱し、SDS−ポリアクリルアミドゲル上にTris−グリシンバッファーを用いてロードした。mini-protean system (Bio-Rad)を用いたゲル電気泳動によりタンパク質を分離し、Trans-Blot SD Semi-Dry Transfer Cell (Bio-Rad)を用い、40mAで45分、ニトロセルロースメンブレン(Bio-Rad)に移した。ニトロセルロースメンブレンを、5%脱脂乳を含有するTween 20 TBS (TBST)で30分ブロッキングした。その後、メンブレンを、3%BSAを含有するTBSTバッファーで調製した対応する一次抗体とともに4℃で一晩インキュベートした。使用した抗体は、抗UCH−L1(AB5937, Chemicon)、抗UCH−L1(MCA−2084, Serotec)、抗β−アクチン(クローンAC−74, Sigma)、抗19S 1βプロテオソーム(PA1−973, Affinity BioReagents)、抗11Sαプロテオソームアクチベーター(PA1−960, Affinity BioReagents)、抗20SXプロテオソーム(PA1−977, Affinity BioReagents)および抗20SYプロテオソーム(PA1−978, Affinity BioReagents)であった。抗体は1:500希釈で用いた。一次抗体とともにインキュベートした後、メンブレンを室温にて5分、TBSTバッファーで3回洗浄し、その後、対応する二次抗体、すなわち、ペルオキシダーゼで標識されたIgG(Dako)の1:1,000(β−アクチンの場合は1:5,000)希釈液とともに室温で1時間インキュベートした。次に、メンブレンを室温にて5分、TBSTバッファーで4回洗浄し、化学発光ECLウエスタンブロッティングシステム(Amersham/Pharmacia)で現像した後、オートラジオグラフィーフィルム(Hyperfilm ECL, Amersham)に露光した。UCH−L1レベルの定量は、血清または脳脊髄液などの末梢組織で行ってもよい。図4に示されるように、内在UCH−L1レベルはこれらの体液において検出することができる。
ウエスタンブロットバンドの濃度測定的定量は、TotalLab v2.01ソフトウェアを用いて行った。統計分析には、Statgraphics Plus v5ソフトウェアを用いた。
Claims (26)
- 患者から得られたサンプルを解析して、ユビキチンカルボキシ末端ヒドロラーゼ L1(UCH−L1)遺伝子の発現レベルまたはこの遺伝子によってコードされるタンパク質の酵素活性を決定することを含んでなる、認知症の発症が疑われる患者におけるレビー小体型認知症(DLB)の診断法。
- 解析された前記サンプルが、RNAまたはタンパク質である、請求項1に記載の診断法。
- 解析および定量された前記サンプルが、UCH−L1遺伝子によってコードされるタンパク質またはその断片である、請求項1または2に記載の診断法。
- 前記サンプルが、生検またはその他の抽出法によって得られた神経組織の細胞から単離される、請求項1〜3のいずれか一項に記載の診断法。
- 前記サンプルが、生検またはその他の抽出法によって得られた末梢神経内分泌細胞などのその他の組織の細胞から単離される、請求項1〜3のいずれか一項に記載の診断法。
- 前記サンプルが、脳脊髄液、血清、または尿などの体液から単離される、請求項1〜3のいずれか一項に記載の診断法。
- 検出が、UCH−L1転写産物レベルの定量を可能にするcDNAのPCR増幅、SDA増幅、またはその他の増幅法によって実施される、請求項1〜6のいずれか一項に記載の診断法。
- 検出が、いずれかの機構に寄託されているオリゴヌクレオチドを用いて作製されたDNAバイオチップにより実施される、請求項1〜6のいずれか一項に記載の診断法。
- 検出が、フォトリソグラフィーまたはその他の機構によりin situ合成されたオリゴヌクレオチドを用いて作製されたDNAバイオチップにより実施される、請求項1〜6のいずれか一項に記載の診断法。
- 検出が、ウエスタンブロットによる存在するタンパク質の量の解析、UCH−L1に対して特異的な抗体を用いる「タンパク質チップ」、または質量分析によるか、もしくはUCH−L1タンパク質レベルの定量を可能にするその他の機構により得られるタンパク質プロフィールにより、実施される、請求項1〜6のいずれか一項に記載の診断法。
- 検出が、UCH−L1のin vitroまたはin vivoでの酵素活性(ユビキチンのエステルおよびアミドの加水分解活性、ならびにユビキチン−リガーゼ活性を含む)の直接または間接解析により実施される、請求項1〜6のいずれか一項に記載の診断法。
- 解析および定量された前記サンプルが、UCH−L1リガーゼ活性による産物である二量体または多量体型のポリユビキチニンである、請求項11に記載の診断法。
- 解析および定量された前記サンプルが、UCH−L1ヒドロラーゼ活性による産物である遊離型のモノユビキチニンである、請求項11に記載の診断法。
- UCH−L1タンパク質レベルを増大させるレビー小体型認知症の進行を停止または逆行させるための、化合物の使用。
- 前記化合物が、UCH−L1遺伝子転写の特異的プロモーターである、請求項14に記載の使用。
- 前記化合物が、UCH−L1遺伝子によってコードされるタンパク質の分解の特異的阻害剤である、請求項14に記載の使用。
- レビー小体型認知症の進行を停止または逆行させるための、酵素UCH−L1のリガーゼ活性を消失または低減させる化合物の使用。
- 前記化合物が、UCH−L1遺伝子によってコードされるタンパク質のユビキチン−リガーゼ活性の競合的または非競合的な特異的阻害剤である、請求項17に記載の使用。
- 細胞培養物などのようなin vitro系におけるUCH−L1のユビキチン−リガーゼ活性の変化の解析に基づいて、レビー小体型認知症の治療的可能性を有する化合物を解析する方法。
- 前記化合物が、アンチセンスもしくはRNA干渉オリゴヌクレオチド、またはmRNAもしくは、リガーゼ活性は与えるがタンパク質には翻訳されない対立遺伝子によって生じるmRNAの、不安定化および消失に基づく他のもの、などの配列情報に従って作製される、請求項19記載の方法。
- レビー小体型認知症の進行を停止または逆行させるための、UCH−L1酵素の加水分解活性を増加させる化合物の使用。
- 前記化合物が、ウィルス発現ベクターまたは他のタイプの発現ベクターに挿入され、適当なプロモーターの制御下で、UCH−L1酵素に加水分解活性を付与してレビー小体型認知症の進行を停止かまたは逆行させるmRNAの過剰発現または過剰安定化を引き起こす、遺伝子または機能のコード単位等の配列情報に従って作製される、請求項21に記載の化合物の使用。
- 前記化合物が、UCH−L1遺伝子によってコードされるタンパク質の加水分解活性を増加または増進させるのに特異的である、請求項21に記載の使用。
- 細胞培養物などのようなin vitro系におけるUCH−L1の加水分解活性の変化の解析に基づいて、レビー小体型認知症の治療的可能性を有する化合物を解析する方法。
- 前記化合物が、タンパク質と結合する際の親和性または最大率という意味においてその酵素活性を増加させる特異的抗体などについての配列情報に従って作製される、請求項23記載の使用。
- 前記化合物が、アンチセンスもしくはRNA干渉オリゴヌクレオチド、またはmRNAもしくは、正常な加水分解活性を有するその対立遺伝子に対して途中で干渉するかまたは減少させるが、タンパク質には翻訳されない対立遺伝子によって生じるmRNAの、不安定化および消失に基づく他のもの、についての配列情報に従って作製される、請求項22記載の使用。
