Description
本発明は、放射線、トポイソメラーゼII阻害剤及びビス-ジオキシピペラジン(bis-dioxypiperazine)の3つの組み合わせを用いた脳腫瘍の治療、並びに脳腫瘍に罹患し放射線療法を受けている患者の治療方法において使用するための、トポイソメラーゼII阻害剤とビス-ジオキシピペラジンの組み合わせに関する。
最も多頻度の原発部位としての肺(50%)又は乳房(20%)において癌の経過中に脳転移があると診断された癌患者は癌患者の25%である(1-3)。局所的な積極的治療に適していない患者は、依然として対症療法の中心となっている全脳放射線療法(WBRT)を受けている。診断後の生存期間中央値はわずか3〜4ヶ月間であり、約40%の症例において、脳での進行は死への過程である(4-6)。
発明者らは、放射線療法を受けている被験体の脳腫瘍の治療における、デクスラゾキサンと組み合わせたエトポシドの作用を研究している。試験動物の生存期間は、デクスラゾキサン単独又は放射線療法単独のいずれかのみと併せたエトポシドの使用と比較して、この3つの組み合わせによって驚くほど伸びたことが見出されている。
本知見は、トポイソメラーゼII毒及びビス-ジオキシピペラジンの使用が、放射線療法にさらされている環境にこれらが投与される場合により効果的であることを示唆する。
に示す構造を有するビス(3,5-ジオキソピペラジン-l-イル)アルカンである。
通常、ビス-ジオキシピペラジンは体重kg当たり約10〜約100 mg、例えば20〜50 mgの用量で投与することができる。
動物:
Taconic M&B (Ry, Denmark)のB6D2F1雌マウスを、9つのケージの中で、水及びBrogaarden (Gentofte, DK)のAltromi実験食へのアクセスを自由にして飼育した。プロトコルに含めたこれらのマウスの重さは19〜22グラムであった。実験中臨床的に悪化した動物は安楽死させた。この実験はDanish Animal Experimentation Inspectorateの承認を得た。
0日目に、短い時間のCO2麻酔中に、30μLの生理食塩水中の総量15 x 104のEhrlich Ascites腫瘍細胞を右側大脳半球中に移植し、3日目に治療した。治療は以下のとおりにスケジュールした:デクスラゾキサンをt=0分、エトポシドをt=20分に投与し、放射線治療をt=50分に行った。放射線療法の手順の前に、コンプライアンスを達成するため軽い麻酔を投与した。
実験的治療の後に得た生存期間中央値を、全ての症例において最も有効な治療法であった放射線療法後に得た生存期間中央値(MS; median survival)のパーセントとして表した。このようにして、発明者らは簡単で信頼できる比較方法を得た。

個別の実験的治療グループ内の生存期間をまとめて、Kaplan-Meierプロットで示した。放射線療法単独と比較した、実験グループでの生存を比較するLogrank試験からのP値を示す。
エトポシドを用いた腹腔内治療の後、エトポシド投与後150分以内で、総エトポシド曲線下面積(AUC)の約95%を占める(自身の実験、データ公表せず)。In vivoでの効果がin vitroで再現できるか否か試験するため、発明者らはin vivoでの薬物動態をできる限り厳密に模倣した。照射前に、Ehrlich Ascites腫瘍細胞をエトポシド又はデクスラゾキサン中で30分間インキュベートし、その後照射後の薬物インキュベーションをさらに120分間行い、結果として曝露の全持続時間は150分となった。細胞は、軟寒天にプレーティングする前に新鮮な培地の中で2回洗浄した。3週間後に生存コロニーの数(50細胞以上)を測定した。
生存率を、100%=未治療対照となるように正規化し、片対数プロットで示した。
X-放射線を、Stabilipan (Siemens, Germany)により300kV、12 mAで発生した。線量速度は、毎分4.7 Gyであった。
デクスラゾキサン(Zinecard(登録商標))をPharmacia(Kalamazoo, MI)から入手し、さらにRinger-lactate(乳酸リンゲル液)中で希釈した。エトポシド(Vepesid(商標))を、投与準備済みの溶液としてPharmacia A/S(Copenhagen, DK)から入手し、等張な生理食塩水中で最終濃度まで希釈した。3H-エトポシドをMoravek(CA,USA)から入手し、-20℃に保持した。薬物は全て腹腔内(ip.)経路により投与した。
hypnorm/dormicumによる麻酔の後に、25μLのエバンスブルー色素又は80μLのリサミングリーンを尾静脈経由で投与した。色素を循環させた後、動物を、4%のホルマリン溶液で5分間経心的に灌流させた。脳を4%のホルマリン中に保持し、200μMの厚さの切片をビブラトーム上で切り取り、染色を視覚的に評価した。動物を脳及び腫瘍の染色の評価に含める前に臓器のポジティブ染色の確認を行った。エバンスブルー色素とリサミングリーンをSigma-Aldrich (Vallensbaek, DK)から入手し、等張な生理食塩水中で2%溶液としてフレッシュな色素溶液を調製した。
トリチウム標識したエトポシドを、「非放射性の(cold)」エトポシド 9.0 mg/ml、「非放射性の」エトポシド 0.9 mg/ml又は等張な生理食塩水のいずれかと混合し、最終量を全て200μLとした。投与の10分後、マウスをCO2で麻酔し、血液を採取して血漿を分離した。さらに、治効実験と同様に、1つのグループのマウスをデクスラゾキサン125 mg/kgで前処理し、その後、続くエトポシド分布に及ぼす影響を記載のとおりに評価した。
用量設定及び実現可能性調査:
初期実験で、脳腫瘍に罹患しているマウスを5 Gy〜20 Gyの照射量の単発の放射線療法単独により治療した。最長の生存期間中央値は10 Gyの照射後に得られた。生存期間は15 Gyの照射後変化せず、20 Gy照射後は、10 Gyの照射後に得られた生存期間と比較して生存期間が事実上減少することが観察されたので、腫瘍を抑制するよりも毒性を増した。
全ての実験において、放射線療法単独を施した後の生存期間は化学療法単独を施した後の生存期間よりも優れていたため、方法に記載の通り、他の治療法の組合わせと比較する「標準生存期間」として用いた。同時放射線療法を伴うエトポシド90 mg/kg及びデクスラゾキサン125 mg/kgの併用後の生存期間は、化学療法単独及び放射線療法単独と比較して著しく増加した(式1)(図1A)。
治効実験を目的として、記載通りの前処理を行い、3日目にエバンスブルー色素、リサミングリーン及び3H-エトポシドを用いた実験を行った。
脳及び腫瘍の3H-エトポシド取り込みの評価:
エトポシドの投与量の増加とデクスラゾキサンを用いた前処理のいずれも脳/血漿の比率(分散分析(ANOVA): p=0.37)を変化させなかった(表2)。
増加濃度のエトポシドを1.25 Gy又は7.5 Gyと組合わせたところ、発散する生存曲線から、7.5 Gyとの組合せで相乗的殺細胞が得られたことが示された(図2A)。
脳転移(BM)の診断はほぼ常に致死的であり、約40%の患者は脳における進行の直接の結果として死亡する。臨床成績を改善するためにWBRTの投与の種々の用量及びスケジュールが研究されている。しかし、20、30及び40 Gyを2〜4週間にわたって照射した後の生存率は、20 Gyを1週間照射した後の生存率より良くはなく(6)、10 Gyの単発照射及び6 Gy x 2回照射の後の反応持続時間は20 Gyを1週間照射した後より少し短いが、2つの治療法は、その他の点では同程度であった(21)。腫瘍領域に対するフィールドを30 Gyからそれ以上へ縮小することを伴う、脳転移を有する153人の患者における70,4 Gyまでの線量増加放射線療法の試みは、脳での進行による死亡に影響を及ぼさなかった(22)。

- 治療を必要としている被験体に、有効量のトポイソメラーゼII毒をビス-ジオキシピペラジンと組み合わせて投与し、放射線を用いて該被験体をさらに治療することを含む、腫瘍細胞の治療方法。
- トポイソメラーゼII毒がエトポシドである、請求項1に記載の方法。
- ビス-ジオキシピペラジンがデクスラゾキサンである、請求項1又は2に記載の方法。
- 腫瘍がCNS腫瘍である、請求項1、2又は3のいずれか1項に記載の方法。
- 放射線治療を受けている患者の腫瘍治療における同時使用、個別使用又は連続使用のための複合製剤としての、トポイソメラーゼII毒及びビス-ジオキシピペラジン。
