Abstract
Description
本発明は、心臓弁の修復を促進しかつ簡単にするためのデバイスおよび方法に関する。
ヒトの心臓は、血液循環において血流の方向を制御する4つの弁を有する。大動脈弁および僧帽弁は、「左」の心臓の一部であり、肺から体への酸素豊富血液の流れを制御するが、肺動脈弁および三尖弁は、「右」の心臓の一部であり、体から肺への酸素欠乏血液の流れを制御する。大動脈弁および肺動脈弁は、拍出チャンバー(心室)と主要な動脈との間に位置し、血液が血液循環へと放出された後に心室へ再度漏出するのを防ぐ。僧帽弁および三尖弁は、受容チャンバー(心房)と心室との間に位置し、血液が放出の間に心房へ再度漏出するのを防ぐ。
本発明は、心臓弁を修復するためのデバイスおよび本デバイスを使用する方法を含む。本発明は、2型弁機能不全(弁尖逸脱)を有する逆流性の僧帽弁および三尖弁を修復するのに特に適している。

Claims (42)
- 少なくとも一つの弁尖が逸脱区を有する二つまたはそれ以上の弁尖および弁下構造体を有する逆流性心臓弁を修復するための移植可能なデバイスであって、以下の構造体を含むデバイス:
逸脱弁尖への接着のための構造体であって、該構造体が収縮期の心臓の収縮の間に反対側の弁尖が接合する接合表面を定義し、それによって弁尖間の接合が正常化される構造体。 - 構造体が剛性または半剛性である、請求項1記載のデバイス。
- 構造体が柔軟性のある、請求項1記載のデバイス。
- 構造体が弾力性のある、請求項1記載のデバイス。
- 構造体が逸脱弁尖への貼り付けのために構成された近位端を有する、請求項1記載のデバイス。
- 近位端が二分岐形態を有し、デバイスが手術的に逸脱弁尖に貼り付けられる際に、逸脱弁尖の自由縁がそこに配置される、請求項5記載のデバイス。
- 構造体が弁内に手術的に移植される際に、逸脱弁尖の自由縁を超えて自由に伸張する遠位端を有する、請求項1記載のデバイス。
- 構造体が弁下構造体への貼り付けのために構成された遠位端を有する、請求項1記載のデバイス。
- 構造体が実質的に平面である、請求項1記載のデバイス。
- 構造体が、曲がっているかまたは湾曲している、請求項1記載のデバイス。
- 曲がった構造体が約75°から180°未満の範囲の角度を定義する、請求項10記載のデバイス。
- 接合表面が正常に機能する弁尖を実質的に模倣するように構成されている、請求項1記載のデバイス。
- 表面が、少なくとも約25mm2の領域を定義する、請求項1記載のデバイス。
- 構造体が約2mmから約10mmの範囲の厚さを有する、請求項1記載のデバイス。
- 構造体が約5mmから約40mmの範囲の長さを有する、請求項1記載のデバイス。
- 逸脱弁尖がうねる区域も有し、かつ表面がうねる区域を不動化するのに十分な領域を有する、請求項1記載のデバイス。
- 弁が収縮期に逸脱弁尖と反対側の弁尖との間に間隙を引き起こす拡張弁輪も有し、かつ構造体の一部が、間隙を埋めるのに十分な長さを有する、請求項1記載のデバイス。
- 少なくとも一つの弁尖が逸脱区を有する二つまたはそれ以上の弁尖および弁下構造体を有する逆流性心臓弁を修復するためのシステムであって、以下を含むシステム:
逸脱弁尖への接着のために構成された構造体であって、該構造体が収縮期の心臓の収縮の間に反対側の弁尖が接合する接合表面を定義し、ここで弁尖間の接合が正常化される構造体;
該構造体を逸脱弁尖に接着するための固定法。 - 固定法が、縫合、止め金、クリップ、ファスナー、および接着剤からなる群より選択される、請求項18記載のシステム。
- 少なくとも一つの弁尖が少なくとも一つの逸脱区を有する二つまたはそれ以上の弁尖および弁下構造体を有する逆流性心臓弁を修復するための方法であって、以下の段階を含む方法:
逸脱弁尖への接着のための構造体を提供する段階であって、ここで該構造体が弁尖接合表面を定義する段階;および
該構造体を逆流性心臓弁に移植し、ここで移植に際して該構造体の少なくとも一部が二つまたはそれ以上の弁尖の間を伸張する段階であって、ここで収縮期の心臓の収縮の間に該逸脱弁尖の反対側の弁尖が該構造体の該接合表面と接合し、それによって二つまたはそれ以上の弁尖の間の接合が正常化される段階。 - 移植が構造体を逸脱弁尖でのみ弁に貼り付ける段階を含む、請求項20記載の方法。
- 構造体が逸脱弁尖の上面に貼り付けられ、それによって該上面の少なくとも一部を覆う、請求項21記載の方法。
- 構造体が逸脱弁尖の下側に貼り付けられる、請求項21記載の方法。

- 第一の構造体が第一の逸脱弁尖に貼り付けられ、第二の構造体が第二の逸脱弁尖に貼り付けられる、請求項31記載の方法。
- 移植が経皮的に施される、請求項20記載の方法。
- 経皮的な移植が構造体を修復されるべき弁まで送達するためにカテーテルを使用する段階を含む、請求項33記載の方法。
- カテーテルを介した送達のために構造体を圧縮する段階をさらに含む、請求項34記載の方法。
- 修復されるべき弁までの送達に際して構造体を膨張させる段階をさらに含む、請求項35記載の方法。
- 心臓弁が僧帽弁である、請求項20記載の方法。

- 弁が収縮期の間に逸脱弁尖と反対側の弁尖との間に間隙を引き起こす拡張弁輪も有し、かつ該弁尖の間を伸張する移植された構造体の一部が、間隙を埋める、請求項20記載の方法。
- 構造体が逸脱区の少なくとも約50%と接触する、請求項20記載の方法。
- 少なくとも一つの弁尖が少なくとも一つの逸脱区を有する二つまたはそれ以上の弁尖および弁下構造体を有する逆流性心臓弁を修復するための方法であって、以下の段階を含む方法:
逸脱弁尖への接着のための構造体を提供する段階であって、ここで該構造体が弁尖接合表面および下面を定義する段階;
該構造体を逸脱弁尖に貼り付ける段階であって、ここで該構造体の下面が逸脱区を覆う段階;および
