Abstract

本発明は、股関節プロテーゼであって、この股関節プロテーゼにはセラミックから成る内側のスライドシェル（１）が設けられており、このスライドシェル（１）の外面がプラスチックカバー（２）により取り囲まれており、股関節プロテーゼが、外側の金属シェル（６）内へ挿入するため又は骨セメントにより直接にインプラントするために設けられており、この場合に内側のスライドシェル（１）内でボールヘッドが関節をなしており、このボールヘッドがシャフトに配置されており、このシャフトが大腿骨内に固定可能になっている。傾動及び転倒に対する強さを高めるためには、本発明によれば、スライドシェル（１）が外面に構造化部を有している。

Description

本発明は、請求項１の上位概念部に記載の股関節プロテーゼに関する。
人工股関節は、一般にスライドシェルから成っており、このスライドシェルは、直接に、又はプラスチックカバーを介して外側の金属シェル内に挿入されている。この金属シェルは骨盤骨内にインプラントされる。スライドシェルとプラスチックカバーとの組合せはサンドイッチインサートとも呼ばれる。
このためには大腿骨内にはシャフトがインプラントされ、このシャフトにはボールヘッドが配置されており、このボールヘッドはスライドシェル内で関節をなす。
人工股関節では、股関節臼へのボールヘッドシャフトの衝突が繰り返し起こり得る。衝突力が十分に大きい場合には、このことは機械的な人工股関節結合部の分解につながりかねない。特にこの場合にはサンドイッチ関節臼システムが危険にさらされている。なぜならば、主に使用されるＰＥ（ポリエチレン）は前記衝撃力に対して不十分な抵抗しか提供することができないからである。
サンドイッチインサートは種々異なった形式で製造される。
１つのシステムの場合には、セラミックのスライドシェル若しくはインサートがプラスチックにより押出被覆され、この場合にはスライドシェルには凹部が配置されている。この場合には加熱により生じる劣悪なポリエチレン（ＰＥ）特性が欠点である。さらにセラミックのスライドシェルには熱衝撃が起こる。成形型及び熱い部材の処理による押出成形手間と並んで、大きい所要構成スペースが欠点である。
択一的なシステムでは、複合構成部材の強度が部分的にわずかな場合に、スライドシェルが円錐状のクランプによりプラスチックカバー内に固定される。この場合にも大きい所要構成スペースが欠点である。
有利にはプラスチックカバー内へのスライドシェルの熱間圧入（warme Einpressung）も用いられる。しかしながら、この場合に複合構成部材の部分的にわずかすぎる強度が生じる。さらに圧縮結合に基づく狭い許容公差にも注意すべきである。
そこで本発明の課題は、請求項１の上位概念部に記載の股関節プロテーゼを改良して、所要構成スペースが小さい場合に高い傾動に対する強さが得られているようにすることである。
本発明によれば、この課題は、スライドシェルが外面に構造化部を有しており、これにより、所要構成スペースが最小限の場合に傾動又は転倒に対する強さが著しく高められていることにより解決される。なぜならば、前記構造化部は所要構成スペース拡大をほとんど必要としないからである。このような股関節プロテーゼはポリロックインサートとも呼ばれる。
前記構造化部は、有利には溝底部に大きい半径を設けられており、これにより、場合によっては生じる溝応力が最小化される。構造化の次のような可能性、すなわち、
・波形の凹部、又は
・半円形の凹部、
が考えられる。
この場合に、高い構成部材安定性を得るためには底部の溝半径は０．５ｍｍよりも大きいことが望ましい。
波形の凹部は、有利にはスライドシェルの外面に環状に配置されている。
スライドシェルは、有利には外面に球面状の、又は段付けされた構成形式を有している。球面状の構成形式は極めて小さい所要構成スペースを有している。
有利な実施態様では、プラスチックカバーはスライドシェルの開かれた端部を包囲把持しており、この場合に、有利にはスライドシェルの上面に位置する、プラスチックカバーのカラーは上縁のほぼ半分を覆っている。
サンドイッチインサートは、有利にはプラスチックカバー内にスライドシェルを圧入することにより製造される。
有利な実施態様では、スライドシェルの内側形状は、スライドシェルの外側形状に対して偏心的に配置されている。
同心率（偏心率）に関する偏差は、有利には少なくとも０．００１ｍｍである。
本発明のさらなる特徴を図面につき以下に説明する。
図１は、球面状の構成形式の、セラミックから成るスライドシェル１を有するサンドイッチインサート１を示している。製造時には、このスライドシェル１はプラスチックカバー２内へ圧入される。このプラスチックカバー２は、有利にはポリエチレン（ＰＥ）から成っている。このサンドイッチインサート１は外側の金属シェル６内へ挿入されている。
図２は、スライドシェル１のが外面に段付けされた構成形式による択一的な実施態様を示している。
プラスチックカバー２の開かれた端部がスライドシェル１を包囲把持しており、これにより、固定が改良されている（図１参照）。スライドシェル１の上面に位置するカラー５が、上縁のほぼ半分を覆っている。
図１及び図２に示した両方のサンドイッチインサート１は、スライドシェル１の外面に本発明による構造化部を示していない。
本発明の有利な実施態様では、スライドシェル１の内側形状１０が、スライドシェル１の外側形状１１に対して偏心的に配置されており、この場合に、同心率（偏心率）に関する偏差は、有利には少なくとも０．００１ｍｍである。
図３は、プラスチックカバー２内へ挿入されたサンドイッチインサート、すなわちセラミックのスライドシェル１を示している。このスライドシェル１の外面には構造化部が配置されており、この構造化部は横断面図で見て波形の凹部８から成っている。この場合、前記構造化部には溝底部に大きい半径が設けられており、この半径が０．５ｍｍよりも大きくなければならないことが重要である。溝底部のこれらの半径ははじめてプラスチックカバー２内のスライドシェル１の良好な傾動及び転倒に対する強さをもたらす。
波形の凹部８は、スライドシェル１の外面に環状に配置されている。
図４は、半円形の凹部９から成る構造化部を示している。ここでも溝底部には０．５ｍｍよりも大きい溝半径を有する凹部が設けられている。
図１から図４までに示したサンドイッチインサート１は、ポリロック（Ｐｏｌｙｌｏｃｋ）・インサートとも呼ばれ、例えば外側の金属シェル６内へ挿入され、この金属シェル６は次いで骨盤骨内にインプラントされる。択一的には、このインサートは骨セメントにより骨盤骨内に直接にインプラントすることもできる。
スライドシェル１内には、ここには図示していない、有利にはセラミックから成るボールヘッドが関節をなしている。このボールヘッドは、大腿骨内にインプラントされたシャフトに固定されている。

Claims (12)

  1. 股関節プロテーゼであって、該股関節プロテーゼにセラミックから成る内側のスライドシェル（１）が設けられており、該スライドシェル（１）の外面がプラスチックカバー（２）により取り囲まれており、前記股関節プロテーゼが、外側の金属シェル（６）内に挿入するか、又は骨セメントにより直接にインプラントするために設けられており、内側のスライドシェル（１）内でボールヘッドが関節をなしており、該ボールヘッドが、シャフトに配置されており、該シャフトが、大腿骨内に固定可能になっている形式のものにおいて、スライドシェル（１）が、外面に構造化部を有していることを特徴とする、股関節プロテーゼ。
  2. 構造化部が、溝底部に大きい直径を備えている、請求項１記載の股関節プロテーゼ。
  3. 溝底部の溝半径が、０．５ｍｍよりも大きい、請求項２記載の股関節プロテーゼ。
  4. 構造化部が、横断面図で見て波形の凹部（８）から成っている、請求項１から３までのいずれか１項記載の股関節プロテーゼ。
  5. 横断面図で見て波形の凹部（８）が、スライドシェル（１）の外面に環状に配置されている、請求項４記載の股関節プロテーゼ。
  6. 構造化部が、半円形の凹部（９）から成っている、請求項１から３までのいずれが１項記載の股関節プロテーゼ。
  7. スライドシェル（１）が、外面に、球面状の、又は段付けされた構成形式を有している、請求項１から６までのいずれか１項記載の装置。
  8. プラスチックカバー（２）が、スライドシェル（１）の開かれた端部を包囲把持している、請求項１から７までのいずれか１項記載の装置。
  9. スライドシェル（１）の上面に位置する、プラスチックカバー（２）のカラー（５）が、上縁のほぼ半分を覆っている、請求項８記載の股関節プロテーゼ。
  10. スライドシェル（１）が、プラスチックカバー（２）内への圧入により該プラスチックカバー（２）に結合されている、請求項１から９までのいずれか１項記載の股関節プロテーゼ。
  11. スライドシェル（１）の内側形状（１０）が、スライドシェル（１）の外側形状（１１）に対して偏心的に配置されている、請求項１から１０までのいずれか１項記載の股関節プロテーゼ。
  12. 同心率（偏心率）に関する偏差が、少なくとも０．００１ｍｍである、請求項１１記載の股関節プロテーゼ。
