Description

【発明の詳細な説明】 （産業上の利用分野） 本発明は人工股関節の臼蓋ソケット（外殻）に関する。
（従来の技術） 人工股関節は第１図の如く寛骨Ｈと大腿骨Ｆとの間の股
関節を人工部材によって置換修復するものであって、寛
骨Ｈ側に凹設したソケット受凹所Ｒに臼蓋ソケットＡを
嵌合着座させると共に大腿骨Ｔの髄腔内に植立したステ
ムＳの先端のステムヘッドＳＨを上記臼蓋ソケットＡ内
に高密度ポリエチレン製受座Ｃを介して転勤自在に嵌合
してなる。臼蓋ソケットＡ、ステムヘッドＳＨ及びステ
ムＳは高強度にして生体為害性のないチタン金属、チタ
ン合金もしくはＣｏ−Ｃ’ｒ合金などのメタル材もしく
はアルミナセラミック、ジルコニアセラミックなどのセ
ラミック材よりなるが．臼蓋ソケットＡは大腿骨Ｆのソ
ケット受凹所Ｒ内に嵌合固定されてステムヘッドＳＨを
転勤自在に支承するものであり、生体と人工部材の界面
にあって大腿骨Ｔの動荷重を受けとめる重要な役割を果
す。ところで従来の臼蓋ソケットＡは第６図に示すよう
に倒椀状殻もしくは半球状殻をなして凹所Ｒ内に嵌納さ
れ止ねじＢ・・・によって寛骨Ｈにねじ着されるもので
あるが、次のような問題点を含んでいた。
（発明が解決しようとする課題） （ｉ）第７図に示す如く正常な寛骨Ｈの表面の皮質骨Ｈ
１は硬質層でその内側の海綿骨Ｈ２は軟質層であり前者
は外力の支承体となり得るが後者はその役に耐えない。
しかるに従来のものに於ては臼蓋ソケットＡの開口周縁
ａｌ（第２図参照）は半球面上にあるため第６図の如く
ソケットＡの主要範囲は海１綿骨Ｈ２と圧接しており且
つ開口端縁ａ１は凹所Ｒの開口周縁に相当する皮質骨Ｈ
１１とは何等の接触関係を持たないのでソケットＡを皮
質骨によって剛直的に受は止める範域は十分とは云えな
い。
（ｉｊ　）人工股関節を着用して経時すると股関節運動
の累積と、この際に生ずる摩耗粉とによってＥｌｌｌ蓋
ソケットＡは寛骨Ｈの内側に次第に陥入移行して所謂マ
イグレーション（ｍｉｇｒａｔｉｏｎ）を起して正常な
股関節機能を阻害したり、この部分に応力集中が起きて
二次的な疾患を誘発する。この原因の一つとして（ｉ）
で述べたような開口周縁ａ１と皮質骨Ｈ１１との構造的
か＼わりの不在なことが上げられる。
本発明は以上の問題点を解決することを目的としている
。
（課題を解決するための手段） 以下に望ましい実施例を第２図、第３図によって本発明
の詳細な説明すると、臼蓋ソケットＡは全体としてほゞ
倒椀状殻もしくは半球状殻をなしてその開口周縁ａ１に
は周方向に間隔おきに外方張出縁ａ２を備えると共に外
方張出縁ａ２はその表面に縦横凹溝ａ３を備えて成る。
望ましい実施例に於いて外方張出縁ａ２は平面円弧状を
なすと共に下向漸増山状とされ外方張出縁ａ２が周縁ａ
１の円周方向につき約１２０″′の間隔で設けられてい
る。なお、符号ａ４は止ねじ取付孔を示す。
（作用） 以上のような構成よりなる本臼蓋ソケットＡの使用要領
と作用とにつき以下に述べる。臼蓋ソケッｌ−Ａを寛骨
Ｈに取付ける要領は従来例と変りなく、第５図に示すよ
うに凹所Ｒ内にソケットＡを嵌納し止ねじＢ・・・にて
ねじ着する。この場合本発明臼蓋ソケットＡには開口周
縁ａ１には周方向間隔おきの外方張出縁ａ２が形成され
ているので外方張出ｉａ２は寛骨Ｈの皮質骨Ｈ１ｌを含
んで凹所Ｒの開口周縁を施蓋するように取付けられる。
従って、この外方張出縁ａ２及びその背后に密着する皮
質骨Ｈ１ｔによって臼蓋ソケットＡの硬質支持域が従来
のものに較べて増大することになり、ステムヘッドＳ　
Ｈから該ソケットＡに伝達される関節荷重に対する抗性
が高まる。また、この外方張出９ａ２は臼蓋ソケットＡ
の回旋やティルティングに対する抵抗力を大幅に高める
作用を持っているにの外方張出縁ａ２をして回倒の如く
平面円弧状片となすことによって寛骨Ｈの対応骨形状に
対する適合性がよくなり、下向漸増山状とすることによ
ってソケットＡの開口周縁ａ１に向って順次強度補強が
図られ堅固となると共に外方張出縁ａ２の表面に縦横凹
溝ａ３を形成することによって皮質骨Ｈ１１に対する接
触面積が増大して界面の密着性が高まる。そして外方張
出縁ａ２の別の特筆すべき点として．臼蓋ソケットＡが
経時によって寛骨Ｈ側に順次せり上って移動するのを倒
椀状殻もしくは半球状殻に対して斜めに張出した外方張
出縁ａ２が皮質骨Ｈ１ｌに係止することによって阻止し
、もってマイグレーションを防止することである。
（実施例） 望ましい実施例図に於て外方張出縁ａ２は円周方向には
ゾ１２０°をもって発現する３個の例を示している。そ
して、この外方張出縁ａ２の扇角θ１は第２図に於て４
８°、下向漸増内形状は第３図に於て約４０°の拡開角
θ２をもって実現しており、且つこの外方張出＠ａ２の
突出長は２〜１０ｍｍが適当である。更に外方張出縁ａ
２の表面及び臼蓋ソケットＡの半球状もしくは倒椀状の
殻の表面にはアパタイト等の生体活性材料よりなるコー
ティング層（不図示）を形成しておくことにより骨との
結合が促進されると共に強固な結合が達成される。縦横
凹溝ａ３の深さは約０．７５＋ｍ＋とじてあり．臼蓋ソ
ケットＡの材質はチタン合金（Ｔｉ−６Ａｆｆ−４Ｖ）
である。
以上の実施例は１例にすぎず症例に合わせて種々の態様
が採られ得ることは勿論である。
（発明の効果） 以上説明したところから明らかなように、本発明によれ
ば臼蓋ソケットの開口周縁沿いに間隔おきの外方張出縁
を具備させることにより該ソケットと皮質骨との接触範
囲を増大して臼蓋ソケットの支持力を高めると共にソケ
ットの回旋やティルティングを防止し且つマイグレーシ
ョンを阻止し得るので、従来の人工股関節の適正を増大
する優れた効果がある。

Claims (3)

    【特許請求の範囲】
  1. １．臼蓋ソケットＡ、ステムＳ及びステムヘッドＳＨを
    含む人工股関節に於て、上記臼蓋ソケットＡは全体とし
    てほゞ倒椀状殻もしくは半球状殻をなしてその開口周縁
    ａ１には周方向に間隔おきに外方張出縁ａ２を備えると
    共に該外方張出縁ａ２の表面に縦横凹溝ａ３を備えて成
    る人工股関節に於ける臼蓋ソケット。
  2. ２．外方張出縁ａ２が周縁ａ１の円周方向につき約１２
    ０°の間隔で設けられている請求項１記載の臼蓋ソケッ
    ト。
  3. ３．外方張出縁ａ２の表面及び臼蓋ソケットＡの半球状
    もしくは倒椀状部分の表面にアパタイト等の生体活性材
    料のコーティング層が形成されている請求項１もしくは
    ２記載の臼蓋ソケット。
