、組換えFVIIa、血漿交換、および大投与量または中間投与量(それぞれ、200−300 IU/kg体重、または25−50 IU/kg体重)のFVIII注入など、いくつかの治療法が用いられる。しかしながら、これらの方法はいずれも満足の行くものではなく、すべて大変な費用がかかる。

明において使用される薬剤は、FVIIIのC2ドメインに対する阻害抗体を有する血友病患者における出血を低下、および/または予防、および/または治療するための手段である。前記リガンドは、例えば、静脈内、動脈内、非経口、またはカテーテル化など、当技術分野において周知の方法で提供することができる。

(実施例)
第VIII因子特異的ヒトIg4κ BO2C11抗体を、マウスにおいて抗イディオタイプ抗体を作製するために使用した。BO2C11の特性は、Jacquemin MGら、(1998)Blood 92:496−506に記載されている。BO2C11の可変部軽鎖および重鎖の塩基配列およびアミノ酸配列は、PCT特許出願:WO01/04269に開示されている。

細胞株14C12から得られる抗イディオタイプ抗体(Ab 14C12)のインビトロにおける性質を、インビトロアッセイ系で調べた。Ab 14C12は、BO2C11に結合して、BO2C11がその標的抗原であるFVIIIのC2ドメインに結合するのを用量依存的に阻害することが示された。さらに、BO2C11のFVIII阻害特性を中和するAb 14C12の能力を機能的凝血アッセイ法においても測定した。発色(第VIII因子発色試験(デイドベーリング社(Dade Behring)、ドイツ、マールブルク(Marburug))は、トロンビンによって特異的に切断される無色の基質を変換することに基づく。このテスト系は、FVIII以外のトロンビンを産生するのに必要なすべての試薬を含んでいる。したがって、長時間にわたる発色の強度は、この系に加えられたFVIIIの量に比例する。BO2C11(0.1μg/ml)を加えると、基質のFVIII依存型変換(0.3 IU/ml)が完全に阻害される。この検査系でFVIII(0.3 IU/ml)を100%阻害できる最低のBO2C11濃度であると決定されている0.1μg/mlに最終濃度がなるよう、BO2C11を発色アッセイに加えた。FVIIIを加える前に、さまざまな濃度のAb 14C12を、この固定した量のBO2C11とインキュベートした。そして、この混合液を発色アッセイに適用した。
濃度でセファロースビーズを加える。遠心分離および洗浄を行った後、テストされるFVIII断片に対する特異性もつ抗体の存在量に比例するサンプルの放射活性を計測する。このようにして、血縁関係にない6人の血友病A患者のポリクローナル抗体と、C2ドメインに対するインヒビターを、C2ドメインが翻訳されるアッセイに一定の濃度で加えた。そして、すべてのサンプルに濃度を上げながらAb 14C12を加えた。6サンプルのうち3つで、ポリクローナル抗体の阻害特性が有意に(およそ50%)中和され、Ab
14C12が、ヒト抗C2抗体上で広く発現されるエピトープを認識することが示された。
Ab 14C12が、BO2C11のFVIII阻害特性をインビボで中和することができるかを、ヒト組換えFVIIIによって再構築されたFVIII−/− C57Bl/6マウス(Singh Iら、(2002)Blood 99:3235−3240)で調べた。
Ab 14C12の徹底的な生化学的評価を行った。Ab 14C12は、表面プラズモン共鳴システムを用いて測定すると、Kon値およびKoff値がそれぞれ105m-1S-1、10-5S-1という高いアフィニティーでBO2C11に結合する。常法によってAb 14C12のVHおよびVLドメインの配列を決定した(図3)ところ、Ab 14C12は、κ軽鎖をもち、IJHV1重鎖抗体ファミリーに属する。Ab 14C12が、FVIII C2ドメインへのBO2C11による結合を阻害できることから、発明者らは、Ab 14C12のVHおよびVLドメインの配列を、CD2の配列と比較してみようと
思い立った。VHのCDR1およびCDR2は、それぞれ、C2ドメインと同一または相同なアミノ酸残基を6個および7個含んでいる。CDR2は、C2領域のV2223〜T2237にほぼ同一であるが、この領域は、重要なリン脂質結合部位と、BO2C11に対する接触残基をいくつか含んでいる。さらに、BO2C11の可変部の配列(Jacquemin MGら、(1998)Blood 92:496−506)、およびC2と結合したその結晶構造(Spiegel P.C.Jr.ら、(2001)B1ood 98:13−19)に基づいて、BO2C11とAb 14C12の間の推定接触残基を同定したところ、その中には、BO2C11の可変部の枠組みの中に位置しているものがある。まとめて考えると、これらのデータは、Ab 14C12の重鎖の可変部は、13個の同一または相同なアミノ酸残基からなるC2内部イメージと、BO2C11の可変部に対する接触残基をいくつか含むことを示している。
それぞれ実施例2および3に記載されたインビトロおよびインビボにおける抗イディオタイプ抗体(Ab 14C12)の性質(上記参照)は、抗イディオタイプ抗体の可変部とFVIIIのC2ドメインとの間に広範な相同性が存在することを示唆していた。C2ドメインの結晶構造が得られている(Prattら(1999)、Nature 402:439−442)。そのため、Ab 14C12の重鎖および軽鎖の可変部の配列を、C2ドメインの配列と並べてみた。
ある抗原に対する抗体の特異性は、通常、1つか2つの相補性決定領域(CDR)、大抵の場合はVH領域に位置する残基によって決まる。結合は、最も好適な熱力学条件に対応して生じる(Manivelら、(2000)Immunity 13:611−620)。その後、抗原に曝露すると、抗体の反応は成熟すると言われており、VH領域およびVL領域にランダムに変異を導入することを含むプロセスになる。この後、抗原に対する結合活性に基づいて選択が起こる。したがって、完全に成熟した抗体の結合活性(この場合はFVIIIに対する、患者のB細胞の天然のレパートリーから検出または単離できる結合活性)は、VH領域およびVL領域全域にわたって存在する残基によってもたらされる協調力(cooperative forces)によって決まる。
当業者に周知の方法に従い、パパインアガロースビーズ(ピアス社(Pierce)、イリノイ州ロックフォード(Rockford,IL)を用いた分解によってAb 14C12のFab断片を調製し、プロテインAセファロースカラムを通過させて精製した。
FVIIIのC2ドメインは、リン脂質(PL)の結合部位、およびフォン・ヴィルブラント因子(vWF)の主要結合部位を含む。PLへの結合は、FVIIIの生理活性にとって必須である。すなわち、FIXおよびFXと第X因子活性化複合体(tenase
complex)を形成する。vWFは、シャペロンタンパク質として働き、FVIIIを早期分解およびクリアランスから防御する。
循環血からのFVIIIのクリアランスは、少なくとも部分的には、LRPレセプターにC2ドメインが結合することに起因する(Saenkoら、(1999)J Biol
Chem 274:37685−37692;Lentingら、(1999)J Biol Chem 274:23734−23739)ため、Ab 14C12によって抑制できることがある。このことを、尻尾に1 IUのヒト組換えFVIIIを注射して再構築したFVIII−/− C57Bl/6マウスで調べた。以前の計算では、このモデルにおけるヒトFVIIIの平均t1/2は165分であることが示された。Ab 14C12がFVIIIのクリアランスに変化を与えなかったことを確認するために、C57BL/6 FVIII−/−マウスに10μg/mlのAb 14C12、または無関係な特異性をもつIgG2aモノクローナル抗体を注射し、その15分後、1 IUのヒト組換えFVIIIを静脈注射した。3時間後と6時間後、静脈穿刺によって血液サンプルを回収した。機能的発色アッセイ法(第VIII因子発色試験、デイドベーリング社(Dade Behring)、ドイツ、マールブルク(Marburug))を用いて残留FVIII活性を評価した。図8に示すように、Ab 14C12が存在するとき、FVIII t1/2に有意な差は見られなかった。したがって、C2ドメインに対するインヒビターを有する患者を治療する方法として、Ab 14C12、またはその派生抗体を投与しても、FVIIIの生理学的クリアランスを変化させることはありえない。
Ab 14C12を用いて、記憶B細胞、および、FVIII機能を阻害する抗体を産生するいくつかのB細胞株を標的することができる。このようなBリンパ球は、その表面に抗体の分泌型の可変部領域と同じ可変部領域をもつ抗体分子を発現する。Ab 14C12のその標的細胞への結合を用いて、細胞死すなわちアポトーシスをもたらすシグナルを伝達することができ、それによって、抗FVIIIインヒビターを産生する細胞を免疫系から極めて特異的に排除することができる。
Claims (21)
- ヒト第VIII因子阻害抗体に対するモノクローナル抗イディオタイプ抗体であって、前記阻害抗体が第VIII因子のC2ドメインに対するものである、モノクローナル抗イディオタイプ抗体。
- さらに、FVIIIのC2ドメインに対する阻害抗体によってもたらされるFVIIIの凝血活性阻害を少なくとも50%中和することができることを特徴とする、請求項1に記載のモノクローナル抗イディオタイプ抗体。
- リン脂質および/またはvWFへのFVIIIの結合を阻害しない、請求項1または2に記載のモノクローナル抗イディオタイプ抗体。
- 前記第VIII因子阻害抗体が、VH1遺伝子ファミリーに由来するDP5 VH遺伝子セグメントによってコードされている可変部重鎖VHドメインを有する、請求項1〜3のいずれか1項に記載のモノクローナル抗イディオタイプ抗体。
- 第VIII因子阻害抗体がBO2C11である、請求項1〜4のいずれか1項に記載のモノクローナル抗イディオタイプ抗体。
- 前記抗体の可変部重鎖および軽鎖の相補性決定領域が、配列番号:5、配列番号:6、配列番号:7、配列番号:8、配列番号:9、および配列番号:10に示されたアミノ酸配列の1つに70%以上の配列同一性を有する、請求項1〜4のいずれか1項に記載のモノクローナル抗イディオタイプ抗体。
- 前記抗イディオタイプ抗体の可変部重鎖が、配列番号:1に示された塩基配列もしくは配列番号:1に70%以上配列が同一である塩基配列にコードされ、および/または、抗イディオタイプ抗体の可変部軽鎖が、配列番号:3に示された塩基配列もしくは配列番号:3に70%以上配列が同一である塩基配列にコードされている、請求項1〜5のいずれかに記載のモノクローナル抗イディオタイプ抗体。
- F(Ab’)断片、Fab’断片、Fab断片、または前記断片を改変したものである、請求項1〜6のいずれかに記載のモノクローナル抗イディオタイプ抗体。
- ヒト化モノクローナル抗イディオタイプ抗体である、請求項1〜7のいずれかに記載のモノクローナル抗イディオタイプ抗体。
- 14C12、またはそれから派生した抗体である、請求項1〜9のいずれかに記載のモノクローナル抗イディオタイプ抗体。
- 配列番号:5、配列番号:6、配列番号:7、配列番号:8、配列番号:9、および配列番号:10より選択されたアミノ酸配列を有するペプチドに、アミノ酸配列において70%以上同一である、配列番号:5、配列番号:6、配列番号:7、配列番号:8、配列番号:9、および配列番号:10より選択されたアミノ酸配列を有する、単離精製ペプチド。
- 請求項1〜10のいずれかに記載のモノクローナル抗イディオタイプ抗体を発現する、モノクローナル細胞株。
- BCCMに寄託番号LMBP5878CBとして寄託された細胞株14C12である、
請求項12に記載のモノクローナル細胞株。 - 請求項1〜10のいずれかに記載のモノクローナル抗イディオタイプ抗体、または請求項12に記載の精製単離ペプチドを、少なくとも1種類の医薬的に許容される担体と混合して含む、医薬組成物。
- 請求項1〜10のいずれか1項に記載のモノクローナル抗イディオタイプ抗体、または請求項12に記載の精製単離ペプチドの薬剤としての使用。
- FVIII阻害抗体の作用を患う患者を治療する方法であって、治療上有効な用量の請求項14に記載の医薬組成物を前記患者に投与することを含む、方法。
- FVIII阻害抗体を有する患者における抑制できない出血を治療または予防する方法であって、治療上有効な用量の請求項14に記載の医薬組成物を前記患者に投与することを含む、方法。
- FVIIIを前記患者に投与することをさらに含む、請求項17に記載の方法。
- 前記患者が血友病患者である、請求項16〜18のいずれか1項に記載の方法。
- FVIIIインヒビターに対する薬剤を製造するためのモノクロナール抗イディオタイプ抗体を開発するための方法であって、第8因子のC2ドメインに対する阻害抗体によって動物を免疫することと、a)FVIIIインヒビターの抗凝血活性を50%以上中和し、b)FVIIIがvWFおよびリン脂質に結合することと相互作用しない抗体を作製するために、前記動物の不死化脾臓細胞をスクリーニングすることとを含む、方法。
- 請求項1〜10のいずれか1項に記載の抗イディオタイプ抗体の、FVIII阻害抗体を検出または精製するための使用。
