Abstract
Description
本発明は、一般に、医療用または外科的治療のための方法およびデバイスに関する。より詳細には、本発明は、下部食道括約筋のような括約筋、または女性の尿道のような身体管腔を増強するための方法およびデバイスに関する。
一般に、本発明は、ヒト身体中の括約筋を処置するためのデバイスおよび方法を提供する。より詳細には、本発明は、括約筋の壁内に2つ以上の磁性デバイスを移植、注入、またはそうでなければ位置決めすることにより括約筋を増強するデバイスおよび方法を含む。これら2つ以上のデバイス間の磁性誘引により、処置される括約筋の緊張は増大される。
食道中に配置され、LESの位置またはその近傍位置にある食道の管腔内の圧力を測定する。この圧力が所望の圧力に接近している場合、手順は終了し得る。その一方、この圧力に対して調節をすることが所望される場合、磁石またはその他の磁化デバイスが挿入および位置決めされ得、移植されたデバイスの磁力を増加または減少する。上記に記載のようなその他の実施形態では、移植されたデバイスは、食道壁中に配置されるとき磁力をもたなくても良く、そして磁化デバイスよって磁化され得る。いくつかの実施形態では、調節デバイスは、後日、移植されたデバイスの磁力を増加または減少するために使用され得、GERDまたは食道のその他の症状を処置するためにLESを、より適切に増強する。
Claims (16)
- 患者の身体管腔を処置するための方法であって、
身体管腔の壁内の予備選択された位置に、少なくとも2つの磁性または磁化可能なデバイスを配置する工程を包含し、ここで該磁性デバイスが互いに誘引し、そしてここで該磁性デバイス間の誘引が、少なくとも部分的に該管腔を締め付ける、方法。 - 前記磁性デバイスを配置する工程が、
送達デバイスを有する内視鏡を前記身体管腔中に挿入すること;
該内視鏡を、前記予備選択された位置の近傍に位置決めするために進めること;および
該送達デバイスで、該身体管腔の壁中に少なくとも2つの磁性または磁化可能なデバイスを移植することをさらに包含する、請求項1に記載の方法。 - 前記少なくとも2つのデバイスが、前記患者の食道の壁中に移植されるとき磁化されない請求項2に記載の方法であって、
前記移植された少なくとも2つのデバイスの近傍の位置に磁化デバイスを位置決めする工程;および
該2つのデバイスの少なくとも1つを磁化する工程をさらに包含する、方法。 - 前記移植された少なくとも2つの磁石の近傍の位置に圧力検知デバイスを位置決めする工程:
前記身体管腔内の該位置における圧力を検知する工程;および
該検知された圧力を基に、該2つのデバイスの少なくとも1つの磁性力を調節する工程をさらに包含する、請求項3に記載の方法。 - 前記検知する工程が、前記送達デバイスまたは内視鏡上の圧力センサーを用いて実施される、請求項4に記載の方法。
- 前記移植された少なくとも2つの磁石の近傍の位置に磁力調節デバイスを位置決めする工程;および
該少なくとも2つの磁石の磁力を、前記食道内で検知された圧力を基に調節する工程をさらに包含する、請求項4または5に記載の方法。 - 前記調節する工程が、前記送達デバイスまたは内視鏡上の電磁石で実施される、請求項6に記載の方法。
- 前記少なくとも2つの磁性デバイスが、前記患者の食道の対向する側面に配置される2つの磁石を備える、請求項1に記載の方法。
- 前記少なくとも2つの磁性デバイスが、保持リング内に含まれる少なくとも4つの磁石を備え、ここで、該保持リングが、前記食道の壁内の配置のための形状であり、そして該少なくとも4つの磁石が半径方向パターンに互いに誘引し、該保持リングを収縮する、請求項1に記載の方法。
- 前記2つの磁性デバイスの少なくとも1つが、磁性粒子を備える、請求項1に記載の方法。
- 患者の身体管腔を処置するためデバイスであって、該管腔の壁中への配置のための2つ以上の磁石または磁化可能なコンポーネントのセットを備え、ここで該デバイスが、好ましくは、熱分解性カーボンのような生体適合性コーティングでコートされる鉄粒子である、デバイス。
- 患者の身体管腔中の括約筋を処置するためのデバイスであって、
近位端および遠位端を有する細長いカテーテル;および
該管腔の壁内に少なくとも2つの磁性部材を配置するために、該細長いカテーテルの遠位端に配置される磁性送達デバイスを備える、デバイス。 - 前記磁性送達デバイスが、
前記カテーテルの長さに沿って配置される細長いチューブであって、近位端および遠位端を有するチューブ;および
該管腔の壁中に磁性粒子を注入するために該チューブの遠位端に位置する少なくとも1つのニードルを備える、請求項12に記載のデバイス。 - 身体管腔の壁中に配置される磁化可能なコンポーネントまたは粒子を磁化するためのデバイスであって、
該身体管腔に導入されるよう適合されるカテーテル;および
該磁化可能なコンポーネントまたは粒子を磁場に曝すために該デバイス上に配置された永久磁石または電磁石を備える、デバイス。 - 前記コンポーネントまたは粒子の磁化の前、磁化の間、または磁化の後に、身体管腔中の圧力を試験する手段をさらに包含する、請求項14に記載のデバイス。
- 身体管腔を進むためのデバイスであって、保持リング内に配置された磁石または磁化可能なコンポーネントを備え、該保持リングが該管腔の周りに移植されるとき該コンポーネントが該管腔に対して閉鎖力を奏するように該身体管腔の壁中に該コンポーネントを配置するよう適合される、デバイス。
