Abstract
【解決手段】損傷膀胱組織を、膀胱状形状に形成された温血脊椎動物の粘膜下組織からなる組織グラフト構成物で置換する。この粘膜下組織は、粘膜層の内腔部分と筋層とから剥離された粘膜下層を含んでいる。

Description
小腸粘膜下組織を米国特許第4,902,508号に記載されている方法によって作った。つまり、ブタ空腸の切片を安楽死後10分以内に採取し、直ちに0.9%生理食塩液中に入れた。これらの切片を10ないし20cm長さに切り、その小腸切片から腸間膜組織を取り外した。小腸を裏返し(inside out)、粘膜を機械的に取りはずす。小腸切片を再び返して(すなわち元の配置の内腔側の緻密層)漿膜および筋層を外側表面から取り外した。組織を生理食塩液ですすぎ、グラフト材料として使用するまで10%硫酸ネオマイシン溶液に入れた。グラフト材料のの保存時間は2週間ないし3カ月であった。粘膜下組織の作成はソーセージ注入成形法に似た機械的プロセスであり、酵素反応段階は含まない。
イヌ腸粘膜下組織を異種移植材料としてブタに用いた。グラフトのための腸粘膜下組織を採取後1ないし5日以内に用いた。注入用腸粘膜下組織は、18ゲージ針によって懸濁液を注入できるように組織を機械的に乳化することによって調製した。
体重250ないし300mgの Sprague Dawley ラット 22匹をネンブタール(15mg/kg)で麻酔し、腹部正中線切開を行い膀胱を露出させた。膀胱頸部を露出させ、薄い付着物である膀胱周囲の脂肪を注意深く取り除いた。膀胱を取り扱いやすくするために、5−0絹の懸濁縫合糸4本を膀胱の4comers に置いた。マイクロ鋏を用いて膀胱の全背側表面(10mm×10mm)を切開し(膀胱表面積の約1/3)、微小焼灼器で止血した。1片の粘膜下組織グラフトを欠損部(膀胱内腔に面する緻密層表面)よりわずかに大きく形づくり、連続 non-interlocking 縫合を用いて7−10マイルド クロミックで正しい位置に縫い付けた。コンピテント(応答性)閉鎖を確実にするために、希メチレンブルー溶液を30ゲージ針で膀胱の腹側面に注射した。色素が溢出した縫合縁を断続7−0マイルド クロミック縫合でかがり縫いした。それから今後グラフトの正確な位置を比較するために、粘膜下組織グラフトのへりに4本の永久標識縫合糸(プロリン)で印しをつけた。その後腹部傷を筋肉および皮膚の両方共を連続5−0デキソン(Dexon)で2層に閉鎖した。手術直後の期間に尿路変更もドレーンも行わずにすんだ。 研究期間中に抗生物質を投与した動物もいなかった。
肉眼で見て、膨張した膀胱試料は均一に拡張しており、グラフト領域に憩室形成の兆候はなかった。グラフトは周囲の本来の膀胱と比較してわずかに白っぽい領域で、ほとんど見分けがつかなった。取り巻く膀胱周囲脂肪、 またはその他の腹部内構造への癒着はごくわずかであった。グラフトに固着していない、自由に浮遊する黄褐色の結石が12週間目に採取した4匹動物のうちの3匹の膀胱に認められ、24週目に採取した2匹のうちの1匹の膀胱に、 および48週目に採取した4匹の動物中3匹の動物の膀胱に認められた。その他の動物では膀胱結石の証拠は見つからなかった。日常的結石分析で100%ヒドロキシアパタイトであることが明らかにされた。水尿管腎盂症のはっきりした兆候はどの動物にもなかった。2匹の動物が術後の早い時期に縫合線漏れによって死亡した。これらに代わる動物によって、動物の総数を22匹に維持した。その他の動物は研究期間中死ぬことはなかった。
14匹のイヌで、 膀胱の30−70%を過酢酸滅菌粘膜下組織材料で置換する研究を行った。動物を手術の1週間、2週間、4週間、8週間、12週間、 6カ月および14カ月後に殺した。概要を述べると、 すべての動物は術後2日以内に尿保有能力および正常な排尿能力を示した。膀胱は非常に短期間(1−2週)に再生し(グラフト組織は内因性膀胱組織で置換された)、正常な移行上皮、基底膜、粘膜下組織、および比較的薄くて正常な、だが明白に組織化された平滑筋細胞層を含んでいた。感染したり、発熱したり、異種材料拒否の兆候を示す動物は1匹もいなかった。
Claims (6)
- 膀胱の損傷または疾病部分の置換のための組織グラフト構成物であって、膀胱の20%以上を置換するための膀胱状形状に形成された温血脊椎動物の粘膜下組織のシートからなる前記構成物。
- 前記組織グラフト構成物が、尿管及び尿道との液体連絡及び外科的付着のための開口部を有する請求の範囲1に記載の組織グラフト構成物。
- 前記組織グラフト構成物が、1温血脊椎動物の2つ以上の粘膜下組織片から形成され、前記グラフト構成物が、前記グラフト構成物の形成に用いた個々の片のどれよりも大きい表面積を有する請求の範囲1に記載の組織グラフト構成物。
- 粘膜下組織が、粘膜層の内腔部分と筋層の両方から剥離された粘膜下層を含む腸粘膜下組織である請求の範囲1に記載の組織グラフト構成物。
- 複数の粘膜下組織層を有することを特徴とする請求の範囲1に記載の組織グラフト構成物。
- 2つ以上の粘膜下組織片から形成されることを特徴とする請求の範囲1に記載の組織グラフト構成物。
