Abstract
【解決手段】 本半月板修復デバイスは、生体適合性を有し、生体適合性を有する組織修復スカフォールドと細胞成長導管フラップとを含んでなる。組織修復スカフォールドは、半月板中の欠損部に接触して配置するのに適しており、半月板組織を支えるための構造および/または組織の成長を促進する構造を与えられることが好ましい。組織修復スカフォールドに取り付けられた細胞成長導管フラップは、滑膜と組織修復スカフォールドとの間の受け渡しを可能とする。

Description
50/50 PDS/VICRYLの不織材料から造られた組織修復インプラントを使って、組織インプラントの滑膜の近傍への配置を比較評価した。インプラントは、約11mm×7mm×1mmの寸法と、236.6mg/mLの密度を持っていた。
(1)生体適合性を有する半月板修復デバイスであって、半月板の欠損部と接触して配置するのに適した、生体適合性を有する組織修復スカフォールドと、当該組織修復スカフォールドに取り付けられており、脛骨表面に接触して、半月板組織欠損部へ生体物質を受け渡すのに適した細胞成長導管フラップとを含んでなる修復デバイス。
(2)前記細胞成長導管フラップが、滑膜から前記組織修復スカフォールドならびに前記半月板組織欠損部への細胞および栄養素の移動を可能とするための導管を提供する、実施態様(1)に記載の修復デバイス。
(3)前記細胞成長導管フラップが滑膜に接触するのに適したものである、実施態様(2)に記載の修復デバイス。
(4)前記組織修復スカフォールドが、生体吸収性物質から造られたものである、実施態様(1)に記載の修復デバイス。
(5)前記組織修復スカフォールドと細胞成長導管フラップとが、相異なる材料から造られている、実施態様(1)に記載の修復デバイス。
(7)前記組織修復スカフォールドと細胞成長導管フラップとが、グリコリド、ラクチドおよびジオキサノンからなる群から選ばれたモノマーに由来する少なくとも一つのポリマーを含んでなる、実施態様(6)に記載の修復デバイス。
(8)前記組織修復スカフォールドと細胞成長導管フラップとがポリジオキサノンを含んでなる、実施態様(1)に記載の修復デバイス。
(9)前記組織修復スカフォールドと細胞成長導管フラップとがグリコリドとL−ラクチドとのコポリマーを含んでなる、実施態様(1)に記載の修復デバイス。
(10)前記組織修復スカフォールドが、天然ポリマー、合成ポリマーおよびこれらの組み合わせからなる群から選ばれた少なくとも一つの材料から形成されたものである、実施態様(1)に記載の修復デバイス。
(12)さらに、細胞の成長を促進するのに有効な少なくとも一つの生理活性物質を含んでなる、実施態様(1)に記載の修復デバイス。
(13)前記生理活性物質が、多血小板血漿、軟骨由来の形態形成タンパク質、ヒトの組み替え成長因子およびこれらの組み合わせからなる群から選ばれたものである、実施態様(12)に記載の修復デバイス。
(14)前記組織修復スカフォールドと細胞成長導管フラップとが単一片から形成されたものである、実施態様(1)に記載の修復デバイス。
(15)前記細胞成長導管フラップが、前記組織修復スカフォールドに対し実質的に垂直方向にあるような向きに配置されている、実施態様(1)に記載の修復デバイス。
(17)前記細胞成長導管フラップと組織修復スカフォールドとが、Lの字状になるように、それぞれに対し配置されている、実施態様(1)に記載の修復デバイス。
(18)前記細胞成長導管フラップが、前記組織修復スカフォールドの厚さより薄い、実施態様(1)に記載の修復デバイス。
(19)前記細胞成長導管フラップの密度が約150mg/mL〜350mg/mLの範囲にある、実施態様(1)に記載の修復デバイス。
(20)前記細胞成長導管フラップの空隙体積が約50%〜95%の範囲にある、実施態様(1)に記載の修復デバイス。
(22)さらに、細胞成長導管フラップを配置する前に半月板にやすり掛けする工程を有する、実施態様(21)に記載の方法。
(23)さらに、細胞成長導管フラップを配置する前に滑膜にやすり掛けする工程を有する、実施態様(21)に記載の方法。
(24)さらに、細胞成長導管フラップを滑膜と接触するように配置することを含む、実施態様(21)に記載の方法。
(25)半月板欠損部の外科的修復方法であって、細胞成長導管フラップを与え、当該細胞成長導管フラップを、半月板組織欠損部と脛骨表面とに接触して配置し、細胞成長導管フラップを適切な場所に固定することを含み、当該細胞成長導管フラップが、細胞および栄養素を半月板欠損部に移動させ、これによって半月板の治癒を促進することが効果的にできる、方法。
(27)前記細胞成長導管フラップが、滑膜から半月板組織欠損部へ細胞および栄養素の移動を可能にする導管を与える、実施態様(25)に記載の方法。
(28)さらに、前記細胞成長導管フラップを配置する前に、半月板にやすり掛けする工程を有する、実施態様(25)に記載の方法。
(29)さらに、前記細胞成長導管フラップを配置する前に、滑膜にやすり掛けする工程有する、実施態様(25)に記載の方法。

Claims (21)
- 生体適合性を有する半月板修復デバイスであって、
半月板の欠損部と接触して配置するのに適した、生体適合性を有する組織修復スカフォールドと、
当該組織修復スカフォールドに取り付けられており、脛骨表面に接触して、半月板組織欠損部へ生体物質を受け渡すのに適した細胞成長導管フラップと
を含んでなる修復デバイス。 - 前記細胞成長導管フラップが、滑膜から前記組織修復スカフォールドならびに前記半月板組織欠損部への細胞および栄養素の移動を可能とするための導管を提供する、請求項1に記載の修復デバイス。
- 前記細胞成長導管フラップが滑膜に接触するのに適したものである、請求項2に記載の修復デバイス。
- 前記組織修復スカフォールドが、生体吸収性物質から造られたものである、請求項1に記載の修復デバイス。
- 前記組織修復スカフォールドと細胞成長導管フラップとが、相異なる材料から造られている、請求項1に記載の修復デバイス。
- 前記組織修復スカフォールドと細胞成長導管フラップとが、同一の材料から造られている、請求項1に記載の修復デバイス。
- 前記組織修復スカフォールドと細胞成長導管フラップとが、グリコリド、ラクチドおよびジオキサノンからなる群から選ばれたモノマーに由来する少なくとも一つのポリマーを含んでなる、請求項6に記載の修復デバイス。
- 前記組織修復スカフォールドと細胞成長導管フラップとがポリジオキサノンを含んでなる、請求項1に記載の修復デバイス。
- 前記組織修復スカフォールドと細胞成長導管フラップとがグリコリドとL−ラクチドとのコポリマーを含んでなる、請求項1に記載の修復デバイス。
- 前記組織修復スカフォールドが、天然ポリマー、合成ポリマーおよびこれらの組み合わせからなる群から選ばれた少なくとも一つの材料から形成されたものである、請求項1に記載の修復デバイス。
- さらに、前記組織修復スカフォールド上または組織修復スカフォールド内に配置された生育可能組織であって、組織修復スカフォールドに隣接する本来の組織と一体化するのに有効な生育可能組織を含んでなる、請求項1に記載の修復デバイス。
- さらに、細胞の成長を促進するのに有効な少なくとも一つの生理活性物質を含んでなる、請求項1に記載の修復デバイス。
- 前記生理活性物質が、多血小板血漿、軟骨由来の形態形成タンパク質、ヒトの組み替え成長因子およびこれらの組み合わせからなる群から選ばれたものである、請求項12に記載の修復デバイス。
- 前記組織修復スカフォールドと細胞成長導管フラップとが単一片から形成されたものである、請求項1に記載の修復デバイス。
- 前記細胞成長導管フラップが、前記組織修復スカフォールドに対し実質的に垂直方向にあるような向きに配置されている、請求項1に記載の修復デバイス。
- 前記細胞成長導管フラップと組織修復スカフォールドとが、Tの字状になるように、それぞれに対し配置されている、請求項1に記載の修復デバイス。
- 前記細胞成長導管フラップと組織修復スカフォールドとが、Lの字状になるように、それぞれに対し配置されている、請求項1に記載の修復デバイス。
- 前記細胞成長導管フラップが、前記組織修復スカフォールドの厚さより薄い、請求項1に記載の修復デバイス。
- 前記細胞成長導管フラップの密度が約150mg/mL〜350mg/mLの範囲にある、請求項1に記載の修復デバイス。

当該組織修復スカフォールドを適切な場所に固定する
ことを含み、
当該細胞成長導管フラップが、細胞および栄養素を半月板欠損部に移動させ、これによって、半月板の治癒を促進できる、
方法。
