Abstract
【解決手段】電動スクリュードライバで作動され、トルクの吸収のためのホルダーが形状適合態様でペディクルスクリュー1の先端に接している。ホルダーはクランプねじの誘導のためにその内径と合わされる管状部分を有し、心立て部分はクランプねじの代わりにペディクルスクリューの先端において固定され得るので、ホルダーが取り付けられると、ペディクルスクリューは係合することが可能で、挿入されたペディクルスクリューの接点も目視できない。即ち、より小さな手術範囲が、同じホルダーまたは他のホルダーと取り付けるために複数の時間維持され得る。

Description
第1のペディクルスクリュー
a1)手術範囲の外でペディクルスクリューの先端上に心立て部分をねじ込んで固定する。第1のホルダーを形状適合態様で先端に位置するまで取り付け、軸方向の心立て部分においてホルダのよりよい事前張力を保証する。ペディクルスクリューを脊椎本体へねじ込む。
第2のペディクルスクリュー
a2)「a1)第1のホルダーを置く…」と同様である。
中間ステップ
・所定の事前張力下で第1及び第2のペディクルスクリューの間隔を判定し、ケーブル上へ押されるべきスペーサーに対して必要な長さを画定する。(同じアプリケーションデータを有する同等のアプリケーションがこの装置に対して出願されている)。拡張器具は、第2のペディクルスクリューのねじ込まれた心立て部分及びケーブルにおける位置決めのために第1のペディクルスクリュー上で方向付けられてもよい。

Claims (15)
- ペディクルスクリュー(1)用計器システムであって、ペディクルスクリューはその先端(2)に内部ねじ山(3)を有し、前記ねじ山はねじ軸(21)の方向に外径D1のグラブねじ(5)を収容して、前記ねじ軸(21)に対して横方向に先端へ突出する接続部分(4)をクランプで締めて固定し、突出部を有するホルダー(6)が前記ペディクルスクリューの前記先端において形状適合態様にて係合し、直径D4のスクリュードライバ(10)がグラブねじ(5)に位置決めできるよう成されており、管状部分(8)を備えた少なくとも2つのホルダー(6a、6b、6c、6d)が設けられ、前記管状部分(8)は前記ペディクルスクリューの前記先端(2)において支持されることを特徴とし、前記グラブねじの代わりに挿入可能な心立て部分(12)があり、前記心立て部分(12)は、同様に前記内部ねじ山(3)へねじ込まれ、前記ホルダ(6a、6d)が取り付けられると係合されることが可能で、同じホルダー(6a、6d)または他のホルダー(6b、6c)を取り外すことを可能にし、且つ所望どおり何度でもペディクルスクリューの形状が適合する係合面にホルダーを誘導することを可能にするために、心立て部分(12)の外径Dがグラブねじ(5)の直径D1と対応しており、前記管状部分(8)は前記直径D1よりわずかだけ大きい内径D2を有し、スクリュードライバ(10)を用いてグラブねじ(5)を位置決めするか、または前記ペディクルスクリューの前記先端(2)に前記管状部分(8)を通して導かれた心立て部分を位置決めして、そのねじ山が傾斜のない態様にて前記内部ねじ山(3)と係合することを特徴とする計器システム。
- 前記内径D2は前記直径D1の1.3倍より小なることを特徴とする請求項1記載の計器システム。
- 前記内径D2は前記直径D1の1.1倍より小なることを特徴とする請求項1記載の計器システム。
- 前記心立て部分(12)は長手軸から20°またはそれ以上の角度まで弾性的に偏向し得る前記長手方向の中心領域(13)を有することを特徴とする請求項1乃至3のいずれかに記載の計器システム。
- 前記心立て部分(12)は、その中心領域(13)の下の方の直径D3を有し、曲げばねとして想定された偏向を可能にすることを特徴とする請求項1乃至4のいずれかに記載の計器システム。
- 前記心立て部分(12)は、前記中心領域(13)に続いて前記長手方向に緩やかで段差のない遷移を有する実質的に円形の断面及び被覆物ラインを有することを特徴とする請求項4または5のいずれかに記載の計器システム。
- 管状ホルダー(6a)はペディクルスクリュー用の位置決め装置として心立て部分(12)と共に形成され、取り付けられたホルダー(6a、6d)は軸方向にブロックする前記心立て部分(12)の方へ固定要素(14)を有し、前記心立て部分(12)が順番に前記ペディクルスクリューの前記先端(2)へねじ込まれることを特徴とする請求項1記載の計器システム。
- 前記取り付けられたホルダー(6d)はクランプねじ(73)の形式で固定要素を有し、それは前記ホルダー(6d)上の先端(72)において支持され、前記心立て部分(12)の後部において内部ねじ山(76)へ前記軸方向に係合することを特徴とする請求項7記載の計器システム。

- 前記偏向弧(18)はその基部(19)に沿って3mmより大きい湾曲の半径を有することを特徴とする請求項10記載の計器システム。
- 前記取り付け座金(17)は偏向弧の代わりに偏向ローラ(18a)を有することを特徴とする請求項10記載の計器システム。

管状部分(8)を備えた少なくとも2つのホルダー(66、68)が設けられ、前記管状部分(8)は前記ペディクルスクリューの前記先端(2)において支持可能であることを特徴とし、前記スクリュードライバに加えて、心立て部分(12)があり、前記心立て部分(12)は前記管状部分(8)を通して挿入され、前記内部ねじ山(3)へ同様にねじ込まれ、前記ホルダ(66、68)が取り付けられるときに係合され、同じホルダー(66)または他のホルダー(68)を取り外して所望するだけ何回でも形状が適合する係合面にホルダーが誘導されるために最大径D’をスクリュードライバ(10)の直径D4’に対応させ、前記管状部分(8)がスクリュードライバの直径D4’よりわずかだけ大きい内径D2’を有し、スクリュードライバ(10)の保持器具(59)を用いて前記クランプねじ(5’)を前記スクリュードライバの軸(62)と整列させ、前記スクリュードライバ(10)を用いて間接的に前記ねじ山(3)において前記クランプねじ(5’)を位置決めするか、または前記ねじ山(3)において前記管状部分(8)を通して直接誘導された心立て部分を位置決めするか、またはねじ込まれた心立て部分(12)においてペディクルスクリューをへホルダーを誘導することを特徴とする計器システム。 - 前記直径D2’は前記直径D4’の1.2倍より小なることを特徴とする請求項13記載の計器システム。
