Abstract
【解決手段】短時間型経口低血糖剤の使用、及び食事共に短時間型経口低血糖剤を投与することによって、内因性のインシュリン分泌が食事により刺激される、2型糖尿病を治療するための新規療法で、メトフォルミンのみでは調節が不良なNIDDM患者に、レパグリニド、グリキドン等とメトフォルミンを併用することにより、血糖調節を顕著に改善せしめる方法である。

Description
糖尿病は、グルコース代謝障害の発現、とりわけ治療を受けていない糖尿病患者での血中グルコースレベルの上昇を特徴とする。基礎となる欠陥によって、糖尿病は、大きく二つのグループ:患者が、患者の膵臓腺中に、インシュリンを産生するβ細胞を欠損するときに発症する1型糖尿病、又はインシュリン依存性糖尿病(IDDM)と、広範な他の異常に加えて、β細胞の機能障害を有する患者に発症する2型糖尿病、又はインシュリン非依存性糖尿病に分類される。
インシュリンに対する患者の組織感受性を増大させる物質は、典型的には経口投与されるので、これらの物質は、経口低血糖剤又はOHA(oral hypoglycaemic agent)と総称されている。
ある好ましい側面では、本発明は、NIDDMにおける食後の高血糖を治療するために食事によるインシュリンの分泌を刺激するように適合された医薬を製造するための、β細胞からのインシュリン分泌を刺激し得る短時間低血糖剤の使用に関する。
健康な人は、24時間のインシュリン基礎分泌を有している。食事と関連して、インシュリンに対する需要が増大し、複雑なフィードバック機構を介して、膵臓は需要を満たすように刺激を受ける。しばらくした後、インシュリンのレベルは、再びベースレベルにまで減少する。
レパグリニドは、2型糖尿病の患者に、柔軟な投薬療法で与え得る
本研究によって明らかとなったように、作用の持続が短い(T1/2=1時間)ために、レパグリニドは、食事に近接した投薬療法に適しており、糖尿病患者に、より柔軟な毎日の生活を与える。
毎日3回食事に近接してレパグリジドを投薬することにより、NIDDMの血糖制御が改善される
要約
レパグリニドは、化学的に新しいクラスのインシュリン分泌促進薬に属し、短時間型であり、迅速に作用するインシュリン放出物質である。食事の直前にレパグリニドを日に3回投薬した場合と、同じ投薬量を日に2回投与した場合とを比較する研究で、インシュリンの放出を節食に合わせることの潜在的な影響を調べた。18人のOHAナイーブNIDDM患者が、4週間のシングルセンター、ダブルブラインド研究に参加し、朝食、昼食、及び夕食前に0.25mgのレパグリニドを与えるか(REP3)、又は朝食前に0.5mg、昼食時にプラセボ、夕食前に0.25mg(REP2)の何れかに、無作為に割り振った。2週間後に、用量を2倍にした。ベースラインでは、血糖値、インシュリン、及びCペプチドプロフィールは両群間で同じであった。4週間後に、空腹時血糖値は、両群で有意に減少した(REP2:11.2から9.6mmol/L、REP3:11.2から8.4mmol/L)。総血糖調節は、REP2で血糖値(AUC0-24h)が8.91mmol/Lであり、REP3で7.00mmol/L(P<0.05)だったので、REP2と比較すると、REP3の方が良好であった。同じ有意差は、グルコースAUC(0-16時間)でも見られた。血糖調節の改善におけるこの差は、REP3でのHbA1Cレベルが7.5から6.5%(P<0.05)へと有意に減少したのに対して、REP2のではHbA1Cは有意に減少しなかった(7.1から6.8%)ことに反映されていた。両群ともに、血漿インシュリンは、次の食事の前に、処置前のレベルに減少し、処理前のレベルに比べて、夜間の血漿インシュリンは増加しなかった。
レパグリニドは、膵臓β細胞中に存在するATP感受性のカリウムチャネルに作用する新規インシュリン分泌刺激薬であるが、スルフォニル尿素とは異なる部位に結合する。レパグリニドは、食事だけでは血糖値が十分に調節されないNIDDM患者を治療するために開発された。レパグリニドは、消化管から迅速に吸収されるので、短い血漿半減期を有しており、食事に近接した投与に極めて適している。本研究は、毎日同じ用量で、朝と夜か、又は3回の主食の前の何れかにレパグリニドを与えたときの血糖調節に対する効果を調べるようにデザインした。
これは、年齢40〜70、25kg/m2のBMI(body mass index)、6.5〜13mmol/Lの空腹時血糖値(FBP)、11%未満のHbA1c、0.3pmo/mLを超える絶食時CペプチドのNIDDM患者が関与するダブルブラインド、プラセボ対照研究であった。3度の主食の前に、毎日3回、0.25mgのレパグリニドを4週間処置するか(REP3)、又は朝食の前に0.5mgのレパグリニド、昼食の前にプラセボ、夕食の前に0.25mgのレパグリニドを4週間処置する(REP2)という何れかの処置に、参加した18人の患者のうち、17人を無作為に振り分けた。2週間後に用量を2倍にして、各食事の前に0.5mgまで(REP3)、1mg+0.5mgまで(REP2)を増やした。各患者は、4週間の研究期間中に3回診察を受けた。ベースライン及び28日目に、24時間のホルモン及び代謝プロフィールを調べた。
REP3群の8人の患者及びREP2群の9人の患者は研究を終了した。
4週間の治療後、REP3とREP2の両群で血糖値が減少した(P<0.01)(図1)。しかしながら、食前血糖値は、REP2よりもREP3の方が1〜2mmol/L低く、食後値は、約2.5mmol/L有意に低かった(P<0.05)。
HbA1c(±SEM)も減少していたが(REP3:7.51±0.78% vs. 6.51±0.64%;REP2:7.12±0.24% vs 6.84±0.34%)、減少が統計学的に有意だったのは、REP3群のみであった(P=0.004)。
研究中には、REP3とREP2群の間には、食前又は食後の血漿インシュリン又は血漿C-ペプチド値に有意な差は存在しなかった。標準化された血漿C-ペプチドのAUC0-24hは、処置後4週目に、両群で増加していた。標準化された血漿インシュリンのAUC0-24hは、REP2群で20%増加し、REP3群で35-40%増加していたが(図3)、差は有意でなかった。両処置群で、血漿インシュリンは、次の食事の前には、処置前のレベルに減少していた。処置前のレベルに比べて、夜間中、血漿インシュリンが増加していた。
レパグリニドの薬物動力学的プロフィールは、REP2群では朝に高いピーク値を示し、REP3群では午後に高いピークを示すことによって特徴づけられた。しかしながら、レパグリニドの平均AUC0-9hとAUC0-24hは、両群で同程度であり、両群が、同じレベルの一日当たり総薬物曝露を受けたことを示している。
何れの処置群についても、深刻な副作用は全く報告されなかった。唯一の深刻でない副作用は、穏やかな低血糖エピソードと1症例のインフルエンザであった。
レパグリニドは、NIDDM患者の血糖調節に有意な改善をもたらし、使用した用量レベルでは、穏やかな副作用を伴うにすぎなかった。両処置療法(食前に1日2回及び1日3回)は、同じようなインシュリン分泌速度を有し、24時間、高インシュリン血症を生じせしめなかったが、データは、朝食と夕食前だけにレパグリニドを投薬した場合と比べて、3回の主食の前に投薬する方が、より大きい代謝調節が達成されることを示している。
レパグリニドを追加する治療は、メトフォルミンでは調整が不良なNIDDM患者の血糖調節を有意に改善する
要約
該多施設無作為試験は、レパグリニド(REP)とメトフォルミン(MET)の組み合わせによる血糖調節効果を、METのみでは十分に調節されないNIDDM患者を何れかの薬物のみで治療した場合(平均HbA1c:8.5%)と比較するためにデザインした。83人の患者が、3アーム(three-armed)、二重盲検、ダブルダミーパラレルグループ研究に参加した。通常どおりMETを投薬する4〜5週の導入期間が経過した後、患者をREP若しくはMETの単一治療、又はREP+MET組み合わせ療法に、無作為に振り分けた。METの用量は、研究中一定に保った(1〜3g/日)。REP用量は、4〜8週の滴定相の間に決定した(初期REP用量:0.5mg、食前、1日3回;最高用量:4mg、食前、1日3回)。滴定相の後は、3ヶ月の維持期間である。ベースラインから最後の通院まで、MET+REPの組み合わせ療法は、REP又はMETの単一治療と比べて、血糖調節を有意に改善した(P<0.005)(HbA1Cの平均変化:-1.41%(MET+REP)、-0.38%(REP)、-0.33%(MET);空腹時血糖値(mmol/L)の平均変化:-2.18(MET+REP)、0.49(REP)、-0.25(MET))。空腹時インシュリンとC-ペプチドレベル、及び脂質プロフィールに関しては、2つの単一療法とMET+REPの組み合わせ療法の間には、統計学的な差は見られなかった。MET及びMET+REP処置は、REP処置に比べて、消化管の副作用をより生じせしめた。重い低血糖事象は、何れの群でも観察されなかった。結論的には、REP処置は、消化管の副作用がより軽く、METと同一の血糖調節を与えた。REP+MET療法は、REP又はMETの何れかと対比して、代謝調節の顕著な改善をもたらし、許容可能な範囲の調節域までHbA1Cを減少せしめた。該データは、REPとMETの組み合わせは、この種の患者に相乗的な特性を有し得ることも示唆している。
レパグリニド(REP)は、食事療法だけでは血糖値が調節されないNIDDM患者を治療するために開発された新規経口低血糖罪である。該薬物は迅速に吸収され、血症半減期が短く、スルフォニル尿素類とは異なり、膵臓β細胞上のATP感受性カリウムチャネルに結合し、胆管を介して排泄される。レパグリニド(REP)は、生理的な食後状態に類似したインシュリン放出プロフィールを刺激する。メトフォルミン(MET)とREPは、相補的な作用機序を有しているので、METのみでは十分治療されない患者に対する、METとの組み合わせ療法としてのREPの薬効及び安全性を調べることが本研究の目的であった。
本研究は、オーストラリアの9施設で行われた無作為、二重盲検、パラレルグループ研究であった。年齢40〜75才でBMIが21kg/m2を超え、調節が不十分な(HbA1C>7.1%)83人のNIDDM患者が、6ヶ月を超えるMET治療の後に参加した。4〜5週にわたるMET治療のオープンベースライン期間後に、通常の用量(1〜3mg/日)でMETを継続するか、又はMETとREPの組み合わせ、若しくはREP単独での処置するか、何れかに患者を無作為に振り分けた。4〜8週の滴定期間の間に、REPの用量を決定した(初期REP用量:0.5mg、食前、1日3回(食事前に、1日3回)、最高用量:4mg、食前、1日3回)。3ヶ月の維持期間の間、最後の滴定ステップで到達した用量を継続した。8回の予定された通院で、患者を診察した。
合計83人の患者が本試験に参加し(MET+REP:27;REP:29;MET:27)、そのうち74人が研究を終了した((MET+REP:27;REP:26;MET:21)。
MET+REP群の患者では、平均HbA1cと空腹時血漿グルコース(FPG)は、ベースラインと最後の通院の間で、それぞれ、8.32から6.91%へ(p<0.005)、10.22から8.04mmol/L(p<0.005)へと有意に減少した(図4及び図5)。METとREP群に関しては、何れのパラメーターについても有意な変化はなかった(表1)。
合計339の副作用が報告されたが、おそらく又は多分、そのうち27が研究薬に関係していると考えられた。
REPとMETの組み合わせ療法は、メトフォルミンのみでは十分に調節されないNIDDM患者に、REP又はMETの何れかの単一療法を施したときよりも、より良い血糖調節を与える。実際には、MET+REP処置は、アメリカ糖尿病教会によって推奨されている目標値(<7%)まで、該患者群のHbA1cを減少せしめた。
Claims (19)
- 食事に近接したインシュリンの分泌を刺激するように適合された、NIDDM患者の食後の高血糖を治療するための医薬を製造するための、β細胞からインシュリンを分泌し得る短時間型低血糖剤の使用。
- レパグリニドの請求項1に記載の使用。
- N-[(トランス-4-イソプロピルシクロヘキシル)-カルボニル]-D-フェニルアラニンの請求項1に記載の使用。
- グリキドンの請求項1に記載の使用。
- 長時間型低血糖剤をさらに具備する医薬を製造するための請求項1〜4の何れか1項に記載の使用。
- このような治療を必要とする患者に、食物と共に、有効量の短時間型低血糖剤を投与することによって、食事に伴うインシュリンの分泌を刺激することを具備するNIDDMの治療法。
- 短時間型低血糖剤がレパグリニドである請求項6に記載の方法。
- 短時間型低血糖剤がN-[(トランス-4-イソプロピルシクロヘキシル)-カルボニル]-D-フェニルアラニンである請求項6に記載の方法。
- 短時間型低血糖剤がグリキドンである請求項6に記載の方法。
- 療法が、短時間型低血糖剤の食事に近接した投与に加え、長時間型低血糖剤による処置を具備する請求項6〜9の何れか1項に記載の方法。
- 短時間型低血糖剤の食事に近接した投与が、1日に1又は2回の長時間型低血糖剤の投与によって補充される請求項10に記載の方法。
- 長時間型低血糖剤が、メトフォルミン、クロルプロパミド、トルブタミド、グリベンクラミド、グリボルヌリド、グリクラジド、グリピジド、及びトログリタゾンを含む群から選択される請求項10又は11に記載の方法。
- 療法が、メトフォルミンによる処置を具備する請求項7に記載の方法。
- レパグリニドとメトフォルミンが同一の投薬ユニット中に含有される請求項13に記載の方法。
- NIDDM患者の血糖調節の改善を達成するための使用に適した薬学的キットであって、NIDDM患者への投与のために調合された一定量のレパグリニドと、NIDDM患者への投与のために調合された相乗効果を示す有効量のメトフォルミンとを具備するキット。
- 適切な担体と共にレパグリニドとメトフォルミンを具備する薬学的組成物。
- 錠剤の形態で与えられる請求項16に記載の薬学的組成物。
- カプセルの形態で与えられる請求項16に記載の薬学的組成物。
- 投薬単位当たり約0.01mg〜約8mgのレパグリニドと、約50mg〜約1000mgのメトフォルミンを含有する請求項16〜18の何れか1項に記載の薬学的組成物。
