Abstract

<P>PROBLEM TO BE SOLVED: To provide a pillow for a practical prone position which is improved for medical treatment, and a mattress for prone position which is used while being combined with the pillow for the prone position. <P>SOLUTION: This mattress is equipped with this pillow for the prone position including a pillow main body for which a filler article for pillow is filled in a flexible external bag forming an approximately horseshoe-shape, and a plurality of band-shape members which are provided on the lower surface side of the flexible external bag for the purpose of fitting-fastening a rigid supporting body having a roughly similar shape as the pillow main body. The pillow comprises a combination to which an upper and lower position adjusting mechanism is added. Then, the mattress is provided with a pair of spectacle-like recesses having an approximately circular shape for reducing the pressure to breasts under the prone position and for facilitating the thoracic respiration. Also, an area which comes into contact with the abdominal region under the prone position is provided with a recess having an approximately shell-like or a drop-like shape for reducing the pressure to internal organs, which reaches the terminal end of the mattress. 本発明は枕、特定的には伏臥位用枕、さらに特定的には医療において使用される伏臥位用枕、ならびにそれと都合よく組合わせて併用される伏臥位用のマットレス構造に関する。殊に眼科治療において使用される顔面位置上下調節可能な伏臥位用枕に関する。

さらに詳しくは、本発明は、概略馬蹄形をなす柔軟性外袋に枕用充填物を充填してなる枕本体;柔軟性外袋の下面側に設けられた複数の帯状部材で固定装着される概略馬蹄形の剛性支持体;該剛性支持体を昇降調節具を介して支持するために床面に配置される、略H字形の平面形状を有するソール;を含む伏臥位用枕であって:該昇降調節具が2本の金属棒材または管材からなる支持桿を貫通孔を中心にして回転軸によりX状に交差組合せてなり、それら支持桿の4つの先端折り曲げカギ形部分を剛性支持体の下面及びソールの上面のH字横棒交差部附近に軸受け固定/ラッチ取り付けして、ラッチでの先端折り曲げカギ形部分の固定位を変位させることによりX状交差支持桿の交差角を変動させ、枕の上下位置を可変調節できるようにしたことを特徴とする伏臥位用枕に関する

【従来の技術】
従来伏臥位枕は、医療分野、殊に眼科における目の手術後に目に圧力を掛けずに長期間、場合によっては数週間にわたり回復を待つ必要がある場合の患者によって使用されてきた。

現代の人々は、一般的には仰臥姿勢で就寝するが、人類発達史の観点からは、ヒトは元来四足状態での活動が長く続いた後に二本足での立ち位置による活動姿勢を採るように発達したものであるから、ヒトが伏臥位で就寝し、睡眠をとることは身体構造的(骨格、臓器の配置等)に自然の理にかなったことといえよう。また仰臥位姿勢ばかりでなく、伏臥位姿勢をとることによって、内臓の働きを良くするであろう。欧米では伏臥位で就寝、睡眠する人の割合が多いといわれる。日本人についてみると、寝姿勢は仰臥位姿勢、側臥位姿勢が主で、伏臥位姿勢は少ないといえる。伏臥位での就寝睡眠姿勢が内臓、呼吸器官、その他の臓器に良い効果があることは、多くの研究成果として発表されてきている。また伏臥位就寝は、高齢化社会の到来とともに、寝たきり状態に原因がある褥瘡や下肢関節拘縮、その他多くの疾患の改善に役立つとの研究がなされ、老人医療、介護医療の分野、さらには一般医療の分野においても関心を集めている。

理学治療(はり、灸、マッサージ等)用の特別仕様ベッドは、専用器具として伏臥位姿勢をとるのに種々の工夫がなされているが、しかしながらこれらベッドは比較的短時間の治療に使用されるものであるから、もしもこれらのベッドの上で長時間にわたり伏臥位姿勢をとり続けたとしたならば、呼吸器官、内臓器官等への圧迫あるいはそれによる苦痛が生じるであろう。そのようなの理学治療用の特別仕様ベッドについては、呼吸器官、内臓器官等への圧迫を排除のために充分な設計上の配慮がなされているとは、必ずしもいえない。また一般の布団またはベッドについては、伏臥位姿勢での就寝あるいは睡眠に関して重要視されておらず、従って長時間にわたり伏臥位姿勢での快適な就寝あるいは睡眠を可能とするための特別な工夫が施されているとはいえない。すなわち、一般の布団またはベッド上で伏臥位姿勢をとれば、鼻が圧迫されあるいはつかえて呼吸もままならず、腰が伸びきってその附近の部位にストレスがたまり、長時間同じ姿勢をとり続けることができない。また顔を横に向けて伏臥位姿勢をとり続けると頚部の凝りや損傷の原因ともなり得る。

実開昭58−24269号公報には、略楕円形の空気入り柔軟浮き袋状の俯伏休息枕が開示されているが、この枕には剛性がないからこの枕が床面(ベッド面)よりも高い位置(elevated level)で使用されるべき時には、顔面部の重量を平均的に支えることができず、顔面を適正位置に維持することができないであろう。実開昭61−134369号公報には、伏臥位での顔面を傾斜面に保持できるようにした伏臥枕が記載されている。この枕は、台座板と顔面保持用逆U字形部材の両脚部とを軸回転可能に取付け、台座板と顔面保持用逆U字形部材とのなす角度を両者間のネジ式上下調節器で調節するものであるが、これを使用する場合には使用者の下向き顔面が斜めに持ち上げられ、頚部のところで頭部が後方へ折り曲げられた状態となり長時間の使用によって頚部に疲労が生じてしまうであろう。実開昭64−46030号公報には、床面よりも高い位置に架設された一対の上下位置調節可能な平行支持棒の間に張り渡された額支持部及び顎支持部を備えた顔支持台が記載されている。しかし顔面が額、顎の狭い部分で支えられるから顔面に対する単位面積当たりの圧迫が大きく、また支持部から顔面が移動し易く(縦方向の滑り移動、横方向の回転移動及び滑り移動等)、顔面保持が不安定となるから、同一姿勢をとり続けることは苦痛となるであろう。実開平2−91530号公報には、略逆U字形のスポンジ製本体部を有し、その中央凹スペース部に顔面を納めるうつぶせ用枕が開示されているが、それを床面よりも高い水準位置(level)で使用するための工夫は何ら施されておらず、そのような使用態様は認識されていない。実開平2−16162号公報には、胸部用枕と額用枕とを連結アーム手段で結合してなる伏臥位用枕が開示されている。この枕の胸部用枕の上辺にU字形の凹部が設けられていて、ここに伏臥位で顎を嵌め込むと、額用枕の上に額が支えられる構造となっている。この枕においても、使用者の下向き顔面が斜めに持ち上げられ、胸部から頚部にかけて後方へ折り曲げられた状態(やや仰け反る状態)となり長時間の使用によってそれらの部分に疲労が生じてしまうであろう。さらには、この場合も枕を床面よりも高い水準位置(level)で使用するための工夫は何ら施されておらず、そのような使用態様は認識されていない。実開平3−13125号公報には、アルミ製角形の密閉タンクの上面中央部に後頭部(仰臥位)または顔面部(伏臥位)を受け入れるための卵形陥没穴を設け、全面をレザー被覆してなる腰痛、猫背等の予防用の枕が開示されている。しかしながら、伏臥位を長時間続けた場合の頚部のストレス疲労を防止するための工夫はなされていないし、床面より高い位置で使用することも認識されていない。実開平4−80521号公報には、凹部または貫通孔からなる顔面収容部を有する頭部マット部材と、胸部を支えるための胸部マット部材とを備えた伏臥用マットが記載されているが、両部材は折り畳み可能に結合されており、その顔面収容部を有する頭部マット部材を床面より高い位置で使用するものではない。実開平7−7631号公報には、仰臥姿勢での網膜剥離患者用の頭部固定用枕が開示されている。この枕には合成樹脂製ビーズ小球が充填され、後頭部を固定するためのドーナッツ状の支持部及び左右側頭部を固定するための一対の彎曲状支持部が備えられている。このような構造では伏臥位姿勢での顔面を支持したときに、楽な呼吸ができない。ビーズが充填されているのであるから枕自体の自己支持性がなく、この枕を高い位置に配置しても、顔面を固定して安定に維持する機能を有しない。実開平7−36836号公報には、うつ伏せ用枕が記載され、この枕は、平らな頭部載置面より連接するスロープ状の胸受け部を有し、その頭部載置面の下のところに左右から人体の腕を挿入するためのトンネルを設けたことを特徴とする。しかしながら、顔面の安定維持のための工夫や胸部を圧迫せずに呼吸を楽にするための工夫、顔面下のスペース(殊に長期間にわたる治療目的に便宜なスペース)の確保の重要性については認識されていない。実開平10−117905号公報には、うつ伏せ寝用枕が開示されており、このものは額支持ブロックと顎支持ブロックとを顔面を支持するのに適当な距離で有しており、両ブロックの間に顔面を圧迫しない深さの凹状顔面周囲空間部を形成したものである。しかしながら、顔面を安定に維持するための工夫はなされておらず、支持部から顔面が移動し易く(縦方向の滑り移動、横方向の回転移動及び滑り移動等)、顔面保持が不安定となるから、長時間の伏臥位姿勢をとり続けることは使用者に疲労をもたらすであろう。また使用者の下向き顔面が持ち上げられ、頚部のところで頭部が後方へ折り曲げられた状態となり長時間の使用によって頚部に疲労が生じてしまうであろう。特開2000−51035号公報には、逆U字形ブロックの凹部に張り渡した額載せ用バンドを備えた構造の医療用枕が開示されている。この枕では比較的幅の狭いバンドによって額が支えられるので、額への圧迫が大きく長時間の使用には適さないであろう。特開2000−93276号公報には、人体頭部を受け容れるのに適度な間隔の一対の平行な水平梁(棒)部材に懸架取付けられた布またはベルト状のブランコ様の柔軟支持部材に、中央部に顔面支持孔を備えた弾性材料製矩形枕本体を載置してなる医療用枕が記載されている。

【発明が解決しようとする課題】
かくして、本発明の課題の一つは、改善された伏臥位用の枕を提供することである。

疾病の治療の際、殊に眼科手術後に目に圧力を掛けずに長期間にわたり回復を待つ必要がある場合、伏臥位用の枕を使用する必要が有り、鼻、口(歯茎)、顎を優しく支えると共に、楽な呼吸の確保、顔面への均等かつ柔軟な圧力の維持、正常適切な顔面角度、方向の維持、鼻水や痰等の排除の容易性、顔面清浄化の容易性、顔面掻痒容易性等が達成されなければならないが、この要求に応えるためには枕は床面(ベッド面)よりも高い位置になるようにして、手、好ましくは両手の差込み及び所要の動作を可能とするのに充分なスペース(間隔)を設けることが必要である。しかしこのようにすると床面よりも高い位置となる枕と敷き布団(またはベッド)面との間の垂直方向のギャップ(落差あるいは乖離)が必然的に生じてしまい、使用者の疲労を誘い、長時間ないしは長期間の使用に耐えられない状態となるから、このような伏臥位用の枕と組合わせて、上記ギャップを解消して楽に伏臥位姿勢がとれるようにする伏臥位用マットレスを開発する必要がある。従って、本発明の第2の重要な課題は、伏臥位用枕と組合わせて使用される伏臥位用マットレスを提供する。

【課題を解決するための手段】
本発明の第1の態様によれば、伏臥位で横たわった際の人体頭部顔面を下向きに保持するための伏臥位用枕にして:
(イ)平面で見て概略馬蹄形をなす長袋状の柔軟性外袋(1)の内側に対応する立体形状の軟質発泡樹脂成形物を挿入充填配置し、あるいは該柔軟性外袋(1)内側に枕用粒状充填物を充填してなる枕本体と;
(ロ)その枕本体の捻じれ、広がり、その他の変形を防止してその馬蹄形を保持して顔面を安定に収受し当接させるようにするために、該枕本体の輪郭線のやや内側に内包される大きさでありかつ該枕本体に略相似する形状の剛性支持体(5)を該柔軟外袋(1)に装着係止する目的で、該柔軟性外袋(1)の下面側に設けられた複数の帯状部材(2、3、4)と;
を備えた伏臥位用枕が提供される。

本発明の枕本体の平面形状は、略馬蹄形ないしはU字形あるいはそれらに類似する形状であって、内側の透孔部の大きさは使用者の下向き顔面を安定に心地よく支持するのに適当な大きさであり、使用者の顎部がその平面形状開口部附近に当接するようにして使用される。このような枕は、顔面を安定に保持し続けなければならないから、使用中に通常遭遇する程度の外力によって簡単に捻じれたり、広がったり、その他の変形を受けてはならない。従って、本発明の枕においては、枕本体の輪郭線のやや内側に内包される大きさでありかつ枕本体に略相似する形状の剛性の支持体を枕の柔軟外袋に装着係止する。そのために、枕をなす柔軟性外袋の下面側に複数の帯状部材を設けて、上記剛性支持体を取外し自在に、しかも確実に固定する。使用者の顔面は、枕の外袋の中の厚みのある充填材あるいはクッション材で隔てられてこの剛性支持体に対するので、顔面が支持体の硬さや凹凸した感じを受けることはない。

その剛性支持体は、典型的には金属パイプ材や金属棒材を折り曲げ加工し、必要に応じて溶接加工し、添付図面に例示するように、平面においては枕と相似な形状の枠構造とする。このような支持体を枕の下面側に装着係止するための帯状部材は、例えば幅広のベルト通し(belt loop)の形態、ポケット形態、マジックテープ(登録商標)(ナイロン製付着テープ、例:Velcro(R))付きの止め帯の形態、あるいはスナップボタン(凹型と凸型のスナップ式のボタン)付きの止め帯の形態等とすることができる。

本発明によれば、装着高さ及び傾斜角度調節可能な伏臥位用枕も提供される。このタイプの枕は医療用として特に適しており、また好ましくは、以下に説明される本発明による伏臥位用マットレスと都合よく組合わせて実用に供されうる。

このような高さ及び傾斜角度調節可能な伏臥位用枕は、該枕本体に略相似する形状の枠構造剛性支持体(5)の両肩部分及び両先端部分に対応する4つの位置のそれぞれにネジ式上下位置調節具(6、7及び8’、9’)を垂直に備えた台座(10)の上に前記本発明の該支持体付き伏臥位用枕を載せ、安定保持のために該両肩部の上下調節ネジ(6、7)に対してそれぞれのネジの頂部附近に設けられた鉤型係合溝(6’、7’)内へ枠構造支持体(5)をその枠構造材の軸に関して回転可能に係合した状態で支持し、該両先端部においては上下調節ネジ(8’、9’)の頂部に設けた水平回転自在の座板(8、9)を介して支持体(5)の対応先端部をそれぞれ支持するようにした構造である。すなわち伏臥位用枕は、4本の柱状の上下調節ネジ(6、7及び8’、9’)の先端部に略水平な状態に架設保持される。このようなネジ式上下位置調節器具の採用によって、枕の上下位置ならびに枕の仰伏角度、左右傾斜角度が容易に調整設定できる。

さらに本発明によれば、伏臥位での枕使用者の顔面を床面より上へ上昇させて床面と顔面との間に空間を生じさせるようにする脚部(12)を一体化して備えるように折り曲げ溶接加工した金属パイプまたは金属棒製の該相似形状の枠構造の支持体(11)を、該複数の帯状部材(2、3、4)によって該柔軟性外袋(1)の下面側に装着係止して備えた前記伏臥位用枕も提供される。この枕の変形態様として、伏臥位での枕使用者の顔面の縦方向傾斜度(仰伏角度)を一定の角度位置に固定すること無く所望により別の角度位置へ可変とするため枕使用者の顔面の縦方向において枕を揺動し得るようにするため該脚部(12)を安楽椅子式彎曲脚部とした伏臥位用枕も提供される。

本発明のさらに別の態様によれば、伏臥位で横たわった際の人体頭部顔面を下向きに保持するための伏臥位用枕にして:
(イ)平面で見て概略馬蹄形をなす長袋状の柔軟性外袋(1)の内側に対応する立体形状の軟質発泡樹脂成形物を挿入充填配置し、あるいは該柔軟性外袋(1)内側に枕用粒状充填物を充填してなる枕本体;
(ロ)その枕本体の捻じれ、広がり、その他の変形を防止してその馬蹄形を保持して顔面を安定に収受し当接させるようにするために、該枕本体の輪郭線のやや内側に内包される大きさでありかつ該枕本体に略相似する平面形状を備え、該柔軟性外袋(1)の下面側に係止付着される板状剛性支持体(5’);
(ハ) 伏臥位での枕使用者の顔面を床面より上へ上昇させて床面と顔面との間に空間を生じさせるようにするため該剛性支持体(5’)を昇降調節具を介して支持するために床面に配置される、略H字形平面形状を有するソール(15);を備えた伏臥位用の上下位可変顔面枕であって:

両端に同一方向に直角に折り曲がったカギ形端部(a,b,a’,b’)及び中央部にそれらのカギ形端部と同一方向に穿った貫通孔(c)をそれぞれ有する2本の金属棒材または管材からなる支持桿(26、26’)を、全てのカギ形端部(a,b,a’.b’)が同一方向を指すように貫通孔(c)を中心にして交差組合せ、両者を上記貫通孔を通る鋲状材(27、例えばリベット、ねじ等)によって、その鋲状材の軸を支点として洋バサミ様に回動自在であるように固定組み付けた昇降具アッセンブリーから主として構成され、さらに、
ソールのH字横棒右交差部(または左交差部)附近の上面に一方の支持桿(26)のカギ形端部(a)を回動自在に挿入固定するために設けられた軸受け部材(28);ソールのH字横棒左交差部(または右交差部)附近の上面に該支持桿(26)のカギ形端部(b)を着脱可能に噛み合わせ係止するために設けられた複数の歯(29、29…)を有するラック;
枕本体用の剛性支持体の裏面の左肩部(または右肩部)の、上記軸受け部材の位置に対応する位置に、他方の支持桿(26’)のカギ形端部(a’)を回動自在に挿入固定するために設けられた軸受け部材(28’);を備え、

(i)伏臥位の人体の胸部乳房に当接する部位に胸部乳房に対する圧迫を軽減し、胸式呼吸を容易化あるいは助長するための一対の眼鏡状のそれぞれ概略円形状の窪み(19);及び
(ii)伏臥位の人体の腹部に当接する部位に腹式呼吸を容易化あるいは助長し、内臓への圧迫を軽減するために、マットレスの末端にまで達する概略砲弾形状の窪み(20+22)(この窪みの形状はマットレスの平面における形状であり、滴形状とも表現できる。);
を設け、人体の胴体部を支えるに足りる長さを有する、上記伏臥位用軟質発泡樹脂マットレスが提供される。このマットレスは、長さ方向において略半分に二分割(X、Y)した形とし、個々の使用者の身長値に対応して両分割部分(X、Y)を適切な間隔で配置組合わせて併用できる。長時間ないし長期間にわたり足首部分を伸ばしたまま伏臥位をとり続けると、疲労はもちろんであるが、尖足の発−生を見ることがある。従って、伏臥位用軟質発泡樹脂マットレスには、伏臥位使用者の足首部分を折り曲げた状態で支持して、足首部の伸びきりによる疲労を予防し、尖足の発生を防止するために伏臥位使用者の足首部分を折り曲げた状態で支持する横断面が山形の脚部支持マットレス部分(Z)を分離し、または結合して備えるのが好ましい。
