【発明の詳細な説明】
【０００１】 （技術分野） 本発明は、独立請求項１、３７および３８の前文に定めるような、骨の、特に
脊椎、融合のための装置、とりわけ骨または特に脊椎を融合するための移植片用
の末端部材に関する。
【０００２】 （背景技術） 骨欠陥の処置と取り扱いは、整形外科の最も挑戦的な局面の一つである。骨欠
陥は臨床的状況の幅の広い種類で生じる。骨欠陥を有する一切の長骨の自然な解
剖学的長さと形状を修復することは問題がある。更に、脊椎を取り除かなければ
ならないときはいつも、脊柱の自然の長さと曲線を修復し、並びに骨融合を促進
するために脊椎スペーサーを挿入することが必要である。脊椎の構造と安定性を
修復し、骨融合を促進することは、コーペクトミー（ｃｏｒｐｅｃｔｏｍｙ）、
一つまたはいくつかの脊椎の部分が取り除かれる外科手術の手法、においてより
困難でさえある。
【０００３】 多くの異なった移植片が骨欠陥と共に用いるために開発されている。いわゆる
「メッシュ移植片」が特に効果があることを示している。このような移植片の一
つは、ペンシルバニア州パオリのＳｙｎｔｈｅｓ（ＵＳＡ）から入手可能なＳｙ
ｎＭｅｓｈTMである。ＳｙｎＭｅｓｈTMはチタンのメッシュの円筒で、これは多
数のこの表面に分散した均一な大きさの、むらなく配置した開口を備えている。
骨欠陥が生ずる臨床的状況へ向けるために、ＳｙｎＭｅｓｈTMは様々な長さと直

【０００４】 メッシュ移植片と同種移植片の臨床的な結果は一般に前向きであるが、一つの
可能性のある臨床的な難しさは移植片の存続である。これらが接触する骨の中へ
移植片の端が過度に沈み込むことは、骨が極度の多孔性であるときはいつも特に
厄介である。機械的および生理学的観点から、移植片と骨の間の表面接触面積を
増すこともまた有利である。Biedermann et al.へのU.S. Patent No. 5,702,451
は、これらの関心事へ向けようと試みる末端の輪を開示している。しかしながら

【００１５】 図１と２の説明に戻り、上表面１８の表面積は骨融合移植片２２の上面と底の
面の表面積より大きい。この大きな表面積のため、移植片２２の上面と底にある
であろうより多くの、上表面１８と周囲の骨との間の接触面積がある。接触面積
の増加は、移植片２２が存続に耐えるのに役立つ。この増加した接触面積は、末

【００１６】 代りに、図７に示すように、上表面１８は中実の表面であることができ、上表
面１８と周囲の骨との間の接触面積を最大とする。中実の上表面は、陥没が特に
関心事である臨床的状況で望ましいであろう。
【００１７】 図３は本発明に係る末端部材１１０の第二の実施態様を示す。一般に、末端部
材１１０の構造の大部分（並びに以下に記載する実施態様）は末端部材１０の構
造と同様または類似であり、従って同じ参照番号が同様の成分に用いられ、これ
らの同様な成分の議論は必要ないと信じる。末端部材１１０は楕円形または長方
形の形状を有し、周囲の骨（および従って骨融合移植体）が、実質的に楕円また
は長方形である状況で用いられるであろう。この末端部材が末端部材１１０のよ
うに非対称の形状を有するとき、第一の溝２０をいかなる方向でも提供できる。
例えば、第一の溝２０は末端部材１１０の長軸に沿って走るように示されている
が、周囲の骨に関して異なって方向付けられるいかなる方向にも走ることができ
る。第一の溝２０の配置での変え易さは、第一の溝２０を、用いる特定の外科手
術の入口として最も適した位置とできることを意味する。特に、もし末端部材１
１０が短軸を前方／後方向に移植されれば、その場合は第一の溝２０は側方の外
科手術の入口として理想的である。もし前方のまたは後の入口が期待されるなら
、そのときは第一の溝２０は前方−後方向へ走るべきである。
【００１８】 図４ａは、末端部材２１０の長軸に関しある角度で走る第一の溝２０を有する
末端部材２１０を示す。第一の溝２０のこの方向は前横の外科手術の入口として
好まれ、この入口は脊柱の外科手術で用いられる。図４ｂは末端部材２１２を示

示し、これはまた第一と第２の溝２０、２１を含む。第一の溝２０は末端部材２
１４の長軸に平行に走り、第二の溝２１は末端部材２１４の長軸を横切って走る
。末端部材２１４上に第一と第二の溝２０、２１の両方を含むことは、末端部材
２１４の移植配向または外科手術の入口に関係なく、少なくともの一つの第一と
第二の溝２０、２１への利用を提供する。
【００１９】 図４ａ、４ｂおよび４ｃに示された末端部材のそれぞれに、上表面１８は多数
の歯３２を有し、これは端部材と周囲の骨の間の機械的かみ合いを提供する。歯
３２は骨に侵入することにより機械的かみ合いを提供する。歯３２により与えら
れた初期の機械的安定性は、末端部材の術後の引き抜けまたは排除の危険を最小

【００２０】 図５に見られるように、末端部材３１０は楔状輪郭を有する。楔状輪郭は脊柱
またはいかなる骨の自然の湾曲を回復するために脊柱の用途に有用となる。脊柱
またはいかなる骨の自然の湾曲も回復する。楔状輪郭はまた、病理学、臨床的状
況、または間違って骨を除去したことにより影響される垂直でない切骨術切断を
補償するために役に立つであろう。骨融合移植変のそれぞれの端に通常用いられ
る末端部材として、異なった楔状輪郭を有する末端部材を一緒に用いることがで
きることに気付くべきである。この楔状形状を得る一つの方法は、第一の部分で
の第一の端３４から第二の端３６への厚さの除々の減少の結果により生じ、その

Claims (38)

    【特許請求の範囲】
  1. 【請求項１】 Ａ）骨、特に脊椎の終板に接触するための縦軸（２７）と上表面（１８；８１２
    ）を有する第一の部分（１２）； Ｂ）移植片の端に取り付ける形状とし寸法とした、縦軸（２７）へ同軸な第二の
    部分（１４）； Ｃ）前記第一と第二の部分（１２；１４）を結びつけ、該第二の部分（１４）が
    前記移植片に取り付けられたとき、該移植片の端で支えられる大きさとした肩（
    １６）、 を含む骨、特に脊椎の融合のために移植片と共に用いる末端部材であって、 Ｄ）前記上表面（１８）が外科手術器具を受取るための第一の溝（２０）を有す
    る、 ことを特徴とする末端部材。
  2. 【請求項２】 前記第二の部分（１４）を、移植片の穴へ挿入される形状と
    寸法としたことを特徴とする請求項１に記載の末端部材。
  3. 【請求項３】 前記第二の部分（１４）が、移植片の端を受取る形状と寸法
    としたスリーブ（８１４）を含むことを特徴とした請求項１に記載の末端部材。
  4. 【請求項４】 前記上表面（１８；８１２）が、外科手術器具を受取るため
    の第二の溝（２１）を有することを特徴とする請求項１から３のいずれか一項に
    記載の末端部材。
  5. 【請求項５】 前記溝（２０）が、第一の部分（１２）を実質的に縦軸（２
    ７）へ直角に貫通することを特徴とする請求項１から３のいずれか一項に記載の
    末端部材。
  6. 【請求項６】 前記第二の溝（２１）が、第一の部分（１２）を本質的に縦
    軸（２７）へ直角に貫通することを特徴とする請求項４または５に記載の末端部
    材。
  7. 【請求項７】 前記第一の溝（２０）が、上表面（１８；８１２）へ向かっ
    て開いていることを特徴とする請求項１から６のいずれか一項に記載の末端部材
    。
  8. 【請求項８】 前記第二の溝（２１）が、上表面（１８；８１２）へ向かっ
    て開いていることを特徴とする請求項４から７のいずれか一項に記載の末端部材
    。
  9. 【請求項９】 前記第一の溝（２０）が、縦軸（２７）に関して対称に形成
    されていることを特徴とする請求項１から８のいずれか一項に記載の末端部材。
  10. 【請求項１０】 前記第二の溝（２１）が、縦軸（２７）に関して対称に形
    成されていることを特徴とする請求項４から９のいずれか一項に記載の末端部材
    。
  11. 【請求項１１】 前記第一の溝（２０）が、第一の部分（１２）の横断面の
    ２０％と８０％の間を覆うことを特徴とする請求項１から１０のいずれか一項に
    記載の末端部材。
  12. 【請求項１２】 前記第二の溝（２１）が、第一の部分（１２）の横断面の
    ２０％と５０％の間を覆うことを特徴とする請求項４から１０のいずれか一項に
    記載の末端部材。
  13. 【請求項１３】 前記第一の溝（２０）が、第二の溝（２１）に関してある
    角度で走ることを特徴とする請求項４から１２のいずれか一項に記載の末端部材
    。
  14. 【請求項１４】 前記第一の溝（２０）が、第二の溝（２１）に平行に走る
    ことを特徴とする請求項４から１２のいずれか一項に記載の末端部材。
  15. 【請求項１５】 前記上表面（１８；８１２）が更に、骨とかみ合うための
    多数の歯（３２）を含むことを特徴とすることを特徴とする請求項１から１４の
    いずれか一項に記載の末端部材。
  16. 【請求項１６】 前記歯（３２）がピラミッド形状を有することを特徴とす
    る請求項１５に記載の末端部材。
  17. 【請求項１７】 前記上表面（１８；８１２）が中実であることを特徴とす
    る請求項１から１６のいずれか一項に記載の末端部材。
  18. 【請求項１８】 前記上表面（１８；８１２）が環状であることを特徴とす
    る請求項１から１７のいずれか一項に記載の末端部材。
  19. 【請求項１９】 前記上表面（１８；８１２）が円形であることを特徴とす
    る請求項１から１７のいずれか一項に記載の末端部材。
  20. 【請求項２０】 前記上表面（１８；８１２）が楕円であることを特徴とす
    る請求項１から１７のいずれか一項に記載の末端部材。
  21. 【請求項２１】 前記上表面（１８；８１２）が長方形であることを特徴と
    する請求項１から１７のいずれか一項に記載の末端部材。
  22. 【請求項２２】 前記上表面（１８；８１２）が平らな平面の表面であるこ
    とを特徴とする請求項１から２１のいずれか一項に記載の末端部材。
  23. 【請求項２３】 前記末端部材が楔形状の輪郭を有することを特徴とする請
    求項１から２２のいずれか一項に記載の末端部材。
  24. 【請求項２４】 前記第一の部分（１２）が第一の端（３４）と第二の端（
    ３６）を有し、該第一の端の高さが第二の端の高さより大きく、楔形状の輪郭を
    作ることを特徴とする請求項２３に記載の末端部材。
  25. 【請求項２５】 前記末端部材が、前記移植片へしっかりと固定できること
    を特徴とする請求項２４に記載の末端部材。
  26. 【請求項２６】 前記末端部材が、前記移植片と緩めることができるように
    連結可能であることを特徴とする請求項２５に記載の末端部材。
  27. 【請求項２７】 前記第二の部分（１４）が留め具を受取る穴（２４）を含
    み、前記末端部材を前記移植片へ固定することを特徴とする請求項２６に記載の
    末端部材。
  28. 【請求項２８】 前記第二の部分（１４）の外部表面が、前記移植片の開口
    に適合する大きさの少なくとも一つの突起部（２８）有し、前記末端部材を移植
    片へ固定することを特徴とする請求項１または２に記載の末端部材。
  29. 【請求項２９】 前記第二の部分（１４）が多数のタブ（４０）を含み、前
    記末端部材を移植片へ固定することを特徴とする請求項１または２に記載の末端
    部材。
  30. 【請求項３０】 前記多数のタブ（４０）のそれぞれが弾性であり、第二の
    部分（１４）を移植片に挿入時に内側へ曲がり、前記末端部材を移植片へ固定す
    るために外側へ曲がり戻ることを特徴とする請求項２９に記載の末端部材。
  31. 【請求項３１】 前記末端部材が移植片と同じ材料で作られていることを特
    徴とする請求項１から３０のいずれか一項に記載の末端部材。
  32. 【請求項３２】 前記末端部材が同種移植片の骨から作られていることを特
    徴とする請求項１から３０のいずれか一項に記載の末端部材。
  33. 【請求項３３】 前記第一の部分（１２）へ回転可能に連結した突起部（９
    ５２）、前記第二の部分（１４）のねじ山を切った穴（９５６）およびねじ山を
    切った穴へねじ止め可能に挿入できる固定ねじ（９５４）を更に含み、ここでね
    じ山を切った穴（９５６）へ固定ねじ（９５４）の挿入が突起部（９５２）の外
    側への動きを引き起こし、前記末端部材を移植片へ固定することを特徴とする請
    求項１または２に記載の末端部材。
  34. 【請求項３４】 前記第一と第二の部分（１２；１４）がそれぞれ二つの部
    分（８１６；８１８）を含み、これらがスリーブ（８１４）により移植片の端を
    受取るのを容易にするのための開いた位置および移植片へ末端部材を固定するた
    めの閉じた位置、並びに末端部材を閉じた位置へ固定するための固定機構の間で
    回転可能であることを特徴とする請求項３から３３のいずれか一項に記載の末端
    部材。
  35. 【請求項３５】 前記固定機構が、お互いに協力する第一の鋸歯状の刃（８
    ２２）と第二の鋸歯状の刃（８２４）を含むことを特徴とする請求項３４に記載
    の末端部材。
  36. 【請求項３６】 前記第二の部分（１４；９１０）が、傾斜のついた表面を
    持つ、とがった部分（９２０）を有する外側の輪（９１４）および外側の輪（９
    １４）に関して回転可能であって少なくとも一つの突起部（９１８）を含む内側
    の輪（９１２）を含み、この突起部（９１８）が回転時にとがった部分（９２０
    ）の傾斜のついた表面に接触し、これによりとがった部分（９２０）を半径方向
    に外側へ押し出すことを特徴とする請求項１から３２のいずれか一項に記載の末
    端部材。
  37. 【請求項３７】 骨融合の移植片と共に用いるための末端部材であって Ａ）大きさと形状で、骨または特に脊椎の終板に一致する上表面（８１２）；お
    よび Ｂ）該上表面（８１２）から伸び、移植片の末端を受取る形状と大きさとしたス
    リーブ（８１４） を含む骨融合の移植片と共に用いるための末端部材であって、 Ｃ）前記末端部材が更に、第一と第二の部分（８１６；８１８）であってこれら
    の間で回転可能である部分とスリーブ（８１４）により移植片の端を受取るのを
    容易にするための開いた位置と該末端部材を移植片へ固定するための閉じた位置
    、および閉じた位置で該末端部材を固定するための固定機構を含む ことを特徴とする前記末端部材。
  38. 【請求項３８】 Ａ）傾斜のついた表面を持つ、とがった部分（９２０）を有する外側の輪（９１
    ４）；および Ｂ）該外側の輪（９１４）に関し回転可能で、少なくとも一つの突起部（９１８
    ）を含む内側の輪（９１２）で、この突起部（９１８）は回転時にとがった部分
    （９２０）の傾斜のついた表面へ接触し、これによりとがった部分（９２０）を
    半径方向外側へ押し出す、 ことを特徴とする骨融合の移植片と共に用いるための末端部材。
