Abstract

【発明の分野】
本発明は一般に双極式電気外科手術装置において使用するための改善した双極
式電極構成に関し、特に、一定の高さの組織スロット部の周囲に配列された電極
を含む双極式電極構成に関する。

【発明の背景】
ＲＦ（高周波）電気は外科処理において組織を焼灼および凝固するために数十
年間にわたって使用されてきた。ＲＦエネルギーを組織に供給するために使用さ
れる装置は概ね２種類、すなわち、単極式（mono-polar）および双極式（bipola
r)に分けられる。双極式電気外科手術装置はその手術装置内に活動電極および帰
還電極を内蔵しており、これらの電極の間に配置される組織に対する電流の流れ
を概ね制限する。一方、単極式電気外科手術の場合は、帰還電極が患者の体の外
側すなわち患者の皮膚上に置かれる。それゆえ、単極式電気外科手術装置の場合
は、活動または処理電極から患者の体内を通って帰還電極に電流が流れる。これ
らの単極式および双極式の電気外科手術は共に、少なくとも部分的に、組織を処
理（例えば、焼灼および／または切断）するための抵抗による発熱に依存してい
る。すなわち、電流が組織内を流れ、組織の電気的な抵抗が熱の形で電力を消散
させる。組織の温度が上昇するにつれて、電気抵抗を含むその諸特性が変化する
。組織の温度が約６７℃乃至７０℃に到達すると、凝固が始まる。さらに付加的
なエネルギーが組織の基質の基本的構成を形成している組織コラーゲン内に消散
すると、この組織が破壊して「融解（melt）」する。コラーゲンが破壊し始める
と、組織が凝固し始める。それゆえ、コラーゲンの破壊が開始する時に組織を圧
縮すると、圧縮された組織層が融合して血管の近傍を密封する。さらに、組織温
度が１００℃に到達すると、大半の流体（水を含む）が組織から消失または蒸発
して、組織が完全に乾燥して組織の電気的な抵抗が相当に増加する。その後、こ
のように乾燥した組織は僅かな処理で切断または分離できるようになる。エネル
ギーが組織内に消散する速度は組織の電気抵抗および組織内を流れる電流密度を
含む多くのファクターに依存する。電気外科手術装置は一般に種々の組織の治療
に用いられるように構成されているので、電流密度はその構成における重量な配
慮点であり、特に双極式電気外科手術装置の場合には、電流密度は特定の組織種
の場合に、装置電極の数、寸法、形状および配置の関数として決まる。
【０００３】 多くの外科的用途において、組織を切断および／または凝固する手段として双
極電気エネルギーを使用することが望ましい。一般に、双極式電気外科手術装置
の場合に、電流の流れを装置内の組織に制限して、装置の近傍の組織に対する（
電流が流れる）程度を相当に低くすることが望ましい。従来技術の双極式電気外
科手術装置の場合は、一般に、電気外科手術エネルギーの供給の前に組織を把持
する装置を構成することによりこれらの目的を達成していた。このような双極式
の電気外科手術装置は当該技術分野において周知であり、特に、組織の切断の前
または当該切断の処理中のいずれかにおいて組織を凝固させる外科手術装置につ
いて多くの構成が示唆されている。このような装置の大半において、電気外科手
術エネルギーの供給の前に組織に対して圧力を加える顎部により組織がまず把持
される。このような装置においては、この把持用の顎部が電気外科手術エネルギ
ーを供給する電極を構成しているか含んでいるが、一部の構成においては、１個
以上の電極が、例えば、切断要素等の装置における別の要素に内蔵可能な場合も
ある。それゆえ、このような双極式電気外科手術用の把持装置においては、処理
される組織がまず把持され、その後に、電気外科手術エネルギーが電極により供
給され、次に、組織が切断または分離され、さらに、組織が放出されて、把持装
置が新しい組織に移動してこの処理が繰り返される。このような処理は多くの外
科処理において極めて効果的であるが、腸間膜組織のような特定の種類の組織に
作用する場合に、装置を組織内で移動しながら継続的に（組織を）把持して放出
するのに相当な手間と時間がかかる場合がある。しかしながら、腸間膜組織のよ
うな組織は脈管組織であって、血管が適当に密封処理されていないと出血するの
で、切断線の両側の組織内の血液を組織の分離前に完全に凝固させることを確実
に行うことが重要である。さらに、最近の外科手術処理の多くは極めて小さな空
間内で行われるので、電気外科手術エネルギーの供給処理の後毎に開口する必要
がある顎部を備えている装置を使用するための十分な余裕がない場合がある。
【０００４】 それゆえ、組織内を継続的に移動しながら組織を凝固および切断するのに適し
た双極式電気外科手術エンド・イフェクタを構成することが有利である。さらに
、組織内にエンド・イフェクタを継続的に移動させながら脈管構造を含む組織を
凝固および切断するのに適した双極式電気外科手術エンド・イフェクタを構成し
、この場合において、凝固領域が顎部組立体の幅に概ね制限されることが有利で
ある。さらに、電気外科手術エンド・イフェクタが組織内を移動している時に組
織を継続的に受容し、凝固させ、さらに、凝固した組織を分離するのに適した双
極式電気外科手術エンド・イフェクタを構成することが有利である。さらに、組
織を継続的に受容し、凝固させ、さらに、組織を分離するのに適した双極式電気
外科手術エンド・イフェクタを構成し、この場合に電気外科手術用の電流が概ね
自己制限的に組織内を流れることが有利である。

【発明の概要】
本発明は医療装置において使用するための双極式電気外科手術エンド・イフェ
クタに関する。本発明による双極式エンド・イフェクタは、第１の組織表面部と
、当該第１の組織表面部の第１の面上の第１の細長い電極と、第１の組織表面部
の第２の面上の第２の細長い電極を備えている。この第２の電極は第１の電極に
対してほぼ平行である。本発明による双極式エンド・イフェクタは、さらに、第
２の電極から第１の電極を分離する第１の中央絶縁領域と、第１の電極および第
２の電極から第２の組織表面部を分離する組織スロットと、第１の組織表面部お
よび第２の組織表面部の間に配置された組織分離装置を備えている。この組織分
離装置は、一般に、当該組織分離装置の先端部における分離端部と、当該分離端
部から上記第１の面に向けて基端側に延出する第１の組織ガイドと、分離端部か
ら上記第２の面に向けて基端側に延出する第２の組織ガイドを備えている。

対の位置に配置されており、上記第２の電極が上記第４の電極に対してほぼ正反
対の位置に配置されている。加えて、本発明の実施形態は双極式電気外科手術エ
ンド・イフェクタを備えており、上記第１の電極および第２の電極が電気的に接
続しており、上記第２の電極および第３の電極が電気的に接続している。

【０００９】 ［発明の詳細な説明］ 図１は本発明による双極式電気外科手術装置１０の側面図である。この図１に
示す本発明の実施形態において、双極式電気外科手術装置１０はハンドル１２、
エクステンダー・チューブ１４、顎部組立体１６および電力コード１８を備えて
いる。図２は双極式電気外科手術装置１０の側面図であり、電気外科手術用発生
装置２０およびフット・スイッチ２２が概略的な形態で含まれている。電気外科
手術用発生装置２０は発生装置用電力コード２５を介して交流ウォール・プラグ
２４に接続できる。例えば、発生装置２０はモデルＩＣＣ３５０としてERBE Ele
ktromedizin GmbH社から販売される発生装置のようなＲＦ電気外科手術用の発生
装置とすることができる。この電気外科手術用発生装置２０はフット・スイッチ
・コード２３により当該発生装置２０に接続できる従来のフット・スイッチ２２
により制御および駆動できる。さらに、この電気外科手術用発生装置２０は電力
コード１８により双極式電気外科手術装置１０に接続でき、このコード１８は双
極電気外科手術エネルギーを双極式電気外科手術装置１０に供給するように構成
されている。
【００１０】 図３は双極式電気外科手術装置１０のエンド・イフェクタ３６の斜視図であり
、第１の電極組立体３０、第２の電極組立体３２、中央絶縁装置組立体３４、組
織分離装置４０および組織スロット５０を含んでいる。組織分離装置４０は、図
示の実施形態のように、好ましくは、中央の楔形状部分により構成されており、
当該楔形状部分は先端部４２、第１の楔形壁部４４および第２の楔形壁部４６（
図１０において見ることができる）を備えている。本発明による組織分離装置の
実施形態は外科的に鋭くて組織を裂くことなく健康な組織を切り込むのに十分に
鋭い先端部４２を備えているが、特定の場合においては、先端部４２が外科的に
鈍くて健康な組織を容易に切断できないが電気外科手術的に処理された（例えば
、完全に乾燥した組織）組織を切断するのには十分に鋭い組織分離装置を利用す
ることが好ましい場合がある。図４は第１の電線２６および第２の電線２８を含
むエンド・イフェクタ３６の斜視図である。これらの第１の電線２６および第２
の電線２８は電力コード１８に電気的に接続されて、電気外科手術用発生装置か
らエンド・イフェクタ３６に電気外科手術エネルギーを伝達するように構成され
ている。第１の電線２６は第１の導電体２７を備えており、第２の電線２８は第
２の導電体２９（図５において見ることができる）を備えている。第１の導電体
２７は、例えば、当該第１の導電体２７を第１の電極組立体３０に銀はんだ付け

第２の電極６２を備えており、第２の電極組立体３２は第３の電極６４および第
４の電極６６を備えている。

断面図であり電極組立体の極性および間隔を示している。図１１において、電気
的に共通の電極は同一の極性標識により識別されている。従って、支持部材３３
を介して電気的に接続している電極６４および６６は共に正の符号で標識されて
おり、支持部材３１を介して電気的に接続している電極６０および６２は共に負
の符号で標識されている。電極６４および６６は、双極式電力コード１８により
電気外科手術用発生装置２０の第１の出力に接続している第２の電線２８に接続
している。一方、電極６０および６２は、双極式電力コード１８により電気外科
手術用発生装置２０の第２の出力に接続している第１の電線２６に接続している
。これらの電極６０，６２，６４および６６は導電性の材料により構成されてい
るが、中央絶縁装置６８および７８は非導電性の材料により構成されている。本
発明のこの実施形態において、エンド・イフェクタ電極の作用部分（すなわち、
内部接触表面部）は概ねＣ字形状に構成できる。電線２６および２８を電極組立
体３０および３２にはんだ付けした後に、電極組立体３０および３２が絶縁装置
組立体３４と共に重ね合わされてエンド・イフェクタ３６が形成される。その後
、楔形状の組織分離装置４０が先端部４２を絶縁装置組立体３４の中心に配置し
た状態で組織スロット５０の後ろ側の中に傾斜して組み込まれる。なお、組織分
離装置４０は楔形状を有するものとして図示および説明しているが、組織スロッ
ト５０の中心部の中にほぼ配置可能な細長い部分を含む多くの別の形状が当該組
織分離装置４０を形成するのに適し得る。さらに、組織を２部分に分離するよう
に作用する別の適当な形状は当該技術分野における熟練者において明らかである
。

あり、Ｗe は電極接触表面部の幅であり、Ｗi は電極接触表面部を分離する中央
絶縁装置組立体３４の部分の幅である。特にＷi は組織スロット５０の長さに沿
う電極接触表面部７０，７４および電極接触表面部７２，７６の間の公称距離で
ある。なお、このＷi は電極間の誘電破壊を防ぐのに十分な大きさであることが
重要である。さらに、組織が乾燥する速度はこの幅Ｗi により制御することがで
き、Ｗi が大きいほど装置が組織を乾燥する速度が遅くなり、Ｗi が小さくなっ
て約０．０２０インチの最適な最小値に至るまで装置１０による組織の乾燥速度
が高まる。このＷi は組織スロット５０の長さに沿ってほぼ一定であるのが好ま
しく、この理由は、電極６０が組織スロット５０の長さに沿って電極６４に対し
てほぼ平行であるのが好ましく、電極６２が組織スロット５０の長さに沿って電
極６６に平行であるのが好ましいからである。また、スロットの高さＷG は組織
スロット５０の全長に沿って一定であるのが好ましく、この理由は、接触表面部
７０および７４が組織スロット５０の全長に沿って接触表面部７２および７６に
対してそれぞれ平行であるのが好ましいからである。なお、一定のＷG が好まし
い構成であるが、特定の状況においては、組織スロットの幅がその長さに沿って
変化する組織スロット５０を構成することが望ましい場合があり、例えば、組織
スロット５０の中を組織が移動する際に処理される組織上の圧力が増加するよう
に、組織スロットがその先端開口部からその横方向の端部における組織分離装置
にわたって狭くなっている組織スロットを構成することが望ましい場合がある。
しかしながら、本発明による双極式電気外科手術装置においては、当該装置は組
織スロット５０の長さに沿う各点における高さＷG がほぼ一定であって組織が組
織スロット５０の中を移動して処理されている間に変化しないように構成されて
いる。本発明による典型的な装置において、好ましいＷG の値は０．０２０イン
チ乃至０．０３０インチの範囲内であり、この範囲において、血管が組織スロッ
ト５０の中およびこれに沿って移動する際に約１ミリメートル乃至４ミリメート
ルの直径の大きさの範囲の血管を最適に圧縮することが期待できる。より大きな
高さＷG を有する装置１０ほど、より太い血管（例えば４ｍｍ乃至７ｍｍ）また
はより厚い組織を許容および処理することができる。このような血管は、例えば
、動脈のバイパス手術において使用するために伏在静脈が採取される場合に密封
する必要がある、例えば、伏在静脈の側分岐部において見ることができる。例え
ば、伏在静脈の側分岐部のような組織が組織スロット５０の中に挿入されると、
電気外科手術エネルギーが電極６０，６４，６２および６６を介してこの静脈の
側分岐部に加えられる。さらに、以下に述べる理由により、ＷG がＷe の２倍よ
りも小さい場合に組織スロット５０の中の組織の最適な焼灼が行えると予想でき
る。また、組織スロットの長さが長いほどスロットにより係合される組織の量が
多くなり、それだけ全体のインピーダンスが低下して組織の焼灼速度が大きくな
る。
【００１７】 図１２は図１１の状態と同様の概略的な断面図であり、双極式電気外科手術装
置１０の顎部組立体１６により係合された組織内を通過する際の電気外科手術用
の電流の主な流路を示している。典型的な電流の経路が図１２に示されている。
図１３は外科手術処理の一例の斜視図であり、この場合に、本発明による双極式
電気外科手術装置１０は腹部外科手術中における腸間膜組織内の血管９２を含む
腸間膜組織９０を焼灼および切断するために使用されている。
【００１８】 動作において、腸間膜組織９０のような組織が外科医により組織スロット５０
の先端部の中に挿入される。その後、この組織が上方傾斜表面部８１および下方
傾斜表面部８３により組織スロット５０の中に案内され、上方切開装置８０およ
び下方切開装置８２が周囲組織を組織スロット５０から分離するように作用し、
処理する組織を周囲組織から切り離して、周囲組織が組織スロット５０の中に押
し込まれることを防ぐ。組織を組織スロット５０の中に押し込むと、外科医は、
例えば、フット・スイッチ２２を用いて、例えば、電気外科手術用発生装置２０
を起動することにより電極６０，６２，６４および６６に電気外科手術エネルギ
ーを供給する。電気外科手術用発生装置２０が起動すると、この発生装置は各電
極６０，６２，６４および６６において交流電位を発生する。図１１に示すよう
に、電極６４における瞬間電位は電極６６における瞬間電位に等しく、電極６０
における瞬間電位は電極６２における瞬間電位に等しい。さらに、電極６０およ
び６２は電極６４および６６に加えられる電位とは異なるがほぼ同じ大きさで反
対の極性の瞬間電位を有する。それゆえ、異なる電位の電極間に一定の電圧が生
じるので、組織のような導電性の物質が接触表面部７０と接触表面部７４との間
に接触した状態で置かれると、電流が電極６０と電極６４との間に流れる。同様
に、組織のような導電性の物質が接触表面部７２と接触表面部７６との間に接触
した状態で置かれると、電流が電極６２と電極６６との間に流れる。しかしなが
ら、電流は抵抗の最も小さい経路、すなわち、他の全ての変数が一定である場合
に最も短い経路において流れるので、組織スロット５０の中の組織内の流れる電
流は図１２に示すような接触表面部７０と接触表面部７４との間の第１の主経路
、および接触表面部７２と接触表面部７６との間の第２の主経路内に流れようと
する。電流はさらに別の経路内にも流れるが、本発明によるエンド・イフェクタ
３６内の主な初期電流経路は図１２に示すものとなる。さらに、組織スロット５
０の中の組織内の最も短い経路は当該組織の表面近傍の領域に沿っているので、
電流密度は初期的に組織の表面に沿う領域および直下の領域内において最も高く
なる。組織スロット５０の上部および底部の近くの組織が発熱して乾燥し始める
と、その電気抵抗値が増加して各主電流経路が第１の組織表面部５２と第２の組
織表面部５４との間の中間点に向かって組織内部により深く進入する。この現象
は組織スロット５０内の組織に加えられる圧力により助長され、この圧力は組織
スロット５０の中に組織が押し込まれる時に組織から水および血液を含む流体を
押し出して、組織スロット５０の中に挿入される血管の凝固および密封処理を容
易にするように作用する。組織スロット５０は処理する組織を受容する一方で、
当該組織スロット５０の中に供給された組織に相当な圧力を生じるように構成さ
れている。さらに、本発明によるエンド・イフェクタ３６はまた組織内の電流を
自己制限的にするのに有効に作用する。
【００１９】 組織が適当に乾燥すると、電流の流れが減少または停止されて電力が伝達しな
くなるので、組織内の抵抗による発熱も生じなくなる。本発明によるエンド・イ
フェクタにおいては、組織スロット５０内の組織は極めて速く乾燥し、組織分離
装置４０に到達した組織はほぼ完全に乾燥していて、この組織内のあらゆる血管
は組織スロット５０の両側において密封されている。それゆえ、処理する組織が
組織分離装置４０の先端部分を通過する際に、先端部４２により二つの部分に分
離されて第１の楔形壁部４４および第２の楔形壁部４６により組織スロット５０
の外に送り出される。
【００２０】 さらに一般的に言えば、本発明による双極式電気外科手術装置は抵抗による発
熱により組織の外端部から中心部に向けて垂直に組織を焼灼するように作用し、
組織が組織スロットの中を連続的に移動する際に組織スロット５０の中央部分か
らその端部に向けて横方向に作用する。組織スロット５０の上部および底部の近
く、および絶縁装置６８および７８の間の組織が発熱し始めると、処理領域内の
組織の抵抗値が上昇して電流が抵抗値の比較的低い別の経路に分流する。それゆ
え、電流密度が組織の元の空間内において低下し、抵抗値の低い領域内において
上昇するために、これらの領域が加熱される。このような現象は組織が完全に処
理されてその抵抗値が処理領域全体にわたって上昇するまで継続する。組織の抵
抗値が上昇すると、電気外科手術用発生装置により生じるインピーダンス負荷が
増大して、組織により示される負荷が発生装置の出力に対して釣り合わなくなる
。さらに、相当量の負荷が釣り合わなくなると、付加的なエネルギーが組織内に
おいてほとんど、あるいは、全く消費されなくなる。
【００２１】 本発明によるエンド・イフェクタ３６は、最も重要な領域、すなわち、先端部
４２が通過する組織スロット５０の中心部内の領域が処理される最初の領域であ
って最も完全に処理される領域であるという意味で特に効果的である。特に、組
織スロット５０の高さＷG を電極の幅Ｗe の２倍よりも確実に小さくすることに
より、組織スロット５０の中央部の中の組織が最初に効果的に焼灼される。さら
に、本発明によるエンド・イフェクタ３６内の電流が自己制限的であるので、顎
部組立体１６の外側の組織に対してほとんど、あるいは、全く損傷を与えず、切
断線の両側の組織を適当に凝固および密封すること確実に行うことが必要な領域
に対する熱的な損傷が制限できる。さらに、エンド・イフェクタ３６により係合
される組織に対するエネルギーの転移が自己調整的であるので、外科医は装置に
対するエネルギー供給を継続して行うことができ、顎部組立体１６の周囲の組織
に著しい横方向の熱的な損傷を与えることなく外科医に都合のよい速度で組織内
に横向きに装置を移動できる。また、顎部組立体１６の外側の組織に対して横方
向の熱的な損傷をほとんど、あるいは、全く与えることなくエンド・イフェクタ
３６内の組織を処理した後に、電気外科手術用発生装置２０を作動し、組織に供
給される電流を自己制限的に保った状態で、外科医は組織内の装置の移動を停止
することもできる。特に、凝固した組織からの熱の伝導により生じる横方向の熱
的な損傷は一般的に顎部組立体１６の外側において０．５ｍｍ乃至１ｍｍ程度で
あると予想できる。さらに別の効果として、この装置の使用方法は技術的に鋭い
感性を必要としないので、既に説明した理由により、同様の組織においてこの装
置を使用する異なる外科医はほとんど同様の結果を得ると考えられる。
【００２２】 本発明によるエンド・イフェクタを内蔵する双極式電気外科手術装置は特に個
々の血管を密封および切断する場合の使用に適している。一般に血管は丈夫であ
るが、電気外科手術エネルギーによるコラーゲンの破壊およびスロット５０の中
に押し込まれる際の圧力により血管組織が正常な場合に比して著しく弱まり、組
織分離装置４０により処理した血管を機械的に結紮することが容易になる。それ
ゆえ、本発明によるエンド・イフェクタ３６を使用することにより、組織スロッ
ト５０内に挿入した血管が単一工程内において圧縮および凝固され、血管が密封
されてその切断時の漏れが防止できる。その後、血管は凝固した領域の中央部に
おいて分離され、２個の密封された端部が残されて、切断処理の前に血管を密封
するための２個以上のクリップを使用する必要性が減少または排除できる。さら
に、外科的に鈍い組織分離装置は健康な組織が組織スロット内に挿入された場合
にこのような組織を切断しないので、外科的に鈍い先端部４２を有する組織分離
装置４０を使用することは有利であり、それゆえ、外科医による電気外科手術エ
ネルギーの供給前に健康な組織が組織スロット内に供給される場合にこの組織を
偶然に切断することが防止できる。スロット内の組織が電気外科的に処理される
と、この組織は乾燥して組織分離装置４０により容易に分割できるようになる。
【００２３】 本明細書において図示し説明した各構造に置き換えることのできる（別の）等
価な構造が存在し、本発明の上記の実施形態が本発明を実施するために使用でき
る唯一の構造ではないことが理解されると考える。本発明を実施するために使用
できる等価な構造の一例として、組織スロット５０の内部表面上に蒸着した導電
性の材料により構成されて電線のような導電体を介して第１の電線２６および第
２の電線２８に接続される電極６０，６２，６４および６６を伴って、顎部組立
体１６が非導電性の材料により形成できる。また、別の例として、電極６０，６
２，６４および６６の１個以上が組織スロット５０の内部にそって配列された複
数の導電性の領域または表面部により構成されていて、本明細書に図示し説明し
た各電極とほぼ同一の領域を組織スロット５０の内部表面上において覆う電極領
域を形成している。さらに別の例として、電極６０，６２，６４および６６の１
個以上が組織スロット５０の中に配置された組織に対して接触表面部７０，７２
，７４および７６を付勢するように構成されたスプリング機構を備えていて、ス
ロット５０内の組織との間の接触が改善される。本発明の別の実施形態は第１の
電極６０および第２の電極６２のみを備えていて、第２の組織表面部が電極を備
えておらず、例えば、非導電性の材料により構成されている。
【００２４】 本発明の組織に対する作用は完全に特徴付けられていないことが理解されると
考える。電極間を流れる電流の作用により組織スロットの中心部の組織は完全に
乾燥すると考えられる。一方、組織スロットの中心部から離れた組織内において
温度勾配が減少するので、当該中心部の両側における組織は凝固するが完全には
乾燥しない傾向が強い。さらに、中心部の組織において発生する蒸気が当該組織
の分割を補助して、組織分離装置による処理を行いやすくすると考えられる。
【００２５】 以上において、本発明の好ましい実施形態を図示し説明したが、当該技術分野
の熟練者において、これらの実施形態が例示的目的のためにのみ開示されている
ことが明らかに理解できる。すなわち、（本明細書の開示を参照した）現時点に
おいて、当該技術分野の熟練者であれば本発明を逸脱することなく多くの変形、
変更および置換えが可能になる。従って、（本明細書における）特許請求の範囲
に定める本発明の趣旨および範囲によってのみ本発明を制限するものとする。

    【特許請求の範囲】
  1. 【請求項１】 医療装置において使用するための双極式電気外科手術エンド
    ・イフェクタにおいて、 第１の組織表面部と、当該第１の組織表面部の第１の面上の第１の細長い電極
    と、第１の組織表面部の第２の面上の第２の細長い電極とから成り、当該第２の
    電極は前記第１の電極に対してほぼ平行であり、さらに、前記第２の電極から前
    記第１の電極を分離する第１の中央絶縁領域と、組織スロットにより前記第１の
    電極および第２の電極から分離されている第２の組織表面部とから成り、当該組
    織スロットが前記第１の組織表面部と前記第２の組織表面部との間の一定の距離
    により定められていて、当該距離が一定であり、さらに、前記第１の組織表面部
    および第２の組織表面部の間に配置された組織分離装置から成り、当該組織分離
    装置がこの組織分離装置の先端部における分離端部と、当該分離端部から前記第
    １の面に向けて基端側に延出する第１の組織ガイドと、分離端部から前記第２の
    面に向けて基端側に延出する第２の組織ガイドとにより構成されている双極式電
    気外科手術エンド・イフェクタ。
  2. 【請求項２】 さらに、前記第２の組織表面部の第１の面上の第３の細長い
    電極と、前記第２の組織表面部の第２の面上の第４の細長い電極とから成り、当
    該第４の細長い電極が前記第２の細長い電極に対してほぼ平行であり、さらに、
    前記第３の電極を第４の電極から分離する第２の中央絶縁領域から成る請求項１
    に記載の双極式電気外科手術エンド・イフェクタ。
  3. 【請求項３】 前記第１の電極が前記第３の電極に対してほぼ正反対の位置
    に配置されており、前記第２の電極が前記第４の電極に対してほぼ正反対の位置
    に配置されている請求項２に記載の双極式電気外科手術エンド・イフェクタ。
  4. 【請求項４】 前記組織スロットの高さが当該組織スロットの長さに沿って
    ほぼ一定である請求項１に記載の双極式電気外科手術エンド・イフェクタ。
  5. 【請求項５】 前記組織分離装置がさらに楔形状の領域により構成されてい

    ・イフェクタにおいて、 当該エンド・イフェクタの第１の面上の第１の細長い電極と、前記エンド・イ
    フェクタの第１の面上の第２の細長い電極とから成り、前記第２の電極が前記第
    １の電極に対してほぼ平行であり、さらに、前記エンド・イフェクタの第２の面
    上の第３の細長い電極から成り、当該第３の電極が前記第１の電極および第２の
    電極に対してほぼ平行であり、さらに、前記エンド・イフェクタの第２の面上の
    第４の細長い電極から成り、当該第４の細長い電極が前記第１の電極、第２の電
    極および第３の電極に対してほぼ平行であり、前記第１の電極および第２の電極
    がほぼ一定の空隙を介して前記第２の電極および第３の電極から分離されており
    、さらに、前記第１の電極を第３の電極から分離する第１の中央絶縁領域と、前
    記第２の電極を前記第４の電極から分離する第２の中央絶縁領域と、前記第１の
    中央絶縁領域および第２の中央絶縁領域の間において前記エンド・イフェクタの
    先端部よりも基端側に配置されている組織分離装置とから成り、当該組織分離装
    置が、 この組織分離装置の先端部における分離端部と、当該分離端部から前記エンド
    ・イフェクタの第１の面に向けて基端側に延出する第１の組織表面部と、分離端
    部から前記エンド・イフェクタの第２の面に向けて基端側に延出する第２の組織
    表面部とにより構成されている双極式電気外科手術エンド・イフェクタ。
  9. 【請求項９】 前記組織分離装置がさらに楔形状の領域により構成されてい
    る請求項８に記載の双極式電気外科手術エンド・イフェクタ。
  10. 【請求項１０】 前記分離端部が外科的に鈍い請求項８に記載の双極式電気
    外科手術エンド・イフェクタ。
  11. 【請求項１１】 前記第１の電極が前記第３の電極に対してほぼ正反対の位
    置に配置されており、前記第２の電極が前記第４の電極に対してほぼ正反対の位
    置に配置されている請求項８に記載の双極式電気外科手術エンド・イフェクタ。
  12. 【請求項１２】 前記第１の電極および第２の電極が電気的に接続していて
    、前記第２の電極および第３の電極が電気的に接続している請求項８に記載の双
    極式電気外科手術エンド・イフェクタ。
  13. 【請求項１３】 医療装置において使用するための双極式電気外科手術エン
    ド・イフェクタにおいて、 前記エンド・イフェクタの基端部から当該エンド・イフェクタの先端部まで延
    在している第１の内面部、および第２の内面部から成り、当該第１の内面部およ
    び第２の内面部の間の空間が組織を収容する空隙部としての組織スロットを形成
    し、当該組織スロットの高さがその長さに沿う任意の位置において一定であり、
    さらに、前記第１の内面部の第１の面上の第１の細長い電極と、前記第２の内面
    部の第１の面上の第２の細長い電極とから成り、当該第２の電極が前記第１の電
    極に対してほぼ平行であり、さらに、前記第１の内面部の第２の面上の第３の細
    長い電極と、前記第２の内面部の第２の面上の第４の細長い電極とから成り、前
    記第２の電極が前記第３の電極に対してほぼ平行であり、さらに、前記第１の電
    極を前記第３の電極から分離する前記第１の内面部上の第１の絶縁領域と、前記
    第２の電極を前記第４の電極から分離する前記第２の内面部上の第２の絶縁領域
    と、前記第１の内面部および第２の内面部の間の空隙部内に配置された中央楔形
    組織分離装置とから成り、当該中央楔形組織分離装置が、この中央楔形組織分離
    装置の先端部分における先端部と、当該先端部から前記エンド・イフェクタの第
    １の面に向けて基端側に延出する第１の組織表面部と、前記先端部から前記エン
    ド・イフェクタの第２の面に向けて基端側に延出する第２の組織表面部とにより
    構成されている双極式電気外科手術エンド・イフェクタ。
