Abstract
(57)【要約】
【課題】 膝を曲げて移植片が揺動されたときにねじ頭
部が移植物体に係止し、ねじ頭部が移植片の作用点とし
て作用して移植片の繊維組織を傷つけかねない。 【解決手段】 外科用ねじ5は頭部6を有し、頭部は少
なくとも一部を半球形とされる。滑らかな連続結合がね
じ5の頭部5の頭部6と軸部7との間に形成される。ね
じをねじ込むために、駆動ソケット11が頭部と同心に
備えられて側面11に開口され、これによりねじ5がワ
イヤー(図示せず)または同様部材に沿ってねじ込める
ようになされる。軸部7は滑らかな柔らかいねじ山を有
し、骨に準備加工した穴の中に靱帯移植片の骨端部を相
互固定して取り付けられるが、両方の骨組織を損傷しな
いようになされる。半球形部分8は、靱帯がその上を疲
労や応力集中を生じることなく曲がり且つ動けるように
する滑らかな連続した湾曲面を形成する。

Description
【0001】
本考案は前部十字靭帯(Anterior Cruciate ligament)を再構成する間の膝蓋 靭帯移植片(Patellar Tendon Grafts)の固定に特に好適な外科用ねじに関する 。
【0002】
損傷した十字靭帯は、両端が骨組織塊に対して生理的に結合されている膝蓋靭 帯を膝関節内で大腿骨と脛骨との間に移植することで修復できる。この移植片は 準備加工した穴の中に挿入され、最初は移植片の骨に並べてこの穴内に挿入され たねじによって結合される。時間が経過すると移植片は骨実体と癒合する。この 技術は一般に良好な効果を示すが、膝を曲げて移植片が揺動されたときにねじ頭 部が移植物体に係止し、ねじ頭部が移植片の支点として作用して移植片の繊維組 織を傷つけかねないという問題が起こり得る。従来のねじ山は移植片に食い込み 、それ故に損傷させるという他の問題も発生し得る。また、従来のねじは或る種 の方法では膝関節内に失われかねず、必要以外の作動をして更に外傷を与えかね ない。

広義の一態様によれば、本考案は、長手方向軸線を有する軸部と、前記軸部の 一端から該軸部の長さに沿って延在し、該長さを通じてその外周には切り込み線 (すなわち、外科用ねじの前進にあって、靭帯移植片の繊維組織に食い込む(切 り込みを入れながら前進する)ような鋭い縁)がない複数のねじ山と、前記軸部 の一端とは反対側の他端に設けた頭部と、工具を挿入するために前記頭部に形成 した凹所とを有し、前記頭部は、前記軸部の頭部隣接部分から滑らか且つ連続的 に延在する外側表面を有するとともに、少なくとも前記頭部隣接部分の最大直径 に等しい直径を有する半球形端部を含んで成る外科用ねじを提供する。

以下、単なる例として、本考案の1好適例を図面を参照しながら説明する。 損傷した十字靭帯を修復するとき、盲穴1が大腿骨2の膝関節部分の内部にド リル加工され、膝裏靭帯移植片4の骨端部3が穴1の中に挿入されて、ねじ5を ねじ込むことで所定位置に固定される。ねじ頭部6は一般に靭帯移植片4の下側 に位置付けられ、従って膝が再び通常の使用状態になされると、靭帯移植片4は ねじ頭部6の上に導かれてそれら両者間に或る程度の相互運動が生じる。
【0005】 ねじ5は靭帯移植片4の骨端部3に実質的に食い込まないが、骨端部3の大腿 骨2に対する癒合に必要な時間にわたって骨端部3を剛性的に位置決めされた状 態に保持するために、ねじ部分と骨端部3および穴1の内壁面との間を相互固定 する。従って、第2図に見られるように、ねじ5のねじ山形は、長手方向軸線に 沿った断面においてシヌソイド形になされ、その外周にはねじ稜線を螺旋状にた どる切り込み線がない。
【0006】 ねじ頭部6は、滑らかに延在し且つ全体的に軸部7から半球形端部8へ続く表 面を有する。頭部6の、ねじ5の長手方向軸線と整合した中央には、ねじ5を所 定位置にねじ込むための組み合う6角形駆動部を受け入れるためのソケット11 を形成した。半球形端部8の半径は概ね4mmであり、この作動に関する典型的な ねじの直径は全長にわたって8mmもしくはそれに近い。
【0007】 第3図に示されるように、頭部8はねじ軸部7よりも大きな直径にもでき、こ の頭部8は軸部7へ向けて細くなるようにテーパーが付された截頭円錐形の円錐 形カラー部分10(遷移領域)まで滑らかに続く半球形以上の球形部分になされ ている。
【0008】 勿論、ねじ5は直径が大きく異なるねじが好ましいとされる別の作動条件の下 でも使用でき、頭部6は軸部7に滑らかに連なるほぼ半球形に維持される。
【0009】 ねじ5を穴1の中に初めて挿入するときにその導入をおよび整合をいっそう容 易にするために、中央通し穴9をねじ5の長さに沿って形成した。この通し穴9 は、大径キルシェナー(Kirschener)ワイヤーまたは類似物によって取扱いでき るようにするためのものであり、中央通し穴9の径は典型的には約2.4mmにな される。

Claims (6)
- 【請求項1】 長手方向軸線を有する軸部と、 前記軸部の一端から該軸部の長さに沿って延在し、該長
さを通じてその外周には切り込み線がない複数のねじ山
と、 前記軸部の一端とは反対側の他端に設けた頭部と、 工具を挿入するために前記頭部に形成した凹所とを有
し、 前記頭部は、前記軸部の頭部隣接部分から滑らか且つ連
続的に延在する外側表面を有するとともに、少なくとも
前記頭部隣接部分の最大直径に等しい直径を有する半球
形端部を含んで成る外科用ねじ。 - 【請求項2】 前記頭部隣接部分にはねじが切られてお
らず、また前記外側表面の直径が該頭部隣接部分の最大
直径に等しくなされている、請求項1に記載された外科
用ねじ。 - 【請求項3】 前記外側表面の直径が前記頭部隣接部分
の最大直径より大きく、前記外側表面が截頭円錐形の遷
移領域を経て前記軸部に向けて細くなるようにテーパが
付されている、請求項1に記載された外科用ねじ。 - 【請求項4】 前記ねじ山が前記軸部の長手方向軸線を
通る平面で見てほぼシヌソイド形状をしている、請求項
1に記載された外科用ねじ。 - 【請求項5】 前記軸部の長手方向軸線に沿って前記凹
所を貫通して延在する通し穴を形成した、請求項1に記
載された外科用ねじ。 - 【請求項6】 前記外側表面の直径が約4mmであるこ
とを特徴とする、請求項1に記載された外科用ねじ。
