【発明の属する技術分野】
本発明は、一方の関節部分が、他方の関節部分に対し、回転運動、転動運動又
は摺動運動あるいはこれらの運動の組合せであってよい運動を実行することがで
きる、人工の関節インプラント用の滑り対要素からなる平面状の複数の対に関す
る。

【従来の技術】
このタイプの関節インプラントは、先ず第１に、人工の股関節、膝関節、肩関
節、距骨関節である。しかし、本発明は、一般的には、２つの滑り対要素が互い
に対面するすべての人工関節に適用される。 一般的には、相対移動する複数の関節部分の間の摩擦を出来る限り少なく保つ
努力がなされている。このことのプラスの副次的効果は、粒子(Partikel)の摩耗
が摩擦係数の減少に連れて著しく減ることである。このことはインプラントの長
期間の安定性に役立つ。 複数の人工関節の移植後に、関節部分の周りは、自然の関節液、いわゆるシノ
ヴィアによって濡らされる。人工股関節の、嵌合いの正確度をもって作動される
関節部分、例えば、球関節及び関節窩の場合、液膜が相対移動する関節部分同士
の間で取られ、かくて、最早、全く潤滑機能を果たすことができないことに至る
。このことによって、関節部分同士の摩擦は、突然著しく大きくなって、人工関
節の操作可能性が損なわれ、滑り対要素の粒子の摩耗が早められる。このことに
よって、患者の身体中のインプラントの長時間残存に関するかなりの諸問題が結
果として生じることがある。
【０００３】 GB 1 527 498 に記載の従来の技術では、この問題を解決することが既に提案 された。例えば、人工の骨盤用窩中の滑り対要素と、人工の股関節頭との間の境
界面に、互いに接続されている複数の通路の形の複数の窪みが設けられており、
これらの窪みで、体液が関節の潤滑をもたらすことが意図されている。全く類似
の構造体はDE 43 36 932 C2及びDE 44 23 020 A1から知られている。液体を導く
複数の通路が互いに連通していることによって、複数の剥き出しの箇所で複数の
通路が乾くことについての諸問題が生じることになる。このとき、この乾きの後
には、人工股関節の不規則な潤滑の問題が生じる。このことによって、人工股関
節の操作可能性はマイナスに損なわれる。
【発明が解決しようとする課題】
このような背景を前にして、本発明の課題は、かように記載された、人工の関
節インプラント用の滑り対要素からなる平面状の複数の対を、以下のように、す
なわち、シノヴィアの液膜が、複数の関節部分同士のギャップで、高い確率をも
って取れないので、シノヴィアが関節用滑液としての自らの機能を確実に果たす
ことができるように、改善することである。

【課題を解決するための手段】
この課題は、請求項１に記載の平面状の複数の対によって、解決される。好ま
しい改善は複数の副請求項に記載されている。 従って、複数の滑り対要素同士の間の、シノヴィア用のギャップの他に、少な
くとも一方の滑り対要素の表面が、規則的に設けられた複数の窪み、互いに関連
なく、すなわち、互いの連絡通路なしに作られる窪みを有することが、提案され
る。一方の滑り対要素の表面に形成された複数の窪みは、シノヴィア用の液体収
集機能を果たし、シノヴィア自体は他方の滑り対要素に対する緩衝作用を果たす
。複数の窪みの液体収集機能は、複数の滑り対要素同士の間の液膜の多かれ少な
かれ突然に生じる亀裂が起こらないこと、をもたらす。複数の滑り対要素が直接
には互いに転動又は摺動せず、これらの間のシノヴィアの膜が常に摩耗を減らす
ことが、常にもたらされる。少ない摩耗粒子は直接的な結果であり、これによっ
て、インプラントの長時間安定性は一層改善される。
【０００５】 好都合な実施の形態では、複数の窪みが少なくとも１つの滑り対要素の表面の
８０％までを占めることが、規定されている。 このことによって、シノヴィアのかなり大きな容量は、かように緩衝作用を達
成するために、いわばリザーバ内に備えられる。 複数の窪みは、多角形の開口部を有することができ、すなわち、立面図では、
ハネカム状の外観を有することができる。これの他に、複数の開口部は、円形に
形成されても良い。一方の可能性か他方の可能性かの選択は、特に、全面と、見
込まれる表面圧力と、当該の関節部分の表面での配置とに拠る。
【０００６】 複数の窪みの横断面が半球状に形成されていることは好ましい。このことによ
って、これらの窪みは、一方では、シノヴィア用の十分に大きなリザーバを提供
し、他方では、シノヴィアによる十分な周囲の洗浄、及び複数の窪みに蓄えられ
たシノヴィアと、後に流れるシノヴィアとの交換という可能性を提供する。
【０００７】 好ましい実施の形態では、複数の窪みは、他方の固定式の関節部分に対して動
く、関節部分のうちの一方の滑り対要素の表面に、分布して配置されている。換
言すれば、複数の窪みは、この場合、他方の関節部分に対して転動運動、回転運
動又は摺動運動を実行する関節部分の、その表面に形成されている。
【０００８】 人工股関節の場合、複数の窪みは、この実施の形態では、球関節に設けられて
いるだろう。これに対し、滑り対要素は、寛骨臼内に、平滑面状に形成されてい
ることができる。 複数の滑り対要素からなる複数の対が、金属対金属、セラミック対セラミック
、セラミック対金属、ポリウレタン対金属又はポリウレタン／セラミックコンパ
ウンド対金属という複数の対のグループから選択されていることは好ましい。

【発明の実施の形態】
複数の図面を参照して本発明の複数の実施の形態を詳述する。図１の（Ａ）は
第１の概観図を示している。自然の寛骨にある削り取られた寛骨臼に入れられか
つそこで固定される金属ソケット１１からなる人工の関節窩が図１の（Ａ）に示
されている。固定式の関節部分２の滑り対要素(Gleitpartner) としては、金属 ソケット１１に嵌め込まれているインレット１２が、用いられている。このイン
レット１２に形成された半球形のリセスには、人工の球関節１３が可動の関節部
分１として係合している。この球関節１３と、インレット２に形成された半球形
のリセスとの間には、ギャップ１０が形成されており、このギャップには移植後
にシノヴィアが満たされる。この結果形成される液膜が取れないように（そうな
れば関節が乾燥状態で作動するだろう）、球関節13の表面には、規則的に配置さ
れた複数の窪み３が設けられており、表面の中に凹んでいる。これらの窪みは、
移植後にシノヴィアで満たされ、かくて、シノヴィア用のリザーバを形成する。
これによって、ギャップ１０内の液膜が取られないことが保証されている。
【００１０】 前記球関節１３の表面上の複数の窪み３が、どの程度の大きな領域を占めるか
は、球関節１３が示されている図１の（Ｂ）から非常に明白である。球関節１３
の表面の９０％までが複数の窪み３によって占められている。表面の残りの部分
は、インレット１２との実際上の摩擦面を形成している。図示した実施の形態で
は、複数の窪みの開口部は円形である。これの代わりに、複数の窪みは、多角形
で形成されることもできる。しかし乍ら、このとき、実際にインレット１２内の
半球部（Kalotte）で転動する表面部分が変化する。

めに用いられている。 図２の（Ａ）には、人工膝関節の第１の関節部分１の、大腿骨の顆部分１４の
側面図が示されている。複数の顆は、ここでは、至る所に、複数の窪み３を有す
る。特に、後方の顆は、図２の（Ｂ）から明らかなように、複数の窪みを有する
。何故ならば、これらの窪みは、膝関節の（図示しない）脛骨部分の片割れの表
面に沿った回転運動及び摺動運動における決定的な割合を占めているからである
。図２の（Ｃ）は、大腿骨の顆部分１４を上から見た図であり、全体像を完全に
する。
【００１２】 図３の（Ａ）は人工膝関節の第２の（固定式の）関節部分２としての脛骨用プ
レート部分１５を下から見た状態で示している。より正確に言えば、脛骨用プレ
ート１５は人工膝関節の図示しない脛骨ステム部分の第２の（固定式の）関節部
分２の滑り対要素を形成している。
【００１３】 図示した実施の形態では、１本の回転ピンが係合するときに通る穿孔１７を有
する。複数の開口部１６は、（図示しない）脛骨プレート部分と脛骨プレート１
５との間の間隙へもシノヴィアを通すために用いられる。このことは図３の（Ｂ
）から明らかである。換言すれば、これらの開口部１６は、プレート１５を脛骨
ステム部分で浮動式に支持するのを促進するために、シノヴィアを脛骨ステム部
分（図示せず）に通すのである。すなわち、このためには、（図示しない）回転
ピンが穿孔１７を通るので、プレート１５は或る程度脛骨ステム部分に対して動
くことができる。例えば平滑な複数の顆に向いた、脛骨プレート１５の上側には
、複数の窪み３が設けられている。ここでも、複数の窪みはシノヴィア用のリザ
ーバとして用いられるので、摺動面を有する脛骨用プレートと、大腿骨の顆部分
との間の十分な液膜が常にもたらされている。
【００１４】 脛骨用プレート１５に形成された複数の窪み３の、脛骨用プレートの表面にお
ける大きな厚みは、図３の（Ｃ）に明らかである。滑り対要素の表面全体の９０
％までを占めることができる窪み３の、その大きな数は、窪みにおけるシノヴィ
アの、相応に大きな貯えと、従ってまた、シノヴィアの十分に大きなストックと
をもたらす。この実施の形態では、複数の窪み３構成は、図１の（Ａ）乃至図１
の（Ｃ）に示した人工股関節における窪みの構成に対応している。

【０００３】 GB 1 527 498 に記載の従来の技術では、この問題を解決することが既に提案 された。例えば、人工の骨盤用窩中の滑り対要素と、人工の股関節頭との間の境
界面に、互いに接続されている複数の通路の形の複数の窪みが設けられており、
これらの窪みで、体液が関節の潤滑をもたらすことが意図されている。全く類似
の構造体はDE 43 36 932 C2及びDE 44 23 020 A1から知られている。液体を導く
複数の通路が互いに連通していることによって、複数の剥き出しの箇所で複数の
通路が乾くことについての諸問題が生じることになる。このとき、この乾きの後
には、人工股関節の不規則な潤滑の問題が来る。このことによって、人工股関節
の操作可能性はマイナスに損なわれる。 US 5,462,362 Aからは、関節インプラントにも用いられることが意図されてい
る耐摩耗性の複数の滑り対要素が公知である。この公報では、複数の関節部分の
表面に、複数の窪みが嵌め込まれており、これらの窪みは相対移動する部分の間
に液膜を形成する体液の、そのためのリザーバである。複数の窪みはすべてシリ
ンダ穴又は盲穴として形成されている。この場合の問題は、この関連で、体液の
緩衝機能が不十分にしか果たされないことである。結局は、このことは、各窪み
がシリンダ状に形成されていることの故の圧力分布の結果なのである。このこと
によって、シノヴィアは自らの機能を確実に果たすことができない。

【課題を解決するための手段】 この課題は、請求項１に記載の平面状の複数の対によって、解決される。好ま
しい改善は複数の副請求項に記載されている。 従って、複数の滑り対要素同士の間の、シノヴィア用のギャップの他に、少な
くとも一方の滑り対要素の表面が、規則的に設けられた複数の窪み、互いに関連
なく、すなわち、互いの連絡通路なしに作られる窪みを有することが、提案され
る。一方の滑り対要素の表面に形成された複数の窪みは、シノヴィア用の液体収
集機能を果たし、シノヴィア自体は他方の滑り対要素に対する緩衝作用を果たす
。複数の窪みの液体収集機能は、複数の滑り対要素同士の間の液膜の多かれ少な
かれ突然に生じる亀裂が起こらないこと、をもたらす。複数の滑り対要素が直接
には互いに転動又は摺動せず、これらの間のシノヴィアの膜が常に摩耗を減らす
ことが、常にもたらされる。少ない摩耗粒子は直接的な結果であり、これによっ
て、インプラントの長時間安定性は一層改善される。 複数の窪みの横断面が半球状に形成されていることは好ましい。このことによ
って、これらの窪みは、一方では、シノヴィア用の十分に大きなリザーバを提供
し、他方では、シノヴィアによる十分な周囲の洗浄、及び複数の窪みに蓄えられ
たシノヴィアと、後に流れるシノヴィアとの交換という可能性を提供する。

Claims (7)

    【特許請求の範囲】
  1. 【請求項１】 人工の関節インプラント用の滑り対要素からなる平面状の複
    数の対であって、一方の関節部分（１）が、他方の関節部分（２）に対して回転
    運動、転動運動又は摺動運動あるいはこれらの運動の組合せを実行し、滑り対要
    素同士の間には自然の液膜（シノヴィア）用のギャップ（１０）が形成されてお
    り、少なくとも１つの滑り対要素の表面は、規則的に配置されかつ互いの接続通
    路のない複数の窪み（３）を有する、複数の対。
  2. 【請求項２】 前記複数の窪み（３）は、前記少なくとも１つの滑り対要素
    の表面９０％まで占める、請求項１に記載の滑り対要素。
  3. 【請求項３】 前記複数の窪み（３）は、多角形の開口部を有する、請求項
    １又は２に記載の滑り対要素。
  4. 【請求項４】 前記複数の窪み（３）は、円形の開口部を有する、請求項１
    又は２に記載の滑り対要素。
  5. 【請求項５】 前記複数の窪み（３）の横断面は、半球形に形成されている
    、請求項１乃至４のいずれか１に記載の滑り対要素。
  6. 【請求項６】 前記複数の窪み（３）は、前記固定式の関節部分（２）の表
    面に対し可動な、一方の滑り対要素の表面上に、分布して配置されている、請求
    項１乃至５のいずれか１に記載の滑り対要素。
  7. 【請求項７】 滑り対要素は、前記金属対金属、セラミック対セラミック、
    セラミック対金属、ポリエチレン対金属又はポリウレタン／セラミックコンパウ
    ンド対金属という複数の対のグループから選択されている、請求項１乃至６のい
    ずれか１に記載の滑り対要素。
