For therapeutic combinations. In elderly patients, the therapeutic effects associated with co-administration of calcium and vitamin D are well known, for example, Marie C. Chapuy et al., Calcium and Cholecalciferol for 3 Years, for Elderly Women with Hip Fractures. Effect of treatment ", British Medical Journal, 308, 1081-1082 (April 23, 1994), Marie C. Chapuy et al., "Vitamin D3 and Calcium for Preventing Hip Fracture in Elderly Women," New England Journal of Medicine, 327, 1637-1642 (December 3, 1992), and "Vitamin D3 and calcium supplementation Preventing Hip Fractures in Elderly Women ", Nutrition Reviews, Vol. 51, 6, pp 183-185. These papers also show the change in the therapeutic effect of the combination as a function of calcium and vitamin D dosage, and that for optimal osteoporosis prevention and treatment indicators, the optimal daily dose of calcium is It is around 1000-1200 mg, and vitamin D3 is 800 UI. Numerous galenic forms are known which combine calcium and vitamin D. Accordingly, WO-A-96 / 09036 (Laboratoire Inotera) describes the above combination of galenic single tablet forms, especially powdered tablets, with respect to the absolute and relative dosages of calcium and vitamin D given in the above references. It has been described. The powder tablets described are administered in a manner that allows the desired daily dose of calcium and vitamin D by taking two tablets daily. In addition to the dry forms of calcium / vitamin D combinations (laminated tablets, sugar-coated tablets, drinking or chewing tablets, effervescent granules, powder bags), for example, soluble calcium salts (such as gluconate or lactate) Liquid forms have been proposed, packaged in ampoules containing a beverage solution. The solution has a certain acidity, as a result of the solubility of the calcium salt, that can inhibit the absorption of the main component and is very intolerable to the patient. In particular, vitamin / calcium dietary supplements consisting of calcium and one vitamin D have also been proposed, for example by WO-A-96 / 31130 (Abbott Laboratory), and are intended to be diluted with water or fruit juices A concentrate in the form is described. However, again, the calcium salt is always in a soluble or solubilized form, and the low pH results in the inconveniences mentioned above. One object of the present invention is to combine calcium and vitamin D, provided in a single formulation of a beverage liquid without strong acidity, to give good tolerability and good absorption of the active ingredient Is to propose a combination of minerals and vitamins. A "single formulation of a beverage liquid" refers to a liquid formulation for a beverage, e.g., in a bag, bottle or ampoule, without the need for any handling by the patient, as in the case of effervescent granules or soluble powders, for example. It should be noted that ready-to-drink liquid formulations are intended. Another object of the invention proposes the above formulation, wherein the single dosage corresponds to a single daily dose (about 1000-1200 mg of elemental calcium per day and 800 Dl of vitamin D) within the above mentioned values. This is because once a day dosage is sufficient and facilitates treatment compliance. In the dry form as described in WO-A-96 / 09036, it has been said that it is difficult to administer a dose of 2 tablets or less per day. Single dose mini-tablets exhibit a consistency that is too thick in the mouth and almost non-exhilarating properties, tend to make patients uncomfortable, and can be used for beverages that can administer larger amounts of each ingredient without difficulty It differs from the case of liquid preparations. It is a further object of the present invention to provide, as described at the outset, calcium / vitamin D in liquid beverage form with an optimal relative dosage between calcium and vitamin D, especially for the prevention and treatment of osteoporosis. It is to propose a combination. The proposed calcium / vitamin D combination in a single formulation of a beverage liquid has already been proposed for the indication in the prevention and treatment of osteoporosis, in particular taking into account the desired optimal proportions given in the above-mentioned literature. We can state that there is not. This may be due to the fact that these combinations are generally prescribed for other indicators (eg, rickets prevention, osteomalacia, specific products overdosed with vitamin D) or for therapeutic purposes ( Vitamin D content is relatively lower than a true pharmaceutical specialty (with no food supplement or medical prescription) in case of overdose to avoid any risks associated with this active ingredient's inherent toxicity Comes from the fact that it acts as an "OTC" specific product sold at To this end, the present invention proposes a therapeutic combination of minerals and vitamins in the form of a single formulation of a drinking liquid, especially for the prevention and treatment of osteoporosis in elderly patients, said combination comprising an active ingredient A calcium salt essentially consisting of elemental calcium and at least one vitamin D and insoluble in the aqueous liquid medium dispersed in the aqueous liquid medium; at least one vitamin D; a solubilizer of vitamin D in the aqueous medium; It is intended to increase the viscosity of the aqueous medium and consists of a gelling agent to keep the insoluble calcium salt in a homogeneous suspension in this medium. According to various advantageous properties:-Ca element in mg and vitamin D in UI, the ratio of elemental calcium to vitamin D is 1.0 to 1.5, preferably 1.2 to 1.3. The combination is packaged in a single daily dosage form, each dosage consisting of 800-1200 mg of Ca element for 600-1000 UI of vitamin D; the insoluble calcium salt is calcium carbonate; Is selected from vitamin D2 or ergocalciferol, vitamin D3 or cholecalciferol, or a mixture thereof; the solubilizing agent for vitamin D is polyoxyethylene glycol and its ester, propylene glycol, ethanol, ether of diethylene glycol, Glycerin and saturated and unsaturated polyglycol glycerides, Selected from liquid fructose-type saccharides, liquid maltitol, esters of polyhydroxyethyl sorbitan, hydrogenated and non-hydrogenated polyoxyethyl castor oil, and maltodextrin, preferably esters of polyhydroxyethyl sorbitan; Guar gum, xanthan gum, tragacanth gum, gum arabic, agar, sodium carboxymethyl starch, colloidal anhydrous silica, cellulose derivatives, typically hydroxypropyl cellulose, sodium hydroxyethyl cellulose, sodium carboxymethyl cellulose, acrylic acid polymer, aluminum silicate And magnesium silicate, preferably xanthan gum; the combination further comprises p-hydroxybenzoic acid, its methyl and Pill esters, sodium derivatives of these esters, preservatives selected from benzoic acid and its salts, sorbic acid and its salts, bisulfite and metabisulfite; the combination further comprises aspartame, saccharose, acesal Consisting of a sweetener selected from fame potassium, sodium saccharinate, sodium cyclamate, and fructose; and the combination has the following percentage formula: 16.67 g calcium carbonate (equivalent to 6.67 g elemental calcium) Amount corresponding to calciferol 5333.33 UI Polyhydroxyethyl sorbitan ester 0.05 g Polysorbate 80 Xanthan gum 0.75 g Fructose 8.67 g Preservatives sufficient Flavors sufficient Pure water 100 ml Final pH is 7-9. Preferred, but non-limiting examples that make the invention effective will now be described. First, each component of the formulation, namely, vitamin D, insoluble calcium salt, solubilizer, gelling agent, sweetener, preservative, and flavor of vitamin D will be described in detail. 1. Vitamin D This vitamin is cholecalciferol (vitamin D3, Roche) in oily solution (vegetable oil) form at 1 million UI / g, or in granular form of 100 CWS ™ from Roche at 100,000 UI / g. It is. In both cases, the presence of d1-α-tocopherol confers great stability to the vitamin and prevents its oxidation. 2. As has been shown, the salt is always in a dispersed state in the liquid medium and does not disturb the equilibrium of the salt, where it does not change its properties (and therefore good absorption) The solution must be essentially insoluble (and not solubilized) in this formulation in a manner that does not acidify the solution. Final pH is 7-9. The insoluble salt of calcium is preferably calcium carbonate of the type Scoralite ™ 1B from Scora, which has a fine particle size analysis of about 12 μm mean diameter, high density (about d = 1.3 g / m 3 ) white It is a powder. 3. Vitamin D Solubilizer This acts to make the lipophilic vitamin (vitamin D3) compatible with the hydrophilic environment, ie, in other words, it is compatible with the solvent (ie, the vitamin is soluble in this solvent). And remains stable) and acts as a solvent or co-solvent for vitamin D3, which is compatible with water. The solubilizer also has an acceptable taste, must not destabilize the selected gelling agent (see below), and must not attack the formulation components of the formulation liquid. Among the various solubilizers meeting these various criteria are, inter alia: polyoxyethylene glycol (PEG), typically PEG 400 and its esters, typically the lauric ester of PEG 200 Ethers of propylene glycol and diethylene glycol, typically diethylene glycol monoethyl ether (Transcutol ™); ethanol; both saturated [typically PEG-8 glyceryl caprylate / coupler ( Glycerin and polyglycol glycerides of Labrasol (TM)) and unsaturated [typically PEG-6 glyceryl monooleate (Labrafil (TM) M1944CS)]-sugars, typically liquid fructose and liquid maltitol (Lycasin (trademark Esters of polyhydroxyethylsorbitan, typically polysorbate 80, which is currently the preferred solubilizer, polysorbate 80 being 0.03 to 0.05% by weight of the final formulation And preferably 0.05% by weight; hydrogenated polyoxyethyl castor oil (typically Croduret ™ 50 special, Cremophor ™ RH 40) and non-hydrogenated polyoxyethyl castor oil (typically And Etocas ™ 35HV); and maltodextrin (typically Glucidex ™). 4. Gelling agent The role of the gelling agent is to keep the calcium salt in a homogeneous suspension in the aqueous medium without the solubilizing agent driving out the salt. To do this, the viscosity of the aqueous medium is increased by the polymer. The polymer must have a good structure to disperse and keep the calcium carbonate dispersed and have a good taste and pleasant structure in the mouth. Among suitable gelling agents, there may be mentioned: gums, typically guar, xanthan, tragacanth, gum arabic, currently preferred gelling agents are 0.1 to 1.0% by weight Agar; sodium carboxymethyl starch (typically Explotab ™); colloidal anhydrous silica (typically Aerosil ™ 200); Cellulose derivatives, typically hydroxypropylcellulose (Klucel ™), sodium hydroxyethylcellulose (Natrosol ™), sodium carboxymethylcellulose (Blanose ™); acrylic acid polymers (typically Carbopol® Aluminum silicate and magnesium silicate (typically Veegum ™ HV) 5. Sweeteners Various types of sweeteners can be chosen and the choice is not critical to the operation of the present invention. Particularly mentioned are aspartame, saccharose, potassium acesulfame (Sunet ™), sodium saccharinate, sodium cyclamate, and fructose (Fructofin ™ C type A121). Advantageously, fructose is chosen, which is a natural sweetener that does not contain saccharose and has advantages not contraindicated in non-insulin dependent diabetics. 6. Preservatives Various preservatives, such as p-hydroxybenzoic acid, its methyl and propyl esters, sodium derivatives of these esters, benzoic acid and its salts, sorbic acid and its salts (especially potassium sorbate), bisulfite, and meta The bisulfite can be selected but is not critical to the operation of the present invention. 7. Flavor This is selected from conventional additives as having the function of imparting the desired taste (lemon, orange, etc.) to the formulation. For production, the operating protocol is as follows: solubilize vitamin D3 in solubilizer, pour this solution into water, stir mix, stir to dissolve preservatives, stir sweetener Then, the flavor (liquid) is added by stirring, the calcium carbonate is stirred and dispersed, and the gelling agent is stirred and dispersed, and packaged. A general preferred formulation is as follows (percentage formulation): 16.67 g of calcium carbonate (equivalent to 6.67 g of elemental calcium) Amount corresponding to cholecalciferol 5333.33 UI Ester of polyhydroxyethyl sorbitan 0.05 g ( Polysorbate 80) Xanthan gum 0.75 g Fructose 8.67 g Preservatives Sufficient Flavor Sufficient Sufficient Purified water Sufficient up to 100 ml The resulting formulation has a pH of 7 to 9 (thus not acidic) and a viscosity η = 2000 mPa ± 20% . As a variant, 0.05 g of polysorbate 80 was added to PEG 400 (1 g), propylene glycol (1 g), Transcutol ™ (0.5 g), ethanol (10 g), glycerin (3 g), liquid maltitol (1 g) or hydrogen Polyoxyethyl castor oil (1 g). Similarly, 0.75 g of xanthan gum was added to guar gum (1 g), hydroxypropyl cellulose (1 g), hydroxyethyl cellulose (1 g), Veegum® HV (5 g), sodium carboxymethyl cellulose (1 g), Carbopol® 974P ( 0.3 g) or agar (0.5 g).
