本発明は、ネファゾドン(nefazodone)による外傷後ストレス障害
(PTSD)の治療に関する。
背景技術
本発明は、化合物2−[3−[4−(3−クロロフェニル)−1−ピペラジニ
ル]プロピル]−5−エチル−2、4−ジヒドロ−4−(2−フェノキシエチル
)−3H−1、2、4−トリアゾール−3−オン又はその製薬上許容できる付加
塩を使用する薬剤生体影響身体治療方法に関する。
この化合物は、以下の構造式(I)を有し、そしてネファゾドンとして知られ
ている。
この塩酸塩は、文献ではMJ 13754−1及びBMY 13754、並びに
ネファゾドン塩酸塩と呼ばれ、それは、「USP Dictionary of

ネファゾドンの臨床研究は、抗うつ剤としてその有用性を示し、そしてネファ
ゾドン塩酸塩は、うつ病にかかった患者の使用に米国食品医薬品局により承認さ

外傷後ストレス障害(PTSD)は、重大な外傷性事象に曝された後に生ずる
不安な障害としてDSM−IV(Diagnostic and Statis
tical Manual,4版)に定義されている。PTSDの診断は、以下
の中心的な症状が外傷に曝された後に伴うときなされる。外傷性事象の持続的再
経験(思考挿入、夢、フラッシュバック、又は内在性及び外在性の暗示)、外傷
又は一般化された心理学的無感覚及び隔離に伴う症状の持続する回避の存在、並
びに外傷前に存在しなかった広くいきわたった生理学的な覚醒。
PTSDの精神薬理学的治療は、主として症候に基づくものであり、そして中
枢神経作用薬のすべのカテゴリーからの薬剤を包含している。PTSD患者のた
めの薬物療法の成果は、残念ながら一般に不適切である。抗うつ薬療法は最も広
く利用されているPTSDの治療であるが、この適応について最近承認されてい
る薬剤はない。PTSDの治療のための文献中のコントロールされた臨床実験の
データの利用性は限定されている。公開されているこれらの研究は、三環抗うつ
薬(イミプラミン、アミトリプチリン、デシプラミン)、モノアミンオキシダー
ゼ阻害剤(MAOI)、フェネルシン又はクロニシンの何れかによる治療後のP
TSD症候学において少しばかりの改善を示唆している。これらの治療の選択は
しばしば使用されるが、それらはPTSDに関する薬物療法に標準的に認めれら

発明の開示
本発明の方法は、障害に苦しみそしてさらに複雑な症候反応が予想される患者
のPSTDの軽減を目的としている。方法は、本質的に、この治療を必要として
いるヒトにネファゾドン又はその製薬上許容可能な酸付加塩を投与することを含
む。この方法で使用されるためには、ネファゾドン塩酸塩の経口投与は、約10
0−600mg/日に及ぶ。分割した投与で毎日約300−600mgの投与は
、好ましい投与の処方であるとして期待される。種々の中枢神経作用薬の治療は
PTSD患者の症候の緩和に使用されてきたが、ネファゾドンは、セロトン作用
性伝達に対するその二重の効果のために従来の治療から重要な臨床上の区別を有
するようである。ネファゾドンは、5−HT2受容体の選択的阻害ばかりでなく
5−HT再取り込みの阻害によりセロトニン受容体を調整する。ネファゾドンの
この新規な薬理は、PTSDの治療における独特な治療活性をもたらすと思われ
る。
発明を実施するための最良の形態
外傷後ストレス障害は、殆どすべてのヒトに顕著な窮迫を生じさせる外傷性エ
ピソードの発現を伴う。その例は、暴力的な対決、例えばレイプ又は暴行未遂又
は重大な事故の事件を含む。戦争行為及び天然の災害、特に仲間又は患者に近い
個人の損失がこれらの事件の間に経験されるとき、また障害の誘発物である。外
傷性事象後直ぐに、患者は他の人たちから離されるか又は疎遠にされたとしばし
ば感じ、さらに環境への一般的な反応性が減退する。うつの感じ及び個人間関係
の相互作用は、事象後しばしば経験される他の症状である。さらに詳細な診断の
基準は、DMS−IVに見いだされるが、一般にPTSDの患者は、不安、敵意
、うつ及び引きこもりの症状を示すだろう。PTSDの症状のこの型は、現在利
用できる薬物療法に抵抗する。
前臨床上の見地から、作用の新規な二重のセロトン作用性メカニズムを有する
ネファゾドンは、PTSDの患者のセロトン作用性伝達を増強することが予想で
きる。この効果は、セロトニン作用物質が逃れられないショックへのストレス反
応を逆転することが動物のストレステスト用モデルで示されているため、障害を
改善するだろう。セロトニン放出は、また攻撃性の阻害剤及び衝動のコントロー
ルの調節剤として認識されている。これらの効果は、PTSDの行動上の要素、
例えば性的に外傷をおった女性における回避の症状及び/又はうつ病の帰還兵に

PTSDの侵入性症状、例えば夢魔、睡眠障害及びはっとするような驚きの反
応は、またネファゾドンに特に反応性であろう。ネファゾドンは、抗不安性及び
睡眠を正常にする性質を有することが示され、顕著な不安及び睡眠障害の症候を
伴ううつ病の障害の治療に臨床上の有効性を立証している。
ネファゾドンの投与が診断された患者のPTSDを軽減することが臨床上観察
されている。PTSDの診断用のDSM−IV基準に適合する患者の二三のパイ
ロットスタディが行われた。最近、規模の大きなコントロールされた研究のプラ
ンが作成されている。一つのパイロットスタディでは、2週間向精神性治療を受
けなかった患者、及び有意な心臓又は医学上の症状のない患者には、ベースライ
ンの評価を与えた。評価は、二三の精神測定装置の完了、並びに患者の外傷経験
のテープ録音された説明を聴きつつ個人の心拍数及び血圧をモニターすることを
含む。研究されるヒトは、次に10週間ネファゾドン(個人の有効量により異な
る300−600mg/日)により処理され、次にベースラインテスト法を使用
して再評価された。一般に、外傷後ストレス症候の低下、及びテープ録音を再び
聴くことに対する生理学的反応におけるかなり改善が、研究を完了した患者につ
いて観察された。数人の患者は、PSTDの睡眠障害要素における顕著な改善を
報告した。
他の小さな研究は、爆弾事故の被害者のオープンラベル治療を含んだ。患者の
大多数は、うつ病及び/又は全身性の不安を伴うPTSDを有した。ネファゾド
ンにより治療されたすべての患者は、優れた反応を示した。患者の或るものは、
最初に、選択性セロトニン再取り込み阻害剤例えばProzac又はPaxi1
により始めたが、これらの薬剤は患者をあまりに活動的にすることを示した。
他のコントロールされていないパイロットスタディは、12週間の医薬の投与
期間後数度の等級のスケールによるベースライン測定を含んだ。ネファゾドンを
最初に50mgの投与物として投与され、次に個々に1日当たり最大600mg
の有効量を与えた。患者の反応は、ネファゾドン投与後の等級のスコアの低下に
より測定した。すべてのパイロットスタディでは、PTSDの患者はネファゾド
ンに十分な耐性があるように思われた。
本発明の方法は、PTSDにかかった患者に、ネファゾドン又はその製薬上許
容できる酸付加塩を投与することを本質的に含む。ネファゾドンの製薬上許容で
きる酸付加塩及び製薬処方の方法は、Temp1eらの米国特許第433831
7号の特許に記述され、それはその全体を参考として本明細書に引用される。
本発明によるネファゾドン塩酸塩の投与は、非経口、経口又は直腸内の経路に
より行うことができる。経口の経路が好ましい。PTSDの軽減のための臨床上
の投与は、1日当たり100−600mg、一般に200−600mgそして好
ましくは1日当たり300−600mgの範囲にあると予想される。投与量は個
々の患者に調整すべきであるが、通常のやり方は、1日1又は2回投与される約

Claims (1)
- 【特許請求の範囲】 1.治療を必要とする患者に非毒性の治療上有効な投与量のネファゾドン又はそ の製薬上許容できる酸付加塩を投与することからなる外傷後ストレス障害(PT SD)を軽減する方法。 2.ネファゾドン塩酸塩が使用され、投与が経口の経路によりなされる請求項1 の方法。 3.うつ病がPSTDにかかった該患者の主な要素である請求項1の方法。 4.敵意がPSTDにかかった該患者の主な要素である請求項1の方法。 5.睡眠障害がPSTDにかかった該患者の主な要素である請求項1の方法。 6.該患者が成人であり、約300−600mgの1日当たりの投与量が使用さ れる請求項2−5の何れか一つの項の方法。 7.前記の1日当たりの投与量が分割されそして1日2回投与される請求項6の 方法。
