Claims (1)
- 【特許請求の範囲】 1.人間の指のMP関節(中手骨/指節骨間関節)と置換するように構成された プロテーゼ装置であって、 略半球状関節面を有する頭部と中手骨の空洞内に受け入れられるように形作ら れる幹部分とを有する中手骨エレメントと、 前記半球状関節面に合わせる凹面を有するソケット形状の頭部と指節骨の空洞 内に受け入れられるように形作られる幹部分とを有する指節骨エレメントとを備 え、 前記指節骨エレメントは、前記幹部分を取り囲む平坦な略楕円状のカラー部と 、前記楕円状カラーの外周から内側に収斂した手掌面および外側面で形成される 前記ソケット形状の頭部の前記凹面を取り囲む略円形のリムと、を含んでいるこ とを特徴とするプロテーゼ装置。 2.前記中手骨エレメントの幹部分が矢状面内の回転中心に対して背面側に所定 距離だけ偏位し、前記指節骨エレメントの幹部分が、該所定距離の少なくとも約 半分だけ前記回転中心に対して背面側に偏位していることを特徴とする請求項1 に記載のプロテーゼ装置。 3.前記リムが、円環状表面部として形成され、前記側面及び手掌面が直線状で あって、前記幹部分の長手方向軸線に対して約95°から約105°の間の背面 角度で配された平面内にある前記円環状リムに略接していることを特徴とする請 求項1に記載のプロテーゼ装置。 4.前記リムが、略円環状の表面部として形成され、前記側面及び手掌面が直線 状であって前記略円環状のリムに接しており、前記円環状のリムがその軸線がプ ロテーゼ装置の矢状面内に位置している円柱に接していることを特徴とする請求 項1に記載のプロテーゼ装置。 5.前記円柱が前記半球状関節面の半径の約2.5倍から約4倍の半径を有する 円筒であり、該円筒の軸線が前記指節骨エレメントの幹部分の長手方向軸線に対 して約95°から約105°の背面角度で配されていることを特徴とする請求項 4に記載のプロテーゼ装置。 6.前記指節骨エレメントの頭部の側面が直線状であり、前記楕円状カラー部の 平面に対して約65°から約75°の間の角度で配されていることを特徴とする 請求項1から5のいずれかに記載のプロテーゼ装置。 7.前記中手骨エレメントの頭部が、前記幹部分の軸線に対して約110°から 約120°の角度で配された前記幹部分の端部に、フラットカラー部と共に形成 され、側面に位置するレリーフ手段が前記フラットカラーおよび半球状頭部の残 部と側面の2箇所で交差して、前記カラーの位置において前記頭部が前記交差部 の間の背面側で約30°から90°の球面弧を有し、前記交差部の間の手掌面側 で約120°から170°の球面弧とを有し、前記装置に取り付けられる人間の 指の側副靭帯が前記中手骨エレメントの頭部の背面/基端側縁から前記指節骨エ レメントの頭部の手掌面/末端側縁まで干渉することなく延在させる自由通路を もたらすことを特徴とする請求項1から6のいずれかに記載のプロテーゼ装置。 8.前記レリーフ手段の平坦面が、前記フラットカラーで前記半球状表面の半径 の約85から160%に等しい長さを有していることを特徴とする請求項7に記 載のプロテーゼ装置。 9.溝手段が前記指節骨エレメントの凹面状関節面に形成され、該溝手段は、互 いに交差する二つの溝を含み、各溝は前記曲げ/伸ばし運動の前記平面を横切っ て配され、前記関節面の端まで延在していることを特徴とする請求項1から8の いずれかに記載のプロテーゼ装置。 10.溝手段が前記指節骨エレメントの凹面状関節面に形成され、該溝手段は前 記曲げ/伸ばし運動の前記平面に対して約45°の角度で配された二つの略直交 する溝を含んでいることを特徴とする請求項1から8のいずれかに記載のプロテ ーゼ装置。 11.人間の指のMP関節(中手骨/指節骨間関節)と置換するように構成され たプロテーゼ装置であって、 中手骨の空洞内に受け入れられるように形作られる幹部分と関節頭部とを有す る中手骨エレメントであり、前記頭部が中手骨の末端側関節部分と置換されるべ き略ボール形状の表面、および、前記幹部分の端に該幹部分の軸に対して約11 0°から約120°の間の背面角度で配されたフラットカラー部を有する中手骨 エレメントと、 指節骨の空洞内に受け入れられるように形作られる幹部分と関節頭部とを有す る指節骨エレメントであり、前記頭部が前記ボール状面に台う凹面を有し、指節 骨における対応する関節部分と置換するように構成されている指節骨エレメント とを備え、 前記中手骨エレメントの関節頭部は、前記ボール形状頭部が前記カラーの位置 に赤道の背面側で約30°から90°の球面弧とを、手掌面側で約120°から 170°の球面弧とを有するように前記ボール形状表面を切る平坦面状のレリー フ手段を各側面に備え、それによって、中手骨エレメントの関節頭部の背面/基 端側の端から指節骨エレメントの関節頭部の手掌面/末端側の端まで干渉なしに 側副靭帯を通す自由通路がもたらされることを特徴とするプロテーゼ装置。 12.前記レリーフ手段の前記平坦面が、前記フラットカラーに、前記半球状表 面の半径の約85から160%に等しい長さを有することを特徴とする請求項1 1に記載のプロテーゼ装置。 13.前記指節骨エレメントの前記基端側関節頭部が、(a)手掌面側の亜脱臼 に充分対抗し得る中手骨エレメントの関節頭部の捕捉をもたらす背面前突と、( b)曲げ運動の際に指の手掌面の靱帯との干渉を防止するように前記背面前突の 方に 充分に離れている手掌面側リム部と、(c)約75°から約85°の間の背面角 度で配された幹部分の端にフラットカラー部とを有することを特徴とする請求項 11に記載のプロテーゼ装置。 14.前記中手骨エレメントの前記幹部分が矢状面内の回転中心に対して背面側 に所定距離だけ偏位し、前記指節骨エレメントの前記幹部分が前記回転中心に対 して少なくとも前記所定距離の約半分だけ背面側に偏位していることを特徴とす る請求項12に記載のプロテーゼ装置。 15.前記指節骨エレメントの凹面状関節面が、前記MP関節の曲げ/伸ばし運 動平面に対して約45°の角度で配向された二つの略直交する溝を含むことを特 徴とする請求項11に記載のプロテーゼ装置。 16.前記指節骨エレメントの頭部が前記凹面を取り囲んで略円環状表面部とし て形成される基端側リムを有し、前記頭部が、前記円環状リムに略接する直線状 の側面および手掌面を有し、前記円環状リムは円筒に接し、該プロテーゼ装置の 矢状面内にある該円筒の軸は、前記指節骨エレメントの幹部分の長手方向軸線に 対して約95°から約105°の間の背面角度で配されていることを特徴とする 請求項11から15のいずれかに記載のプロテーゼ装置。 17.人間の指のMP関節(中手骨/指節骨間関節)と置換するように構成され たプロテーゼ装置であって、 中手骨の空洞内に受け入れられるような形作られる幹部分と、中手骨の基端側 関節部分と置き換えられるべき略ボール状関節面を有する関節頭部とを有する中 手骨エレメントと、 指節骨の空洞内に受け入れられるように形作られる幹部分と、前記ボール状表 面に合う略ソケット形状の関節面を有し、指節骨における対応する関節部分を置 き換えるように構成された関節頭部とを備えた指節骨エレメントとを備え、 前記関節面の一方に設けられ、前記溝手段はMP関節の曲げ/伸ばし運動の平 面を過って配され、前記合同関節部分の端まで延在している溝手段を除いて、前 記中手骨及び指節骨エレメントの前記合わさる関節面は、実質的に合同であり、 滑液の流体を端で進入させ、前記中手骨及び指節骨エレメントにおいて一致する 関節面間の内部接触領域から外に到達可能にしていることを特徴とするプロテー ゼ装置。 18.前記中手骨エレメントの前記関節面が凸面状球面であり、前記指節骨エレ メントの前記関節面が凹面状球面であることを特徴とする請求項17に記載のプ ロテーゼ装置。 19.前記溝手段が前記指節骨エレメントの前記関節面に形成され、互いに交差 する二つの溝を含み、該各溝は曲げ/伸ばし運動の平面を横切って配され、前記 合同関節面の縁まで延在していることを特徴とする請求項17に記載のプロテー ゼ装置。 20.前記溝手段が前記指節骨エレメントの前記関節面に形成され、曲げ/伸ば し運動の平面に対して約45°の角度で配された二つの略直交する溝を含んでい ることを特徴とする請求項17に記載のプロテーゼ装置。 21.前記指節骨エレメントの関節頭部が、(a)前記指節骨エレメントの手掌 面側の亜脱臼/脱臼を受けにくいように前記中手骨エレメントの関節頭部を捕捉 するリム状背面前突と、(b)前記リム状背面前突に対し充分に離れ、人間の指 の側副靭帯に対する間隙をもたらすと共に、曲げの際に該手掌面側靱帯との干渉 を防止する手掌面側リム部とを備えていることを特徴とする請求項17から20 のいずれかに記載のプロテーゼ装置。 22.前記中手骨エレメントの関節頭部が、半球形状部分以外となる該頭部の各 側面に形成された平坦なレリーフ面を有し、該レリーフ面は、前記中手骨エレメ ントの関節頭部の背面/基端側の縁から前記指節骨エレメントの関節頭部の手掌 面/末端側の緑まで干渉無しに前記側副靭帯が通る自由通路をもたらすことを特 徴とする請求項16に記載のプロテーゼ装置。 23.前記指節骨エレメントの関節頭部が、(a)手掌面側の亜脱臼に充分耐え 得る中手骨エレメントの関節頭部の捕捉をもたらす背面前突と、(b)曲げ運動 の際に指の手掌面靱帯との干渉を防止するように前記背面前突に対し充分に離れ る手掌面側リム部と、(c)幹部分の長手方向軸に対して約75°から約85° の間の背面角度で配された幹部分の端にある平坦なカラー部と、を有することを 特徴とする請求項17に記載のプロテーゼ装置。 24.前記指節骨エレメントの頭部が前記ソケット状表面を取り囲み、略円環状 表面部として形成された基端側リムを有し、該頭部は前記円環状リムと接する直 線状の側面および手掌面を有し、前記円環状リムは、前記プロテーゼ装置の矢状 面内に位置し、前記指節骨エレメントの幹部分の長手方向軸に対して約95°か ら約110°の間の背面角度で配されている軸線を有する円筒に接することを特 徴とする請求項17に記載のプロテーゼ装置。
