Description
CAMPO DELL 'INVENZIONE
L'invenzione riguarda l'uso dell'enzima glutatione reduttasi (GSSG reduttasi) per la preparazione di medicamenti per il trattamento di pazienti HTV sieropositivi o già affetti da AIDS conclamato, a scopo sia preventivo sia curativo dell'AIDS.
ARTE PRECEDENTE
L'AIDS (Acquired Immune Deficiency Syndrome)può essere considerato una patologia immunodeficiente secondaria indotta dai virus umani dell'immunodeficienza (HIV).
Il profilo immunologico esibito in AIDS, dall'infezione iniziale agli stadi terminali della malattia, mostra gli effetti della devastazione graduale del sistema immunitario.
Nell'AIDS la comparsa dell'immunità cellulare e della sorveglianza immunitaria attraverso la distruzione dei linfociti T CD4+ consente l'invasione di organismi opportunistici, con la conseguente comparsa di infezioni di origine virale, fungina e protozoaria, così come la comparsa di tumori di origine virale legata all!assenza delle cellule del sistema immunitario preposte alla soppressione della loro insorgenza.
La terapia dell'AIDS è attualmente basata sull'uso di agenti anti retrovirali che possono essere divisi in tre categorie:
inibitori della HIV proteasi;
inibitori nucleosidici della transcrittasi inversa (NRTI);
inibitori non-nucleosidici della transcrittasi inversa (NNRTI).
Tali farmaci possono essere utilizzati da soli o più spesso in associazione tra loro.
L'approccio terapeutico basato sulla vaccinazione è al momento ancora a livello sperimentale, con risultati interlocutori. La terapia dei pazienti affetti da AIDS comporta inoltre la somministrazione degli opportuni agenti atti alla cura delle summenzionate infezione opportunistiche.
Un aspetto noto dell'infezione da HIV è l'aumento dello stress ossidativo che può contribuire a potenziare la replicazione dell’HIV stesso ed almeno in parte spiegare anomalie immunologiche associate alla patologia da HIV.
E' stato osservato che nei pazienti infettati da HIV l'aumento dello stress ossidativo è associato alla deplezione degli antiossidanti naturali ed in particolare del glutatione ridotto (GSH).
A livello sperimentale è stato poi dimostrato che GSH ed N-acetilcisteina possono inibire il processo di transcrittasi inversa dell'HIV-1 (M. Kameoka et al., AIDS Res. Hum. Retroviruses 12, 1635-8 (1996)) e che GSH somministrato in un modello murino di immunodeficienza è in grado di ridurre il carico di DNA provirale nel primo periodo dell'infezione (A.T. Paiamara et al., AIDS Res. Hum Retroviruses 12, 1373-81 (1996)). Nonostante tali risultati, per quanto ci è noto, l'uso di GSH non è mai stato associato a trattamenti terapeutici diretti alla cura dell'AIDS.
E' stato ora sorprendentemente trovato, ed è oggetto della presente invenzione, che la somministrazione di glutatione reduttasi è in grado di ripristinare le difese immunitarie naturali e può validamente contribuire alla prevenzione dell'insorgere della patologia indotta dal virus HIV ed al trattamento della stessa nei pazienti già infettati.
RIASSUNTO DELL'INVENZIONE
La presente invenzione riguarda quindi l’uso dell'enzima glutatione reduttasi (GSSG reduttasi) per la preparazione di un medicamento per la cure e la prevenzione delle infezioni da virus HIV.
Un altro aspetto dell'invenzione è costituito da composizioni farmaceutiche contenenti una quantità efficace di glutatione reduttasi in miscela con veicoli opportuni.
DESCRIZIONE DETTAGLIATA DELL'INVENZIONE
La glutatione reduttasi o GSSG reduttasi è sorprendentemente in grado di inibire il ciclo vitale dell'HIV, come è stato possibile dimostrare in prove in vitro su colture di cellule infettate.
Si ipotizza che la GSSG reduttasi sia in grado di contrastare rallosterismo citotossico che insorge in alcune proteine di HTV, in particolare nella proteina NCp7, a seguito di scompensi in vie metaboliche cellulari dipendenti dal rapporto NAD/NADPH. La validità dell'invenzione non è comunque in alcun modo legata alla verifica sperimentale di tale ipotesi.
La glutatione reduttasi è un enzima ubiquitario che catalizza la riduzione di glutatione ossidato (GSSG) a glutatione (GSH). ,
La glutatione reduttasi è un enzima omodimerico appartenente alla famiglia delle flavoproteine disolfuro ossidoriduttasi che può essere estratto da più fonti (eritrociti umani, fegato di mammiferi, batteri o lieviti) e ha peso molecolare compreso tra 100000 e 118000 dalton.
La glutatione reduttasi è disponibile in commercio oppure può essere otenuta con metodi convenzionali estrativi o di DNA ricombinante.
In alternativa all'enzima, si può ipotizzare l'uso di composti ad attività agonista o capaci di stimolare la produzione di glutatione redutasi, agendo ad esempio sugli elementi di controllo trascrizionale del gene corrispondente.
Per il previsto impiego terapeutico, l'enzima può essere formulato in opportune formulazioni farmaceutiche ricorrendo a tecniche ed eccipienti convenzionali, come descritto ad esempio in "Remington's Pharmaceutical Sciences Handbook", Mack Pub. Co., XVII ed., NY, USA.
Le formulazioni farmaceutiche dell'invenzione sono preferibilmente somministrate per via endovenosa o inalatorià. Esempi di formulazioni adate comprendono fiale di uso immediato o da ricostituire. Non è da escludersi la possibilità della somministrazione orale, ricorrendo ad esempio a formulazioni liposomiali o ad altre tecniche note per favorire l'assorbimento orale di sostanze di natura proteica.
Nel primo caso, oltre alla GSSG reduttasi, la fiala può contenere opportuni eccipienti quali sodio edetato, sodio glutammato, poligelina oltre a una soluzione tampone organica, ad esempio trometanolo o inorganica (fosfati misti) che garantisca un pH tra 6.5 e 7.6.
Nella formulazione da ricostituire l'enzima sarà preferibilmente in forma liofilizzata in presenza di supporti, quali ad esempio latosio, mannite, fosfati e simili, mentre la fiala solvente potrà comprendere oltre alla soluzione fisiologica contenente sodio cloruro allo 0,9%, opportuni preservanti, quali ad esempio alchil pidrossibenzoati e le soluzioni tampone sopra citate.
La posologia giornaliera può essere compresa nell'intervallo di 1-10 mg di enzima, preferibilmente 1,5-2, 5 mg una o più volte alla settimana, preferibilmente due volte alla settimana, fino alla normalizzazione dei valori plasmatici di GSH. La somministrazione della GSSG reduttasi potrà essere effettuata da sola o anche in associazione con le altre classi di farmaci in uso -per la cura dell· AIDS e delle concomitanti infezioni . opportunistiche .

- 3. Uso di glutatione reduttasi per la preparazione di medicamenti per il trattamento e la profilassi di infezioni da HIV.
