Abstract
본 발명은 Pneumocystis carinii 폐렴의 1차 및 2차 예방과 치료를 위한 테르비나핀의 사용에 있어, 이 종류의 폐렴으로 고통받는 환자에게 테르비나핀 3-20mg/kg/일을 경구투여 하는 것을 포함한다.
Description
본 발명은 테르비나핀[(E)-N-(6,6-디메틸-2-헵텐-4-인일)-N-메틸-1-나프탈렌-메탄아민]의 새로운 치료학적 용도에 관한 것이다. 보다 정확하게 말하면, 본 발명은 Pneumocystis carinii 폐렴, 또는 간질형질세포성 폐렴(pneumocystosis)의 치료방법을 위한 테르비나핀의 사용에 관한 것이다.
Pneumocystis carinii 폐렴은 영양부족의 신생아와 백혈병 또는 림프종환자, 뿐만아니라 면역억제 요법을 받는 이식수용자 또는 환자에게서 가장 빈번하게 대면하는 질병중의 하나이다. 또한, 그것은 AIDS 환자의 가장 빈번한 전염병이며 주요 사망원인이다.
치료는 본질적으로 중요한데, 왜냐하면 미치료된 질병은 100%에 가까운 매우 높은 사망률을 가지기 때문이다. 즉시 치료되는 경우, 간질형질 세포성 폐렴은 25 내지 30% 사망률 (첫번째 증상발현) 을 갖지만 재발이 빈번하며 (6개월째에 35%, 12개월이내에 60%), 50 내지 60% 사망률이 관련된다. 평균 수명 예상은 영국에서는 첫번째 증상발현으로부터 12.5개월, 그리고 미국에서는 9개월로 계산하고 있다.

펜타미딘은 3주 동안 4mg/kg/일의 투여량으로 정맥내 또는 근육내 투여된다. (후자의 경로는 무균 종기를 유발할 수 있다). 주요 부작용은 저혈당증, 빈박, 동맥 저혈압, 쇼크, 혈청 크레아틴 상승, 혈청 트랜스아미나제 상승, 환각, 미각장애, 오심, 구토, 설사, 피부발진, 백혈구 감소증 및 혈소판 감소증이다.
또한 펜타미딘은 P. carinii 폐렴의 1차 및 2차 예방에 있어 에어로솔에 의해 사용될 수 있다. 이 경우 폐포공간내 고농도 (〉 10배) 와 저혈액 레벨 (간 경로에 의한 600ng/mℓ에 대하여 〈 10mg/mℓ) 에 도달한다. 이 투여방법으로 혈액부작용은 관찰되지 않았다. 그러나, 감기, 기관지 경련, 상부 폐엽내 재발 및 기흉이 일어날수 있다. 약제는 매 30일 300mg의 투여량으로 투여된다.
예방 목적을 위해 사용될 수 있는 다른 약제는 연속적으로 또는 매주 3일 연속으로 경구투여되는 코트리목사졸(160/800mg 1일 2회) 과 팬시다(Fansidar, 1주 1정) 이며, 그러나 후자의 약제로 얻어진 결과는 모순된다.
다른 요법은 21일 동안 1일 1회 경구투여되는 트리메토프림-다프손(20/100mg/kg/일)이다. 주요 금기는 글루코스 6-포스페이트 탈수소효소 결핍과 지도부딘의 동시 흡수이다.
드물게 사용되는 치료법은 트리메토트렉세이트(30/60mg/㎡정맥내) + 루코보린(20/80mg/㎡근육내 또는 경구) 에 기초한 것이다. 피부발진 또는 혈액 이상이 일어날 수 있다. 에플로르니틴(100mg/kg 매 6시간 정맥내) 은 감소된 Hb, 혈소판 감소증, 피부발진 및 오심을 유발할 수 있다.
클린다마이신과 프리마퀸의 복합제제 (클린다마이신 900mg 매 8시간 정맥내 + 프리마퀸 30mg/일) 은 오심, 혈액 이상 또는 간 기능 매개변수 변화를 일으킬 수 있다.
간질형질세포성 폐렴의 급성상태 동안에 코르티코스테로이드가 투여될 수 있으며, 그것은 단핵성 침윤과 2형 폐포세포 과형성에 대해 일정한 제한 작용을 주는 것으로 생각된다. 또한, 그것은 포스포리파제 A의 작용 (계면활성제 분해) 을 억제하는 것으로 생각된다.
현재, 테르비나핀은 오로지 천재성 진균증의 치료에 사용되고 있다. 테르비나핀에 관하여 얻어진 지식은 지금까지는 P. carinii 폐렴의 치료에 대한 그것의 사용은 제외된 것 같다 (Balfour J.A., Faulds D. Terbinafine: E.R. review of its pharmacodynamic and pharmacokinetic properties and therapeutic potential in superficial mycoses. Drugs, 43:259-284, 1992). 더욱, 폐국균증과 효모균증에실행된 연구에서 보고된 결과에 따르면 고질적인 진균증의 치료에서 그것을 사용하려는 시도는 실패하고 있다 (Haeuser M, Schmitt HJ, Bernard EM, Armstrong D.A. new bioassay for Terbinafine: Eur. J. Clin. Microbiol. Infect. Dis., 7: 531-538, 1988; Polak A., Dixon D.M. Chemotherapeutic activity in a mouse model of cryptococcosis with cutaneous and nasal involvement : Mycoses, 31: 501-507, 1988; Schmitt H.J., Andrade J., Edwards F. et al. Inactivity of terbinafine in a rat model of pulmonary aspergillosis, Eur. J. Microbiol. Infect. Dis., 9: 832-835, 1990). 이들 데이터에 기초하면, 테르비나핀은 면역 저하된 숙주의 고질적인 진균 감염 또는 간질형질세포성 폐렴의 치료에 효과적인 것으로 간주되지 않을 수 있다.
그러나, 놀랍게도 테르비나핀은 5 내지 20mg/kg/일의 범위의 경구투여량으로 이식 수용자, 암환자 또는 영양부족환자와 후천성 면역결핍증(AIDS) 또는 면역억제제로 치료받거나 방사선치료 받은 속발성 면역결핍증 환자에게서 P. carinii 감염의 예방 및 치료에 대하여 강력하게 효과적이라는 것을 이제 발견하였다.
테르비니핀의 효능은 동물과 사람에게서 다양한 연구를 테스트하고 있다. 또한, 상술된 투여량으로의 테르비나핀 투여는 어떤 앞서 언급된 독성이나 원치않은 부작용을 유발하지 않는다.
동물연구
코르티코스테로이드로 치료된 래트의 P. carinii외 감염을 예방하는데 있어 테르비나핀의 효능을 평가하는 것을 목적으로 연구를 실행하였다.
체중이 180 내지 200g 인 31마리 Sprague-Dowley 래트로 구성된 두가지 뱃치를 고려하였다. 각각의 래트를 코티손 아세테이트(25mg/kg 피하) 로 면역억제 처치하고 12주동안 저단백질(8%) 식이요법을 행하였다. 4주째에, 뱃치 A 래트를 경구 테르비나핀으로 처치하고 또한 코티손 처치하였으며; 반면에 뱃치 B 래트는 죽이기전까지 코티손아세테이트만을 투여하였다. 1주 간격을 두고 각각의 래트의 체중을 측정하였다. 12주 끝무렵에, 모든 래트를 죽이고 폐를 무균적으로 제거한 후 사용할때까지 -70℃에서 저장하였다. 각각의 폐에 대하여 세균과 곰팡이를 분리시키기 위한 조직학적 표본외에 P. carinii- 특이적인 세포학적 염색에 폐 조직을 사용하기 위해 슬라이드로 만들었다(Giemsa 및 Gomori-Grocott). 또한 폐조직의 부분을 분쇄하고 HBSS (행크스 균형 염용액) 에 균질화시키고 콜라게나제와 히알루로니다제로 분해하였다. 이와같이 얻어진 물질을 반복해서 세척하고, HBSS에 재현탁하고 면역세포화학적 염색을 하였다 (P. carinii 모노클론 항체로의 면역형광법). 얻어진 결과는 다음 표에 주어진다.

뱃치 B의 다섯마리 래트(12.1%) 가 7주 및 8주 경과중에 죽었으며 이들 경우의 어느것도 폐조직의 여러가지 적당하게 처치되고 염색된 부분에서는 P. carinii의 존재가 검출되지 않았다. 그러나, 후자에서 수많은 그램-양성 및 그램-음성 유기체가 분리되었다. 이 뱃치의 나머지 래트중에 P. carinii는 18마리 (58%)에게서 발견되었으며 폐 절개면에는 Pneumocystis의 만연된 침윤(20 내지 30cysts/필드) 이 나타났다.
120 내지 160g 범위의 체중감소가 기준과 비교했을 때 발견되었다.
뱃치 A의 래트 중에서 P. carinii는 단지 1마리(3.2%)에게서 검출되었으며 감염도는 약간나타났다 (4 내지 9cyst/필드). 체중감소는 이들 래트중 어떤 것에서도 관찰되지 않았다. 뱃치 A래트는 모두 죽지 않았다.
사람연구
연구 샘플을 AIDS와 전에 P. carinii 폐병 (제 1증상발현) 을 가진 7명의 환자로 구성하였으며, 그중 4명 (남성 3명, 여성 1명, 평균연령 37세, 모두 이성애자) 은 코트리목사졸 (술파메톡사졸 800mg + 트리메토프림 160mg 3일 동안 1일 2회 투여) 로 2차 예방하였으며 3명 (남성 1명, 여성 2명, 평균연령 29세, 모두 정맥내 약물 남용자) 은 펜타미딘 에어로솔 (매달 300mg)로 예방하였다. 모든 환자에게 여러번 CD4 + 림프구 〈 200㎣ (141 ±7.9)를 제공하였다. 각각의 환자를 11개월 추적검사를 하고, 그 기간중에 방사선 조사와 유도객담 검사를 30일 간격을 두고 행하여 P. carinii를 검출해서 치료효능을 평가하였다. 또한, 다수의 실험 매개변수도 평가하였다 (전체 혈구 카운트, 간 및 췌장기능테스트, 혈청 LDH 분석등) 트리메토프림-술파메톡사졸로 예방 조처된 환자에게서는 어떠한 변화도 방사선 사진에서 검출되지 않았으며 유도 객담에서 P. carinii가 존재한다는 증거도 발견되지 않았다. 펜타미딘 예방조처된 3명의 환자중, 1명은 치료 12주 무렵에 약물의 비내성을 보였으며, 반면에 다른 2명에게서는 유도 객담내 P. carinii의 존재에 대한 반복된 증거가 있었다. 방사선 의학에서 이들 환자는 임상 징후의 부재시 폐조직의 범발성 항진을 나타내었다. 이들 데이터에 기초하여 상기 언급된 3명의 환자는 테르비나핀 치료를 시작하기로 동의하였다. 약제는 500mg/일의 투여량으로 경구 투여하였다.
최초 투여한지 8개월 후, 특이체질 현상이 기록된 바 없으며 다양한 실험 매개변수는 트랜스아미나제와 LDH 값의 약간의 상승(1명) 을 제외하고는 정상적인 한도내에 있었다. 치료기간을 통하여 객담내 P. carinii에 대한 반복된 음성 현미경 조사와 폐방사선 사진의 정규화는 주목할만하다.
