The device according to the invention is particularly intended to secure and improve medication management for all patients and particularly those dependent and / or suffering from long-term and / or poly-medicated conditions.
Description
The present invention relates to a medical device for automatically dispensing adaptable and secure tablets to the final patient. For the sake of simplicity the term tablet will generally designate tablets, tablets or capsules.
The purpose of the present invention is to guarantee, trace, communicate and archive that not only the right medicine has been dispensed at the right time, the right patient and the right way, but also that the delivered dose matches perfectly , not only to the medical prescription, but also and above all is adapted to the actual state of health and not supposed of the patient. The present invention indeed has physical and intelligent means for delivering the right dose of medication by adapting it in real time based on internal and external data.
By internal data is meant, by non-limiting examples, a database of existing drugs and their conditions of use, the initial prescription of the doctor, the identity of the patient, etc ... By external data, we mean, by non-limiting examples the biological data of the patient in relation to his state of health (acquired and transmitted by complementary devices), the prescription updated by the doctor, etc ... In addition, the ability of the invention to protect and to trace the drugs from their diet by a qualified person until they are actually taken by the patient, helps to secure the drug circuit.
These new functionalities of adaptation and therefore of real-time optimization of the delivered dose thus removes the present invention from the current pillboxes (whether connected, intelligent or not), and contribute to the inventive and novel nature of the invention. They make it possible to improve the patient's compliance with the drug treatment, but also to enable the doctor to optimize the treatment according to the patient's state of health (decrease in sub-optimal prescriptions) and to avoid iatrogenic and self-medication risks. Indeed, a large number of products exists with the purpose of organizing the various drug intake of patients. They are all in the form of identified cells. An alveolus corresponding to a temporal take of drugs Each alveolus is filled according to the prescription of the doctor. Thus, each cell frequently comprises one or more tablets of different pharmaceutical specialties. A pharmaceutical specialty is defined as a drug prepared in advance, presented in a special package and characterized by a special designation. Each pharmaceutical specialty is therefore present at a dose defined by the prescription of the doctor and that it is possible to modify that by repackaging the pill. At the appropriate time, the patient must open the correct cell, and ingest all the tablets it contains. New developments exist on this type of device, allowing for example to inform the patient of the cell to open and when. Some connected devices also allow a third party to be notified if the outlets are not respected (ie when the cells have not been opened at the right time). These devices are therefore organizers of tablets. And if they appreciably improve the observance of the treatment by the patient they present certain defects. First observed defect, in the case of poly-medicated patients the preparation of the cells is tedious and especially source of potential errors. It is indeed complex to fill a large number of cells with different amounts of tablets of several pharmaceutical specialties. In addition, this preparation requires additional human manipulation of drugs that can cause hygiene problems or even drug contamination. Second defect observed, once the prepared cells, a multitude of different pharmaceutical specialties are in contact with each other. This can cause chemical interactions between drugs with consequences for the patient potentially harmful to his health. Third defect observed, once the cells are filled, their contents are frozen. If the dose of only one of the specialties has to be corrected for various reasons, such as a bad reaction to a drug (abnormal biological data) or maladjustment of the intended dosage, the preparation of the cells is to be repeated from the prescription updated by the doctor. This update requires a new medical consultation and therefore a delay detrimental to the health of the patient. This update also requires a new packaging pill and thus a new human manipulation with the risks mentioned above and the creation of a stock of drugs to destroy. Thus, to overcome these drawbacks described above, according to a first aspect, the invention proposes a multitude of reservoirs in which the tablets to be dispensed are placed. Here, unlike existing devices, the tablets are not arranged by setting (a dose being defined by a number / precise dose of different tablets and different pharmaceutical specialties to be taken at a given moment), but by pharmaceutical specialty. In other words, each reservoir contains the tablets of one and the same pharmaceutical specialty. The number of tablets placed in each reservoir is consistent with the prescription established by the physician (s). Thus the proper filling of each tank is much simpler and safer than that of the cells of the pillboxes. Thanks to this simplification of filling by an authorized person, a simple and reliable automation of this operation 15 could therefore be envisaged. The only variable concerns the number of tablets present in each tank, and even in the event of a filling error, the present invention will be able to detect and correct it. According to a second aspect, the invention proposes a system for identifying and controlling the reservoirs and tablets they contain. Whether physical or digital, each tank has a unique identification linked to all the information of the pharmaceutical specialty it contains. The tanks can be fixed on the inventions and filled directly on it or removable to facilitate filling and handling. According to a third aspect, the invention provides a motion generation system which allows the tablets to be moved from the inside to the outside of the tanks. This generation of movement can be powered indifferently by electrical energy, mechanical, pneumatic or any other. The movement to move the tablets 30 is indifferently a movement of rotation, translation, vibration or any other. According to a fourth aspect, the invention has counting means for measuring and counting the number of tablets that have been moved from the inside to the outside of each tank.
These control means are indifferently, optical, mechanical or any other. In a fifth aspect, the invention provides a buffer chamber system prior to final dispensing to the patient. Indeed, if the aforementioned control means detect an abnormality (a bad counting of the tablets), a motion generating system 5 will move the tablets to a compartment inaccessible to the patient. A new dispensing cycle can be relaunched if necessary. In the event that no abnormality is detected, the motion generation system will move the set of tablets to a compartment accessible to the patient.
According to a sixth aspect, the present invention provides an artificial intelligence system. The said artificial intelligence is capable according to all the available information (either in its memory, or remotely in an external database, or provided by the diagnostic accessories, or by another source) to calculate and interpret the the most appropriate dose to provide at the time of taking drug and this, within the limits 15 defined by the prescriber. The prescriber may at any time modify and optimize his initial prescription remotely or locally. The so-called artificial intelligence also has means for controlling the mechanical elements in order to guarantee a perfect match in real time between the calculated optimum dose and the dose actually delivered. And this without any new human manipulation of the drugs. Another aspect of this artificial intelligence is its ability to statistically record and analyze all incoming and outgoing information to determine, for example, the effectiveness rate of a specific treatment or a combination of treatments. This must be done in accordance with the regulations on the protection of privacy and freedom.
USB).According to a seventh aspect, the invention provides one or more Machine-to-Machine communication systems. These systems can communicate up and down with other remote devices. These can be diagnostic systems, telephone systems, computer systems. The communication support can be Bluetooth or wifi type or other protocol, either physical (eg USB).
These systems also allow, thanks to an ergonomic interface, the exchange of real-time information between the patient and the network of caregivers (request for advice from the patient to his doctor or pharmacist, request news from the patient by in a particular embodiment of the invention, a multitude of removable tablet reservoirs are inserted, after a prior filling of the tablets, in the locations of the invention.
The identification carried out during the preliminary filling phase enables the artificial intelligence of the invention to know the nature and the quantity of the tablets present in each of the tanks and therefore the locations. Once parameterized by local or remote communication means, the artificial intelligence will know the timing and composition (i.e., the nature and amount of the tablets) of all the patient's drug intake. At the right moment, the artificial intelligence, via the interface man / machine will alert the patient for the taking of drugs. Depending on the internal information parameterized in the artificial intelligence, but also on the basis of external data from, for example, a biological constants sensor, the artificial intelligence can calculate and optimize the exact dose of each pharmaceutical specialty to be delivered within limits. predefined by the prescriber. It will then drive each motion generation system corresponding to the 20 locations in which the tablets to be delivered are located. The motion generated by the motion generation systems will be transmitted to the tablet reservoirs. Each tank will adequately transform this movement to release the tablets one by one. When the counting system has noticed the fall of the expected number of tablets, the generation of movement will be stopped.
If all the counts of all the tanks are in accordance with the order, the tablets previously stored in a buffer chamber will be delivered to the patient. Otherwise they will be put off. Artificial intelligence, will record dispensing data of pharmaceutical specialties (nature, number and moment). It may, if necessary, communicate them to any other third-party product or application in accordance with the regulations on the protection of privacy and freedom.
Other objects and advantages of the invention will become apparent from the following description, with reference to the accompanying figures, in which: - Figure 1 is an overall perspective view of the present invention; Figure 2 is an exploded perspective view of the invention; FIG. 3 is a perspective view of the only activation subassembly; - Figure 4 is an exploded perspective view of the only subset of activation; FIG. 5 is a perspective view of a single and only compressed reservoir + geareducer assembly; FIG. 6 is a perspective view, an exploded view and a sectional view of a tablet reservoir and its dispensing Referring more particularly to Figures 1 and 2, the present invention is constructed in a vertical pattern. Each floor having a particular function. The removable cover (1) allows easy and quick loading of the tablet reservoirs (13) in the invention. Once closed, the lid (1) secures access to the tablet reservoir (13) and contributes to the proper preservation of said tablets. The base (2) has shapes materializing the different locations of the tanks; in this particular embodiment the invention comprises nine locations. The activation subassembly (3) is the motorized stage transforming the rotation of the geared motors into one-to-one delivery of the tablets. It is therefore composed of all the elements of transmission of power necessary for this transformation of movements. Figures 3, 4 and 5, to better understand the construction and operation of this subassembly. Indeed, the tablet reservoirs (13) are fixed through the upper support (17) and on the lower support (16). This lower support (16) is also the place of attachment of the gears (18) and counting means (15). The geared motors (14) are fixed on the upper support (17). The upper support (17) and the lower support (16) are precisely positioned one by the other by a centering system and screwed pins.
Thus, once inserted through the upper support (17), the tablet reservoirs (13) become jammed in the proper shapes of the lower support (16). In this way, the toothed wheels (18) engaged with the geared motors (14) are meshing in the 3032350 compressed tanks (13) through their plug (21) on the separating wheel (20). Figure 6 illustrates this feature more clearly. In fact, the rotation of the geared motors (14) will cause the rotation of the gears (18) and thereby the rotation of the separation wheels (20) of the tablet reservoirs (13). The rotation of the separating wheels (20), by their internal forms, but also by the associated forms of the plugs (21), will result in the drop of the tablets vials (19) through the plugs (21). The counting means (15) being placed directly opposite the outlets of the plugs (21), they are able to count all the drops of the tablets.
Once delivered, the tablets fall into the receptacle (4), whose function is to direct all the tablets to the buffer chamber whose distribution valve (5) is the bottom. This dispensing valve, according to the instructions given by the artificial intelligence, will have the function of moving the tablets either on the dispensing side or on the reject side of the valve support (7). Thus, the tablets will be found respectively in the dispensing drawer (9) or in the scrap drawer (10). These drawers being housed, as well as two storage drawers (8), in the drawer support (11). The base (12) meanwhile will serve to maintain the whole of the invention in stable equilibrium position on a flat surface, such as a table.
The invention also has a man / machine interface (6) consisting of the latest existing technologies for upstream and downstream communication. As non-limiting examples, a touch screen and a camera, a speaker and a microphone, but also potentially biometric sensors such as digital sensors or retina.
Thus, in particular, it is not possible to depart from the scope of the invention: by modifying the number of tablet reservoirs or by uniting the tablet reservoirs with the invention, by modifying the principle of Activate tablet reservoirs o Change the number of motion generating systems o Change the buffer chamber number o Change the principle of moving tablets from the inside to the outside of the drug reservoirs by removing the buffer chamber and / or its motorized valve o by modifying the invention to deliver other dosage forms of drug 5 than those mentioned (non-limiting examples syrup, powder, etc ...) o by adding a system of detection of opening of the reservoir by issuing o Machine-to-Machine communication or information exchange devices with other devices o Adding communication or information exchange devices between the patient and other services or other services such as personal assistance The device according to the invention is particularly intended to secure and improve the effectiveness of medication management for all patients and especially for: - dependent persons, whether at home or in care facilities. and / or those suffering from long-term conditions and / or poly-medicated persons
