L'invention concerne de façon générale un dispositif global pour la pose d'un système de protection de broche antimigration pour la chirurgie osseuse complété par une pince coupante sertisseuse. GLOBAL DEVICE FOR PLACING A PROTECTIVE PLUG WITH CUTTING CRIMPING FORCEPS FOR ANTIMIGRATION PIN. 10 La pose d'une broche en chirurgie plus particulièrement osseuse est un geste quotidien effectué par les chirurgiens orthopédistes. En effet, l'un des moyens les plus utilisés pour réunifier deux fragments d'os disjoints par une fracture par exemple est l'utilisation de broches (tiges droites avec une ou des extrémités pointues) en inox biocompatible. Cette broche de diamètre 15 adapté à la taille de l'os concerné va, lors de sa mise en place, transfixier les deux fragments et ainsi les réunifier intimement afin que puisse commencer la repousse osseuse génératrice d'un cal qui va solidariser ces deux fragments. Après avoir procédé à une légère ouverture cutanée afin d'accéder à l'os, le premier geste du chirurgien sera de mettre en place la broche soit avec un porte broche dit pièce à main ou par le biais 20 du mandrin d'un moteur. Le second geste consistera à mettre à la bonne longueur la broche afin qu'elle ne ressorte que de 2 ou 3 mm au-dessus de l'os. Cette mise à longueur nécessite de couper le broche avec une pince coupante. Le fait de couper la broche, comme dans toute coupe de métal à l'aide d'une pince, induit que cette extrémité n'a pas une coupe franche et peut donc être extrêmement agressive, blessante. 25 Ce d'autant plus que le chirurgien va refermer la légère ouverture pratiquée dans la peau et que cette extrémité agressive, coupante, va alors se trouver plus particulièrement dans le poignet, siège principal apte à recevoir une ou des broches, en contact direct avec les tendons nombreux à cet endroit. Les résultats de cette confrontation des tendons avec l'extrémité coupante de la broche sont bien connus et se 30 traduisent soit par la rupture totale du tendon soit son endommagement. La `conséquence immédiate est la paralysie totale ou partielle d'un doigt. En conclusion, l'os fracturé a certes été réparé mais au prix de la perte de mobilité d'un doigt. Par ailleurs, la même broche indépendamment d'être agressive par son extrémité coupée, une fois installée dans l'os, 35 a une fâcheuse tendance par son autre extrémité à migrer c'est à dire à se déplacer. Cette migration peut aller jusqu'à transpercer la peau et ainsi se retrouver à l'air libre. Bien évidemment le patient n'apprécie pas cet état de fait et encore moins le chirurgien. L'inconvénient majeur de ce sertissage effectué 45 avec la pince coupante est bien entendu le risque de couper ce capuchon. A l'usage un autre risque est rencontré: le bouchon métallique sur tout son pourtour extérieur est parfaitement lisse aussi vu sa petite taille et son polissage, il est particulièrement malaisé de le saisir avec la pince et de le serrer fortement afin de le sertir fermement..5 Le résultat est soit un mauvais sertissage soit la destruction du bouchon avec tous les inconvénients qui en résultent. Un autre inconvénient notable de ce bouchon de protection toujours à cause de sa petite taille et de son polissage est la grande difficulté qu'ont les chirurgiens à le saisir entre deux doigts pour l'amener sur le bout de la broche. En effet, il ne faut pas oublier qu'à l'instant de mettre en place le bouchon, les chirurgiens ont des gants ce qui ne facilite pas la préhension et de plus bien souvent ces gants sont maculés de sang d'où des effets de glissements. Si l'on se réfère au dessin explicatif (Figure 1 ), on peut penser qu'à chacune de ses extrémités elle débute et finit progressivement c'est à dire qu'elle présente une forme incurvée concave et convexe sensée donner une certaine progressivité. Cependant à l'usage cette progressivité va parfois à l'encontre du but recherché c'est à dire le blocage dans l'os et si ce dernier n'est pas effectif l'effet antimigration ne fonctionnera pas avec toutes les conséquences qui en découlent. Pour pallier à tous ces inconvénients et dans le mode de réalisation préféré, l'un des objets de la présente invention afin de pallier à la difficulté qu'ont les chirurgiens à saisir le bouchon consiste en un système appelé préhenseur ( Figure 1) . Constitué d'un corps cylindrique, ou de toutes autres formes, suffisamment long pour pouvoir être pris en main, il comporte à une de ses extrémités un logement apte à recevoir le bouchon (Figure 2) destiné à être placé sur l'extrémité coupée et blessante de la broche. Ce logement en creux est réalisé de manière à ce que le bouchon y prenne place mais seulement sur un tiers environ de sa longueur. Les deux autre tiers de ce bouchon dépasseront à l'extérieur du logement de manière à être visible (Figure 2) mais aussi et surtout de manière à ce que la gorge, objet d'une autre invention décrite plus loin, soit bien apparente pour en assurer le sertissage. Le logement du bouchon est conçu de manière à ce qu'il soit maintenu fermement jusqu'au moment de l'utilisation c'est à dire qu'il faut que le diamètre interne soit le plus justement calculé pour éviter que le bouchon ne se désolidarise du préhenseur avant son utilisation par l'effet d'un choc ou sous les contraintes liées à la stérilisation puisque cet ensemble bouchon/préhenseur est destiné à être implanté donc stérilisé. Idéalement concernant la matière à utiliser, une polymère tel un polyéthylène ou un propylène par exemple sont à retenir car ils possèdent des propriétés pseudo-élastiques qui sont avantageusement mises à profit pour la contention du bouchon. Bien évidemment toutes autres matières possédant de tels avantages peuvent être utilisées. La partie terminale du préhenseur ( Figure 2) contenant le bouchon se termine de manière effilée, en forme de cône, ceci afin de faciliter la vision du chirurgien qui ne doit pas avoir la vue de l'extrémité de la broche masquée par une masse trop importante du préhenseur. De plus, cette terminaison effilée permet au chirurgien une meilleure précision dans son geste lors de la mise en place du bouchon qui va coiffer la broche. Le second est le plus important, car en ayant effectué cette gorge, du métal a été ôté et ainsi cette portion a été affaiblie ce qui facilite le sertissage car la pince va rencontrer une moindre résistance. Il est préférable dans un mode de réalisation idéal que cette gorge ne soit pas réalisée de manière abrupte dans le bouchon mais au contraire en se creusant d'une manière progressive vers l'intérieur de sa cuvette. Cette disposition permet à la pince sertisseuse de trouver aisément l'emplacement idéal pour sertir. De plus, en n'ayant que des rayons au lieu d'angles, cela permet d'éviter d'implanter un élément qui présenterait des risques de traumatismes ou de blessures avec les tissus avoisinants. Cela n'allait pas sans quelques inconvénients dont le principal était de couper le bouchon si la pression exercée était trop importante. Pour y pallier, la nouveauté consiste en I'adjonction au milieu ou à la base des mors coupants d'une fente destinée à sertir. Cela permet au chirurgien dans la continuité de son premier geste de coupe par un léger déplacement en avant, de sertir par pincement le bouchon qu'il a placé sur l'extrémité de la broche qui vient d'être coupée. Cette pince revendique d'être à la fois coupante et sertisseuse et ce grâce à une encoche située au milieu ou en arrière des mors coupants. Comme toute pince coupante ; cette pince par le biais de la pression exercée par le chirurgien sur les deux branches qu'il a en main va par ses deux mors coupants parallèles cisailler la broche mais dans ces deux mors parallèles vers le milieu ou à l'arrière, un peu de matière a été enlevée sur chacun d'eux. Ainsi, lors de la réunification de ces mors on obtient une encoche transversale. Cette encoche lorsqu'elle va se refermer par pression sur les branches de la pince sertit par écrasement mais sans le couper, le bouchon posé sur l'extrémité de la broche par le biais du préhenseur qui peur alors être retiré laissant en place le bouchon solidement solidarisé à la broche.
