The invention relates to a specific method of manufacture of the orthodontic box described in part of the application no. 0708328 by the same author, from a French patent. This will serve as a reference for the manufacture of a housing integral with the tooth either via a base glued to it, or a ring which is sealed thereto. It therefore includes the base and a structure. Conventionally, this structure comprises a groove in which is placed a portion of a rectifying arc and one or two fins disposed on each side of the groove. An elastic ligature (Fig. 8) in the form of 0 (54) or metal passes under the fins (53) and above the arc (50). Conventionally, the walls of the groove are rigid and fixed, either by construction or by the contribution of external elements incorporated by the practitioner. For the description of the method of construction, the case is placed on the anterior surface of an upper incisor of a standing patient, head straight. The action of the ligature is materialized by the direction of the arrow B which tends to drive the arc in the bottom of the groove and that of the arrow A which tends to reduce the height of the groove (Fig. 6). To recall the idea described in this application No. 0708328, the vertical force A is used in a housing where part of the groove is movable and the other is attached to the base. Thanks to the tension of the ligature the throat passes from 0,76mm to 0,50mm automatically and progressively during the treatment, without any external intervention and as the tooth migrates. The clinical functioning of the finished housing is described further. The vertical dimension or occlusal gingivo of the groove of this structure is conventionally fixed and between 0.40 mm and 0.55 mm.
A structure will be made whose vertical dimension of the groove is variable, measuring 0.76 mm in the maximum open position and 0.50 mm in the minimum open position, these two conjugate values, preferred by the author, being not exhaustive. We take a classic case, amputated from the occlusal part of its host structure. A vertical lateral schematic section, FIG. 1, shows the relationships of the elements between them.
Drawn in fine lines, only the gingival part of the structure, the module A (10) is secured to the base (11) of the housing. This module may comprise from top to bottom one or two ligation fins (12) - two wings will have been chosen here -, at least the fixed horizontal gingival face (13) of the groove and the bottom (14) thereof with its projection (15) which overhangs the base. A median passage (16) between the feet of the fins is provided for the sliding of the module B. This passage is closed forward by a strip (17) stretched between the feet of the fins. The base can be either cast with the fixed structure or welded to it. In this choice of construction it is soldered (18) to the module A. The direction of the horizontal face on the plane of the base is either perpendicular or inclined as here: the groove is said to be torqued. On this same section, the mobile module B (20) drawn in thick line, may comprise from bottom to top one or two ligation fins (22), at least the other face (23) horizontal antagonist of the throat, sometimes the bottom of the latter, not shown here, since already present on the fixed module, and a vertical extension (21) which will slide in a housing (16) of the fixed module A (10). Fig. 2 shows the groove in the maximum open position and the extension of B in its housing: it is delimited by the feet of the fins of the module A (10), the base of the housing (11) and its coverslip (17) . A crimping point widens the end of this extension to create a stop (24) on the outlet of the housing. The height of the groove (30) is then 0.76mm. Fig. 3 shows the groove in the maximum closed position: the module B abuts on the projection (15) of the bottom of the groove; the height of the groove (40) is then 0.50mm. FIG. 4, anterior view of the housing in the open position, allows for: - the module A (10) fixed to the base (11) with its fin (s) (12) bent (s) back, the projection of its horizontal face (13), the bottom of the groove (14) overhanging the base and the projection of its -2-lower projection (15). - The mobile module B (20) with its fin (s) (22) bent (s) back, its sliding extension (21) on the base, in contact with the foot (s) of the wings of A and the stop set with its end (24) which prevents it from going out. A hatch (17) hatched in the drawing is attached to the module A to limit forward the housing of the extension (21). When B slides, its moving horizontal face (projection 23) approaches or moves away from the fixed face (projection13) of A along a path of 0.26mm. Fig.5 shows the assembly on a horizontal section at the housing of the movable extension. In the variant of FIG. 5A, the covering strip (17) of the extension and the module (10) form a single piece: the extension is arranged, then the base (11) is welded (S) to the module (10). . In the variant of FIG. 5B the base and the module form a single piece: the extension is arranged and the lamella (17) is welded (S) to the module. The variant of FIG. 5C shows the same section with a fixed module having only one foot of fin (s) (10) and a mobile module whose two extensions (21) slide on each side, above the base (11). ). Fig. 6 shows the operation of this finished housing in a clinical case chosen as extreme. It is schematically represented by the successive lateral sections A, B and C during the orthodontic treatment. A. The case comprises a ligature whose directions of force are represented by the arrows B and A; an arc portion 50 is inserted into its groove and maintained by this ligature. The force A is lower than the voltage at 51 of the arc on the mobile module and the groove remains in its maximum open position: 0.76mm; the mobile module remains retained by its crimping point 24. Section B. The proximal teeth have migrated, the leveling progresses, the voltage 51 of the arc on the mobile module decreases gradually until the force A of the ligation exceeds that of the fixed module on the arc. The system seeks its position of equilibrium: the horizontal faces (13 and 23) of each module are brought closer to each other until they come into contact with the arc. A couple appears whose moment in 51 and 52 will tilt the case so the tooth in the direction of torque already seen.
The module B has finished its race because it comes up against the projection of the groove (15). The described housing may be placed on an opposing tooth or on the lingual surface of a tooth. Similarly, the mobile module can be gingival instead of occlusal: its method of manufacture remains the same. All biocompatible and different dental materials can be used in this same housing; such, for example, a simple stainless steel wire for the manufacture of at least one of the two modules. Thus, in FIG. 7 there is again the mobile module provided with a fin (22), two portions of wire (23), homologous with the horizontal face of the groove described above and with the two portions of sliding wire, curved and welded (S) behind the foot of the fixed module, This arrangement is similar to that of FIG.
5C. 5C. There are two main advantages to this construction: - It creates a simple housing, of reduced size and less expensive construction, which allows the insertion of a single arc, even definitive, from the beginning of the treatment. The operator only changes it to the contention period by keeping the same boxes. - By using ligatures of different strengths or with the combined use of looser metal ligatures on some casings, it is possible to get some teeth sliding on the arc while keeping others motionless with tighter ligatures.
